Abstract

Background: Myelofibrosis with myeloid metaplasia is a rare myeloproliferative disorder. Epidemiological data on the incidence of Myelofibrosis with myeloid metaplasia are still lacking. In Goteborg, Sweden, from 1983 to 1992, the incidence of the disease was retrospectively calculated at 0.4 per 100,000 population. In population- based leukemia data in the UK, the incidence of Myelofibrosis with myeloid metaplasia is estimated to be 0.73 per 100,000 person-years in men and 0.40 per 100,000 person-years in women. It is estimated that there are 230 new cases each year in England and Wales, 330 in Italy, and 1,600 in the United States. No medical therapies have been shown to be effective against the overall survival of patients. The prognosis of myelofibrosis with myeloid metaplasia varies widely. For this reason, it is necessary to report cases of Myelofibrosis Myeloid Metaplasia Methode: Descriptive research, in the form of case reports of patients with myelofibrosis metaplasia, with data obtained from primary data anamnesa and direct physical examination of patients and secondary data from medical records, the results of examinations of doctors at RSPAL Dr Ramelan Surabaya 2022-2024. Results: Since 13 December 2022 until this date 3 May 2024 (17 months).Patient with a diagnosis of Primary Myelofibrosis and high Uric Acid after undergoing therapy with jakavi 15 mg 1x1. Allopurinol 300mg 1x1 and Nabic 3x1. Experienced improvement in complaints of increased appetite. Resting at night there are no complaints. Activity such as walking is not easily tired, mass in the abdomen that does not cause pain. On physical examination, there was an increase in body weight from 50 kg to 55 kg and still found S IV splenomegaly, from the haematological examination there was a decrease in platelets 136.00 (10^3µL) which was previously 203.00 (10^3µL). Uric acid improved from 10.8 mg/dl to 7.7 mg/dl. Conclusion: Jakavi is a drug containing ruxolitinib that can treat splenomegaly or disease-related symptoms in adult patients with myelofibrosis. Abstract

Medical education is widely recognized for its high academic demands, intensive schedules, and frequent examinations, all of which may contribute to academic stress among students. Unmanaged stress can affect the autonomic nervous system and increase gastric acid secretion through interactions within the brain–gut axis, manifesting as gastrointestinal disturbances, particularly functional dyspepsia. This study aimed to examine the relationship between academic stress and the occurrence of functional dyspepsia among preclinical medical students of the Faculty of Medicine, Hang Tuah University, class of 2022. This study employed an descriptive correlational design with a cross-sectional approach. The sampling technique used was purposive sampling, involving 72 students selected from the 2022 cohort population. Data were collected using PSS-10 to assess stress levels and R4-FDDQ to diagnose functional dyspepsia. Data analysis was conducted using the Chi-square statistical test. The results demonstrated a statistically significant association between academic stress and functional dyspepsia, as indicated by the Chi-square test with a p-value of 0,014. This study concludes that there is a significant relationship between academic stress and the occurrence of functional dyspepsia among preclinical medical students of the Faculty of Medicine, Hang Tuah University, class of 2022. Higher levels of academic stress are associated with an increased likelihood of developing functional dyspepsia.   ABSTRAK Pendidikan kedokteran dikenal memiliki tuntutan akademik yang tinggi, padatnya jadwal, dan ujian yang sering, yang berpotensi memicu stres akademik pada mahasiswa. Stres yang tidak tertangani dapat memengaruhi sistem saraf otonom dan meningkatkan sekresi asam lambung melalui interaksi brain-gut axis, yang bermanifestasi sebagai gangguan pencernaan, khususnya dispepsia fungsional. Tujuan dari penelitian ini untuk mengetahui hubungan antara stres akademik dan kejadian dispepsia fungsional pada mahasiswa preklinik Fakultas Kedokteran Universitas Hang Tuah angkatan 2022. Penelitian ini menggunakan desain deskriptif korelasi dengan pendekatan cross-sectional. Teknik pengambilan sampel menggunakan purposive sampling dengan jumlah responden sebanyak 72 mahasiswa dari populasi angkatan 2022. Pengumpulan data dilakukan menggunakan PSS-10 untuk mengukur tingkat stres dan R4-FDDQ untuk diagnosis dispepsia fungsional. Analisis data dilakukan menggunakan uji statistik Chi-Square. Hasil penelitian menunjukkan bahwa terdapat hubungan yang bermakna antara stres akademik dengan dispepsia fungsional dari hasil uji Chi-Square dengan nilai p-value sebesar 0,014. Kesimpulan dari penelitian ini adalah terdapat hubungan yang signifikan antara stres akademik dan kejadian dispepsia fungsional pada mahasiswa preklinik Fakultas Kedokteran Universitas Hang Tuah angkatan 2022. Semakin tinggi tingkat stres akademik, semakin besar peluang terjadinya dispepsia fungsional.

Abstract

Background: The liver's presence of fat has a histological picture with lipids>5%, and no secondary cause is called NAFLD. Rhizophora apiculata can serve as a therapeutic or prophylactic agent in NAFLD due to its flavonoid content. Objective: to ascertain the impact of Rhizophora apiculata extract administration on SGOT levels in white rats subjected to dexamethasone induction. Methods: The experimental animals were categorized into four groups: K- group, no treatment; K+ group, which was administered dexamethasone at a dosage of 5 mg/kg BW from the 15th to the 21st day; KP1 group, which received Rhizophora apiculata ethanol extract at a dosage of 56 mg/kg BW from the 8th to the 21st day, along with dexamethasone at 5 mg/kg BW from the 15th to the 21st day; and KP2 group, which was given Rhizophora apiculata ethanol extract at 56 mg/kg BW and dexamethasone at 5 mg/kg BW from the 15th to the 21st day. Result: The data were not normally distributed (p < 0.05), so the Kruskal-Wallis test was carried out, which yielded p < 0.05. Consequently, the post hoc test was continued, namely the Mann-Whitney U test. The post hoc test of the K-group and K+ obtained p < 0.05. The K+ group and KP1 and KP2 showed P > 0.05, indicating no significant difference. Conclusion: The Rhizophora apiculata extract at a dose of 56 mg/kg BW for 7 days can reduce SGOT levels in the Kp2 group.
