Abstract

Medical records (RM) as patient health information recorded assessment results, plans and implementation of care, treatment, integrated patient progress, as well as a summary of discharge for inpatients made by PPA. The results of medical record documents tracing at the Nala Husada dental hospital found that the RM was incomplete. This study aims to analyze the completeness of medical records filling that does not align with the accreditation standard of medical records management and health information (MRMIK). This study identified causal factors as a follow-up to implementing patient medical record management. It was conducted quantitatively with a descriptive observational approach. Data was obtained through observation, interviews, and documentation at the medical record unit during July 2022. The data was then processed and analyzed. Based on the analysis of the completeness of the 80 file documents, 86.25% were found to be complete, and 13.75% were incomplete. Incomplete medical record documents with good records were 17.4%, 16% important reports/records, 12.5% author authentication, and 8% patient identity. Factors causing incomplete filling of medical record documents with the management element approach included human error (lack of understanding and discipline, limited number of staff, training), Material and Machine (limited equipment, various documents, unspecific evaluation monitoring review), and method (incomplete regulatory studies), monitoring and evaluation are not continuous), and money (not included in the program budget plan). Recommendations to improve the completeness of medical record filling were training, sanctions, and rewards, regulation review, procedures for filling as per PPA requirements, and standardization of medical record forms.

Abstract

One way to develop health is community empowerment in the field of dental and oral health. Based on the 2018 Basic Health Research, it was reported that the prevalence of dental and oral problems in Indonesia is still very high. As many as 93% of school-aged children experience dental and oral problems. This is due to a lack of knowledge and lack of awareness of the Indonesian population regarding dental hygiene. In a series of activities for the 2023 National Dental Health Month, dental and oral health education programs are carried out in schools/School Health Programs. Method: The method is carried out using a cadre training program / Training of Trainers through zoom meetings for teachers and parents of students by providing dental and oral health material. Next, ask them to provide assistance with morning and evening toothbrushing activities for 21 days, teaching children about the benefits of good oral hygiene and showing them how to brush their teeth twice a day. It is preceded by a pretest and at the end of the delivery of the material a post test is given. The results of the evaluation of understanding of the material presented show that there is a significant difference in the pre-test and post-test of prospective cadres, which means that the cadres understand the material well. The new habit of brushing teeth has been quite successful from the uploaded photo documentation and students have recorded well for 21 days. This shows a change in behavior in maintaining dental and oral health.

Abstract

Background: Digital transformation in dental healthcare has brought forward Smart Odontogram systems that offer structured, electronic, and real-time documentation of dental conditions. This tool is particularly valuable for early detection, monitoring, and prevention of oral diseases. However, its success relies heavily on management readiness and policy.  Objective: This study aimed to review the implementation of the Smart Odontogram application from a managerial and policy perspective in supporting preventive dentistry. Methods: This study used a mixed-methods approach. The sample consisted of 12 selected individuals using purposive sampling. Inclusion criterion was involvement in Smart Odontogram implementation for at least six months. Primary data were obtained from interviews and questionnaires, focusing on organizational readiness, human resource competency and training, and managerial challenges and policies. Results: The hospital management has implemented both the hardware and software of Smart Odontogram, but not much (33.3%) integrated with Electronic Medical Record (EMR) system. Only one participant reported a lack of mentoring (8,3%), and only a few received basic training on Smart Odontogram (16,7%).  Only one experienced an oversight of data input (8,3%), and a few reported about the lack of quality of data system control (16,7%). Most participants considered Smart Odontogram to be a significant support of preventive dentistry, particularly in the early identification of caries and periodontal diseases (83,3%). Conclusion: The implementation of Smart Odontogram, supported by sound managerial strategies and policies, significantly enhanced preventive dentistry at Nala Husada Dental Hospital.

Abstract

Hospitals are required to improve service quality, cost efficiency, patient safety, and user satisfaction in an increasingly complex healthcare environment. Traditional performance measurement systems that focus mainly on financial indicators are often insufficient to evaluate overall organizational performance. The Balanced Scorecard (BSC) offers a comprehensive framework by integrating four perspectives: financial, customer, internal business processes, and learning and growth. This study aimed to review the implementation of BSC in hospitals and identify its benefits, outcomes, and challenges. A systematic review was conducted following the PRISMA 2020 guidelines. Literature searches were performed in PubMed, Scopus, Google Scholar, and DOAJ using the keywords “balanced scorecard,” “hospital performance,” “healthcare management,” and “BSC in hospital.” Articles published between 2010 and 2025 in English or Indonesian were screened according to predefined inclusion and exclusion criteria. Of 312 records identified, 14 studies met the eligibility criteria and were included in the final analysis. The findings showed that BSC positively contributed to hospital performance across all four perspectives. Financial improvements included better cost control, budget management, and operational efficiency. From the customer perspective, hospitals reported higher patient satisfaction and service quality. Internal process improvements included reduced waiting times, enhanced clinical quality, and more efficient workflows. In the learning and growth perspective, BSC supported staff development, organizational culture, employee retention, and professional competence. However, implementation challenges remained, including limited data quality, resistance to change, inadequate information systems, and insufficient understanding of BSC concepts. In conclusion, BSC is a comprehensive and adaptable framework for hospital performance measurement that supports strategic management, continuous quality improvement, and organizational sustainability.
