Abstract

Background: Laser photocoagulation is a crucial therapy for numerous retinal diseases. Laser can be delivered with different machines and modalities (slit lamp, endolaser, indirect laser) at different wavelengths (532 to 812 nm) with varying parameters (power, spot size, duration, number of spots). New developments using semi automatic pattern delivery of retinal laser burns which use smaller amounts of energy and shorter duration have been reported. This literature review is conducted to review various published article that reported efficacy, safety, and duration of medium duration threshold patterned (MDTP) laser photocoagulation.Methods: This literature review was conducted from the Pubmed (NLM) database and Ophsource using the keyword panretinal photocoagulation, and Pascal or medium threshold or short exposure. The inculsion criteria of this review were all of studies which reported the use of medium duration threshold patterned which was applied as panretinal photocoagulation (PRP).Results: Parameter used in MDTP laser were as follow, spot size was between 200 to 400 μm, duration was 20-30 ms, and average laser power was more than 200 to 630 mV. Compare to standard laser treatment, power used in MDTP laser were higher.Conclusion: Medium duration threshold patterned laser photo-coagulation is as effective as conventional laser. MDTP laser can preserve the retinal sensitivity/visual field, also offers less pain and discomfort and safe to perform full scatter PRP in single sitting Keywords: medium duration threshold patterned laser; panretinal photocoagulation

Abstract

Background: To review the literature and report relative efficacy and safety of various tamponade agents used with surgery for rhegmatogenous retinal detachment (RRD) complicated by proliferative vitreoretinopathy (PVR) regarding functional success rate, anatomical success rate, and adverse effects. Methods: We searched articles from PubMed and Clinical Key database starting from the year of 1990 to November 2014. Information needed from all articles were extracted into data extraction sheet. We reviewed the efficacy of both tamponade agents regarding the postoperative visual acuity, macular attachment, and adverse effect. Results: Eight articles were included after fulfilling inclusion and exclusion criteria. Our analysis showed all studies shows an increasing visual acuity and percentage of macular attachment postoperatively of all type tamponade agents. Functional and anatomical success rate of Sulfur hexafluoride gas (SF6) was high in study with PVR stage B or less (94-98%). In patient with complex or recurrent RD associated with PVR, silicone oil had a better functional success (51% vs 32.5%) and better anatomical success (80% vs 60%) than SF6 gas. The anatomical success rate also higher in C3F8 than in SF6 group (73.9% vs 57.5% respectively). Functional and anatomical success rate of HSO as tamponade was high in patients with a severe PVR located mainly in the inferior quadrants (92.3%).Conclusion: As tamponade agents, C3F8 and silicone oil appear to have more visual and anatomic advantages over SF6 in participants with complex or recurrent RD associated with PVR. Silicone oil was better than C3F8 gas to obtain final visual and anatomic successs in patient with recurrent RD associated with higher grade PVR. Heavy silicone oil would be useful in particular cases complicated by recurrent inferior RD with PVR. Retinal redetachment, glaucoma, cataract, hypotony and keratopathy were reported in both gases and silicone group. Abstract

Background: To critically evaluate the efficacy and safety profile of intravitreal bevacizumab (IVB)compared to intravitreal triamcinolone acetonide (IVTA) in patients with diabetic macular edema(DME).Methods: We searched publication related to diabetic macular edema managed by bevacizumab andtriamcinolone acteonide through systematic search of database in a period of January 2002 untilOctober 2013. Information needed from all articles were extracted into a data extraction sheet. Wereviewed the efficacy of both treatment in decreasing central macular thickness (CMT), improvementof visual acuity and the adverse effects following the injection.Results: Twelve articles were included after fulfilling inclusion and exclusion cirteria. Our analysisshowed single IVTA injection were more effective in decreasing CMT and improving visual acuity at1 week to 3 months. There were no significant improvement of CMT in multiple injection of IVTAcompared to multiple injection of IVB.Conclusion: A single intravitreal injection of IVTA appears to offer certain short-term advantagesover IVB for the management of patients with diabetic macular edema, particularly with regard toCMT. Keywords: intravitreal, triamcinolone acetonide, bevacizumab, diabetic macular edema

Abstract

Background: To assess the agreement between a group of trained non-physician ophthalmic careproviders as primary grader, and vitreoretina specialist as secondary grader, in the interpretation ofsingle-field 45o, non-mydriatic digital fundus image for diabetic retinopathy (DR) screening in primaryhealth care.Methods: This is an analytic, retrospective study. A total of 117 fundus images of diabetic patientswere graded by primary grader, followed by secondary grader using the modified National ScreeningCommittee (NSC) as a guideline. Kappa statistics and Intraclass Correlation (ICC) were used for thereliability assessment of photograph quality, DR presence and classification, presence of maculopathyand photocoagulation, severity level, and presence of vision-threatening diabetic retinopathy (VTDR).Diagnostic values of fundus image interpretation by primary grader in detecting DR using secondarygrader as the reference standard were also assessed.Results: Agreement for DR classification and its severity level was good (ICC 0.654; 0.642). Fairagreement was found for presence of maculopathy and VTDR (κ 0.368; ICC 0.595). Slight agreementwas found for photograph quality (κ 0.170). The percentage agreement in identifying DR was 29.9%.Sensitivity and specificity of fundus image interpretation by primary grader in DR and VTDR detectionwere 52.24% and 71.43%, 54.55% and 95.12%, consecutively.Conclusion: The reliability of primary grader for interpreting single-field fundus photography in DRscreening has not reached the ideal result in this study (κ more than 0.6 in all variables). Acomprehensive training with continuing education is essential for non-physician personnel, to ensurethe reliability of DR screening program using single-field fundus photography.Keywords: diabetic retinopathy screening, interobserver agreement, kappa value, intraclass correlation.

Abstract

Background: Diabetic macular edema (DME) is the main cause of visual impairment in diabetic retinopathy (DR). Current gold standard therapy of DME is macular laser photocoagulation (MPC). Growing evidences have shown benefits of intravitreal anti-VEGF agents (i.e bevacizumab) and intravitreal corticosteroids (i.e triamcinolone acetonide). Aim: To compare the visual acuity (VA) improvement of patients with DME, treated with intravitreal bevacizumab (IVB), a combination of IVB and intravitreal triamcinolone (IVB/IVT), and MPC. Method: A comprehensive PubMed® and Cochrane® databases search was conducted on May 4th, 2017 using appropriate keywords (diabetic macular edema, bevacizumab, triamcinolone, and laser photocoagulation using their MeSH terms). Studies were filtered using inclusion criterions (clinical trials, RCT, meta-analysis, systematic review, English, humans, and publication within 10 years) Results: Three studies (2 systematic reviews and 1 RCT) were found suitable. From these results, all studies showed favoring effects of IVB when compared to IVB/IVT combination and MPC in short term period (up to 6 months). However, there was no significant improvement of VA beyond this period in all groups. Conclusion: IVB appears to be superior to IVB/IVT and MPC in improving VA during 6 months follow- up period. Future systematic reviews and meta-analysis are required on the effect of IVB and MPC combination in cases of DME.

Abstract

In the case of rhegmatogenous retinal detachment (RRD), pneumatic retinopexy is an alternative choice besides scleral buckling and pars plana vitrectomy. This case series describes two cases of RRD with superotemporal tear treated succesfully with pneumatic retinopexy. The expandable gas used in this study was 0.4 cc perfluoropropane (C3F8) gas, patient’s head is immediately positioned face down (prone), then slowly turned into an upright position in accordance with the tear for 1-3 days. Laser retinopexy using an argon laser is performed after the retina is properly reattached. Ten months after pneumatic retinopexy, the VA of RE in case I remained at 6/18 with reattached retina. At four weeks, VA of RE in case II was at 3/60 with reattached retina. VA of both cases was relatively satisfactory with retinal reattachment in a single procedure. The anatomical and functional success of pneumatic retinopexy is related to macular status before surgery, retinal detachment area, phakic status, or the presence of PVR and high myopia.

Abstract

Latar Belakang: Vitrektomi pada ablasio retina dapat dilakukan dengan anestesi umum maupun anestesi lokal. Anestesi lokal kerap dipilih atas keunggulannya dalam efisiensi waktu dan biaya. Tujuan: Memaparkan tingkat keberhasilan anatomi dan fungsional vitrektomi dengan anestesi lokal pada ablasio retina rhegmatogen di Divisi Vitreoretina RSCM-FKUI. Metode: Penelusuran retrospektif pada 42 pasien ablasio retina rhegmatogen yang dilakukan vitrektomi dengan anestesi lokal di RSCM-FKUI pada Januari 2017 – Juni 2017.Hasil: Terjadi peningkatan tajam penglihatan pada enam bulan pasca vitrektomi dengan anestesi lokal yaitu visus ?1/60 dari 55% menjadi 77%. Kondisi retina menempel pasca tindakan operasi vitrektomi dengan anestesi lokal sebanyak 93%. Kesimpulan: Tingkat keberhasilan fungsional sebesar 77% dan keberhasilan anatomis 93%.

Abstract

Eales disease is a rare idiopathic occlusive retinal vasculitis with low prevalence in the general population. Its infrequent occurrence limits diagnostic and treatment guidelines, emphasizing the need for ongoing reports to establish effective protocols. This case report describes a patient presenting with symptoms of bilateral retinal vasculitis. Fundus examination revealed vitreous hemorrhage (VH) and peripheral retinal vasculitis. Laboratory and imaging tests were performed to exclude differential diagnoses, leading to Eales disease as the primary diagnosis. Pars plana vitrectomy, panretinal laser photocoagulation, intravitreal anti-vascular endothelial growth factor, and anti-tuberculosis therapy were administered. Long-term follow-up showed satisfactory outcomes. This report suggested Eales disease to be a diagnosis of consideration for ophthalmologists encountering male patients with recurrent VH. Clinical suspicion, including appropriate laboratory testing and imaging, is essential to developing a holistic approach to diagnosis and management, aiming to prevent progression and achieve optimal visual outcomes.

Abstract

Introduction: Age-related macular degeneration (AMD) is a leading cause of blindness globally, with wet AMD being particularly debilitating. The prevalence of AMD is increasing, necessitating a deeper understanding of its clinical characteristics and management. This study aims to investigate the demographic and clinical features, management, and treatment outcomes of wet AMD at Cipto Mangunkusumo Hospital during January-December 2022. Methods: A retrospective descriptive study was conducted, analyzing medical records of wet AMD patients treated during the specified period. Data analysis included patient demographics, clinical characteristics, treatment modalities, and outcomes. Results: Of 129 eyes from 122 patients, 115 eyes met the inclusion criteria. The majority were female (55.7%) with an average age of 67 years. Most patients presented with blurry vision (93.9%) and received Patizra anti-VEGF injections (60.9%). Hypertension was the most common risk factor (28.7%). Anti-VEGF treatment significantly reduced central macular thickness (p < 0.05) but had no significant impact on visual acuity. Avastin and Patizra injections similarly influenced macular thickness but not visual acuity. There were no significant differences between loading and non-loading dose groups in terms of outcomes. Conclusion: Anti-VEGF treatment effectively reduces central macular thickness in wet AMD patients, regardless of the specific agent used. However, it does not significantly improve visual acuity. Further research is needed to explore differences between loading and non-loading dose protocols, compare treatment strategies, and investigate demographic risk factors. Consistency in visual acuity documentation using the ETDRS format is recommended for future studies.

Abstract

Introduction: Neovascular age-related macular degeneration (nAMD) is a leading cause of irreversible vision loss in the elderly. Intravitreal anti-vascular endothelial growth factor (anti-VEGF) therapies have revolutionized the treatment of nAMD. Faricimab, a novel bi-specific anti-VEGF and anti-angiopoietin-2 antibody, has shown promise in clinical trials. This comprehensive systematic review aims to evaluate the real-world efficacy and safety of intravitreal faricimab in the management of nAMD. Methods: A comprehensive search was conducted in major electronic databases to identify studies reporting outcomes related to faricimab treatment for nAMD in real-world settings. A total of 6 studies were included, comprising 800 patients including 874 eyes. The primary outcomes of interest included visual acuity improvements, central subfield thickness of retina, and safety. Result: The review reveals that intravitreal faricimab is associated with significant visual acuity improvements in patients with nAMD, with outcomes comparable to or better than existing anti-VEGF agents. Furthermore, patients receiving faricimab typically required fewer injections, resulting in a potentially lower treatment burden. The findings also suggest that faricimab may offer a longer treatment interval, which could have a positive impact on patient quality of life. Conclusion: Regarding safety, faricimab demonstrated a favorable safety profile in the real-world setting, with a low incidence of ocular and systemic adverse events. This suggests that faricimab is well-tolerated by patients, supporting its long-term use in the management of nAMD.
