Abstract

The Covid-19 pandemic has caused more and more children to lose their parents, thus requiring attention from various interested parties to deal with this problem. This oral health counseling effort needs to be carried out so that the orphanage administrators and the children under them can increase knowledge, awareness, and then are expected to form a behavior that will have a positive impact on the oral health of these children. PkM activities that will be carried out include training, counseling and education on how to maintain oral health in orphanages, including the causes of caries, how to detect early dental caries, how to prevent caries and actions taken if dental caries has occurred. Toothbrush and tongue brush training will also be conducted. It is hoped that this PkM can be a provision for improving oral health in the extended family of the orphanage. Outputs in the form of educational videos, posters and teaching aids will be submitted for Intellectual Property Rights (HKI) and are expected to be used by the orphanage.

Abstract

Obliterasi saluran akar adalah kondisi yang ditandai dengan deposisi jaringan keras dalam ruang saluran akar yang sering kali disebabkan oleh trauma dan dapat menyebabkan tantangan besar dalam diagnosis maupun perawatan. Seorang wanita berusia 27 tahun dengan keluhan diskolorasi kuning pada gigi insisivus sentral atas (gigi 11) akibat trauma 10 tahun sebelumnya. Pemeriksaan klinis dan radiografis menggunakan CBCT menunjukkan obliterasi saluran akar tanpa disertai kondisi patologis periapikal. Uji sensitivitas pulpa menunjukkan tidak ada respons, namun tidak dapat disimpulkan sebagai nekrosis. Untuk manajemen, static guide endodontik berbasis data CBCT digunakan untuk membantu penetrasi akurat ke saluran akar yang terkalsifikasi. Perawatan melibatkan preparasi biomekanik dengan teknik crown down menggunakan instrumen rotary, irigasi larutan natrium hipoklorit, dan medikasi intrakanaldengan kalsium hidroksida. Saluran akar kemudian diobturasi menggunakan teknik single cone dengan bioceramic sealer. Untuk memulihkan estetika, dilakukan perawatan intracoronal bleaching dengan hidrogen peroksida 35% yang dilanjutkan dengan restorasi komposit. Prosedur ini berhasil mengembalikan warna gigi dari warna A3 menjadi A2 tanpa komplikasi teknis seperti perforasi atau patahnya instrumen. Evaluasi menunjukkan jaringan periapikal dalam batas normal tanpa gejala klinis. Laporan ini menunjukkan bahwa penggunaan static guide endodontik meningkatkan akurasi dan efisiensi dalam pengelolaan kasus obliterasi saluran akar, terutama pada gigi anterior dengan diskolorasi yang menuntut estetika. Pendekatan ini direkomendasikan sebagai solusi yang efektif untuk kasus serupa, mengatasiketerbatasan teknik konvensional dan meningkatkan keberhasilan jangka panjang.

Abstract

Latar belakang: Resin komposit nanohybrid merupakan kombinasi antara nanopartikel dan microhybrid, yang memiliki banyak kelebihan. Resin komposit nanohybrid memiliki kekurangan yaitu absorpsi cairan. Bahan makanan atau minuman yang mengandung asam dapat berpengaruh pada kekasaran dan kekerasan resin komposit nanohybrid. Jus jambu biji kemasan mengandung asam dan mempunyai nilai pH yang rendah. Tujuan: Untuk mengetahui pengaruh perendaman jus jambu biji merah kemasan terhadap kekasaran dan kekerasan resin komposit nanohybrid. Metode: Penelitian ini merupakan eksperimental laboratoris dengan rancangan post test with control group design dengan jumlah sampel sebanyak 24 sampel resin komposit nanohybrid yang dibentuk menggunakan stainless steel mold berdiameter 10 mm dan ketebalan 2 mm (sampel dibagi menjadi 2 kelompok; kelompok perlakuan dan kelompok kontrol). Kelompok kontrol direndam selama 24 jam dalam saliva buatan, sedangkan kelompok perlakuan direndam selama 6 jam dalam jus jambu biji merah kemasan dilanjutkan dengan perendaman dalam saliva buatan selama 18 jam. Perendaman sampel dilakukan selama 4 hari dalam inkubator dengan suhu 37°C. Uji kekasaran permukaan menggunakan surface roughness tester dan uji kekerasan menggunakan micro vickers hardness. Hasil: Berdasarkan uji analisa one way ANOVA, kekasaran permukaan (p: 0,001) dan kekerasan (p: 0,003) menunjukkan perbedaan perubahan bermakna (p<0,05). Kesimpulan: Perendaman menggunakan jus jambu biji merah kemasan setelah direndam 6 jam selama 4 hari terjadi peningkatankekasaran dan penurunan kekerasan.

Abstract

Introduction: Endocrown is an indirect restoration following the endodontic treatment made of ceramic material as a substitute for post core crown. The endocrown is cemented to the inner wall of the pulp chamber and to the cavity margins to increase the macromechanical retention. Material commonly used are monolithic zirconia and lithium disilicate; both can be obtained using the CAD/CAM technique. Good endocrown restoration must be able to handle the chewing workload especially on the posterior tooth. Objective: To determine the difference in fracture toughness of monolithic zirconia and lithium disilicate CAD/CAM endocrown. Methods: This study used 6 endocrown CAD/CAM samples, attached to typodont prototype in 3D resin printing model produced by the printing machine. Samples were divided into 2 groups. Group A were monolithic zirconia endocrowns and group B were lithium disilicate endocrowns. The fracture toughness was calculated using the Universal Testing Machine (UTM). Results: Fracture toughness of the monolithic zirconia endocrown were 2.747 N and lithium disilicate were 769 N. The unpaired t test showed that there was significant difference in fracture toughness between groups (p < 0,05). Conclusion: Monolithic zirconia endocrown has a higher fracture toughness compared to the lithium disilicate endocrown, but lithium disilicate is more recommended as an endocrown restoration material because its fracture toughness is closer to masticatory load of posterior teeth.

Abstract

ABSTRAKPendahuluan: Trauma dapat menyebabkan nekrosis pulpa, resorpsi eksternal, gangguan proses penutupan bagian apikal, dan perubahan warna gigi. MTA memiliki aktivitas antimikroba, biokompatibilitas yang sangat baik, dan kemampuan sealing yang unggul. Bleaching intrakoronal dapat digunakan untuk mengembalikan warna gigi setelah perawatan endodontik. Perawatan apikal yang terbuka dapat dilakukan dengan menggunakan MTA sebagai apical plug. Kasus trauma gigi yang disertai dengan diskolorasi dapat dilakukan bleaching intrakoronal serta MTA sebagai apical plug. Laporan kasus ini bertujuan untuk melaporkan penatalaksanaan bleaching intrakoronal serta MTA sebagai apical plug. Laporan kasus: Seorang wanita 46 tahun mengeluhkan adanya fraktur dan perubahan warna pada gigi insisivus sentralis kanan rahang atas. Pemeriksaan klinis gigi 11 tidak responsif terhadap tes vitalitas, positif terhadap tes perkusi. Pemeriksaan radiografi terdapat radiolusensi di daerah periapikal. Diagnosis kasus ini nekrosis pulpa disertai periodontitis apikali simptomatik. Setelah perawatan saluran akar dan medikamen intrakanal menggunakan Ca(OH)2, MTA digunakan sebagai apical plug dan diikuti dengan obturasi dengan teknik warm vertical compaction. Bleaching intrakoronal dilakukan dan resin komposit digunakan untuk restorasi akhir. Setelah dua bulan follow up, prognosisnya baik dengan tidak ada perubahan warna gigi. MTA sebagai barrier di ujung saluran akar (apical plug) pada gigi dengan pulpa nekrotik dan apeks yang terbuka. Bleaching intrakoronal dengan hidrogen peroksida memberikan hasil estetika yang baik dalam waktu singkat dan tanpa efek samping. Simpulan: MTA apical plug dan bleaching intrakoronal efektif sebagai perawatan pada perubahan warna gigi non vital dengan apeks terbuka.Kata kunci: apeksifikasi, penutupan apikal, pemutihan, intra koronal, mineral trioksida agregat, apeks terbukaMineral trioxide aggregate apical plug and intracoronal bleaching on dental trauma: a case reportABSTRACTIntroduction: Trauma can cause pulp necrosis, external resorption, interference with the apical closure process, and tooth discoloration. MTA has antimicrobial activity, excellent biocompatibility, and superior sealing abilities. Internal bleaching can be used to restore tooth color after endodontic treatment. Management of open apex can be done using MTA as an apical plug. In cases of dental trauma accompanied by discoloration, intracoronal bleaching and MTA as an apical plug can be performed. This case report aims to report the management of intracoronal bleaching and MTA as an apical plug. Case report: A 46-year-old woman complained of fractured and discolored on her maxillary right central incisor. On clinical examination, tooth 11 was non responsive to vitality tests, positive to percussion test. Clinical examination of tooth 11 was unresponsive to vitality tests, positive to percussion tests. Radiographic examination showed radiolucency in the periapical area. After root canal treatment and intracanal medicament using Ca(OH)2, MTA was used as an apical plug and followed with warm vertical compaction obturation.  Intracoronal bleaching was performed and resin composite was used for the final restoration. After 2 months follow up, the prognosis was good with no reversal of tooth discoloration. MTA offers a barrier at the end of root canal (apical plug) in teeth with necrotic pulps and open apex. Intracoronal bleaching with hydrogen peroxide provides superior aesthetic results in a short period of time with no adverse effects. Conclusion: MTA apical plug and intracoronal bleaching were effective as management for discolored non-vital teeth with an open apex.Keyword: apexification, apical plug, intracoronal, bleaching, mineral trioxide aggregate, open apex.

Abstract

Teeth that have undergone endodontic treatment often lose significant structure, requiring strong and durable restorations to prevent clinical failure and support long-term function. This study aimed to evaluate the effect of post-curing time on the hardness, diametral tensile strength (DTS), and water sorption of 3D-printed resin composites. An experimental study was conducted using 80 disc-shaped resin composite specimens divided into four groups based on post-curing time duration (5 minutes, 10 minutes, no curing, and control). Hardness was measured using the Vickers Hardness test, DTS with a Universal Testing Machine, and water sorption through immersion in artificial saliva. Hypothesis testing was performed using ANOVA and Post Hoc Tamhane tests. The results showed that increasing the post-curing time significantly improved hardness but did not affect DTS and water sorption. The group with 10 minutes of post-curing time achieved the best results in hardness improvement. The findings indicate that post-curing time positively influences the mechanical and physical properties of resin composites, particularly the hardness of 3D-printed materials. This process enables better polymer densification, resulting in stronger materials resistant to degradation in the oral environment. The duration of post-curing time significantly contributes to optimizing the mechanical and physical properties of 3D-printed resin composites, making it an essential step in manufacturing restorations for the long-term success of dental restorations.

Abstract

Marginal and internal fit accuracy are critical factors for the success of indirect restorations. With advancements in Liquid Crystal Display (LCD)-based 3D printing technology, this study evaluates the effects of build angle and preparation margin design on the accuracy of 3D-printed resin composite overlays. This study aims to analyze differences in the marginal and internal fit accuracy of 3D-printed resin composite overlays based on build angle (150° and 180°) and preparation margin designs (shoulder and functional cusp bevel). This experimental study used 40 overlay samples divided into four groups based on combinations of build angle and preparation margin design. Marginal and internal fit accuracy was assessed using the silicone weight technique, and using the silicone replica technique with a digital optical microscope. Data were statistically analyzed using Two-way ANOVA and Post Hoc Tukey tests. Build angle statistically influenced both marginal and internal fit accuracy (p<0.05). The 180° angulation group showed better results compared to 150°. Preparation margin design did not significantly affect internal fit accuracy, although the functional cusp bevel design exhibited larger marginal gaps than the shoulder design in specific measurements. Conclusion: The build angle affects the marginal and internal fit accuracy for 3D-printed resin composite overlays using the LCD method and 180° yields better marginal and internal fit accuracy. Preparation margin design did not significantly affect internal fit accuracy.

Abstract

Tooth discoloration is categorized into intrinsic and extrinsic types, with the latter caused by external staining agents such as pigmented beverages (e.g., black tea). Whitening toothpastes are commonly used to address extrinsic discoloration, though chemical-based formulations may lead to side effects. Consequently, natural alternatives, including activated charcoal and bentonite, are gaining attention as whitening agents. This study aimed to evaluate the effects of activated charcoal and bentonite toothpaste on extrinsic tooth discoloration. A control group pretest-posttest design was employed, involving 48 premolar teeth divided into four groups. Samples were stained by immersion in a black tea solution (400 mL water, 4 tea bags) for 4 hours daily over 13 days, simulating 7 months of tea consumption. Color measurements were conducted using a spectrophotometer before and after brushing. Brushing was carried out using an electric toothbrush with 0.5 g of toothpaste and 1 mL of distilled water for 2 minutes, twice daily, over 10 days. Repeated ANOVA analysis revealed significant differences in ΔE, ΔL, and ΔC values (p < 0.05). Activated charcoal toothpaste produced a marked lightening effect and increased color intensity, although no change was observed at the cervical region of the teeth. These findings suggest that activated charcoal may serve as an effective natural agent for managing extrinsic discoloration while further research is needed to clarify its long-term effects and clinical applications.
