Abstract

Aspirasi benda asing merupakan masalah global dan seringkali menyebabkan komplikasi yang mengancam jiwa,Aspirasi benda asing lebih sering terjadi pada anak-anak dibanding orang dewasa. Sekitar 75% sampai 85% aspirasi bendaasing terjadi pada anak dibawah 15 tahun yang lebih dari dua pertiga kasus terjadi pada anak-anak usia dibawah tiga tahun(3). Di Amerika angka kematian pada anak usia di bawah 4 tahun mencapai 7% pada tahun 1986(5). Pada kasus ini, penderitaadalah seorang perempuan berumur 14 tahun, datang ke IRD dikonsulkan ke bagian paru dengan keluhan tertelan jarumpentul saat sedang memakai hijab sambil bercanda dengan temannya. Kemudian dilakukan foto Rontgen tampak bentukanmetal jarum di daerah parakardial kanan Pada dewasa bronkus utama kanan lebih pendek dari bronkus utama kiri. Bronkusutama kanan membentuk sudut 25 derajat dari garis median, sedangkan bronkus utama kiri membentuk sudut 45 derajat.Pada anak usia dibawah 14 tahun hal ini tidak berlaku karena ukuran bronkus kanan dan kiri relatif sama, dengan percabanganbronkus kiri tidak sedekat pada bronkus kanan. Selanjutnya dilakukan ekstraksi jarum pentul yang telah teraspirasi denganmenggunakan bronkoskopi serat optik pada penderita. Ekstraksi jarum pentul berhasil dilakukan dengan baik.Kata Kunci : Aspirasi, ekstraksi jarum pentul

Abstract

Kebakaran hutan telah menjadi bencana alam yang rutin di Kalimantan. Hal ini meningkatkan dampak polusi yang berkepanjangan. Salah satu dampak polusi yang paling utama adalah pengaruhnya terhadap kesehatan pernapasan. Tujuan dari studi ini adalaha untuk mencari hubungan antara kebakaran hutan dengan penyakit pernapasan. Mencari hubungan antara kebakaran hutan dengan penyakit pernapasan PPOK, Asma dan ISPA. Metode studi ini menggunakan cohort retrospective dengan total sampel. Data polusi yang diambil pada bulan september hingga november terdapat di dua tempat yaitu Kota Palangkaraya dan Kabupaten kotawaringin Timur. Penelitian dilakukan di RSUD Dr. Doris Sylvanus Palangkaraya dan RSUD Dr. Murjani Sampit. Uji statistik menggunakan korelasi Spearmen dan tes Kruskal-Wallis. Hasil dari 277 responden didapatkan tidak ada korelasi antara peningkatan polusi dengan ketiga penyakit. Untuk antar penyakit dengan tingkat polusi signifikansinya adalah p=0,058 (PPOK), p=0,759 (Asma) dan p=0,594 (ISPA). Sebagai kesimpulan, tidak ada hubungan antara tiga penyakit pernapasan dengan peningkatan tingkat polusi yang diakibatkan oleh kebakaran hutan. Tetapi pada polusi tingkat berbahaya terdapat lonjakan yang berarti jika dilihat secara diagram. Diperlukan penelitian lebih lanjut dalam waktu yang lebih  lama untuk mendapatkan hasil yang lebih akurat.

Abstract

Introduction: Indonesia is still among the top three contributors to the number of Tuberculosis (TB) patients in the world in 2017. The awareness about TB can be affected by the presence of other TB patients in the family. Perception and good knowledge in TB patients can increase obedient in treatment. This study aims to relate to the presence of other TB patients in the family environment with knowledge of TB patients.Methods: This study used cross-sectional design. Research respondents have taken using the consecutive sampling technique. Respondents were TB patients on category one anti-tuberculosis treatment at Siti Khadijah Sepanjang Hospital and several primary health centers in Sidoarjo region during February-March 2019. This study used a questionnaire which included sociodemographic, level of knowledge, and perception about TB.Results: The number of respondents in this study was 50 people aged 22-67 years old. The level of knowledge of the respondents was mostly good (82%) as well as perceptions regarding TB (78%). Of the 50 patients, 17 patients (34%) claimed some families also suffered from TB. The comparison between group with other TB patient and group without other TB patients in the family showed significantly different result in the level of knowledge (p=0,000) and perception (p=0,000). The presence of other TB cases in family increased level of knowledge and perception regarding TB significantly.Conclusion: The presence of other TB patients in the family environment can increase awareness so that it can increase the interest of TB patients to find information about TB.

Abstract

Tuberculosis (TB) is caused by bacteria (Mycobacterium tuberculosis) that most often affect the lungs. The incidence of tuberculosis in Indonesia is high because the success rate of tuberculosis treatment has decreased which causes the chain of transmission continues to occur. The main government programs to decrese the incidence and increasing the success treatment with the Directly Observed Treatment (DOTS) program. One important indicator of the DOTS program to assessing the success of tuberculosis treatment is the conversion of positive AFB sputum to negative at the end of the intensive phase of treatment. AFB sputum conversion to negative at the end of the intensive phase of treatment is influenced by several internal factors such as level of education and income, gender, adherence, patient's nutritional status, and comorbidities. Then external factors can also influence the AFB sputum conversion in anti-tuberculosis treatment such as environmental conditions, smear positivity, drug taking supervisors (PMO), and availability of drugs in health facilities.

Abstract

Background:  Tuberculosis (TB) remains a significant public health concern in Indonesia, with a high prevalence of cases, particularly in patients with comorbidities such as HIV infection, chronic obstructive pulmonary disease (COPD), diabetes mellitus (DM), and chronic kidney disease (CKD). Evidence suggested radiological variations in chest X-ray findings among these complex condition, which may pose significant challenges in accurately diagnosing pulmonary TB in clinical practice.Objective:  This study aimed to provide a comprehensive understanding of the variations in chest X-ray imaging in pulmonary TB patients with specific comorbidities, focusing on DM, CKD, and COPD.Methods: A cross-sectional study was conducted at Siti Khodijah Muhammadiyah Sepanjang Hospital by utilizing standardized medical records and chest X-ray results of 50 pulmonary patients with comorbidities.Results: The most prevalent comorbidity was DM, with radiological findings including fibroinfiltrates, consolidations, and cavities. CKD patients exhibited radiological features such as infiltrates, cavitations, and pleural effusion, while COPD patients presented with infiltrates and consolidations. Conclusion: The study provides valuable insights into the radiological manifestations of pulmonary TB and its comorbidities, offering a basis for improved management and treatment strategies for patients with pulmonary TB and comorbidities. Further research employing longitudinal designs and balanced representation of comorbid conditions is recommended to enhance the understanding of the interplay between TB and associated health conditions.

Abstract

Background: Diabetes mellitus (DM) is a health problem in the world. Currently, the number of DM cases globally has reached 537 million people, and Indonesia ranks fifth highest in the world. Low knowledge and dietary compliance are a problem that continues to contribute to new cases and relapses of old cases characterized by uncontrolled increases in blood sugar. This is caused by low self-efficacy which is characterized by feelings of hopelessness. Objectives: The aim of this research was to determine the effect of Diabetes Self-Management Education (DSME) on self-efficacy in type II Diabetes Mellitus patients. Methods: This study used a Quasy-experimental design, with a pretest posttest equivalent control group design, the sample size was 68 type II DM patients, consisting of 34 intervention groups and 34 control groups. Self-efficacy data was collected using the DMSES (Diabetes Management Self Efficacy Scale) questionnaire. Univariate analysis was used to describe the frequency distribution of respondents, mean, median and standard deviation. Next, bivariate analysis used the Wilcoxon test. Results: There was a significant difference in the difference in mean value self-efficacy between before and after intervention with p-value 0.000 < 0.05. Conclusion: Diabetes Management Self Efficacy Scale is effective increase self-efficacy. Thus, DSME can be an alternative intervention to overcome problems and prevent complications in DM patients.

Abstract

Indonesia is ranked third with the most significant number of pulmonary TB (Tuberculosis) patients in the world, after China and India. In 2017, there were 420,994 new TB cases in Indonesia. The survey showed that 68.5% of male participants smoked, while only 3.7% of female participants smoked. This service activity aims to assist TB patients through the Zero TB application based on a community partnership system for TB prevention and treatment. The training effectively improved participants' knowledge, behavior, and compliance. There was a significant change in the level of expertise, where the pre-test results showed that 50% of respondents had low knowledge, but after counseling using the Zero TB application, 83.4% had high knowledge. Behavioral changes also improved, with 53.3% of respondents showing good changes in the pre-test, which increased to 100% after the intervention. The level of treatment adherence also increased from 93.3% of respondents with low adherence in the pre-test to 96.6% with high adherence in the post-test. It is hoped that participants' increased knowledge, behavior, and adherence can prevent TB while raising public awareness of the importance of TB treatment, especially at Yayasan Bhanu Yasa Sejahtera (YABHYSA).

Abstract

The financial product innovation that exists at the KHAS Kempek Micro Waqf Bank, Cirebon Regency is one of several sharia MFIs for community empowerment programs through the establishment of Sharia MFIs around Islamic boarding schools, which in its establishment was facilitated by the Financial Services Authority (OJK) and the Small Business Business Incubation Center (PINBUK). This study aims to find out how to optimize the use of Islamic products at Micro Waqf Banks for Islamic Financial and Banking Innovation in MSME Development. Method study using a qualitative approach to design descriptive analytical. Source of data used is primary data sources from data obtained by observing and interviewing directly from the source, namely at the KHAS Kempek Micro Waqf Bank, Cirebon Regency as well as collecting documentation from books, articles, journals, the internet and various other sources which are then processed and analyzed with stages of analysis which include data reduction, data display and verification as well as conclusions. Results from study shows that Islamic Financial and Banking Innovation Micro Waqf Bank KHAS Kempek Cirebon Regency for the Economic Empowerment of Micro Enterprise Actors (MSMEs) around Islamic Boarding Schools is operating with sharia principles that are committed to tackling the problem of poverty and inequality by providing access to capital to productive poor people or to micro-entrepreneurs around Islamic boarding schools to improve their standard of living and help them with loans that are unsecured or collateral and most importantly with very low margins, which are equivalent to 3% per year, by maximizing cash waqf funds in providing loans capital, coaching and evaluation. It was concluded that the KHAS Kempek Micro Waqf Bank, Cirebon Regency, had succeeded in empowering the micro-business actors around Islamic boarding schools with sharia principles.
