Abstract

BACKGROUND Women in menopause experience dramatic alterations in gonadal hormone concentrations, including diminished estradiol levels. It has been hypothesized that these hormonal changes may be responsible for the occurrences of menopausal symptoms, such as hot flushes, sleep disruption, night sweats, and irritability, as well as mild cognitive impairment (MCI) in postmenopausal women. Therefore, this study was aimed to determine if there is any association between hormonal changes and menopausal symptoms, including MCI in postmenopausal women.METHODS A total of 245 postmenopausal women were recruited for this cross-sectional study and classified into 2 groups; MCI and non-MCI groups. Diagnosis of MCI was made using the modified Petersen criteria. Variables such as subjects’ age, duration of menopause, menopausal symptoms, anthropometric status, hormone levels, and cognitive status were assessed and statistically analyzed.RESULTS Of the 245 subjects enrolled in this study, 53.9% had MCI. Multivariate analysis found that sleep disruption and estradiol levels were independent risk factors of MCI in postmenopausal women. No significant correlation was found between menopausal symptoms and estradiol concentration.CONCLUSIONS Low estradiol levels were associated with higher risks of experiencing menopausal symptoms and MCI. Sleep disruption and estradiol levels were the most influential factors in the development of MCI in postmenopausal women.

Abstract

The internet today has become an integral part of daily life that facilitates communication, education, and entertainment. The behavioral pattern of excessive internet usage has similarities to substance addiction, such as tolerance, withdrawal, repeated failure to reduce or quit, and impairment in daily life. Yet, there is no consistent physiological change that accompanies excessive use of the internet, as there is in excessive substance use. Neurological and neuroimaging studies of excessive internet users show biological changes in the prefrontal cortex that are similar to those found in other addictive syndromes. Brain structure changes also occur in the temporal cortex and ventral striatum, compromising executive function in planning and reasoning and increasing impulsive risk, resulting in loss of control over internet use. Of all the potential online applications known to cause addiction, only internet gaming disorder has been selected in the Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition (DSM-5) as a condition for further study.

Abstract

This study aimed to analyse the relation of nutritional status, physical activity or oxidative stress with cognitive function of pre-elderly and elderly. A comparative cross-sectional study was conducted on 40 pre-elderly and 35 elderly subjects who were admitted to the Integrated Development Post Program (Posbindu) between September 2014 and January 2015. Anthropometric measurements (weight and height), physical activity, and biochemical data (oxidized low-density lipoprotein[Ox-LDL] and serum malondialdehyd [MDA]) were obtained. Nutritional status was assessed through body mass index (BMI), while the cognitive function was measured by Rey Osterrieth Complex Figure (ROCF) and Digit Span Backward. The data were analysed using an independent t-test and Spearman’s test with a confidence level of 95%. The study showed that the prevalence of malnutrition among the pre-elderly and elderly were 60.0% and 80.0%, respectively. Concerning physical activity, those who belonged to inactive-sedentary activity based on physical activity level (PAL) ratio were 55.0% and 45.7%, respectively. There were no significant differences in Ox-LDL, MDA levels and cognitive function (p>0.05). Significant negative correlations existed between Ox-LDL and immediate visual memory function (r=-0.289; p<0.05), as well as between Ox-LDL and delayed visual memory function (r=-0.288; p<0.05). The conclusion that visual memory function was only correlated with Ox-LDL.

Abstract

Latar belakang. Gangguan Pemusatan Perhatian dan Hiperaktivitas (GPPH) merupakan gangguan psikiatrik paling banyak pada anak, dengan prevalensi 26,2% di Jakarta. Serum feritin, diduga, memengaruhi patofisiologi GPPH terkait dengan aktivitas dopaminergik.Tujuan. Mengetahui hubungan kadar feritin dengan gejala klinis GPPH serta mengetahui adakah perbedaan kadar feritin pada anak GPPH dan bukan GPPH.Metode. Desain penelitian potong lintang, membandingkan 47 anak GPPH dan 47 anak sehat sebagai kontrol yang berusia 7-12 tahun (usia rerata 9,09± 1,29). Uji korelasi Spearman digunakan untuk mengetahui hubungan kadar feritin dengan gejala klinis GPPH. Pemeriksaan serum feritin menggunakan metode Electrochemiluminescent Immuno Assay (ECLIA). Diagnosis GPPH ditegakkan dengan MINI KID, sedangkan gejala klinis GPPH dinilai berdasarkan Skala Penilaian Perilaku Anak Hiperaktif Indonesia (SPPAHI).Hasil. Tidak didapatkan hubungan bermakna antara kadar feritin dengan gejala klinis GPPH, koefisien korelasi 0,108 (p>0,05). Rerata kadar feritin anak GPPH adalah 38,7 ng/mL (median), yang tidak berbeda bermakna dengan kontrol (median 28 ng/mL).Kesimpulan. Tidak terbukti adanya hubungan antara feritin dengan gejala klinis GPPH. Masih diperlukan studi lebih lanjut untuk melihat peran feritin melalui dopamin pada GPPH.

Abstract

Delirium is often associated with factors that worsen the condition and prognosis of a person's health. The Cognitive Reserve Index questionnaire Indonesia version (KICK) is a questionnaire that quantifies the cognitive reserve (CR) into a single index. This study aims to obtain an instrument to assess CR, namely KICK, and test its validity and reliability and finding the role of CR in geriatric delirium cases. The study was conducted in the integrated geriatric outpatient and acute geriatric inpatient of Cipto Mangunkusumo Hospital on June 2019 to September 2019, by validity testing and case control study in geriatric delirium cases to 33 subjects with delirium and 33 controls. Content validity test was carried out through a panel test with the conclusion that KICK is relevant for use as an instrument to examine CR. The construct validity of the three variables showed significant correlation with total score with p-value <0,001). Reliability test showed Cronbach’s Alpha of 0,688. Significant differences in CR score of delirium versus control (p<0,01) with OR 9 (95% CI 2.86 to 28.22).KICK is valid and reliable to measure CR of Indonesian elderly and patients with low CR score have a higher chance of developing delirium.

Abstract

The objective of this study was to analyze the association of nutritional status measured by Body Mass Index (BMI) and Mini Nutritional Assessment (MNA) with lipid profile in pre-elderly and elderly. The study design was cross sectional involving 116 subjects in Poslansia Dahlia Senja, Depok City. Data were collected in September 2014 consisted of weight, height, arm circumference, calf circumference, and blood samples for lipid profile analysis. Blood analysis were conducted at Health Laboratory of Bogor City. The results showed that 61.1% pre-erderly and 54.5% elderly had BMI with category overweight and obese, while nutritional status measured by MNA were 62.7% pre elderly and 70.2% elderly had good nutritional status. There were 54.2% pre-elderly and 63.1% elderly people had dyslipidemia. Statistical analysis with correlation test showed there was significant correlation between BMI with triglycerides (r=0.201; p<0.05) and MNA with total cholesterol (r=0.264; p<0.05).
