Abstract

Background: Indonesia started establishing national health insurance in 2014. Implementation of Indonesian national health insurance has frequent gaps between health expenditures based on hospital tariffs and the INA-CBG tariffObjectives: This review was aimed to compare the hospital tariff of healthcare to the INA-CBGs tariff and to identify the factors contributing to the actual cost. Methods: Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) methodology followed in this research. The keywords "Real Cost and INA CBG'S" were used in Google Scholar, and the keywords "INA CBG'S AND REAL COST" on Pubmed, ScienceDirect, and Scopus between 2014 and 2022.Results: A total of 634 records were found through database searching of Google Scholar (594), Pubmed (1), ScienceDirect (36), and Scopus (3). After the screening process, 24 articles were included in the review. It was found that 13 articles (54.16%) showed a lower hospital tariff than INA CBG's tariff, while 11 articles (45.84%) showed a higher hospital tariff than INA CBG's tariff. The main factors affecting the gaps of hospital tariff versus INA CBGs tariff mentioned in the articles were LOS, severity, and hospitality class. Conclusion: The literature of this review found hospital rates are higher with rates of INA-CBGs in many hospitals, so it requires a review of rates and an efficient strategy of hospitals.

Abstract

Background: Chronic Suppurative Otitis Media (CSOM) is a chronic inflammatory and infectious disease of the middle ear or mastoid cavity that can result in the loss of bodily functions or even death. The higher concentration of CSOM is found in developing nations such as Indonesia, where the population cannot afford treatment. In cases of persistent otorrhea following 3 weeks of treatment with topical ear antibiotics, oral antibiotics may be considered as a second line of treatment. Topical ear antibiotics are insufficiently available at healthcare facilities because they are not included in the Indonesian National List of Essential Medicines (NLEM).Objectives: The objective of this study was to compare the cost-effectiveness of topical ear antibiotics versus oral antibiotics in the treatment of chronic suppurative otitis media (CSOM).Methods: This study was a cross-sectional study using retrospective data from medical records and cost assessments at a rural hospital in the Gowa district of South Sulawesi. The effectiveness of the treatments was evaluated based on the frequency of visits to the ENT department, with antibiotic prescriptions and positive otorrhea. Results: The findings revealed that the Average Cost-Effectiveness Ratio (ACER) for topical ear antibiotics was Rp 161,020, while the ACER for oral antibiotics was Rp 286,968. The Incremental Cost-Effectiveness Ratio (ICER) was determined to be Rp 351, and the Incremental Net Benefit (INB) was positive (Rp 99,857) at threshold Rp 434,500.Conclusion: Our study found that topical ear antibiotics were more cost-effective than oral antibiotics when considering the INB values based on INA-CBGs, along with fluctuations in cost-effectiveness using sensitivity analysis.

Abstract

Background: Breast cancer patients receiving oral chemotherapy must strictly adhere to their treatment regimen for optimal therapeutic outcomes. Various factors, including demographics, environment, social dynamics, and economic circumstances may impact adherence to the recommended treatment. Objectives: This study aims to assess patient’s treatment adherence and decisively identify the factors that affect it.Methods: This cross-sectional study was conducted at a General Hospital in Gianyar, Bali, in May-June 2024, and 70 samples were purposively selected. Patients with complete medical records and consent were recruited; those with incomplete questionnaires were excluded. An adapted questionnaire and data collection sheet were used to measure adherence and gather medical information. The responses to the adherence questionnaire have been effectively processed into a final scoring format. Higher scores clearly reflect better adherence. Then, the data analysis involved univariate tests to examine respondent characteristics and adherence, as well as bivariate tests (Spearman-rho and Mann Whitney U-Test) to analyze the relationship between respondent characteristics and their adherence (with a 95% CI).Results: The majority of respondents were <60 years old (75.71%), female (100%), had completed elementary school (28.57%), were employed (64.29%), lived in rural areas (58.57%), had been diagnosed with cancer for <4 years (100%) with stage 3 cancer (47.14%), underwent oral chemotherapy ≤3 times a month (71.43%), had no comorbidities (78.57%), used national health insurance (100%), and had an average adherence score of 88.77±3.72 (moderate of adherence). None of the respondent characteristics mentioned in the results of the respondent description were related to patient adherence (p>0.005).Conclusion: The patient's adherence to oral chemotherapy is moderate. Other factors suspected to strongly influence better adherence to treatment include consistent treatment monitoring by pharmacists, family support, knowledge, health insurance, and the patient's psychological condition.
