Abstract

Background: Henoch-Schonlein purpura (HSP) is a vasculitis of the small vessels and the most common type of vasculitis in children. Vesicles and haemorrhagic bullae were thought to be rare in HSP and maybe difficult to diagnose. Purpose: To describe the rare case of haemorrhagic bullous lesion in HSP. Case: A 15-year-old Javanese girl was admitted to Dr. Soetomo General Hospital Surabaya with one week history of multiple haemorrhagic blisters on her extremities, accompanied with joint pain, abdominal pain, and severe nausea. Firstly, it’s only a few redness patches then become numerous and changed to haemorrhagic blisters. A dermatological examination shown multiple bullae and vesicles on multiple palpable erythematous purpura. There were hypokalemia on laboratory examination and histopathological examination concluded a leukocytoclastic vasculitis. Discussion: Fluid balance, oral corticosteroid, oral antihistamine, antiemetic injection, and potassium correction were administered to the patient. Dexamethasone was discontinued because there was a presence of melena. Symptomatic treatment was given and significantly improvement was achieved in one month after treatment. Conclusion: Henoch-Schonlein purpura is diagnosed based on history, physical, clinical, and histopathological examination. Vesicles and haemorrhagic bullae are rare in HSP, and it does not seem to have any prognosis value in the outcome of HSP. Key words: Henoch-Schonlein purpura, haemorrhagic bullous lesion, small vessel vasculitis.

Abstract

Background: The prevalence of atopic dermatitis (AD) has been increased for several decades. T-helper cell (Th)1 and Th2 disregulation influence the level of immunoglobulin (Ig)E and interfere Scoring Atopic Dermatitis (SCORAD) index. Management approachment only relieved the symptoms, therapy with probiotic are potentially  improve this disregulation. Lactobacillus plantarum (LP) has been fullfilled as probiotic criteria and it’s safely used in human. Methods: In a randomized, double-blind, placebo-controlled trial, twenty (22) patients were randomized to take LP or placebo for 12 weeks with 2 weeks wash-out period in Pediatric Division, Dermatology and Venereology Outpatient Clinic. Total IgE serum level  and SCORING indeks were evaluted before and after intervention. Results: Twelve (12) patients in probiotic group and 10 patient in placebo group completed the study. Our findings revealed that there is no significant different in total IgE serum between LP (504.533 IU/ml ± 415.686 IU/ml) and placebo (909.580 ± 885.051 IU/ml) respectively (p=0.117). Mean decreased difference of SCORAD index in LP group (36.784 ± 8.705) significant greater than placebo (26.860 ± 5.021) respectively (p= 0.000). Conclusion: Total IgE serum in both group not significantly different. However, SCORAD indeks in LP group showed a significantly greater reduction.  LP is beneficial in the treatment of children with AD.

Abstract

Coronavirus disease 2019 (COVID-19) is an infection caused by severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2). This virus has affected to many countries and territories all around the world, thus in March 11, 2020 World Health Organization (WHO) declares the COVID-19 outbreak as pandemic. The symptoms of COVID-19 are varying, but most patient have mild symptom or asymptomatic. First case was describe as a ptechiae mimicking dengue fever. The most lesion are erythematous rash, followed by urticarial and varicella-like vesicle. These lesion not only caused by virus it self, however inflammatory response and drug reaction should be considered. In addition to skin manifestations in COVID-19 patients, skin problems also occur in health care workers. Health workers whom controlling COVID-19 also prone to skin and mucous membrane injury, which may caused by using Personal Protective Equipment (PPE) in long period. The purpose of this study is to describe various dermatology manifestation in COVID-19 and described various skin problem in health-care workers to controlling COVID-19 pandemic. These cutaneous manifestation combined with clinical symptoms of COVID-19 may contribute made a early diagnosis in COVID-19. Correct use of PPE can reduce skin damage in health-care workers.

Abstract

Anxiety is a global psychiatric phenomenon, while poor sleep quality remains a pressing public health issue. The arduous medical education journey exposes students to prolonged stress, potentially exacerbating anxiety and sleep disturbances. This study investigates the correlation between anxiety and sleep quality in Hang Tuah Faculty of Medicine students, bridging the pre-clinical and clinical phases. Employing an analytical observational approach with a cross-sectional design, this study enrolled students from pre-clinical (batches 2020, 2021, 2022) and clinical years (batches DM45, DM46, DM47). Among the 409 approached participants, 296 met the inclusion criteria after excluding 113 samples. Data collection utilized online questionnaires with acquired informed consent. Sleep quality was evaluated via the Pittsburgh Sleep Quality Index, generating scores from 0 to 21. This study showed significantly robust evidence of an association between anxiety and sleep quality (p-value: 0.0001; r: 0.342) across both pre-clinical and clinical students. This study underscores a direct relationship: elevated anxiety levels correspond to diminished sleep quality. Such findings resonate with the prevailing challenges in medical education and their impact on students' mental and physical well-being. Implementing targeted interventions is crucial to address these concerns. In conclusion, this study underscores the undeniable link between anxiety and sleep quality among Hang Tuah Faculty of Medicine students, encompassing the years 2017 to 2022. The implications are far-reaching, emphasizing the necessity of interventions that enhance both psychological well-being and sleep quality. By recognizing and addressing these challenges, medical institutions can foster healthier academic environments and better equip students for their medical careers.

Abstract

Chronic Kidney Disease (CKD) is a persistent abnormality in kidney function or structure, with or without a decline in glomerular filtration rate (GFR) below 60 mL/min/1.73 m², lasting for more than three months. CKD diagnosis relies on markers of kidney damage and/or a reduction in GFR. Patients with advanced CKD, characterized by a GFR below 15 mL/min/1.73 m², typically require dialysis. In 2018, 60,852 individuals in Indonesia were reported to have CKD requiring lifelong dialysis. This study aims to assess the characteristics of CKD patients undergoing hemodialysis. This research employs descriptive design. The sample consists of all CKD patients undergoing hemodialysis, as recorded in the medical records of RSPAL dr. Ramelan Surabaya between January and June 2022. The findings reveal that more than half of the patients (52.7%) had been on hemodialysis for more than 12 months. Many of the patients (66.2%) were between 40 and 60 years old, with males accounting for 54.1% of the sample. Hypertension and diabetes mellitus were identified as the most common comorbid conditions (31.1%), while pain medication consumption was the least reported cause (2.7%). All patients had hemoglobin levels below the normal range (<13 g/dL for males and <12 g/dL for females) and relied on BPJS (Indonesia’s national health insurance) to cover the cost of hemodialysis. Furthermore, most patients exhibited creatinine levels exceeding 1.5 mg/dL, blood urea nitrogen (BUN) levels above 24 mg/dL, serum iron (SI) levels between 45 and 158 µg/dL, and total iron-binding capacity (TIBC) levels below 250 µg/dL.

Abstract

Morbus Hansen is an infectious skin disease caused by Mycobacterium Leprosy which besomes one of the neglected disease and very common in Indonesia and other subtropical countries. World Health Organization (WHO) reported the number of new cases of Hansen's disease on 2018 in 159 countries was 208,619 cases. Indonesia ranks third in the world with a total of 17,017 cases, after India and Brazil. On this retrospective descriptive study regarding the profile of Morbus Hansen patients at the Dernatovenerology Clinic, dr. Ramelan Hospital on 2016–2021, there were 24 Morbus Hansen patients registered at the RSPAL Dr. Skin and Venereology Polyclinic. Ramelan during 2016-2021. The prevalence of Morbus Hansen is dominant in male patients, with a ratio of 5:1 compared to female patients, the avarage age of the groups is 21-30 years and 41-50 years. Based on the type of morbus hansen, 87,5% were Multibacillary (MB) type.

Abstract

Hansen disease atau yang lebih sering dikenal dengan penyakit kusta merupakan penyakit infeksi kronik yang disebabkan oleh bakteri Mycobacterium leprae dan sering dikenal sebagai “The Great Immitator Disease” karena gejalanya susah dibedakan dengan penyakit kulit lainnya. Menurut World Health Organization (WHO) penyakit kusta utamanya menyerang kulit, saraf perifer, mukosa saluran pernafasan bagian atas. Tujuan dari penelitian ini yaitu untuk mengetahui gambaran karakteristik pasien kusta di RSUD Sumberglagah periode Januari - Agustus 2023. Penelitian ini merupakan penelitian deskriptif retrospektif. Sampel dalam penelitian merupakan semua pasien kusta di RSUD Sumberglagah Mojokerto berdasarkan data yang tercatat di rekam medis periode Januari 2023- Agustus 2023. Hasil penelitian menujukkan bahwa sebagian besar pasien kusta adalah laki-laki (67.70%), berusia > 15thn (98.39%), menjalani pekerjaan berat (64.50%) berpendidikan sampai tingkat SD (45.16%) masih menjalani pengobatan sampai saat ini (51.6%), tipe MB (100%) tidak mengalami reaksi kusta (46.8%) memiliki keluhan utama berupa luka di ekstremitas (24.2%) dan berasal dari kota Mojokerto (59.7%). Sesuai dengan 62 sampel dari data yang telah diambil, dapat disimpulkan bahwa sebagian besar pasien kusta adalah laki-laki, berusia lebih dari 15 tahun, mengerjakan pekerjaan berat, menjalani pendidikan sampai tingkat SD, masih menjalani pengobatan,tipe kusta MB, datang dengan keluhan utama luka di ekstremitas dan berasal dari kota Mojokerto.

Abstract

Background: The exposure to UV-B (280-320 nm) in Indonesia's tropical sunlight leads to the accumulation of free radicals, which in turn causes the formation of dry skin and thickening of the epidermis, a sign of skin photoaging. Topical intervention with moisturizers containing antioxidants is one approach to prevent further damage. The addition of kombucha coffee extract from fermented robusta coffee (Coffea canephora) with SCOBY (symbiotic cultures of bacteria and yeasts) can be a source of antioxidants. Purpose: To evaluate the effect of Coffea canephora extract as an antioxidant moisturizer in reducing epidermal thickness on mice’s back skin exposed to UV-B irradiation. Methods: This research is a true experimental design with a post-test only control design. The sample used in this research is thirty male white mice (Rattus norvegicus), which were divided into six groups and were UV-B irradiated for two weeks with total dosage of 980 mJ/cm2.At the end of the treatment, skin samples were excised and stained histologically with Mollory Azan (MA) to evaluate the thickness of the epidermis. Data obtained were analyzed statically with SPSS. Result: The Kruskal-Wallis test demonstrated significant results (0.0001, p<0.05) across all groups, indicating the efficacy of the kombucha coffee extract antioxidant moisturizer in reducing epidermal thickness. Conclusion: The application of Coffea canephora extract antioxidant moisturizer provided photoprotection against UV-B induced hyperplasia skin epidermis with the concentration of 5% and 10% extract.

Abstract

Introduction: The Cleft Lip and Palate (CLP) is a congenital malformation involving the orofacial region. The failure of frontonasal and maxillary processes to fuse causes a gap through the lips, alveoli, base of the nose, and in the hard and/or soft palate. Case Presentation: This case report illustrates a 2-year-3-month-old female patient diagnosed with bilateral cleft lip and palate at birth. The patient underwent cheilorrhaphy at 4 months old and currently plans to have scar revision and columellar lengthening surgery at Dr. Ramelan Naval Hospital, Surabaya. The patient presents with a flat nose, a hypertrophic scar above the lip, and speech articulation impairment. Her medical history includes prenatal maternal hypertension, neonatal pneumonia, and exposure to cigar smoke. Additionally, there is a family history of cleft lip and palate. This case highlights the vital role of columellar lengthening in enhancing nasal appearance and airway patency for optimal outcomes without complications. Conclusion: Cleft lip and palate (CLP) require multidisciplinary care. Columellar lengthening and scar revision have been proven effective in enhancing nasal functions and aesthetics. The presence of prenatal maternal hypertension, exposure to cigar smoke, and familial history of CLP accentuates etiological complexity. The case shows successful intervention balancing function, aesthetics, and craniofacial growth in CLP repair. This study aims to increase insight for future management concerning nasal deformities in CLP patients.
