Abstract

Bencana merupakan salah satu kasus paling sulit bagi institusi kesehatan yang dapat mengganggu fungsi layanan kesehatan. Rencana penanggulangan bencana sangat penting dalam memastikan kesiapsiagaan dan respon rumah sakit. Penelitian ini bertujuan untuk menganalisis kesiapsiagaan Murni Teguh Memorial Hospital (MTMH) dalam menghadapi bencana. Jenis penelitian ini ialah penelitian deskriftif kualitatif. Teknik dengan wawancara mendalam kepada 10 orang informan, observasi lapangan dengan menggunakan Hospital Safety Index (HSI) dan telaah dokumen kesiapan bencana. Murni Teguh Memorial Hospital memiliki indeks kesiapsiagaan tinggi (0,72; Overall A), menandakan siap memberikan pelayanan saat darurat. Analisis kualitatif menunjukkan kesiapan menghadapi bahaya internal baik, namun kesiapan penanganan bahaya eksternal perlu ditingkatkan. Kesiapan struktural dan non-struktural baik, tetapi perlu evaluasi berkala. Manajemen pengelolaan kondisi darurat dan bencana sudah dipersiapkan dengan baik, tetapi regulasi transportasi evakuasi pasien, pengantaran maupun penjemputan tenaga medis, distribusi obat-obatan dan peralatan medis saat bencana belum tertulis. Tim Siaga Bencana dan koordinasi eksternal ada, namun Memorandum of Understanding (MOU) dengan lembaga-lembaga seperti Damkar, Badan Penanggulangan Bencana Daerah (BPBD) dan Kepolisian belum tersedia. Pengetahuan staf baik, tetapi materi pelatihan kurang variatif dan spesifik tentang penanganan bencana eksternal. Beberapa dokumen kesiapan bencana belum sesuai dengan Pedoman Rumah Sakit Aman Bencana Kemenkes 2024. Murni Teguh Memorial Hospital memiliki kesiapsiagaan tinggi (HSI 0,72 – Overall A) dan dinilai mampu menghadapi kondisi darurat, namun masih terdapat kekurangan pada kesiapan menghadapi bencana eksternal, belum adanya MOU dengan instansi terkait, regulasi teknis evakuasi dan distribusi logistik belum tertulis, serta beberapa dokumen belum sesuai Pedoman RS Aman Bencana Kemenkes 2024

Abstract

ABSTRACT Surgical site infection (SSI) is a major postoperative complication that increases morbidity, mortality, and healthcare burden. The implementation of the Surgical safety Checklist (SSC) is considered a key preventive strategy, yet its effectiveness remains debated. This study aimed to analyze the association between SSC completeness and SSI, as well as other clinical factors, among surgical patients at RSUD Panyabungan. A cross-sectional design was applied with total sampling of 873 surgical patients from January to April 2025. Data were obtained from medical records and analyzed using multivariate logistic regression. The incidence of SSI was 5.6%. Compliance with SSC was relatively high: Sign In 79.4%, Time Out 80.6%, and Sign Out 80.3%. Bivariate analysis revealed that incomplete SSC in all phases was significantly associated with increased SSI risk (crude OR 6–19; p<0.001). Additional factors such as ≥2 comorbidities (OR 7.02; p<0.001), and emergency surgery (OR 2.99; p=0.006) were also associated with SSI. In the final logistic regression model, independent predictors of SSI were: complete Sign In as a strong protective factor (AOR=0.057; 95% CI: 0.027–0.120; p<0.001), ≥1 comorbidity (AOR=3.075; 95% CI: 1.803–5.246; p<0.001), and emergency surgery compared to elective procedures (AOR=4.274; 95% CI: 1.612–11.335; p=0.003). This study highlights the strong association between Sign In completeness and lower SSI risk, and emphasizes the need for optimizing perioperative management among patients with comorbidities and those undergoing emergency surgery. Abstract

Health is one of the basic needs of society which is very important in supporting daily activities. As people's living standards increase, so do people's needs, hopes and demands for quality health. This situation requires health service providers such as hospitals to improve the quality of services better and provide satisfaction for consumers as users of health services. The problem formulation in this research is "Evaluation of the Relationship between Health Service Quality and Patient Satisfaction at UPT. Puskesmas Bestari, Medan City in 2024." The general objective of this research is to evaluate the relationship between the quality of health services and patient satisfaction. This type of research is observational analytics with a crosssectional design, namely research to study the dynamics of the correlation between risk factors and effects, using an approach, observational method, or data collection. This research was carried out from October 2024 to February 2025. The samples in this study were patients who came for treatment at UPT. Puskesmas Bestari in Medan City and received health services for 96 people. the majority of respondents in this study were in the age range of 36 - 40 years, 35.4%. The majority of respondents were women, namely 58.3%. Based on education level, the majority of respondents had a high school/equivalent education, 51.0%. Meanwhile, from the work aspect, the majority of respondents worked as employees or laborers with a total of 47.9%. Respondents who were satisfied were 66.7%, while respondents who were dissatisfied were 33.3%. Based on the results of research conducted at UPT Puskesmas Bestari, it can be concluded that: There is a relationship between the quality of health services from the dimension of responsiveness and patient satisfaction. There is a relationship between the quality of health services from the quality assurance dimension and patient satisfaction. There is a relationship between the quality of health services from the tangible facilities and patient satisfaction dimensions. There is a relationship between the quality of health services from the dimension of concern (empathy) and patient satisfaction. Abstract

Health workers at primary health care facilities, such as community health centers (puskesmas), play a crucial role in all phases of disaster management. However, their preparedness often remains suboptimal due to limited training, experience, and coordination capacity. This study aimed to analyze factors associated with disaster preparedness among health workers at Harian Community Health Center, Samosir Regency. A quantitative research with a cross-sectional design was applied. The total population of 51 health workers was included using a total sampling technique. Data were collected using a structured questionnaire validated for reliability and accuracy, and analyzed using chi-square and multiple logistic regression tests. The results showed significant relationships between disaster management training (p=0.043), involvement in disaster response (p=0.008), participation in the Rapid Response Team (TGC) (p=0.012), knowledge (p=0.035), and attitude (p=0.049) with disaster preparedness. In contrast, age (p=0.471), gender (p=1.000), education level (p=0.323), and work experience (p=0.750) were not significantly related. Multivariate analysis identified participation in the TGC as the most dominant factor influencing preparedness, with Exp(B) = 21.635. These findings highlight the need to strengthen disaster preparedness at the primary care level through regular training, simulation exercises, and empowerment of TGC teams as key components of the health emergency response system

Abstract

Latar belakang: Pelayanan poli rawat jalan yaitu jenis layanan medis di rumah sakit yang dirujuk kepada pasien waktu kurang dari 24 jam, hingga tidak memerlukan perawatan di rumah sakit. Layanan ini bertujuan untuk melaksanakan observasi, menetapkan diagnosis, memberikan terapi, melakukan rehabilitasi, serta menyelenggarakan berbagai layanan kesehatan lainnya Tujuan: untuk mengevaluasi keterkaitan antara mutu pelayanan dengan tingkat kepuasan pasien terhadap layanan rawat jalan di Poliklinik Penyakit Dalam RSU Eshmun Medan Metode: menggunakan pendekatan analitis dengan desain potong lintang (cross-sectional). Populasi melibatkan seluruh pasien rawat jalan yang datang ke Poliklinik Penyakit Dalam RSU Eshmun Medan selama bulan Juni 2023, yang berjumlah 283 orang. Sampel diambil sebanyak 96 orang, menggunakan rumus penentuan sampel untuk estimasi proporsi, dan diseleksi berdasarkan kriteria inklusi dan eksklusi yang telah ditetapkan oleh peneliti. Pengolahan dan analisis data dilakukan secara komputerisasi dengan menggunakan uji Chi-Square pada tingkat signifikansi α = 0,05. Simpulan: bahwa ada hubungan yang signifikan antara mutu layanan dan kepuasan pasien (α < 0,05). Dengan kata lain, semakin baik kualitas pelayanan yang disediakan oleh rumah sakit, maka tingkat kepuasan pasien pun akan semakin tinggi Hasil: diharapkan dapat memberikan saran bagi pihak rumah sakit agar meningkatkan efektivitas pengawasan terhadap semua aspek layanan, baik dari segi struktural maupun fungsional. Setiap unit layanan dapat melaksanakan tugasnya dengan lebih profesional, berkualitas, dan fokus pada kepuasan pasien.

Abstract

Latar belakang: Indonesia merupakan negara dengan beban Tuberkulosis (TB) tinggi dan menempati peringkat kedua setelah India. Rendahnya pencapaian indikator success rate (SR) menjadi tantangan utama bagi tim pelaksana Program TB, yang dipengaruhi oleh faktor organisasi, petugas, lingkungan, dan manajemen program. Tujuan: mengetahui pengaruh perceived organizational support, organizational trust dan work environment terhadap kinerja penanggulangan tuberkulosis di Puskesmas Sekupang Batam. Metode: secara kuantitatif dengan menggunakan pendekatan cross sectional. Populasi dalam penelitian ini adalah seluruh petugas kesehatan di Puskesmas Sekupang Batam yang berjumlah 63 orang. Penentuan jumlah sampel yang digunakan dalam penelitian ini ditentukan dengan menggunakan total sampling sehingga jumlah sampel dalam penelitian ini sebanyak 63 sampel. Analisis data yang digunakan yaitu analisis univariat, bivariat dan multivariat. Hasil: Terdapat hubungan perceived organizational support, organizational trust dan work environment terhadap kinerja penanggulangan tuberkulosis di Puskesmas Sekupang Batam. Variabel yang paling berhubungan terhadap kinerja penanggulangan tuberkulosis di Puskesmas Sekupang Batam work environment (X3).. Kesimpulan: Terdapat hubungan perceived organizational support, organizational trust dan work environment terhadap kinerja penanggulangan tuberkulosis di Puskesmas Sekupang Batam

Abstract

Handwashing with soap represents the mechanical sanitization of dermal surfaces using cleansing agents and clean running water to eradicate pathogens, bacteria, and virus strains that potentially trigger various infectious clinical conditions. The primary goal behind promoting handwashing with soap is to suppress the transmission velocity of communicable agents. This empirical investigation was executed to chart the operational correlation between knowledge levels and the behavioral habit of handwashing with soap among learners at Public Elementary School 013 Simangaronsang, Doloksanggul District, Humbang Hasundutan Regency. Methodologically, This study adopts a quantitative approach using a cross-sectional model, drawing from a target population that simultaneously served as the total sample of 69 students from grades IV and V. Primary and secondary data collection was conducted utilizing a structured questionnaire instrument to evaluate the depth of student comprehension and determine the performance of the habit of handwashing with soap variable among the participants. Statistical hypotheses were verified using a bivariate Chi-Square test. The final research outcomes confirmed a highly significant relationship between health awareness and the practical habit of handwashing with soap, supported by an empirical p-value of 0.001. Based on these dynamics, there is an eye-opening need for tangible interventions to revitalize health infrastructure and deploy systematic educational schedules detailing accurate handwashing with soap steps through visual posters and audio-visual media, supplemented by furnishing robust sanitary assets and ensuring a stable supply of clean water within the school premises
