Abstract

Diabetic foot ulcers are one of the chronic complications of diabetes mellitus (DM) that can lead to serious infections and even amputation. Poor self-care ability is a major risk factor in the development of chronic wounds among DM patients. The Diabetes Self-Management Education (DSME) intervention has been considered effective in improving patients’ self-management abilities; however, contextual evidence in Indonesia remains limited. This study aimed to evaluate the effectiveness of DSME intervention in improving self-care abilities among patients with diabetic foot ulcers at RSUD Banjarmasin. The study employed a quasi-experimental design using a one group pre-test and post-test approach. A total of 30 patients were selected using total sampling. The DSME intervention was delivered over seven consecutive days through a structured, face-to-face educational approach. Self-care ability was measured using the Summary of Diabetes Self-Care Activities (SDSCA) questionnaire before and after the intervention. Data were analyzed using a paired sample t-test. Prior to the intervention, the majority of respondents (70%) were categorized as having poor self-care. After the intervention, 80% of respondents were categorized as having good self-care. The paired sample t-test showed a significant difference in self-care scores before and after the DSME intervention (p < 0.001). The DSME intervention was effective in enhancing self-care abilities among patients with diabetic foot ulcers.

Abstract

Asthma is becoming a serious global health problem and needs to be addressed. Many asthmatics from various countries experience asthma disorders, so if it is not controlled, it can increase the morbidity rate. One way to assess the occurrence of asthma is to conduct an Expiratory Peak Current (APE) assessment. Do Buteyko breath exercises and Blowing Balloons to overcome and alleviate asthma symptoms. This study aimed to compare Buteyko's breath exercises with Blowing Balloons exercises against changes in peak expiratory currents in asthma patients. The research design used was quasi-experimental with pretest and post-test methods in two groups. The study sample of 20 respondents consisted of 2 groups, namely 10 respondents of the Buteyko breath exercise group and 10 respondents of the Blowing Balloons exercise group, with a simple random sampling technique. After Buteyko breath training and Blowing Balloons exercises were carried out 2 times a day for two weeks, the Wilcoxon sign rank test results for Buteyko breath training and Blowing Balloons exercises with a value of p = 0.00 were obtained. The results of the Mann withney test showed no difference in the mean rank of the asthma control test (ACT) score in Buteyko's breath exercise and Blowing Balloons exercise p = 0.21. The results of measuring the expiratory peak current (APE) showed a significant difference with the p-value = 0.00 in Buteyko breath exercises and Blowing Balloons exercises. Based on the study's results, it can be concluded that Buteyko's breath exercises and Blowing Balloons exercise positively impact the management of asthma symptoms. The results of this study can be an input for nursing services to be used as a guideline for developing Buteyko's breath exercises by blowing balloons against peak expiratory currents in asthma patients integrated into the provision of nursing care.

Abstract

The process resulting from chronic diseases is the inability to carry out activities independently and freely, so a caregiver who provides a series of treatments is needed. The optimal role of caregivers requires an appropriate approach to improve these abilities. One model that can be applied to caregivers of stroke patients is family-centered care. The FCC model applied to stroke patients can build a system of collaboration, focus on the strengths and sources rather than the weaknesses of the family, recognize the family's expertise in caring for patients, build empowerment rather than dependence, increase more information sharing with patients, caregivers and health services than information only known to professionals, create flexible and non-rigid programs. This study aims to develop an FCC model for caregivers who treat stroke patients at home and can be used as a guide by nurses and caregivers in treating stroke patients at home. This research design is action research, which consists of the stages of the survey, planning, acting, observing, and reflecting. The data collection method was carried out through FGDs, the distribution of questionnaires, and the observation of participants. The participants who contributed to this study were 31 caregivers of stroke patients—data analysis using quantitative and qualitative methods. Qualitative data were obtained through FGD results and observation results. In contrast, quantitative data was collected through the dissemination of FCC questionnaires, assessing the swallowing status of stroke patients, and observing results were analyzed using descriptive statistical tests. This research has produced an FCC Model, a reference in treating stroke patients with dysphagia disorders. It positively impacts increasing skills, the ability to practice swallowing, and assessing swallowing ability in treating stroke patients with dysphagia disorders. Another impact of developing the FCC Model at the Center is that caregivers can apply FCC values when treating stroke patients with dysphagia

Abstract

The methodology for employing the bedside handover method while deploying patient receiving weighers between shifts is a set of instructions for doing so. This study aims to apply the bedside handover approach to patients receiving weights in hospital inpatient rooms. Action research, which is conducted in one cycle, is the research method used. The data gathering techniques included a focus group discussion guide, a patient satisfaction survey, a nurse work satisfaction survey, and an observation format. The 15 nurses who took part in this study were chosen using the purposive sample method. Both quantitative and qualitative statistical tests were used to assess the data on both a qualitative and quantitative level. This study developed a plan for implementing acceptable patient weights utilizing the bedside handover approach in hospital inpatient rooms. This study advised the hospital administration to test the draft protocol for the bedside handover approach to be used as a standard protocol.

Abstract

The hardest part for patients is adhering to fluid intake restrictions, which can be stressful to the point that patients cannot regulate their fluid intake consumption. The main factor determining the effectiveness of hemodialysis is the patient's adherence to the rules on food and fluid intake. This study aims to determine whether controlling food intake affects how much salt a patient receiving hemodialysis for chronic renal failure feels like they are thirsty. This study is quasi-experimental, using a pre-post test, a control group, successive sampling for sample selection, and up to 15 samples for each intervention and control group. A thirst distress scale (TDS) questionnaire and a visual analog scale (VAS) were used to gauge the respondents' thirst levels. In contrast, the Wilcoxon test and the Mann-Withney test with a significance level of p 0.05 were used in the data analysis for this investigation. There was a difference in thirst as measured by TDS with a meaning fulness of p = 0.008 (p0.05) and VAS with a meaning of p + 0.048 (p0.05) after the intervention, according to the results, which showed that 13 respondents experienced a decrease in TDS values and two people whose TDS values remained with the meaningfulness of 0.000 (p0.05) after treatment in the intervention group. The results of this study allow us to conclude that education is provided continuously. A patient's daily self-care behavior can be improved by the conduct of home visits as a form of supervision of the education that has been given. This behavior includes reducing the patient's daily salt intake to lessen the thirst patients with renal failure receiving hemodialysis frequently experience.

Abstract

One of the effects of diabetes brought on by poor venous and arterial blood circulation is diabetic foot injury (LKD). Buerger Allen exercise is one modality of therapy that uses active postural movements to enhance blood flow in the lower extremities venous and arterial blood vessels. The brachial index ankle (ABI) technique was used to evaluate improved foot circulation. This study sought to assess the impact of Buerger Allen exercise on LKD patients' lower circulatory extremities. A quasi-experiment with a pretest and post-test without a control strategy was used as the research design. 43 respondents made up the 43 samples for this study, which were collected using successive sampling methods. Using bivariate data analysis, Wilcoxon tests were utilized to compare values before and after the Buerger Allen exercise. The findings demonstrated a statistically significant relationship between the average ABI in LKD patients before and after 0.85 (P-value = 0.000). The Buerger Allen exercise intervention is performed twice daily for three weeks. Positional changes and gravitational force variations can assist in emptying and filling blood vessels. At the same time, contraction of the gastrocnemius muscle and the plantar muscles as a muscle pump activates the ship and prevents backflow. Regular exercise can widen collateral circulation channels, boosting the supply of oxygen to peripheral tissues and the distribution of nutrients. Exercise helps increase circulation, according to studies by Buerger Allen. Therefore it can be suggested in the future that nurses teach patients with lower extremity circulatory disorders in LKD patients.

Abstract

Every industry in today's world, including the medical and health sectors, offers rapid gratification. The existence of retaliation by pharmaceutical companies against doctors, particularly when the pharmaceutical company colludes with doctors to promote drugs by paying them commissions or incentives for each prescription they write for patients when there is a mutually agreed-upon arrangement between the two parties. The conspiracy idea comes from the doctor's request and the pharmaceutical industry. This is also a result of monopolistic practices between pharmaceutical companies and doctors, which cause the price of drugs that patients must purchase to become uncontrollably expensive and raises concerns about the quality of the medications for the patient's recovery because of the doctor's financial incentives to see that the drug is quickly sold/sold on the market. The approach of this study is a type of normative juridical research that employs literature sources, pertinent laws and regulations, other media materials, and interviews to collect information or theories regarding the outcome of the research issue. Pharmacies exert influence by giving something to doctors who work for the government to keep them from losing their freedom and independence when prescribing pharmaceuticals to patients. Doctors routinely recommend pharmaceutical companies' prescriptions because they find it rewarding; as a result, the cost of pharmaceuticals rises, monopolies form, and consumer protection is necessary. It can be considered a crime of gratification for the doctor, which is prohibited under Criminal Code Article 12B.

Abstract

Gender power in reducing the risk of cervical cancer needs serious attention because it has a role in decision-making in the household. Many women leave all decision-making in the family to their husbands, even regarding their reproductive health problems, because of the power relations that a husband has as the head of the household. Factors of domination of power in the household, socioeconomic and patriarchal culture are some factors that relate to decision-making for women. This study analyzed the relationship between gender power relations and decision-making to conduct an IVA TEST examination. This type of research is an analytical survey with a cross-sectional research design. The study sample was taken by 100 respondents using probability sampling techniques with a simple random sampling type. The results of the bivariate analysis using the chi-square test obtained that the variables of power dominance in the household were related to the decision-making of the IVA TEST examination (p = 0.042). Socioeconomic variables consisting of education and income have no relationship with the decision-making to perform the IVA TEST examination. In contrast, knowledge relates to the decision-making to perform the IVA TEST examination (p=0.026). The patriarchal cultural variable of access to resources has no relationship with the decision-making to conduct the IVA TEST examination. In contrast, access to health facilities relates to the decision to run the IVA TEST examination (p=0.048).
