Abstract

Introduction: The welfare of the elderly can be reflected through their health conditions, both physically, spiritually and socially, which allows everyone to live a productive life socially and economically. Modern lifestyles have changed human attitudes and behaviors, including diet, smoking, alcohol consumption and drugs as a lifestyle so that patients with degenerative diseases (diseases due to decreased organ function) are increasing and threatening lives. Purpose: The purpose of this community service is to screen the Frequency of Physical Activity in the elderly. Methods: The methods used are measurement and examination as well as providing education. The method of measuring the Frequency of Physical Activity and counseling about a healthy lifestyle for the elderly in Malang. Results: The results of the screening conducted, of the 55 participants who took part in the examination, found 19 people were often active, 11 people experienced problems related to physical activity with infrequent activity, and 15 people experienced problems very rarely active. Conclusions: This community service runs smoothly and can provide benefits to the community. The results of this community service can be used as a reference for improving the health status of the elderly.

Abstract

Stunting is a condition of failure in growth caused by failure of gain weight, disruption of linear growth, head circumference, and malnutrition that cause uncomparable height to the children and peers. In 2022, based on data from the Indonesian Nutrition Status Survey (SSGI), the prevalence of stunted toddlers in Malang City will be 18%. The main factors of this condition are mothers' lack of knowledge about nutrition, parenting patterns, health services, access to clean water sources, socio-cultural factors and incorrect breastfeeding practices. Therefore, efforts are needed to increase maternal knowledge as a form of preventing stunting cases. The aim of this activity is to increase mothers' knowledge and attitudes towards stunting and MPASI in children. The method in this project based learning activity is carried out in several stages, there are the planning stage, implementation stage, and evaluation stage. In this activity, a pretest and posttest were carried out, as well as delivering the material using power point media. The results of the base learning project for Posyandu mothers on street Raya Candi II RW 2, Sukun, Malang City showed that there was an increase in knowledge

Abstract

4-5 year olds should learn to improve personal social development. One effective way to improve personal social development is to play a role. The purpose of this study was to determine the differences in role play in personal social development in preschoolers (ages 4-5 years). The methode this research is Pre-experimental design using One Group Pra Post test design with sample of 30 respondents. The Sampling technique using purposive sampling using statistical test of Wilcoxon. A data Collection is done use Denver II only on the social development sector before intervention and after intervention. The result of the research shows that before the intervention of playing role as much as 25 (80,6%) children experience delay in personal social development. After playing role intervention role play a number of 12 children experience improvement in personal social development, so a number of 10 (32,2%) normal child 18 (58,1%) coution 3 (9,7%) child delayed. The result of statistical test shows that there is a difference of role play to social development in preschool children (4-5 years) with significant value 0,001 (0,001 <0,05). The conclusion in this study is that proper simulation of preschoolers should be done to improve the social development of children.

Abstract

Referral non-compliance represents a critical yet under addressed challenge in healthcare systems, undermining patient safety and amplifying malpractice risks. This mixed-methods study investigates the systemic, demographic, and socio-economic factors driving non-compliance, alongside its clinical and legal consequences. Conducted across urban and rural settings in Indonesia and comparative U.S. jurisdictions, the research integrates quantitative analysis of referral compliance rates and malpractice trends with qualitative insights from provider interviews and legal case studies. Key findings reveal that systemic barriers—such as fragmented electronic health record (EHR) systems, socio-economic inequities, and geographic isolation—disproportionately affect marginalized populations, exacerbating disparities in access to specialized care. Legal analysis highlights the tension between negligence liability and tort reform policies, emphasizing the need for rigorous documentation and patient-centered communication to mitigate risks. The study proposes multi-dimensional solutions, including technology-driven referral tracking, policy reforms to address structural inequities, and provider training in motivational interviewing. By bridging gaps between clinical practice, legal accountability, and ethical advocacy, this research advocates for systemic reforms to transform referral protocols into pillars of equitable, safe healthcare. The findings underscore the urgency of addressing non-compliance as both a clinical imperative and a moral obligation to protect vulnerable patients.

Abstract

The BPJS referral system in Indonesia has been instrumental in expanding healthcare access, yet challenges such as unnecessary referrals, governance transparency, and cost inefficiencies persist. Strengthening primary healthcare infrastructure, enhancing quality control, and improving patient education are critical to optimizing the system's effectiveness. This study explores the impact of targeted interventions on BPJS referral accessibility. This study employed a systematic review approach guided by the PRISMA framework. Peer-reviewed studies published between 2019 and 2023 were identified through databases such as PubMed, Scopus, and Web of Science using terms like "BPJS referral system," "healthcare accessibility," and "governance transparency." Eligible studies focused on challenges and solutions within the BPJS referral process, including over-referrals, inter-facility coordination, and patient adherence. Quality assessments were conducted using the Cochrane Risk of Bias Tool and Newcastle-Ottawa Scale. Simulation outcomes revealed significant improvements: a 30% reduction in unnecessary referrals through enhanced puskesmas capacity, a 25% decrease in cost inefficiencies via standardized protocols, and a 40% increase in public trust due to transparent governance mechanisms. Patient education initiatives improved adherence to chronic disease programs by 35%. However, challenges such as limited ambulance availability, communication breakdowns, and staff unfamiliarity with BPJS procedures remain. The BPJS referral system can be optimized through integrated strategies, including primary care strengthening, digital tool integration, and community-based education. Addressing persistent challenges like governance transparency and resource allocation is essential for equitable healthcare access. Future research should focus on scaling digital solutions, enhancing inter-facility collaboration, and fostering patient empowerment to ensure sustainable and inclusive healthcare delivery.

Abstract

The COVID-19 pandemic placed unprecedented strain on healthcare systems worldwide, necessitating rapid policy interventions to manage patient flow, resource allocation, and referral mechanisms. This study employed a systematic literature review (SLR) using the PRISMA framework to evaluate the impact of health emergency policies on referral mechanisms during the pandemic. The review focused on understanding how policies such as lockdowns, triage protocols, and telemedicine integration influenced access, equity, system efficiency, and resilience in referral networks. A comprehensive search was conducted across databases including PubMed, Scopus, Web of Science, IEEE Xplore, and Cochrane Library, with inclusion criteria encompassing peer-reviewed studies published in English between 2020 and 2023. Studies were rigorously assessed for methodological quality using tools such as the Cochrane Risk of Bias Tool and Simulation Model Quality Assessment Framework. Key themes identified included restricted access to emergency services, healthcare system reordering, operational strain on infrastructure, and coordination gaps. The findings revealed significant challenges, such as ambulance delays, emergency department closures, protocol overload, and inequities in care access, while also highlighting adaptive strategies like AI-driven referral tools, hybrid triage systems, and community health worker networks. This study underscores the need for balanced policy design, robust technological integration, and long-term systemic reforms to strengthen referral mechanisms during health emergencies. The results provide actionable insights for policymakers and healthcare administrators to optimize referral systems and ensure equitable access to care during future crises.

Abstract

Background: The advancement of modern sports demands a comprehensive, rapid, and responsive healthcare system, wherein nurses—as integral members of multidisciplinary medical teams—play a crucial role in safeguarding athlete safety and performance. This study aimed to explore nurses’ experiences, roles, challenges, and expectations as part of medical teams during sporting events.  Methods: A qualitative study employing a phenomenological approach was conducted to gain in-depth insights into the experiences of eight nurses who had served on sports medical teams in Malang, Indonesia. Data were collected through in-depth interviews and analyzed thematically. Results: Five main themes emerged: (1) background and motivation, (2) roles and responsibilities, (3) interprofessional collaboration, (4) practical challenges in the field, and (5) expectations and recommendations. Nurses participated due to personal and professional motivations, fulfilling dynamic roles ranging from first aid provision to health education. However, they encountered limitations such as inadequate equipment, heavy workloads, and unclear authority boundaries. They also expressed desires for specialized training and greater professional recognition. Conclusion: Nurses are essential pillars within sports medical teams, yet systemic support is needed to strengthen their roles and professional standing.

Abstract

The integration of electronic referral (e-referral) systems into healthcare ecosystems offers significant potential to enhance care coordination, reduce inefficiencies, and improve patient outcomes. However, challenges persist across legal, technological, and social dimensions. A PRISMA-based Systematic Literature Review (SLR) highlights jurisdictional barriers, such as GDPR-HIPAA mismatches, with 40% of EU organizations facing hurdles in sharing data with U.S. partners. Stringent data localization laws in countries like Russia and China further complicate interoperability. Ethical frameworks, including beneficence and non-maleficence, are critical for trust in sensitive care areas. Technological gaps include uneven HL7 FHIR adoption, infrastructure limitations, and underutilization of AI and blockchain innovations. Estonia’s X-Road system demonstrates blockchain’s role in reducing referral wait times by 30%, while India’s e-Sanjeevani platform serves remote areas offline. Clinician resistance and digital literacy gaps necessitate tailored solutions. Escalating cyber threats demand robust measures like zero-trust architectures. Public-private partnerships, open-source platforms, and measurable outcomes (?80% clinician adoption, <48-hour approval times) are vital. Pandemic-resilient systems and sustainable cloud solutions further emphasize scalability and equity. Harmonized policies and inclusive stakeholder engagement are essential for global success.

Abstract

Low community health literacy and limited local health cadre capacity remain major public health challenges in peri-urban areas of Malang, Indonesia. To address these issues, Universitas Brawijaya (UB) developed the UB Integrated Door-to-Door Health Clinic Model, combining health education, basic medical services, and cadre empowerment through household-based outreach. This study evaluated the model’s effectiveness in improving community health literacy and cadre capacity in Kalirejo Village. The program involved lecturers, students, healthcare professionals, and local cadres through health education, screening, counseling, and training activities. Quantitative data were analyzed using descriptive statistics and paired comparative tests, while qualitative feedback was analyzed thematically. Results showed significant improvements in community health literacy mean scores (62.4% to 81.7%; p<0.05) and cadre competency scores (58.9% to 84.3%; p<0.05). Participants also reported increased engagement in health promotion activities. The model demonstrates the potential of university-led integrated outreach programs for sustainable community empowerment and health promotion.

Abstract

Abstract: Breastfeeding is the best way to meet the nutritional needs for the growth and development of infants. Breastfeeding plays a role in cognitive, sensory, motor development and protection against infections and chronic diseases, but various obstacles are still found in the community. The study focused on the experience of ineffective breastfeeding among working mothers in Pucangsongo Village. Design of this study was Qualitative research with a phenomenological approach. Data was collected by interviewing five nurses and observing data in the field to obtain primary data and reviewing Pucangsongo Polindes documents to obtain secondary data. Triangulation of research data is done by triangulation of sources and methods. Data analysis through the stages of data reduction, data display, and drawing conclusions and notifications. Results of this study were obstacles faced by working mothers who experience problems with ineffective breastfeeding include the availability of advice and infrastructure for breastfeeding in the workplace. Failure to breastfeed for working mothers, namely busyness during work or company policies that are not cooperative in providing sufficient time for mothers to express breast milk. Health workers such as midwives also affect the experience of ineffective breastfeeding. Health workers have a very important role in protecting, promoting, and supporting breastfeeding businesses and must be seen in terms of their broad involvement in social aspects.
