Abstract

Incompleteness of files and inaccurate diagnosis codes are factors that cause claims to occur. From the results of observations of 10 samples of claim files for outpatient schizophrenia cases, it was found that 3(30%) were approved and 7(70%) were rejected because 2 files had different SEP dates, 2 files did not have the signature of the doctor in charge of the patient on the medical resume sheet and 3 used incorrect diagnosis codes, resulting in delays in the payment process which caused material losses for the Hospital. This study aims to determine pending claims for outpatient schizophrenia cases in terms of completeness of files and accuracy of diagnosis codes. The type of research is observational with a quantitative descriptive design with a population and sample of 68 pending outpatient schizophrenia case files for the period January and February 2024. The research data used is secondary data obtained by observation using an observation sheet, then the data is processed and analyzed univariately using a frequency distribution. The results of this research show that pending claims for outpatient schizophrenia are seen from the completeness of the file and the accuracy of the diagnosis code, namely 16 (23%) pending due to incomplete claim files, 48 (71%) pending due to inaccurate diagnosis codes and 4 (6%) pending due to incomplete and inaccurate diagnosis code. Efforts are needed to solve the problem of pending claims by forming an internal verification team to evaluate and optimize existing management activities and communicate with the person in charge of the medical department

Abstract

Background: Recent research shows that vitamins actively regulate epigenetic mechanisms like DNA methylation and histone changes. Gene regulation, cellular differentiation, and disease development depend on these pathways. Aims: This review aims to synthesize current evidence on how vitamins A, B, C, D, E, and K influence epigenetic regulation, and to elucidate their potential roles in health maintenance and disease prevention. Methods: NCBI and ScienceDirect were used to search English-language literature from 2015 to 2024 for a narrative review. To capture vitamins' direct and indirect impacts on epigenetic pathways, selected peer-reviewed papers were qualitatively examined. Results: Vitamin A affects gene expression via receptor-mediated mechanisms which modify chromatin structure. B vitamins provide methyl donors for DNA methylation and histone changes. Vitamin C cofactors TET enzymes and histone demethylases, supporting active DNA demethylation and histone acetylation. The vitamin D receptor (VDR) affects DNA methyltransferases and histone-modifying enzymes, regulating gene transcription. Vitamins E and K modulate oxidative stress and histone changes to stabilize the epigenome, which may benefit cancer treatment and metabolic regulation. Conclusion: Vitamins' diverse and linked epigenetic regulatory activities are highlighted.  These micronutrients may help prevent and treat cancer, metabolic problems, and developmental anomalies by maintaining genomic integrity and influencing gene expression. Keywords: Vitamins, Epigenetics, DNA Methylation, Histone Modification, Nutritional Epigenetics, Gene Expression.

Abstract

Early character education plays a fundamental role in shaping a generation that is not only intelligent but also physically healthy, emotionally balanced, and morally responsible. The cultivation of these values from an early age is crucial to equip children with the resilience needed to face various social challenges in the future. This community service activity was designed to increase awareness among elementary school students regarding three critical issues: the dangers of drug abuse, the practices of bullying, and the detrimental impact of corruption. The program was conducted at SDN 1 Citeluk with the participation of approximately 80 students from grades IV to VI. The educational materials were delivered through an interactive approach combining lectures, group discussions, role-playing, and quizzes. This approach was selected to ensure that the learning process was enjoyable, engaging, and easily understood by children at the elementary school level. During the sessions, students were introduced to the basic concepts of drugs, their harmful effects on health and life, and the importance of avoiding them. Additionally, the program emphasized identifying the various forms of bullying—whether physical, verbal, or social—and encouraged students to develop empathy and mutual respect. Another important aspect addressed was corruption, where students were guided to recognize the value of honesty and integrity through relatable examples in daily life. The results of the activity revealed that students were able to clearly understand the dangers of drugs, recognize the different forms of bullying, and provide examples of honest and moral behavior. This indicates that early character education delivered through interactive and participatory methods can effectively enhance children’s comprehension of healthy and moral values. It is expected that similar programs can be carried out sustainably in elementary school settings as part of continuous character-building efforts to support the development of a responsible and ethical young generation

Abstract

Accurate diagnosis coding according to ICD-10 to assign accurate diagnosis codes for inpatients also requires additional information such as laboratory results to be taken into account. The results of initial observations made on 10 medical resume sheets for gastroenteritis cases showed that 3 (30%) had accurate diagnosis codes and 7 (70%) had inaccurate diagnosis codes. This can affect the quality of data, information and reports as well as the accuracy of rates for general patients and INA-CBG'S rates which are used as a payment method for BPJS patients so that it can have an impact on reducing the quality of hospital services. This research aims to determine the accuracy of writing diagnoses and the accuracy of gastroenteritis codes for inpatients at Rafflesia Hospital in 2023. The type of research used is descriptive research with a cross sectional method. The population in this study was 214 medical record files for inpatient gastroenteritis cases in 2022 with a sample of 140 files using a simple random sampling technique. The research data used is secondary data which was processed univariately. The results of this study were that from 140 inpatient gastroenteritis medical record files, 100(100%) had the results of supporting examinations but they did not match the diagnosis written on the patient's medical resume because in writing the diagnosis it was not stated whether the gastroenteritis was infected or non-infected, 29(21% ) writing the diagnosis correctly, 43(31%) wrote the diagnosis incorrectly because they used Indonesian and 68(48%) wrote the diagnosis incorrectly because they used non-standard abbreviations and 69(49%) had an accurate diagnosis code and 71(51%) Inaccurate diagnosis code because there is no 4th character.

Abstract

The completeness of the physical examination and the accuracy of laboratory results are crucial in supporting the accuracy of diagnostic codes. A preliminary survey of 10 Dengue Hemorrhagic Fever (DHF) medical records revealed that 4 (40%) were accurate and 6 (60%) were inaccurate. This was due to incomplete and inaccurate physical examination and laboratory results, which affected diagnosis and coding. This resulted in decreased data, information, and reporting quality, as well as the accuracy of INA-CBG rates, which could negatively impact the quality of hospital services. This study aimed to determine the accuracy of DHF diagnostic codes in terms of the completeness of medical information. This study used an observational, quantitative descriptive design, with 86 DHF medical records as the population and sample. The data used were secondary data obtained through observation using a checklist. The data were then processed and analyzed univariately using a frequency distribution. The results of this study are that from 86 DHF medical record files, 33 (38%) were found to have complete physical examinations, 12 (10%) were accurate laboratory test results, 8 (9%) were accurate diagnoses, and 8 (9%) were accurate diagnosis codes. It is expected that medical record officers, especially in the assembling section, will conduct medical record audits through qualitative analysis and improve coder skills through coding classification training.

Abstract

Cancer represents a global health burden characterized by high morbidity and mortality, underscoring the urgent demand for reliable, non-invasive biomarkers to enhance early detection, prognostication, and therapeutic monitoring. Circulating microRNAs (c-miRNAs) are short non-coding RNAs present in body fluids that have attracted attention as potential biomarkers, because of their stability, ease of detection, and close relationship with tumor biology. This review evaluates the diagnostic and prognostic potential of c-miRNAs and their integration into precision oncology. A systematic search was performed across PubMed, Scopus, and ScienceDirect for publications from 2010 to 2025 that investigated c-miRNAs in human body fluids in relation to diagnosis, prognosis, or treatment response. Twelve studies met the eligibility criteria, encompassing breast, lung, colorectal, and gallbladder cancers. Frequently dysregulated c-miRNAs, particularly miR-21, miR-155, miR-210, and Let-7a, demonstrated consistent associations with tumor detection, patient survival, and disease progression. Analytical platforms ranged from quantitative RT-PCR to advanced biosensor-based technologies, enabling sensitive and rapid quantification. Collectively, the evidence highlights c-miRNAs as non-invasive biomarkers capable of complementing conventional clinical tools. Their clinical adoption could reduce reliance on invasive tissue biopsies, support earlier and more accurate diagnosis, assist in tailoring patient-specific treatment strategies, and promote the broader implementation of precision medicine in cancer care.
