Abstract

Background: Acute Respiratory Infection (ARI) remains a leading cause of morbidity among children under five, particularly in agrarian areas characterized by exposure to kitchen smoke, limited ventilation, and agricultural activities that potentially increase ARI risk. Community health cadres for under-five children, as the front line of community empowerment, often have limited knowledge regarding the relationship between household environment and ARI prevention. Aims: To assess the effect of Clean and Healthy Living Behavior (CHLB/PHBS) education on changes in the knowledge of child health cadres in preventing ARI among under-five children in agrarian areas. Methods: A pre-experimental study with a one-group pretest-posttest design was conducted to evaluate the impact of a 90-minute CHLB education session. The intervention combined interactive lectures, discussions, and visual media. Knowledge was measured using a validated 15-item questionnaire. Data were analyzed using the Wilcoxon Signed-Rank Test. Results: The mean knowledge score significantly increased from 6.20 ± 0.97 (pretest) to 12.97 ± 1.03 (posttest). Statistical analysis showed p = 0.000, indicating a significant difference. Before the intervention, 83.3% of cadres were in the low-knowledge category, whereas after the education, 90% achieved the high-knowledge category. Conclusion: CHLB education significantly improved the knowledge of child health cadres in preventing ARI among under-five children in agrarian areas. This approach proved to be effective and contextually relevant, with potential to serve as a model for cadre empowerment in ARI prevention.

Abstract

Pulmonary tuberculosis (TB) imposes not only physical but also substantial psychological burdens, particularly stress, which may compromise treatment adherence and outcomes. While the Health Belief Model (HBM) has been widely applied to predict TB treatment behaviors, its utility in explaining how sociodemographic factors shape stress levels remains underexplored. This cross-sectional study aimed to examine the association between sociodemographic characteristics and stress among 150 pulmonary TB patients in Jombang Regency, Indonesia, using the HBM as a theoretical lens. Stress was assessed using the Depression Anxiety Stress Scale–42 (DASS-42), with 62.7% of participants reporting moderate to severe stress. Chi-square and binary logistic regression analyses revealed that education (p = 0.012; OR = 1.89, 95% CI: 1.10–3.25), marital status (p = 0.021; OR = 1.72, 95% CI: 1.05–2.80), and occupation (p = 0.008; OR = 2.45, 95% CI: 1.30–4.60) were significantly associated with stress levels, whereas age and gender were not (p > 0.05). Occupation emerged as the strongest predictor, with unemployed or informally employed patients more than twice as likely to experience high stress. These findings underscore the role of perceived barriers and self-efficacy—core HBM constructs shaped by sociodemographic context—in influencing psychological distress. Integrating tailored psychosocial support into TB care is essential to mitigate stress and enhance treatment success.

Abstract

Introduction: Nurses’ therapeutic communication is a key element in enhancing patients’ experiences during hospitalization; however, its simultaneous impact on service satisfaction and self-care remains underexplored, particularly in regional hospitals in Indonesia. This study aims to analyze the relationship between patients’ perceptions of nurses’ therapeutic communication and their service satisfaction and self-care levels among inpatients.Methods: A cross-sectional analytical study was conducted on 110 patients at the inpatient ward of a type C hospital. Data were collected using the Nurse–Patient Communication Questionnaire (NPCQ; 15 items), a validated 7-item modified Patient Satisfaction Questionnaire (PSQ-18), and the Self-Care Ability Scale (SCAS; 12 items). Spearman’s correlation and simple linear regression analyses were performed, with statistical significance set at p < 0.05.Results: Patients reported high perceptions of therapeutic communication (mean = 48.2/60), high service satisfaction (mean = 28.6/35), and moderate to high self-care ability (mean = 36.8/48). Therapeutic communication demonstrated a strong positive correlation with service satisfaction (r = 0.68, p < 0.001) and a moderate-to-strong correlation with self-care (r = 0.59, p < 0.001). Regression analysis revealed that therapeutic communication explained 45% of the variance in satisfaction and 34% of the variance in self-care. Notably, while respect and information clarity scored highest (means = 3.6–3.8), patient involvement in care decisions was the lowest-rated dimension (mean = 2.7).Conclusion: Therapeutic communication serves as a dual-impact clinical competency that enhances both emotional satisfaction and functional self-care. These findings call on hospital administrators and nursing educators to institutionalize communication training as a core, measurable component of nursing practice, not as a soft skill, especially in resource-constrained settings.

Abstract

Background: Burnout is a critical occupational issue among nurses, affecting their well-being and the quality of patient care. It is characterized by emotional exhaustion, depersonalization, and reduced personal accomplishment. Nurses working rotating or night shifts are particularly vulnerable due to high workload and limited recovery time. Purpose: To examine burnout levels among nurses. Method: A descriptive quantitative design was used, involving nurses on morning, afternoon, and night shifts. Data were collected using the Maslach Burnout Inventory-Human Services Survey (MBI-HSS) and analyzed to determine overall burnout and its dimensions across shifts. Results: 53.3% of respondents experienced burnout, while 46.7% did not. Most nurses across all shifts exhibited moderate levels of burnout. Emotional exhaustion was the most dominant dimension, followed by depersonalization, which was higher among nurses on rotational shifts or in understaffed units. Personal accomplishment remained high for morning and afternoon shifts, but moderated for night shifts, indicating a negative effect of shift instability on perceived professional competence. Conclusion: Burnout is prevalent among nurses with varying impacts across its dimensions. Interventions that target emotional exhaustion, increase personal accomplishment, and reduce depersonalization, particularly for night and rotating shift nurses, are crucial for supporting nurse well-being and maintaining quality of care.   Keywords: Burnout Levels; Nurse; Work Shift.   Pendahuluan: Burnout merupakan masalah pekerjaan yang kritis di kalangan perawat, yang memengaruhi kesejahteraan mereka dan kualitas perawatan pasien. Burnout ditandai dengan kelelahan emosional, depersonalisasi, dan penurunan pencapaian pribadi. Perawat yang bekerja dengan sistem rotasi atau shift malam sangat rentan karena beban kerja yang tinggi dan waktu pemulihan yang terbatas. Tujuan: Untuk meneliti tingkat burnout di kalangan perawat. Metode: Desain kuantitatif deskriptif digunakan, yang melibatkan perawat pada shift pagi, siang, dan malam. Data dikumpulkan menggunakan Maslach Burnout Inventory-Human Services Survey (MBI-HSS) dan dianalisis untuk menentukan burnout secara keseluruhan dan dimensinya di seluruh shift. Hasil: Sebanyak 53.3% responden mengalami burnout, sedangkan 46.7% tidak. Sebagian besar perawat di semua shift menunjukkan burnout tingkat sedang. Kelelahan emosional merupakan dimensi yang paling dominan, diikuti oleh depersonalisasi yang lebih tinggi di antara perawat dengan sistem rotasi atau di unit yang kekurangan staf. Pencapaian pribadi tetap tinggi untuk shift pagi dan siang, tetapi moderat untuk shift malam, menunjukkan efek negatif dari ketidakstabilan shift terhadap kompetensi profesional yang dirasakan. Simpulan: Burnout lazim terjadi di kalangan perawat dengan dampak yang bervariasi pada dimensinya. Intervensi yang menargetkan kelelahan emosional, meningkatkan pencapaian pribadi, dan mengurangi depersonalisasi, terutama untuk perawat shift malam dan shift bergilir, sangat penting untuk mendukung kesejahteraan perawat dan menjaga kualitas perawatan.   Kata Kunci: Burnout; Perawat; Shift Kerja.

Abstract

Tuberkulosis (TB) paru merupakan salah satu penyakit yang membutuhkan waktu pengobatan lama sehingga rentan mengalami stres. Stres yang dialami pasien tuberkulosis paru sangat berfariatif mulai dari normal, ringan, sedang, parah, hingga sangat parah. Praktik kebersyukur (gratitude) dapat memicu perasaan positif dan mengurangi gangguan emosi yang berdampak pada kesehatan mental, seperti kecemasan, stres, bahkan depresi. Tujuan penelitian ini adalah untuk mengetahui hubungan gratitude dengan stres pada pasien TB paru di kecamatan Mojowarno kabupaten Jombang. Penelitian ini menggunakan metode kuantitatif observasional analitik dengan pendekatan cross sectional. Populasi dalam penelitian ini sejumlah 241 responden dengan sampel 150 responden dan pengambilan sampel menggunakan simple random sampling. Kuesioner Depression, Anxiety, and Stress Scale-21 (DASS-21) digunakan untuk menilai stress dan Gratitude Questionnaire-6 (GQ-6) untuk mengukur gratitude pada pasien. Metode analisis data yang digunakan adalah dengan menggunakan uji spearman rank. Hasil dari penelitian menunjukkan bahwa 58,7% responden memiliki tingkat gratitude tinggi, 39,3% sedang dan 2% rendah. Hasil analisa data stres menunjukkan 52% tidak mengalami stres, 38% stres ringan, 8,7% stres sedang dan 1,3% stres berat.  Hasil uji korelasi spearman rank diperoleh nilai p= 0,005 yang artinya terdapat hubungan antara gratitude dengan stress pada pasien tuberkulosis paru. Hal ini berimplikasi bahwa gratitude menjadi dasar bagi seorang pasien tuberkulosis paru dalam mengurangi stres. Abstract Pulmonary tuberculosis (TB) is a disease that requires a long treatment time, making it susceptible to stress. The stress experienced by pulmonary tuberculosis patients varies from normal, mild, moderate, severe, to very severe. The practice of gratitude can trigger positive feelings and reduce emotional disorders that impact mental health, such as anxiety, stress, and even depression. The aim of this research is to determine the relationship between gratitude and stress in pulmonary TB patients in Mojowarno sub-district, Jombang district. This research uses quantitative observational analytical methods with a cross sectional approach. The population in this study was 241 respondents with a sample of 150 respondents and sampling used simple random sampling. The Depression, Anxiety, and Stress Scale-21 (DASS-21) questionnaire is used to assess stress and the Gratitude Questionnaire-6 (GQ-6) to measure gratitude towards patients. The data analysis method used is the Spearman rank test. The results of the research showed that 58.7% of respondents had a high level of gratitude, 39.3% had a medium level and 2% had a low level. The results of stress data analysis showed that 52% experienced no stress, 38% had mild stress, 8.7% had moderate stress and 1.3% had severe stress. The results of the Spearman rank correlation test obtained a value of p= 0.005, which means there is a relationship between gratitude and stress in pulmonary tuberculosis patients. This has the implication that gratitude is the basis for a pulmonary tuberculosis patient in reducing stress.

Abstract

Clinical skill deficits among vocational nursing students compromise patient safety, particularly when conventional training lacks structured feedback mechanisms. This study investigated the effects of a deliberate practice intervention and self-efficacy on clinical skills performance. A quasi-experimental pretest–posttest control-group design was conducted with 60 second-semester diploma nursing students, who were allocated via matched sampling to the experimental (n=30) or control (n=30) groups. The experimental cohort completed an eight-session deliberate practice program (90 minutes per week for four weeks), while the control group received standard instruction. Blinded assessors evaluated psychomotor skills using a validated Objective Structured Clinical Examination checklist, and self-efficacy was measured with the General Self-Efficacy Scale. Intervention fidelity was rigorously maintained through protocol checklists. The experimental group demonstrated significantly superior post-intervention clinical skills scores compared to controls (mean difference = 12.47, 95% CI [8.40, 15.60], p < .001). Self-efficacy independently predicted skill performance (β = 0.29, p = .008). Combined, the intervention and self-efficacy explained 58.3% of the variance in clinical performance (adjusted R² = 0.55, p < .001). Structured deliberate practice substantially enhances clinical competency in vocational nursing students, with self-efficacy functioning as a critical complementary predictor. Nursing educators should systematically integrate deliberate practice frameworks with targeted confidence-building strategies to optimize psychomotor skill acquisition and strengthen patient safety outcomes.

Abstract

Pendahuluan: Penerapan Pola Hidup Bersih dan Sehat (PHBS) di Lapas Jombang bertujuan untuk meningkatkan kesehatan fisik dan mental warga binaan dengan mengatasi masalah kebersihan, pola makan, dan terbatasnya fasilitas kesehatan. Metode: Metode yang digunakan dalam pengabdian ini adalah pendekatan partisipatif, dengan melibatkan warga binaan dalam pelaksanaan program edukasi mengenai kebersihan tubuh dan lingkungan, pola makan sehat, serta aktivitas fisik. Hasil: Hasilnya menunjukkan peningkatan signifikan dalam kebiasaan mencuci tangan (80%), pemeliharaan kebersihan lingkungan (70%), serta partisipasi dalam olahraga (60%). Meskipun demikian, tantangan utama tetap pada keterbatasan bahan makanan bergizi yang tersedia dan fasilitas olahraga yang terbatas. Program ini juga berhasil meningkatkan kesadaran akan pentingnya kesehatan mental, dengan sebagian besar warga binaan menunjukkan perubahan positif dalam kebiasaan sehat mereka. Kesimpulan: Kesimpulannya, program PHBS di Lapas Jombang efektif dalam meningkatkan kualitas hidup warga binaan, namun perlu adanya peningkatan akses terhadap makanan bergizi, fasilitas olahraga, dan dukungan kesehatan mental untuk memastikan keberlanjutan program. Evaluasi berkala dan partisipasi warga binaan dalam pengelolaan program menjadi kunci untuk mencapai dampak yang lebih luas dan berkelanjutan.

Abstract

Acute pain is one of the most consistent complications after appendectomy and, when left poorly controlled, delays mobilization, disrupts rest, and slows recovery. Slow deep breathing (SDB) is listed among the independent, non-pharmacological interventions in the Indonesian Nursing Intervention Standards, yet detailed, session-by-session accounts of how the technique is delivered at the bedside and how patients respond across successive post-operative days remain scarce in the nursing literature. This study described the nursing care process for a post-appendectomy patient with acute pain and evaluated within-subject changes in pain intensity and physiological indicators before and after three days of SDB therapy at Bangil Regional General Hospital (RSUD Bangil), Indonesia. A quasi-experimental, one-group pre-test post-test case design, embedded in the nursing process and reported in line with single-case reporting conventions, was used. SDB was delivered once daily for three consecutive days: inhalation through the nose for four seconds, breath-holding for two seconds, and exhalation through pursed lips for eight seconds, repeated across three cycles per session. Pain was scored with the Numeric Rating Scale (NRS) before and after each session. Pain intensity declined from severe (NRS 7) at baseline to mild (NRS 3) after the third session, accompanied by a drop in pulse rate from 104 to 87 beats per minute and the resolution of grimacing, restlessness, and protective behavior. SDB is a feasible, low-cost, nurse-initiated intervention that surgical wards can integrate into routine post-appendectomy pain management protocols alongside pharmacological treatment.

Abstract

Introduction: Sleep disturbances are a common and pressing health issue among the elderly, significantly impacting their overall well-being and quality of life. Purpose: To examine the effect of Progressive Muscle Relaxation (PMR) on sleep quality in elderly individuals. A quantitative pre-test and post-test design was employed with 20 elderly participants aged 60 to 90 years. Sleep quality was assessed using the Pittsburgh Sleep Quality Index (PSQI) before and after the PMR intervention. Method: Results showed that prior to the intervention, 90% of participants experienced mild sleep disturbances and 10% had moderate disturbances. Following the intervention, 20% of participants reported good sleep quality, and none remained in the moderate disturbance category. The Wilcoxon signed-rank test revealed a significant difference between pre- and post-intervention PSQI scores (Z = -3.920, p = 0.000), indicating a significant reduction in sleep disturbances after PMR. Conclusion: These findings suggest that PMR is effective in improving sleep quality among the elderly. The study recommends further development of PMR programs as a non-pharmacological intervention to enhance sleep health in older populations across various settings.
