Abstract

ABSTRACTStrategy DOTS is the tuberculosis control programs. The program has implemented in Tanah Kalikedinding Health Center and has expected to reach CDR ≥ 70% and SR ≥ 85%, which closely related to the management of health centers. This study aimed to evaluate the implementation of the DOTS program in health center whose the results associated with indicators of tuberculosis. This was a descriptive design study with the population was all pulmonary specialist, tuberculosis officers and laboratory personnel and pulmonary tuberculosis patients. The number of Tuberculosis patient respondents was 32 respondents. Samples were chosen using purposive sampling. Data collected by interview questionnaire and checklist. The variables were the finding case, the TB treatment, the enabling factor and inhibiting factor, recording and reporting, and result of tuberculosis indicators. This study resulted that CDR in 2013 was 112% already reached the national target ≥ 70%. This success related to the finding case almost all patients > 2 weeks of cought and all (100%) patients were examinated sputum and diagnosed according to the steps of tuberculosis diagnosis in Indonesia Department of Health guidelines. While SR in 2013 was 65.5% did not reach the target ≥ 85%. It was caused of there are patients who did not have a taking drug observer (PMO). All (100%) patients ever forgot taking anti tuberculosis drugs. Change in schedule of visit to the continuation phase be 2×/month caused patients to forget taking anti tuberculosis drugs. The enabling factor was counseling routinely by health care workers in health center. Inhibiting factor was distance to health center by majority (65,5%) patients were > 1 km. So they needed vehicle to go to the health center. Recording and reporting using electronic systems and being reported by online. So it is expected all TB patients were expected have a taking drug observer and optimizing the role of the a taking drug observer to increase success rate.Keywords: DOTS strategy, Case Detection Rate, Success Rate, evaluation,                     Tuberculosis

Abstract

Latar belakang: Tahun 2020 Indonesia dihadapkan pada pandemi COVID-19. Untuk mensiasati kemungkinan dampak negatif yang ditimbulkan oleh COVID-19, maka dibutuhkan kemandirian masyarakat dalam menghadapi masa pandemi. Oleh karena itu, perlu diadakan suatu kegiatan pemberdayaan masyarakat guna meningkatkan kemandirian masyarakat dalam menghadapi pandemi COVID-19. Tujuan: memberikan pengetahuan terkait persebaran dan pencegahan Covid-19 di tingkat RT dan pelatihan cara pembuatan ekstrak jamu dari tanaman toga. Metode: Metode yang digunakan dalam kegiatan ini adalah penyuluhan dan training. Penyuluhan secara bertahap, yakni 2 kegiatan dihari yang berbeda, kegiatan pertama memberikan pengetahuan kepada warga seputar penyakit COVID-19 dan di hari berikutnya dilakukan penyuluhan tentang cara menjaga imunitas tubuh dan mempraktikan cara pembuatan ekstrak jamu dari tanaman toga. Juga dilakukan pre dan posttest. Hasil: Pada kriteria kehadiran peserta, 10 dari 12 peserta (83,3%) hadir dalam penyuluhan secara tatap muka di rumah pelaksana kegiatan. Hal ini membuktikan bahwa kegiatan sudah mencukupi dan sukses dari segi peserta (kehadiran peserta > 75%). Kemudian jika dilihat dari segi peningkatan pengetahuan, terjadi peningkatan pengetahuan sebesar 51% dengan keefektivan berada pada kategori sangat efektif (keefektivan > 1). Untuk praktik pembuatan jamu, sebanyak 6 peserta dari 12 peserta penyuluhan bisa mempraktikan membuat ekstrak jamu di rumah masing-masing (efektif meningkatkan pemahaman peserta bila dapat mempraktikan mandiri ≥50%).Kesimpulan: kegiatan penyuluhan tentang cara meningkatkan imunitas tubuh saat pandemi Covid-19 berhasil, baik dari segi peserta (adequancy of performance) maupun peningkatan pengetahuan (effectiveness).

Abstract

The Directly Observed Treatment, Short-course (DOTS) Strategy is a tuberculosis control program. This study aims to evaluate the implementation of the DOTS program in Public Health Center (PHC) of Tanah Kalikedinding whose results are associated with the achievement of tuberculosis indicators. This study uses a descriptive design with a population that is all lung specialist doctors, tuberculosis officers, laboratory workers, and pulmonary tuberculosis patients. Sampling technique with purposive sampling. Data collection by interview questionnaire and checklist. The variables of this study are case finding, treatment, driving and inhibiting factors, recording and reporting, and outcomes based on tuberculosis indicators. The results showed the Case Detection Rate (CDR) at 2013 was 112% meeting the national target ≥70%. This success was due to the selection of up to 87.5% in suspected tuberculosis (cough> 2 weeks) and all patients were examined according to the diagnosis flow in the guidelines of the Ministry of Health of the Republic of Indonesia, while the Success Rate (SR) at 2013 was 65.5% under the target ≥85%. This is due to the fact that there are still patients who do not have a Drug Supervisor (PMO). The reinforcing  factor is routine counseling by staff at the Public health center (PHC). The inhibiting factor is the distance to the PHC which most of the patients are more than 1 km (65.6%). Recording and reporting using an electronic system and reported online. The conclusion of the study is the need for drug swallowing supervision to improve the success of the treatment is expected that all tuberculosis patients have PMO.

Abstract

Santri pondok pesantren merupakan komunitas berisiko tinggi terserang skabies karena kepadatan dan kontak erat penghuninya. Santri yang terserang mengalami gatal, sulit tidur di malam hari, sampai dengan infeksi sekunder sehingga mengganggu proses belajar. Penyakit ini disebabkan sanitasi lingkungan dan hygiene personal yang buruk akibat pengetahuan santri tentang penyakit terbatas. Pesantren selalu menekankan upaya menjaga kebersihan lingkungan, yang tanpa kesadaran santri tidak akan berjalan optimal. Sehingga perlu dilaksanakan upaya komprehensif dengan fokus peningkatan pengetahuan santri tentang hidup sehat, sehingga timbul kesadaran untuk merubah perilaku menjaga kebersihan lingkungan dan pribadi. Metode kegiatan terdiri dari identifikasi, pendataan dan pengobatan santri penderita skabies, pemberian penyuluhan mengenai penyakit dan pencegahannya termasuk perilaku hidup bersih dan sehat (PHBS), pelatihan wakil santri, pelatihan pembuatan sabun belerang, dan pendampingan perbaikan kebersihan lingkungan ponpes. Kegiatan ini dilaksanakan bulan September-November 2020 di pesantren Nurul Islam (Nuris) Kelurahan Antirogo Kecamatan Sumbersari Jember. Sasaran kegiatan ini adalah santri Madarasah tsanawiyah (MTs), SMA, santri yang tergabung pada kegiatan ekstrakurikuler PMR, serta pengasuh ponpes. Hasil akhir kegiatan ini berupa turunnya angka kasus skabies, peningkatan pengetahuan dan sikap santri, tersedianya produk sabun belerang, terdapat wakil santri yang terlatih mendeteksi dan memberiksan penangan awal kasus skabies ringan, dan adanya rekomendasi kepada pengasuh terkait peningkatan sanitasi dan kebersihan lingkungan pondok.

Abstract

Latar belakang: Pondok pesantren merupakan tempat berkumpulnya para santri dari berbagai daerah. Tempat ini dikenal memiliki banyak problematika kesehatan, mulai dari kebersihan diri hingga kebersihan lingkungan. Kebersihan lingkungan sekitar maupun di dalam kamar ditentukan oleh kebiasaan sehari-hari santri. Kurangnya kepedulian dalam menjaga kebersihan lingkungan dapat berisiko munculnya penyakit, diantaranya scabies (gudik). Tujuan: untuk mengetahui pengetahuan, sikap dan perilaku santri di kawasan pondok terkait kejadian scabies sebagai analisis dasar pemberian training kader kesehatan. Metode: Ini merupakan jenis penelitian deskriptif kuantitatif dengan pengumpulan data wawancara terstruktur dengan kuesioner. Sampel penelitian ini adalah siswa-siswi SMA/MA Unggulan Nuris yang tergabung dalam ekstrakurikuler PMR. Penelitian dilakukan Oktober-November 2019 di SMA/MA Unggulan Nuris Jember. Hasil: Hasil studi ini menunjukkan bahwa sebagian besar responden memiliki pengetahuan yang baik terkait kebersihan diri dan lingkungan. Juga sebagian besar sikap mereka cenderung setuju terhadap aktivitas kebersihan diri dan lingkungan. Namun, dalam hasil perilaku/kebiasaan sehari-hari mereka, terdapat beberapa hal yang perlu menjadi perhatian. Sebagian besar (73%) santri saling meminjam sarung/kerudung dengan temannya; dan sebagian besar (57%) masih sesekali saling pinjam pakaian dengan temannya; juga masih ada yang saling meminjam peralatan mandi pribadi seperti sabun mandi, sikat gigi, dan handuk; hampir sebagian (43%) siswa menggunakan pakaian yang sama lebih dari 1 hari; hampir sebagian (47%) juga tidak mandi setelah beraktivitas/berolahraga; dan hampir sebagian (47%) hanya sesekali menebah tempat tidur saat setelah maupun hendak ditiduri. Oleh karena itu kebiasaan tersebut perlu diubah. Perilaku dapat diubah dengan adanya keteladanan dari sesama santri yang ditunjuk menjadi kader kesehatan agar santri tahu, mau dan mampu hidup bersih dan sehat. Selanjutnya dilakukan training terhadap anggota PMR yang kemudian menjadi kader kesehatan dalam upaya pengendalian kebersihan lingkungan dan menekan angka kejadian scabies di pondok pesantren. Kesimpulan dan rekomendasi: meskipun memiliki pengetahuan dan sikap yang cukup baik, namun beberapa perilaku santri masih berpotensi dalam meningkatnya kejadian scabies atau penyakit kulit lainnya. Diharapkan ada pemantauan berkala terhadap kebersihan lingkungan dan perilaku santri setelah dilakukan training dan dibentuknya kader kesehatan.

Abstract

The number of people living with HIV/AIDS in Indonesia is increasing every year. HIV/AIDS surveillance is the most effective way to control the spreading of HIV/AIDS cases. This study aims to describe the implementation of HIV/AIDS surveillance in Jember. This paper was a descriptive study that included all public health centers (PHCs) conducting HIV/AIDS surveillance. The sample was the Jember District Departement of Health Office and four PHCs using purposive sampling. Variables in this study were input, process, and output. The data was obtained from an interview using a questionnaire. The results indicated that the four PHCs did not have an epidemiologist. Data collection mostly derived from Voluntary Counseling and Testing (VCT) and mobile VCT. Processing HIV/AIDS surveillance data utilized the HIV/AIDS and STDs Information System (SIHA) application and validated by the District Department of Health Office once a month. However, only two PHCs conducted analysis and data interpretation. Dissemination was only done by the District Departement of Health Office and 2 out of 4 PHCs. Hence, the components and process of surveillance needed to be optimized.

Abstract

The World Health Organization estimates that the Covid-19 Pandemic will end in 2022. However, this won’t be easy to realize if every country is unprepared to deal with the spread of Covid-19, considering that new variants of Covid-19 are starting to emerge. Therefore, applying health protocols and living a healthy life should not be reduced. Although the government has been intensively disseminating the Covid-19 vaccine and implementing health protocols, many Indonesians are still reluctant to comply. This Community Service activity was carried out in Karangrejo Village, RT 01/RW04, Sumbersari District, Jember Regency. The results of the problem diagnosis found that residents still did not understand how to live healthy during a pandemic and apply the correct health protocol. There are three activities in this community service, namely ADUHAI Education, GETAS Education, and the distribution of educational posters, masks, and educational banners. The result of this activity is that there is an increase in participants' knowledge from 73.31% (pre-test) to 97.48% (post-test) in the ADUHAI Education program and from 79.92% (pre-test) to 92.72% (post-test) on the GETAS Education program. Furthermore, the community is expected to practice independently in implementing healthy living recommendations and sustainably obeying health protocols. Abstract

There were disposable masks waste that are disposed of carelessly in the Perhutani area of the Selorejovillage. In addition, residents did not know how to process mask waste. There was also a lack ofdiscipline in the use of proper masks and low awareness of hand washing. Following up on these results,solutions that could be done weretraining on how to wear masks correctly, training on 6 steps ofwashing hands,training on waste management of disposable masks. Coordination was carried out withlocal working groups and community leaders, the head of the RT and the head of the RW. Theimplementation of training activities is carried out online, but the implementation of pre-test and posttest,evaluation of self-practice,also documentation is carried out offline. Citizen participation is quitegood, but still needs to be improved, especially when conducting online discussions. Training on usingmasks correctly showed an increase in knowledge of 93%. Training on with 6 steps of washing handswith soap showed an increase in knowledge of 91.8%. Lastly, Training on the waste management ofdisposable masks showed an increase in knowledge of 89.1%, where these indicators has exceeded thetargeted indicators.Keywords: disposable mask, waste management, training, covid-19

Abstract

ABSTRAK Mayoritas masyarakat Desa Candijati Arjasa Jember Jawa Timur bekerja di bidang pertanian  (64%). Desa Candijati merupakan penghasil padi terbanyak di Kecamatan Arjasa. Petani desa Candijati termasuk golongan masyarakat terdampak pandemi corona. Pembatasan mobilitas membuat petani kesulitan memasarkan produk pertaniannya. Harga produk pertanian menjadi anjlok. Pendapatan petani menjadi turun drastis. Hal ini mencetuskan kecemasan dan stress pada petani. Petani juga menghadapi ancaman terpapar covid selama pandemi, karena tidak mendapat informasi mengenai penyakit dan pencegahan yang benar. Berdasarkan permasalahan tersebut, tim pengusul pengabdian menawarkan solusi kepada mitra petani kelompok tani Desa Candijati berupa penyuluhan mengenai penyakit covid dan pencegahannya serta pelatihan membuat masker beraromaterapi. Tujuan pengabdian ini adalah mencegah paparan Covid dan mengurangi stress pada petani serta meningkatkan status ekonomi petani. Metode kegiatan terdiri dari identifikasi keluhan/penyakit petani, pemberian penyuluhan mengenai penyakit dan pencegahannya, dan pelatihan pembuatan tisue  aromaterapi. Tisue aromaterapi yang terbuat dari jerami dan ditambahkan aroma jeruk disisipkan ke dalam masker kain 2 lapis. Selain mencegah paparan virus covid, aroma jeruk pada tisue saat dipakai dalam masker dapat meredakan kecemasan sehingga membantu mengurangi stress yang dialami petani. Sasaran kegiatan ini adalah ibu-ibu petani desa Candijati kecamatan Arjasa. Hasil akhir kegiatan ini berupa produk masker aromaterapi, peningkatan ketrampilan,  pemahaman dan kesehatan masyarakat. Kata Kunci: masker, aromaterapi, tisue, jerami, petani  ABSTRACT The majority of the people of Candijati Arjasa Jember East Java work in agriculture (64%). Candijati village is the largest rice producer in the Arjasa district. Candijati farmers are part of the community affected by the corona pandemic. Restrictions on mobility make it difficult for farmers to market their agricultural products. Prices of agricultural products have fallen. Farmers' incomes have fallen drastically. This triggers anxiety and stress in farmers. Farmers also face the threat of being exposed to covid during the pandemic, because they do not get information about the disease and proper prevention. Based on these problems, the team offered solutions to farmers of Candijati Village in the form of counseling about the covid disease and its prevention as well as training to make aromatherapy masks. The purpose of this program is to prevent exposure to Covid, reduce stress, and improve the economic status of farmers. The method of activity consists of identifying farmer complaints/diseases, providing counseling about diseases and their prevention, training in making aromatherapy tisue. This aromatherapy tissue made from straw and added with the aroma of citrus is inserted into a 2-ply cloth mask. In addition to preventing exposure to the covid virus, the smell of citrus on tissues when used in masks can relieve anxiety so that it helps reduce stress. The target of this activity is the farmers of Candijati village, Arjasa sub-district. The end result of this activity is aromatherapy mask products, skill improvement, understanding, and public health.  Keywords: masks, aromatherapy, tissue, straw, farmers

Abstract

Stunting in the working area of ​​the Jelbuk Public Health Center Jember Regency is high. In addition, the working area of ​​the Jelbuk Public Health Center is also in the second worst position in terms of access to sanitation facilities. This study aims to determine the relationship between water, sanitation, and hygiene (WASH) with stunting in toddlers aged 24-59 months in the working area of ​​Jelbuk Public Health Center. The research was conducted from October 2021 to August 2022 using an observational analytic method with a case-control. The sample is 98 respondents consisting of 49 case samples and 49 control samples. The sampling technique used is simple random sampling. They collected data using interviews, documentation studies, and observation. Data analysis used univariate and bivariate analysis. The results showed that the variables that were related were sources of drinking water (p-value= 0,012 OR = 3,212 95% CI = 1,361-7,581), latrine access (p-value= 0,039 OR = 2,604 95% CI = 1,129-6,004), waste disposal (p-value= 0,009 OR=7,622 95% CI=1,605-36,186), sewerage (p-value=0,002 OR=4,111 95% CI=1,735-9,740), and mother's hand washing habits (p-value= 0,000 OR = 6,923 95% CI = 2,852-16,804), while drinking water treatment is not related (p-value= 0,068). The community is expected to be able to further improve the behavior of consuming protected drinking water sources by always cooking from the water they consume so that the potential for contamination can be minimized, carrying out proper waste disposal, and improving the habit of washing mother's hands using soap and running water at the recommended time.
