Abstract

One plant that can be used as an alternative treatment for hyperuricemia is the cherry plant. The usefulness of these plants is due to the secondary metabolite compounds that can inhibit the enzyme xanthine oxidase. Cherry mistletoe is thought to have the same flavonoids, saponins, alkaloids, terpenoids, and tannins as the cherry plant, which can act as inhibitors. The aim of the study was to determine the inhibitory effect of an ethanol extract of cherry mistletoe leaves on the xanthine oxidase enzyme. This study was conducted as  an in vitro experimental study. Cherry mistletoe leaves were extracted with 96% ethanol. The extract was done with phytochemical screening, followed by an inhibition test on the xanthine oxidase enzyme using a spectrophotometer at a wavelength of 400 nm. Allopurinol was a positive control. The ethanol extract of cherry mistletoe leaves contains flavonoids, saponins, alkaloids, terpenoids, and tannins. The IC50 value of the ethanol extract of cherry mistletoe leaves was 23.44 mg/L. The ethanol extract of cherry mistletoe leaves has an inhibitory effect on the xanthine oxidase enzyme with moderate activity. Accordingly, the cherry mistletoe leaf extract can be considered as an alternative inhibitor of the xanthine oxidase enzyme in the control of hyperuricemia.

Abstract

Hypertension remains a critical health challenge worldwide, with uncontrolled hypertension being a significant contributor to cardiovascular morbidity and mortality. Elderly patients often face difficulties in hypertension management due to limited knowledge, adherence barriers, and lack of education. This study aimed to evaluate the impact of an mHealth-assisted patient education intervention on knowledge and attitude among elderly patients with uncontrolled hypertension. A pre-experimental one-group pre-test and post-test design was conducted among 39 patients aged 46–55 years (48%) and 56-65 years (52%). Patients accessed an mHealth application featuring interactive hypertension management modules. Knowledge and attitude were assessed using validated questionnaires pre- and post-intervention. Results showed a significant improvement in both knowledge and attitude scores post-intervention (p 0.05). The mHealth-based approach effectively increased patient understanding and encouraged a positive attitude towards hypertension management. The study highlights the potential of mHealth applications in enhancing hypertension knowledge and attitude among elderly patients, emphasizing the need for integrating digital tools in routine hypertension care.

Abstract

The outpatient pharmacy department is the last service unit visited by patients. One common concern is the delay in receiving medicine. Improvements in medicine dispensing time were made with a lean hospital. This study aims to analyze the impact of lean hospital interventions on waiting times and Value Added Ratio (VAR). This quasi-experiment was conducted with a non-equivalent control group before-after design. The study population included non-compounded medicine prescriptions with out-of-pocket healthcare from April to July 2024. The sample was 1,266 prescriptions. Direct observation gathered data for value stream mapping in non-compounded medicine dispensing, recording time for prescription screening, preparation, final checks, and delivery, including patient education. Activity time was considered value-added time. Value-added time divided by Cycle Time (CT) was used to calculate VAR. Critical waste identified by the root-cause analysis was conducted through in-depth interviews using the fishbone diagram guide. Data before and after intervention were analyzed using the Mann-Whitney U-test. Inventory waste significantly affected dispensing times. The lean hospital strategy improved the efficiency of pharmaceutical services by reducing the dispensing time from 19.68 to 16.18 minutes (p 0.001) and increasing the VAR from 51.34% to 57.21% (p 0.001) with an automated medicine planning system.

Abstract

Alzheimer's disease (AD) has been recognized as a significant issue affecting population health globally and tended to increase over the years. The utilization of natural products for AD treatments has been widely studied, which possibly offers better outcomes with minimum side effects. Coffee consumption has been subjected as a lifestyle propensity, which offers beneficial advantages including reducing the risk of AD. Bioactive natural compounds contained in coffee such as caffeine and caffeic acid have been experimentally proven to be acetylcholinesterase (AChE) inhibitors, a pivotal target enzyme for AD treatments. This research aimed to explore the dynamics interactions of caffeine and caffeic acid in the AChE active site using the in silico approach. In this study, 100 redocking and docking simulations were implemented before the molecular dynamics (MD) simulations. The 55-ns MD simulations of huprine X, caffeine, and caffeic acid were implemented to study the dynamics interactions. Conformational stability, free energies of binding, and interaction hotspots were identified during the simulations. Our findings informed that caffeine interacted in the active site during the simulations, revealing the importance of the imidazole ring in maintaining the interactions. In contrast, caffeic acid interacted longer in the plausible allosteric site, forming ionic, hydrogen bonds, and aromatic interactions.

Abstract

Tablet is a popular pharmaceutical dosage form among the public. One method of making tablets is direct compression. This method must consider the flow properties and compressibility of the excipients. But many excipients do not meet these criteria, leading to the alternative use of co-processed excipients of jackfruit seed starch and avicel®️ PH 101. This research aims to determine the physical characteristics of direct compression vitamin C tablets using co-processed excipient of PBN-A (Jackfruit Seed Starch and Avicel®️ PH 101) with varying ratios of 1:3, 3:1, and 1:1. The research methods include powder preparation, powder evaluation, tablet compression, and physical evaluation of tablet. Descriptive and statistical data analysis using one-way ANOVA. The research results show that tablets exhibit favorable physical characteristics in F1, but F2 and F3 show suboptimal hardness and friability values. The statistical analysis results indicate a significant difference with a p-value of (0.000) and post-hoc tests explain that these significant differences are particularly noticeable between F1 and F2, K1, and K2. This study concludes that vitamin C tablets using co-processed excipients of PBN-A with ratios of 1:3, 3:1, and 1:1 exhibit good physical characteristics in all evaluations, except for the hardness and friability of F2 (3:1) and the hardness of F3 (1:1).

Abstract

An increase in the ultraviolet (UV) index poses severe health risks by inducing harmful effects on the skin, such as sunburn, erythema, skin darkening, premature ageing, and skin cancer. These risks are mitigated through sunscreens with high Sun Protection Factor (SPF) values (15). Natural sources of high SPF include patchouli leaf essential oil, which contains flavonoids. Flavonoid compounds with double bonds can absorb UV-A and UV-B rays, protecting against their harmful effects. The purpose of this study was to find the best formula and observe its impact on the physical characteristics of the preparation and SPF activity. Formula optimization used Design Expert software 13 with the simplex lattice design method. The optimized variables were TEA and stearic acid as emulsifying agents. The results of the optimum formula composition consisted of 4% TEA and 5% stearic acid. The optimum skin protection lotion preparation has a yellow color, a distinctive smell of patchouli oil, a semi-solid shape and a thick texture. The pH parameters of 7.7±0.09, adhesion of 2.34±0.06 seconds, spreadability of 6.85±0.02 cm and viscosity of 4503.3±219.39 Cp. Patchouli leaf essential oil and lotion have ultra-category SPF values with oil SPF value of 27.86±1.07 and lotion preparation of 27.65±1.75 preparation is also homogeneous with pH parameters of 7.7±0.09, adhesion of 2.34±0.06 seconds, spreadability of 6.85±0.02 cm and viscosity of 4503.3±219.39 Cp. Patchouli leaf essential oil and lotion have ultra-category SPF values with oil SPF value of 27.86±1.07 and lotion preparation of 27.65±1.75.

Abstract

Irrational use of antibiotics is currently still a major health problem in the world. Worldwide around 50% of medicines are sold, distributed, and given to patients inappropriately. Irrational use of antibiotics can trigger antibiotic resistance. Knowledge and behavior regarding antibiotic stewardship are important in preventing antibiotic resistance. This cross-sectional research was conducted to determine the relationship between knowledge and behavior regarding antibiotic stewardship among healthcare workers. This study's 58 respondents were general practitioners/dentists, specialist doctors/dentists, and pharmacists. The instrument used in this study was a valid and reliable questionnaire. The data analysis used was univariate analysis and Spearman analysis with IBM SPSS Statistics 26 and significance set as p0.05. In this study, the results showed that the majority of respondents worked as specialist doctors/dentists (53.44%), aged in the range 26-35 years (36.20%), of the type female (58.62%), have work experience for more than 10 years (39.65%) and all of the surveyed respondents have good knowledge and behavior (100%). Spearman’s rank analysis showed no significant relationship between knowledge and antibiotic stewardship behavior (p0.05). The high level of expertise of respondents does not significantly affect the level of behavior because it can influence several factors, both internal and external. Further study is needed to identify other factors influencing antibiotic stewardship and rational use of medicine.

Abstract

Medication adherence among patients with type 2 diabetes mellitus (T2DM) continues to be a difficult challenge. Providing drug information (PDI) at health centers can enhance medication adherence and help patients achieve glycemic control. A cross-sectional study was conducted in February-April 2024 at the North Denpasar Health Center. Inclusion criteria were patients 18 years old either with or without comorbidities, receiving the same antidiabetic treatment for ≥3 months. Exclusion criteria were patients who withdrew from the study, changed their care, or had unstable health conditions. Medication adherence data were collected using the Probabilistic Medication Adherence Scale (ProMAS) questionnaire, PDI data and fasting blood glucose (FBS) examination results in medical records using data collection documents. Correlation analysis between PDI and medication adherence and glycemic control used the Kendall's tau-b/c test. Results showed patients were mostly male (53.84%), age ≥65 (43.20%), elementary school education (35.50%), unemployed (63.90%), no family history (70.41%), disease duration 5 years (62.72%), with comorbidities (65.09%), and FBS achieved (81.66%). The antidiabetic used was a combination of metformin and glimepiride (54.43%). A total of 81.65% obtained routine PDI with high medication adherence (48.52%). The conclusion of the study was that there was a significant relationship between PDI with medication adherence (p=0.000; r=0.253) and glycemic control (p=0.000; r=0.447).

Abstract

Anemia is a significant health concern, especially among adolescent females in Indonesia. Menstruation and poor nutrition are major contributors to anemia, since menstruation leads to red blood cell loss and poor nutrition hinders production. This study examines the relationship between menstrual characteristics, nutritional status, and anemia among high school female students in Muara Siberut Village, Mentawai Islands. This cross-sectional observational study involved 97 female high school students. We assessed nutritional status using BMI percentiles and categorized menstrual characteristics according to standard gynecological classifications. Anemia was diagnosed via hemoglobin measurements using a digital hemoglobinometer. Statistical analyses (p0.05) with odds ratio (OR) and 95% confidence interval (CI) utilized Chi-square or Fisher’s exact tests to explore associations between menstrual characteristics, nutritional status, and anemia. No significant correlation was observed between nutritional status and anemia (p=0.394). Conversely, a significant correlation was found between the menstrual cycle and anemia (p=0.023) as well as between menstrual length and anemia (p=0.017). Furthermore, a subgroup analysis revealed individuals with a normal menstrual cycle were at a higher risk of experiencing anemia compared to those with oligomenorrhea (OR=4, 95%CI=0.988–16.199). Additionally, the results indicated that individuals with normal menstrual length were at a higher risk of experiencing anemia compared to those with hypomenorrhea (OR=4.624, 95%CI=1.174–18.202). This study provides valuable insights into the predictors of anemia among female adolescents, emphasizing the need for targeted health education and interventions.

Abstract

Flu is one of the diseases that can be treated with self-medications, and information about these drugs is often obtained through advertisements on television, which will influence self-medication behavior. The purpose of this study was to analyze the relationship between perceptions of cold medicine advertisements and self-medication behavior using cold medicines. The method used in this study was an analytic survey with a cross-sectional research design and the sampling technique used was quota sampling. The sample of this study was 94 respondents with the majority of sample aged 17-27 years, female, high school/vocational high school graduates, who work as housewives. The results showed that respondents' perceptions of drug advertisements in self-medications were 10.64 % of respondents were affected and 89. 36 % of respondents were not affected. The self-medication behavior done by the respondents was 65.96% rational and 34.04% irrational. The results of the chi-square analysis showed that there was no significant relationship between perceptions of cold medicine advertisements and self-medication behavior using cold medicines (p value = 0.225). The results of the evaluation and the completeness of advertisements for cold medicine on television showed that some advertisements for cold medicines broadcast on television were still incomplete and inappropriate. The importance of rational use of medications should be a guide in all self-medication for the safety and maintenance of public health.
