Abstract

Purpose: The major challenges of vaccination programs are associated with the vaccine cold chain management in primary health centers that must be evaluated. This paper discusses how vaccine cold chain management can address the challenges of vaccination programs in primary health centers. Specifically, it examines how the vaccine chain is managed. Methods: The design of this observational study was cross-sectional. Evaluations assessed in this study include input aspects, output aspects, process aspects, and outcome aspects of vaccine chain management. Observation methods and direct interviews with the evaluated vaccine chain staff collected primary data. Results: Management of the vaccine chain has been carried out properly with the support of human resources or immunization personnel who are in accordance with their competence, based on research results, the management staff are diploma graduates in nursing and midwifery. The defrosting process is carried out for a certain period, as well as using vaccine carriers and ice flasks, which are still good for vaccine distribution. It is very important to pay attention to the supply of the number and type of vaccines for the next month's needs at the public health center. Conclusion: The three key factors that guide the vaccine cold chain are educated vaccine control staff, vaccine storage, and delivery equipment, and efficient vaccine control procedures.

Abstract

Purpose: This study aims to explore epidemiological characteristics of dengue fever cases in Magelang City during 2019-2023. Methods: Monthly dengue fever cases from 2019-2023 were extracted from the dengue passive surveillance database at the City Health Office. Data was analyzed descriptively based on time, person, and place. Results: There were 992 dengue cases with a mean incidence rate (IR) of 158 per 100,000 population, and the highest IR was observed in 2019 with 359 per 100.000. The annual case fatality rate was 0.5%, ranging from 0% to 1.3%. Males (52,2%) were observed higher than females (47.8%), and most cases were aged 12-25 years old (36.9%), with elderly (>= 60 years old) only counted for 14 (1.4%) cases. Dengue was reported throughout the year across the village within the city, with the peak of dengue season observed in January and February. Most notifications came from private hospitals (21%), and primary health care contributes about 0.5% of case notifications. Conclusion: The dengue case in Magelang City has characteristics similar to other areas in which the most affected were children and has a seasonal pattern, with the peak number of cases found in January to February and all areas within the city being endemic.

Abstract

The aging process leads to a decline in musculoskeletal and neuromuscular function, increasing the risk of falls among older adults. The World Health Organization (WHO) reports that approximately 28–35% of individuals aged ≥65 years experience falls annually, with prevalence in Indonesia ranging from 12–29%. One preventive strategy to reduce fall risk is Tandem Walking Exercise. This study aimed to analyze the effect of Tandem Walking Exercise on balance in older adults, measured using the Timed Up and Go Test (TUGT) as an indicator of fall risk. This study employed a pre-experimental design with a one-group pretest–posttest approach. The sample consisted of 33 older adults selected through purposive sampling in the working area of Bandongan Public Health Center, Magelang Regency. The intervention was conducted for one week with a total of three exercise sessions. Balance was assessed before and after the intervention using the TUGT. Data were analyzed using the Wilcoxon Signed Rank Test. The results showed a significant difference in TUGT scores before and after the intervention (p < 0.05), indicating improved balance and reduced fall risk among older adults. Tandem Walking Exercise has a significant effect on improving balance and can serve as an alternative preventive intervention in community settings. However, the generalizability of the findings is limited due to the absence of a control group, short intervention duration, and small sample size.
