Abstract

The still high maternal mortality rate (MMR) in Indonesia, especially in East Java, is caused by indirect causes or other causes such as prolonged labor, so a method is needed to treat cases of prolonged labor such as auricular acupressure. Auricular acupressure (AKAR) is a non-pharmacological method proven effective for uterine contractions, but its side effects on the fetal heart rate (FHR) are unknown. The research aimed to analyze the AKAR method on uterine contractions, FHR, and labor duration in the first stage's active phase. This type of research is truly experimental, with a pretest-posttest control group design. The research subjects were 41 respondents, primigravida active phase first-stage mothers, selected by consecutive sampling and randomly divided into two groups, namely 21 AKAR groups and 20 deep breathing relaxation control groups. The research results of the Friedman test statistical test ρ-value equal 0.000 in the AKAR group and ρ-value equal 0.000 in the control group, meaning that AKAR influences uterine contractions. The Friedman Test findings show that AKAR does not affect FHR (ρ-value equal 0.618). The Independent Sample Test ρ-value equal 0.29 indicates no statistically significant difference between the two groups' labor lengths throughout the first stage's active phase. The AKAR group's average labor time in the first stage of the active phase was 365.40 minutes, with a minimum of 267 minutes and a high of 395 minutes. Conclusion: The auricular acupressure method affects uterine contractions but does not affect the fetal heart rate. There was no difference in the length of labor in the first active phase of labor between the auricular acupressure and deep breathing relaxation groups, so it is recommended that the auricular acupressure method can be used as an alternative complementary care to prevent prolonged labor.

Abstract

Hypertensive disorders (HD) affect up to 10 percent of pregnancies worldwide. Gestational Hypertension 17 percent will progress to preeclampsia if left untreated. Acupressure has been used for centuries to treat a variety of symptoms associated with pregnancy and is also known to lower blood pressure in hypertension. However, the use of ear acupressure as an adjunct to conventional care for hypertensive disorders in pregnancy is underrepresented in the scientific literature. This study aimed to determine the effectiveness of ear acupressure on gestational hypertension. The research method employed a pre-experimental design with a one-group pre-post design. A sample of 15 people received antenatal care and ear acupressure. Analysis using the Paired T Test. The study's results showed a difference in the average blood pressure of primigravida women in the 2nd and third trimesters, both before and after ear acupressure, of 6 mmHg systolic and 4 mmHg diastolic. The results of the Paired t-test p equal 0.00. The conclusion is that ear acupressure affects the blood pressure of primigravida during the second and third trimesters.

Abstract

Labor risk management plays an essential role in preventing complications that may endanger mothers and newborns. This study aimed to analyze the relationship between midwives’ knowledge and attitudes toward the implementation of labor risk management at Independent Midwife Practices  in the South Surabaya area. The research employed a quantitative cross-sectional design with a total sample of 25 certified “Bidan Delima” midwives. Data were collected using questionnaires and observation sheets, and analyzed using the Spearman correlation test. The results showed that 40% of midwives had moderate knowledge, 52% had good attitudes, and 44% implemented risk management effectively. There was a significant relationship between knowledge and implementation (p < 0.001; r = 0.693) as well as between attitude and implementation (p = 0.002; r = 0.582). Good knowledge and attitudes enhance the effective application of risk management. Regular training programs are needed for midwives to update their knowledge and skills in order to reduce maternal and neonatal mortality rates.

Abstract

Childbirth is a very phenomenal event for prospective mothers, where there are very memorable events in their lives, especially related to pain management and anxiety in facing the labor process. Based on a preliminary study of 5 mothers giving birth, it was found that 4 mothers who were going to give birth could not withstand the pain, so the pain became stronger. Self Healing is a way to fight negativity by saying positive affirmation words to the mother so that the mother feels better and more comfortable. This study aims to determine the Effect of Self Healing on the Level of Labor Pain in the First Active Phase of Primigravida at TPMB Naning Widayanti, S.Keb. The type of research is Quasi Experiment with the One Group Pretest-Posttest Design approach. The study population was 37 people, with a sample size of 34 people according to the inclusion criteria. The sampling technique used Acidental Sampling. Before the intervention was given, a pretest was conducted to measure the level of pain using the Numerica Rating Scale, then self-healing intervention was given with positive sentences/affirmations when pain arose for 5 minutes until the opening was 8 cm, then the level of pain was measured again in the post-test. The results of the study obtained before being given Self Healing were that some of the severe pain levels were controlled (47.0%), while after being given Self Healing, some of the mild pain levels were (50%). The results of the Wilcoxon Test p = 0.000, so it was concluded that there was an effect of self-healing on the level of labor pain during the first active phase of primigravida. Suggestions for self-healing therapy can be applied in obstetric services so that it can be used as an alternative complementary obstetric treatment
