Abstract

Myopia is a common refractive disorder, and its prevalence continues to rise globally, including in Indonesia. In young adults over 21 years of age, the prevalence of myopia with a refractive error greater than -0.5 D reaches 48.1%. Myopia has been associated with various ocular complications, including elevated intraocular pressure (IOP), which can lead to further vision impairment. This study aims to analyze the relationship between the degree of myopia and intraocular pressure in myopic patients. A cross-sectional and consecutive sampling of myopia patients at Dr. Wahidin Sudirohusodo Hospital. Data from medical records and direct examination. Chi square correlation analysis revealed a significant relationship between the degree of myopia and intraocular pressure. In the right eye, the p-value was 0.03 (p < 0.05), while in the left eye, it was 0.01 (p < 0.05). Patient characteristics showed that the majority were female (69.6%), and most patients were young adults aged 21–40 years (79.7%). There is a significant relationship between the degree of myopia and intraocular pressure at Dr. Wahidin Sudirohusodo hospital.

Abstract

Thalasemia affects hemoglobin production. Transfusion-dependent thalassemia is a demanding clinical condition requiring lifelong care and follow-up. A 38-year-old woman is pregnant with her fourth child at 35 weeks gestation. This woman was diagnosed with Hb-E thalassemia-dependent transfusion (Hb electrophoresis showing Hb E 51.5%, Hb F 41.5%, and Hb A 3.5%). The Patient appeared pale with anemic conjunctiva, white sclera, and abdominal enlargement due to pregnancy. The Patient weighs 42 kg, and her height is 150 cm (BMI 18.67). Laboratory results were microcytic hypochromic anemia with hemoglobin 7.4 g/dL, erythrocytes 3.12 million/mL, mean cell volume (MCV) 74.0 fl, mean cell hemoglobin (MCH) 23.7 pg, mean cell hemoglobin (MCHC) 32 g/dL, and platelets with a platelet count of 133,000/µL. A cesarean method was chosen to terminate the pregnancy. There was no intrapartum and postpartum complication in this Patient. Her baby was 1.860 grams and 47 cm in length. There are no abnormalities found in the neonatal examination. Management of pregnant women with thalassemia requires special attention from preconception, during pregnancy, and toward delivery time. Multidisciplinary involvement such as hematologists and obstetricians is the primary key to the safety of mothers and children.

Abstract

Patients with beta-thalassemia major require repeated blood transfusions. Repeated blood transfusions and increased gastrointestinal iron intake can lead to iron overload in various organs, including the pituitary gland. Iron accumulation in the pituitary gland can damage gonadotroph cells and interfere with follicle-stimulating hormone (FSH) secretion, leading to hypogonadism. This study analyzes the correlation between serum ferritin and FSH in patients with beta-thalassemia major. A cross-sectional study was conducted, enrolling adult patients (≥18 years) with beta-thalassemia major undergoing transfusions at Airlangga University Hospital's Medical Hematology-Oncology Clinic. The analysis was performed using Spearman’s Rho correlation test. Out of 51 patients, 29 were female (56.9%) and 22 were male (43.1%), with a median age of 25 years. The mean serum ferritin level was 937.17 ± 210.61 µg/L, and the majority (64.7%) had serum ferritin levels <1000 µg/L. The median FSH level was 14.15 IU/L. 68.18% of male patients had FSH levels above the normal range, whereas 89.65% of female patients remained within the normal range. Spearman’s Rho analysis demonstrated no correlation between serum ferritin and FSH (r = 0.239; p = 0.092; 95% CI). In conclusion, this study found no statistically significant correlation between serum ferritin and FSH in patients with beta-thalassemia major.

Abstract

Latar Belakang: Aktivitas fisik intensitas tinggi memicu perubahan metabolisme energi yang signifikan, khususnya pergeseran dari metabolisme aerobik menuju anaerobik yang menyebabkan peningkatan produksi asam laktat. Asam laktat tidak hanya merupakan produk metabolik, tetapi juga berperan sebagai indikator fisiologis penting dalam menilai respons tubuh terhadap aktivitas fisik. Tujuan: Untuk meninjau dan mensintesis peran asam laktat sebagai respons metabolik pada aktivitas fisik intensitas tinggi. Metode: Kajian ini menggunakan pendekatan literature review terstruktur dengan penelusuran database Google Scholar, PubMed, dan ScienceDirect hingga tahun 2025. Kriteria inklusi meliputi penelitian pada manusia yang membahas hubungan antara aktivitas fisik intensitas tinggi dengan kadar asam laktat. Proses seleksi dilakukan melalui tahap identifikasi, penyaringan, dan evaluasi teks lengkap. Karena adanya heterogenitas desain penelitian, variasi protokol latihan, dan perbedaan metode pengukuran, sintesis dilakukan secara naratif. Penilaian risiko bias dilakukan secara kualitatif berdasarkan kejelasan metodologi dan konsistensi hasil. Hasil: Hasil kajian menunjukkan bahwa peningkatan intensitas aktivitas fisik berkaitan dengan peningkatan produksi asam laktat akibat dominasi metabolisme anaerobik. Kadar laktat meningkat secara signifikan ketika aktivitas melampaui ambang laktat, terutama pada latihan intensitas tinggi seperti sprint dan latihan anaerobik. Selain itu, strategi pemulihan seperti recovery aktif terbukti mempercepat eliminasi laktat, sementara adaptasi latihan jangka panjang meningkatkan efisiensi metabolisme energi dan kemampuan tubuh dalam mengelola akumulasi laktat. Keterbatasan: Heterogenitas desain penelitian, variasi protokol latihan, serta perbedaan metode pengukuran membatasi kemungkinan dilakukan sintesis kuantitatif. Kesimpulan: Asam laktat memiliki peran penting sebagai respons metabolik pada aktivitas fisik intensitas tinggi. Peningkatan intensitas aktivitas meningkatkan produksi laktat, sementara mekanisme pemulihan dan adaptasi latihan berperan dalam regulasi metabolisme energi. Temuan ini memiliki implikasi penting dalam fisiologi olahraga, perencanaan latihan, dan praktik klinis.
