Abstract

The hereditary forms of polycystic kidney disease of autosomal dominant PKD (ADPKD) and autosomal recessive PKD (ARPKD) are the main forms. ADPKD is a multifactorial disorder characterized by bilateral renal cysts and commonly affects adult patients. The most common extrarenal manifestations of ADPKD are liver cysts and is often incidental findings and clinically insignificant. A case report has been reported with polycystic disease in the kidneys and liver with autoimmune hemolytic anemia. ADPKD is a progressive disease and symptoms tend to get worse over time. ADPKD involves managing the symptoms and slowing disease progression. The most serious complication of ADPKD is end stage kidney failure. This aim of this study were to determine the symptoms and laboratory tests that confirm the diagnosis of polycystic kidney disease accompanied by autoimmune hemolytic anemia.

Abstract

The Covid-19 case is a global pandemic outbreak, resulting in the closure of the implementation of education carried out online by students as well as the work on the results of research data, based on filling out an implementation questionnaire with an average result related to effectiveness, there are 44% which shows that it is not good. Students experience fatigue by 56%, student concentration in lectures by 20%, internet use by 72%, indicating that the conversion value of the level of achievement and qualifications is sufficient, resulting in boredom and stress, as well as physical and mental fatigue. The learning process online or online is very important, supported by the need for internet access

Abstract

Background: COVID-19, caused by the SARS-COV-2 virus, is a contagious and potentially fatal disease that is commonly diagnosed using real-time reverse polymerase chain reaction (RT-PCR) assay. Studies have shown that the D-dimer level and the features of hematological exams can offer important prognostic value of the patients. This study aimed to describe data about the routine hematology features and D-Dimer levels of hospitalized COVID-19 patients in Sidoarjo Regional Hospital. Methods: This study used a retrospective, cross sectional, observational approach with descriptive analysis. Medical records of 167 COVID-19 patient were collected based on the criteria. The outcomes of the D-dimer and complete blood count of the patients were summarized. Results: According to the findings, the number of the patients with COVID-19, 53 (31.7%) were between the ages of 31 and 35, and at least 36 (21.6%) were between the ages of 17 and 30. Prominent hematology routine outcomes were the number of patients with anemia (103 or 61.7%), neutrophilia (105 or 62.9%), and lymphocytopenia (129 or 77.2%). The D-dimer value increased by 93 (55.7%) patients among 167 COVID-19 patients. Conclusion: Therefore, monitoring the patient's routine hematological presentation and D-dimer level during treatment is essential for preventing mortality and morbidity.

Abstract

Hepatocellular carcinoma (HCC) is the most common primary tumor of the liver and occupies the top 10 most cancers in Indonesia. Men are likely to develop HCC, and 90% cases of HCC develop in the background of chronic liver disease, which associates with hepatitis B and C virus infection. There are numerous treatment options for HCC, however in intermediate-advanced stage, palliative treatment such as transarterial chemoembolization (TACE) and systemic chemotherapy are the management of choice. A 51 years old male with HCC based on history, physical examination, radiology and laboratory result was referred to interventional radiology department for TACE consideration. Barcelona Clinic Liver Cancer (BCLC) B was concluded in this patient after thorough evaluation. There was history of hepatitis B infection, ascites, low albumin level, high AFP, and from Abdominal CT Scan with contrast showed large lobulated mass on the right lobe, without evidence of portal vein thrombosis. TACE was eligible and done using mixed of doxorubicin 50 mg and lipiodol, and embolization using gel foam. After 8 weeks CT evaluation was done and showed increase area of tumor necrosis, while tumor size was stable. Necrosis area expansion is the standard used for evaluating this patient which fell into partial response criteria. TACE is a palliative therapy of choice in accordance with BCLC B recommendation. Evaluation of TACE performance is more optimal by assessing the size of necrosis area compared to mass measurement.

Abstract

The objective was to investigate nitrite contents of edible bird’s nest product for human consumption. The investigated edible bird’s nest included 19 samples from multiple lots of commercially local products. Nitrite concentrations were determined by spectrophotometry. Nitrite was detected in low concentration (10.752±1.515 ppm). Detection of adulteration which is residue of hydrogen peroxide was conducted in the present study. Hydrogen peroxide was detected by rapid test which specific for hydrogen peroxide. From 19 sample tested, there was no residue of hydrogen peroxide contained in the products. These results provide new information for evaluating nitrite and hydrogen peroxide in local edible bird’s nest products regarding potential public health consequences.

Abstract

Background: Hemorrhoids are the most common anorectal disease, affecting millions worldwide. Hemorrhoids are categorized as "internal" or "external" based on their proximity to the pectinate line, which separates the upper 2/3 and lower 1/3 of the anus. The hemorrhoidal disease can be treated in various ways, from non-invasive methods like rubber band ligation, cryotherapy, sclerotherapy, laser photocoagulation, and phototherapy to invasive methods like surgical excision for patients with severe symptoms, grade III or IV hemorrhoids, or persistent bleeding. This study described the results of Laser Hemorrhoidoplasty (LHP) using reusable bare fiber combined mucopexy, a diode laser with wavelengths of 980 nm and 1470 nm, specifically gallium aluminum arsenide (GaAIAs). Methods: a case of a 28-year-old male with complaints of rectal prolapse mass one day ago and could not be reinserted with a finger, bleeding while passing stool, and rectal pain with a Visual analog scale was 7. The patient had experienced similar symptoms and had a sclerobanding procedure four years ago. However, it has reappeared in the past year. The patient was diagnosed with grade 4 internal hemorrhoids and underwent Conclusion: Laser hemorrhoidoplasty using reusable bare fiber with combined mucopexy. After the laser hemorrhoidoplasty procedure on day 4, the patient felt no pain, rectal bleeding, rectal lumps, or soiling.

Abstract

Giant Condyloma Acuminata (GCA) is a sexually transmitted infection caused by Human papillomavirus (HPV) types 6 and 11. GCA lesions can growth up to 10 cm and form like a cauliflower shape. This infection can affect patient’s quality of life. The goal of therapy in GCA is to remove all skin lesions, which can be done by surgical treatment. Many modalities have been tested in GCA cases, one of which is electrosurgery. The aim of this study was to evaluate effectiveness of electrosurgery for GCA. A 43-year-old woman came with complaints of warts appeared in the genital area since 3 months before. Examination showed multiple papules with a verrucous surface, size around 7 cm in the labia majora, labia minor and perineum. The acetowhite examination and histopathology lead to the diagnosis of condyloma acuminata. Electrosurgery (electrodesication) to remove GCA lesions divided into 2 times. Post-operative evaluation, patient did not complain of pain and erythema, edema, and pus. Electrosurgery is a therapeutic modality that can be chosen in cases of GCA. Electrosurgery, like other surgical modalities, is capable of removing the entire lesion, but the advantages of electrosurgery include not injuring large areas, minimal bleeding and a lower recurrence rate when compared to other therapeutic modalities.

Abstract

At the end of January 2022 as many as 33 countries with new cases of 85% new variant called omicron which is the concern of the World Health Organization. Patients who have been diagnosed with COVID-19 are analyzed for temperature starting from the appearance of fever and until they experience recovery, followed by a decrease in body temperature and loss of fever. This study aims to conduct a mathematical simulation analysis of the observation of the body temperature of covid 19 patients. The research method is descriptive analysis by analyzing using mathematical simulations on the fever of covid 19 patients until the temperature changes to normal again. In February - March 2022, this research was carried out at the general practitioner clinic in Surabaya, Putat Gede, Sukomanunggal sub-district, with positive results from a swab examination and an infrared thermometer. The study population was patients with positive Covid-19 swab results, with a large study sample of 30 Covid-19 patients. The results showed that there was a mathematical simulation with a fever variation of 38 C. – 39.5 C occurs in covid 19 patients. The temperature is decreasing day by day approaching the normal body temperature according to the fever that is gone and healing occurs in Covid 19 patients.

Abstract

Background: Anemia and leukocytosis were found to be associated with conditions that higher the risk for the formation of atherosclerotic plaque which will develop into cardiovascular disease. Basic laboratory parameters such as anemia and leukocytosis are not routinely used to assess Coronary Artery Disease (CAD) severity based on its stenosis. This study aimed to analyze the role of simple laboratory examiantion anemia and leukocytosis as predictors of the severity of stenosis in CAD. Methods: Cross-sectional approach was used in this study. Secondary data on CAD patients treated at Sultan Agung Islamic Hospital Semarang in 2022 were analyzed. Anemia and leukocytosis are the main risk factors analyzed in this study. Other risk factors such as gender, age, smoking status, hypertension and diabetes mellitus (DM) were also included in the analysis of this study. We performed bivariate analysis and multivariate analysis to see the relationship between these risk factors and the CAD stenosis severity. Results: Bivariate analysis with chi-square showed that anemia had a significant relationship with CAD severity (P<0.05). Multivariate analysis between anemia and smoking status reported that anemia was the most significant risk factor for the CAD stenosis severity (P<0.05). Conclusion: No association found between leukocytosis and CAD severity.
