Abstract

Background: Hypertension is a cardiovascular disease that is common to find in elderly. Its prevalence reached to 22% throughout the world in 2020. Pharmacological treatment to hypertension involves lifetime commitment to drug adherence. Ginger extract as complementary therapy has been widely common in Indonesia to reduce common flu symptoms and indigestion. It acts as a therapeutic effect that boosts immune system, improving the cardiovascular system by reducing the level of low-density lipoproteins and cholesterol. Prior studies showed its effect in lowering blood pressure in hypertension patients, while several other said the effect was stronger in less than 50 years old people. The aim of the study was to analyze red ginger extract as main therapy in hypertension in elderly. Methods: This study was a quasi-experimental with one group pretest-posttest without control group. Participants were 45-65 years old hypertension patients in Semper Barat II Primary Health Care. Intervention was 4 grams of minced ginger then added by 2 spoons of honey in 100 ml of water. The drink was consumed in 5 days. Results: Average age of participants were 62,9 (60-65) years old. Before intervention, mean blood pressure was 156,8 (140-180) / 98,03 (85-115) mmHg and lowered to 148,6 (125-170) / 92,57 (80-110) mmHg after 5 days of intervention (p=0,0001). Conclusion: We recommend ginger extract as an alternative to reduce blood pressure in elderly with hypertension. We hoped the results will become a new approach to treat hypertension.

Abstract

Background: Trained and continuous physical activity can increase estrogen levels in women, while irregular activity can increase excessive Estrogen levels. Submaximal exercise is a recommended exercise to improve a woman's physiological function The purpose of this experiment was to determine the effect of submaximal exercise on trained and untrained intensity on Estrogen levels in female white rats (Rattus norvegicus) Wistar strains. Methods: This research was an experimental study using a post-test-only control group design using 30 female Wistar rats which were divided into 5 groups: control group not treated (K1), experimental group of animals with exercise intensity without intervention (K2), group of experimental animals with untrained intensity without intervention (K3), group of experimental animals with exercise intensity with intervention (K4), and experimental group of animals with exercise intensity with intervention (K4). experimental animals with untrained intensity treatment with intervention (K5). The intervention was swimming at moderate intensity with a duration of 85% of the maximum time to swim and a load of 8% of the rat's body weight. After the treatment period, all groups were terminated to take intracardiac blood measured using ELISA. Results: The data results were analyzed using statistical tests. The results of this study found that the trained group had lower Estrogen levels compared to the untrained group, but higher than the control group. The Kruskal-Wallis test show a significance value (p = 0.157), p> 0.05. Conclusion: In this study, there was no effect of submaximal exercise between trained and untrained intensity on Estrogen levels in Wistar strain female white rat.

Abstract

Diet vegan belakangan ini menjadi semakin populer di Eropa dan negara lainnya termasuk Indonesia. Pola seperti itu bila dijalankan oleh ibu hamil dan menyusui berpotensi menimbulkan kekurangan nutrisi dan menyebabkan gangguan kesehatan tulang anak. Review ini bertujuan untuk mengetahui risiko diet vegan pada ibu hamil dan ibu menyusui. Hasil review memperlihatkan bahwa diet vegan pada ibu hamil dan menyusui dapat menganggu kesehatan tulang anak. Kesimpulan review adalah rakitis nutrisi dapat terjadi akibat diet vegan pada ibu hamil dan menyusui. Sehubungan dengan hal diatas perlu dilakukan edukasi pada ibu hamil dan menyusui yang melakukan diet vegan untuk mencegah timbulnya masalah tersebut. Review ini membahas efek diet vegan terhadap kesehatan tulang anak, pencegahan dan tatalaksananya.

Abstract

Background: Air pollution due to motor vehicle emissions causes several serious health problems including respiratory problems. Fumes exposure activated alveolar macrophages and epithelial cells release mediators such as LTB4, TNF-α, and IL-8. TNF-α are known induced acute inflammation and tissue damage. Oats are plant with high dietary fiber content and nutritional value. Oats are contain of antioxidant compounds source such as tocopherols and avenanthramides. Therefore, this research aims to determine the effect of oatmeal on TNF-α of white rats exposed to motor vehicle fumes. Methods: This research used true experiment design with Randomized Post-Test Only Control Group design using 16 white male rats divided into three groups: control group only was given standard fed (K1), second group was given standard fed and exposed with motor vehicle fumes (K2), third group was given standard fed mixed with 10 gr of oatmeal and exposed with motor vehicle fumes (K3) for ten days. After ten days of the treatment, blood of white rats from all groups will be taken intracardial then measured TNF- α levels. Results: Statistical test analyzed using SPSS Software 23. Oatmeal has an effect to decrease TNF-α levels (P<0.001). Conclusion: This study conclude that oatmeal can help reduce TNF-α levels when exposed to motor vehicle fumes.

Abstract

Background: Fatigue on the face can affect the brightness of the face, if the facial muscles function well, the health of the face will be immediately visible. There are many facial care methods currently offered, which can be done with facial acupuncture and acupuncture. This study aims to determine the effectiveness of facial acupressure and acupuncture on the facial muscles, which has never been done before. Methods: Analytical experimental research was divided into two groups, the first group was given facial acupuncture treatment and the second group was given facial acupuncture treatment. The data obtained in the form of facial muscle relaxation is indicated by the disappearance of wrinkles on the face with a score of (1, 2, 3). The dependent variables are facial acupuncture and facial acupuncture. Inclusion criteria were women aged 40 – 50 years, agreed to take part in this study, and had no facial abnormalities. The exclusion criteria are women aged less than 40 years or over 50 years, and there are defects in the facial area. This research will be carried out in May – June 2023 at the Wijaya Surabaya doctor's practice and polyclinic. Results: The results of the study found that 20 minutes of facial acupuncture was useful for relaxing the facial muscles by up to 60%, on the sixth day the face looked brighter and cleaner, while acupuncture therapy on the facial muscles experienced relaxation of up to 60% on the sixth day with 30 minutes. The effectiveness of facial acupressure therapy is 10 minutes faster than acupuncture. Conclusion: Facial acupressure therapy and acupuncture therapy have proven to be effective in relaxing the facial muscles.

Abstract

Background: The study aims to map TB (Tuberculosis) – HIV (Human Immunodeficiency Virus) patients based on clinical, radiological, and laboratory patterns. Objective: This study is expected to provide previous research investigating radiologic and laboratory examinations in TB-HIV patients. Method: We conducted a cross-sectional study based on medical records of TB-HIV patients at RSPI Sulianti Saroso from January 2004 - March 2017. Samples were 240 TB-HIV patients from a population of 1014 People Living with HIV/AIDS (PLWHA). Inclusion criteria were PLWHA, aged over 18 years, diagnosed with TB with completed medical record status. Result: Characteristics of the majority of patients were in the productive age group (99.2%) with an average age of 33 years and male sex (78%). The main symptom is cough (91.7%) and cough with phlegm (59.1%). Most patients (83.37%) have not received ART (Anti-Retroviral Therapy). Pulmonary physical examination showed that most patients had Ronchi (95.8%) while others got wheezing (5%). Laboratory tests revealed an initial CD4 cell count of 71.6 cells/ml and a negative Acid-Fast Bacillus (AFB) smear (59.2%). The patient's radiology was mainly with an infiltrate (82.5%); most infiltrate was presented in apex (52.4%). Conclusion:  The majority of TB-HIV coinfected patients are young males, presenting with common TB symptoms, low CD4 counts, and negative AFB smears, complicating diagnosis. Radiological findings frequently show infiltrates, particularly in the apex. These findings emphasize the importance of early diagnosis and ART initiation to improve outcomes in TB-HIV coinfected patients

Abstract

Background: Gout arthritis is caused by the deposition of monosodium urate crystals in the joints, which can cause significant pain, swelling, and recurring inflammation, particularly in the big toe. Inflammatory mediators are critical to the beginning and duration of the inflammatory response in gout arthritis. Gout is mediated by TNF-α, ILs, and the NLRP3 inflammasome, which promotes macrophages, monocytes, and neutrophils, leading to inflammatory reactions. TNF-α acts as a potent inflammatory mediator by stimulating the expression of other inflammatory cytokines, adhesion molecules, and matrix metalloproteinases (MMPs). TNF-α interacts with particular receptors and activates intracellular signaling pathways, leading to leukocyte recruitment, NLRP3 inflammasome activation, and the release of other inflammatory mediators. Objective: This article is a review article in which the researchers examine the current state of understanding regarding the role of TNF-α as a major inflammatory mediator in gout arthritis, deepen the understanding of TNF-α in gout inflammation, and explore its potential as a therapeutic target. Method: It involvedsystematically searching relevant databases, selecting studies based on inclusion criteria, and synthesizing findings to identify trends, gaps, and the current state of knowledge on the topic. Result: The results of this review article indicated that TNF-α plays a crucial role in joint inflammation by triggering the release of pro-inflammatory cytokines and stimulating neutrophil infiltration into the joints. Conclusion: Thus, Inhibiting TNF-α can reduce inflammation and improve symptoms in people with gout arthritis.

Abstract

Background: Reduced metabolic function can cause the human body to experience a lack of energy, resulting in the possibility of diseases including diabetes mellitus, hypertension, atherosclerosis, and even coronary heart disease. Objective: This study aims to analyze the incidence of obesity through Body Mass Index on the incidence of diabetes mellitus and Lipid Profile at SMA X Surabaya. Methods: The research design used was cross-sectional. Sampling was carried out using non-probability sampling with a random sampling technique. Data collection was carried out at SMA X Surabaya by 64 teachers and employees of SMA X Surabaya. Respondents were gathered to receive health education, then went through the registration stages, and vital signs, and then 5 mL of venous blood was taken to measure blood sugar levels and lipid profiles, namely total cholesterol and triglyceride levels. This research uses the Chi-Square test statistical test. Result: There is a significant relationship based on the chi square test between body mass index and cholesterol levels and triglyceride levels. because the p-value < 0.05 is 0.041 and 0.050 respectively, but there is no significant relationship between body mass index and glucose levels with a p-value of 0.181. Conclusion: There is a relationship between the incidence of obesity through Body Mass Index and the incidence of Lipid Profile but there is no relationship with the incidence of diabetes mellitus at SMA X Surabaya

Abstract

Background: One well-known hypercoagulable state that raises the risk of venous thromboembolism is pregnancy and it is more common in cesarean delivery. Pulmonary embolisms are difficult to identify based solely on postpartum symptoms. Most of the symptoms are non-specific, therefore they are typically underdiagnosed throughout pregnancy and the postpartum period. Objective: This work presented information on the treatment of high-risk pulmonary embolism in the postpartum phase. Case presentation: This paper reported the case of a 33-year-old female, admitted to the emergency room for shortness of breath. Her personal history showed that she had just given birth to her sixth child by caesarian section a month ago and had a history of hysterectomy due to severe infection. Result: She suffered a high-risk pulmonary embolism in the post-partum period. It takes a strong index of suspicion to diagnose and treat patients in a timely manner. Conclusion: Echocardiography and Multislice Computerized Tomography are the best diagnostic methods. Venous thromboembolism prophylaxis should be taken into account, particularly in patients who have risk factors.
