Abstract

The prevalence of refractive error and its relationship with education, age and sex at DR. Wahidin Soedirohusodo general hospital have not been comprehensively assessed. The aim of this study was to examined the distribution and risk factor of refractive errors at DR. Wahidin Soedirohusodo general hospital. Methods of this study used population based cross-sectional study. Respondents were interviewed and underwent standardised clinical eye examinations. Refractive error was determined by an automatic refraction device. Refractive errors are myopia, hyperopia and astigmatism. Myopia and hyperopia were defined as spherical equivalent of -0.50/+0.50 diopter (D) or worse, respectively; astigmatism was defined as cylindrical error >0.50 D. Total of 1760 patients with refractive error from January 2016 to January 2018 with 700 or 39,8% males and 1060 or 60.2% females. Based on the cross-tabulation output, it is known that the respondents with the most elementary level of education experienced mild hypermetropy (10.2%), the junior high school education experienced mild myopia (6.8%), high school and bachelor education experienced mild myopia (8%). Age 6-15 years experienced the most myopia compositus astigmatism (5.7%), 16-25 years mild myopia (10.2%), 26-35 years mild myopia (4.5%), 36-45 years mild myopia (10.2%), 46-55 years mild hypermetropia (10.2%) and 56-65 years mild hypermetropia (5.7%). Kruskal Wallis test it is known that the sig value obtained = 0,000 indicates that there are differences in refractive abnormalities based on education and age. Chi square sig value obtained is 0.021, indicating that there is a relationship between sex and refractive errors. Multivariate analysis revealed female subjects inhibited the risk of mild myopic by 0.157 times the male subjects and simple myopia astigmatism by 0.082 times the male subjects. The relationship of myopia, astigmatism and hypermetropia with age and education is not significant. The risk factor of mild myopia and simple myopia astigmatism decrease in female than male. These findings may help clinicians to better understand the patterns of refractive error and planning for preschool vision-screening programs.

Abstract

Twin pregnancy with Hydatidiform Mole is a rare case, only about 1 in 22.000 to 100.000 pregnancies. Molar pregnancy with triplet pregnancy is even rarer, which is only 6 cases reported and mostly occurred in women who received therapy for infertility. Management in cases of twin pregnancy with hydatidiform mole is dilemmatic both for the patient and physician. We present a case of 29 years old woman with her first pregnancy of twin fetuses complicated with hydatidiform mole. Diagnostic approaches were made mainly by ultrasound examination, continued with laboratory and radiology examinations which some did not performed due to worsened vaginal bleeding. This case ended with premature labor with the result of delivery of two babies and placenta with vesicles which is a characteristic of hydatidiform mole.

Abstract

Vitiligo is a acquired skin disease characterized by a well-defined depigmented macules caused by progressive loss of functional melanocytes. Vitiligo usually appears first during childhood or young adults. Management of vitiligo in children are more difficult because the preference of therapy is limited when compared with adult. Targeted 308 nm UVB phototherapy is used for local vitiligo in children as it can be focused on the lesion and reduce the side effects. A 4-year-old girl, came to dermato-venereology polyclinic of Indera Hospital Denpasar with complaints of white patch on her lips for 3 weeks. Based on the history and physical examination, she was diagnosed with focal vitiligo. She was treated with targeted 308 nm excimer light phototherapy three times a week. Five sessions of phototeraphy resulted good response characterized by repigmentation of the lesion without redness and blisters.The therapy aims to achieve the best results with minimal side effects. Phototherapy targets are recommended for local vitiligo and especially for new small lesions as well as pediatric vitiligo. The mechanisms of this phototherapy is to trigger repigmentation, stabilization, immunosuppression and immunomodulation. The conclusion is targeted 308 nm phototherapy is quite useful for treating vitiligo even though more therapy sessions are needed.

Abstract

Premenstrual syndrome (PMS) in adolescents group is one of the most common disorder in woman at reproductive age. It substantially reduces quality of life for many women of reproductive age, with pharmaceutical treatments having limited efficacy and substantial side effects. A health lifestyle e.g. regular physical activity and high soy dietary intakes has been recommended as a method of reducing PMS symptoms. This study aims to describe the physical activity and high soy dietary intake and PMS symptoms in female students of Vocational High School 10 Surabaya. This study was designed to describe the activity level were recalled in 7x24 hour, soy dietary intake, and severity of PMS symptoms in female students that was administrated by structured questionnaires. The result showed that of all participants (n=59) were defined as having light physical activity level (88.1%), moderate level of soy dietary intake (17.22-28.38 portions per month), and mild degree of PMS symptoms (71.2%). Based on crosstabulation showed that respondences who had light physical activity experienced moderate PMS degree (8.6%), and who had high soy dietary intake experienced moderate PMS degree (8.5%). This recommendation is based on relatively little evidence, and the relationship between physical activity, soy dietary intake, and premenstrual syndrome symptoms remains unclear.

Abstract

Otitis media tuberkulosis adalah peradangan  kronik mukosa telinga tengah yang disebabkan oleh mycobacterium tuberculosis. Gejala klasik otitis media tuberkulosis adalah  perforasi multipel membrane timpani, otore tanpa nyeri dan jaringan granulasi yang banyak. Diagnosis pasti berdasarkan ditemukan BTA dari sekret telinga atau aspirasi telinga tengah dengan atau tanpa kultur mycobacterium tuberculosis serta pemeriksaan histopatologi jaringan granulasi. Tujuan penulisan ini untuk melaporkan satu kasus jarang, yaitu Otitis media tuberculosis dengan kolesteatoma. Kasus: Anak perempuan  usia 4 tahun dengan keluhan utama bengkak di depan dan belakang telinga kiri sejak 20 hari sebelum datang ke rumah sakit. Dilakukan insisi drainase abses, urgent canal wall down mastoidectomy sinistra dan kraniotomi untuk evakuasi abses. Metode penulisan dengan pencarian literatur melalui PubMed didapatkan 3 literatur yang dianalisis. Dari laporan kasus yang didapatkan, diagnosis otitis media tuberculosis dilakukan dengan pemeriksaan otoskopi, audiologi, mikrobiologi dan histopatologi, foto polos dada, CT scan tulang temporal. Kesimpulannya adalah otitis media tuberculosis merupakan kasus yang jarang dan sulit untuk dilakukan diagnose tepat. Sehingga memerlukan tingkat kewaspadaan tinggi pada gejala-gejala klinis otitis media yang disertai dengan komplikasi.

Abstract

Tuberculosis is an infectious disease caused by Mycobaterium tuberculosis which can be influenced by several factors. The physical condition of an unhealthy home environment plays an important role in the transmission and proliferation of Mycobacterium tuberculosis. The aim of the study was to analyzed the relationship between lighting levels, air humidity, and air ventilation with risk factors for positive Acid resistant bacteria pulmonary TB incidence in Jatikalang Village, Krian District, Sidoarjo Regency. The population of the cases in this study were all patients who were declared positive result of Acid resistant bacteria pulmonary tuberculosis who came to the Krian Health Center for the period January 2017 - December 2017 with a sample size of 34 respondents. The data analysis was used is the odds ratio test. The results of the analysis showed that there was a significant relationship between home ventilation and the incidence of tuberculosis with p-value = 0.006 (<0.05) and OR = 7.800 (> 1) are obtained. The result showed that poor home ventilation has a risk of 7,800 times greater to experience the incidence of pulmonary TB.

Abstract

Tuberculosis (TB) is one of the major worldwide threat and global burden in Indonesia. CNS tuberculosis is the most severe form of TB infection. CT evaluation on diagnosing tuberculosis meningitis (TBM) with triad of hydrocephalus, basal meningeal enhancement and infarction was reported to be sensitive. PCR cerebrospinal fluid (CSF) is known to be specific, however negative results have been reported due to presence of PCR?s inhibitors, poor lysis of mycobacteria, and the uneven distribution in specimens. CT Scan plays the vital role in diagnosing TBM patient with the presence of TBM CT triad, especially basal cistern enhancement (BME) which is the specific enhancement pattern in MTB patient. There are 9 must know BME pattern, such as contrast filling the cistern, double and triple line, linear enhancement at MCA cistern, Y sign, posterior infundibular recess enhancement, ill defined border, join the dots sign, nodular enhancement, and asymmetry off all pattern. Familiarizing BME criteria is essential to provide confident diagnosis and reduce morbidity and mortality

Abstract

Penelitian ini bertujuan untuk mendapatkan data lokasi, etiologi, hasil kultur kuman dan uji sensitifitas antibiotik serta komplikasi dari penderita abses leher dalam yang di rawat ruang rawat inap bedah teratai THT-KL RSUD Dr. Soetomo periode Januari sampai Desember 2014. Bahan dan metode penelitian yaitu rekam medik penderita. Hasil: Didapatkan 41 penderita. Lokasi di submandibula 13(31,70%), ruang peritonsil 11(26,82%), kombinasi ruang submandibula, parafaring dan retrofaring 11 (26,82%). Etiologi infeksi gigi 14(34,14%), infeksi tonsil akut 8(19,51%), DM 8(19,51%), DM Tipe 2 dan infeksi gigi 10(24,39%), DM disertai infeksi orofaring 1(2,43%). Dari kultur pus didapatkan Staph. Aureus (33,33%), Strep. viridians (28,75%), Kleb. Pneumonia (9,52%). Dari kultur darah didapatkan kuman Strep. viridians (40,00%), Kleb. Pneumonia (20,00%), Acinobacter humanii (20,00%), Staph. Haemolyticus (20,00%). Kultur urin didapatkan Acinobacter humanii dan Staph. haemolyticus masing-masing 50%. Hasil uji sensitifitas antibiotik dari kultur pus didapatkan angka sensitive terhadap Meropenem (87,5%), Ceftriaxone (73,68%), Cefoperazone-sulbactam (70,00%), Cefotaxime (68,75%). Resitensi kuman terhadap antibiotika tersebut diatas secara berurutan 12,50%, 21,05%, 20,00%, 12,5%. Komplikasi sepsis 12(29,26%), obstruksi jalan napas atas (trakeotomi) 6(14,63%). Kesimpulan: Lokasi tersering di submandibula disusul peritonssil, kombinasi submandibula dan parafaring atau retrofaring. Etiologi tersering infeksi gigi disusul tonsil. Penyakit penyerta tersering yaitu DM. Kuman yang sering ditemukan dari kultur pus adalah Staph. aureus dan Strep. viridians. Kuman dari kultur darah terbanyak Strep. viridians, disusul Kleb. pneumonia, Acinobacter humanii, Staph. haemolyticus. Hasil uji sensitifitas terhadap antibiotik dari kultur pus didapatkan angka sensitif tertinggi adalah Meropenem disusul Ceftriaxone, Cefoperazone-sulbactam dan Cefotaxime. Komplikasi tersering sepsis disusul obstruksi jalan nafas atas dan mediastinitis. Sebanyak 4,87% penderita meninggal.

Abstract

Daun mangrove diekstrak dengan menggunakan etanol 96% dan dipakai sebagai pencegahan peningkatan asam lambung dengan uji hewan coba Mus musculus. Tujuan penelitian untuk mengetahui potensi ekstrak daun Avicennia alba sebagai pencegahan peningkatan asam lambung karena induksi aspirin, dengan melihat kadar asam bebas, asam total, asam organik dan pH lambung Mus musculus. Metode penelitian Mus musculus dibagi dalam 5 kelompok 1). Kelompok kontrol negatif, 2). Kelompok kontrol positif, 3). 3 Kelompok perlakuan dengan dosis berbeda yaitu ekstrak daun Avicennia alba 1,3 mg/20 g BB, ekstrak daun Avicennia alba 2,6 mg/20 g BB, ekstrak daun Avicennia alba 5,2 mg/20 g BB. Hasil menunjukan bahwa Pemberian ekstrak etanol Avicennia alba P2 yang diberikan secara intra oral mencegah peningkatan kadar asam bebas, asam total, asam organik dan pH asam lambung yang diinduksi oleh aspirin yang diberikan secara intra oral. Kesimpulan penelitian adalah ekstrak etanol Avicennia alba mencegah peningkatan kadar asam bebas, asam total, asam organik dan pH lambung. Ekstrak etanol Avicennia alba 2,6 mg/ 20 g BB merupakan dosis yang dianjurkan

Abstract

Head trauma in children are frequent caused by falling, games, traffic accidents and more. Not all of head trauma continues to be brain injury. According with this, all of doctor must be able to established the diagnosis and classified head trauma to determine the optimal management and prognosis especially related with problems of brain development after head trauma. Long-term follow up should be done especially in severe and moderate head trauma to early detection any problems about development progress in this child
