Abstract

Indonesia Health Ministrial Decree No. 1027/2004 has been published as a standard of quality assurance in pharmaceutical care provided by pharmacist. As the time goes by, the newest regulation has been provided by government through PP No. 51/2009 to legitimate pharmacists in their responsibility as role player in pharmaceutical care. The implementation of this policy, however, is still unknown yet and needs to be further evaluated. The study aimed to evaluate the pharmaceutical care standard implementation by pharmacist according to this regulation.  This research involved pharmacists in 32 pharmacies in Bantul as respondents. These pharmacies was selected by proportional random from the total of 108 pharmacies in 16 district of Bantul Residence. The data was collected through questionnaire and analized descriptively according to indicators; administration; drug and prescription service; communication, information, and education, inventory control management, human resources, supporting facilities, and evaluation of service quality. The result was  categorized as good if total score persentation 81-100%, adequate if total score persentation 61-80%, and less if total score persentation ≤60%. The study result showed that generally the pharmaceutical care standard has not been well implemented in Bantul Residence. There were 18 pharmacies (56.25%) in the good category, 13 pharmacies (40.63%) in the adequate category, and 1 pharmacy (3.13%) in the less category. The indicators that have been well implemented were supporting facilities indicator (88.95%), human resources indicator (887.67%), inventory control management (82.29%). Other indicators including Communiation, Information, and Education (75.63%), administration (68.23%), and drug and prescription service (64.95%) were in the less category.Keywords: Pharmaceutical Care Standard, Kepmenkes No. 1027/2004, PP No. 51/2009, Bantul Residence

Abstract

Praktek Kerja Profesi Apoteker adalah tahap penting untuk meningkatkan keterampilan pharmaceutical care mahasiswa. Tidaklah mudah bagi preseptor untuk menilai peningkatan kompetensi mahasiswa dalam waktu yang singkat, sehingga diperlukan key indicators.Penelitian ini bertujuan untuk mengetahui apa saja key indicators kompetensi pharmaceutical care pada Praktek Kerja Profesi Apoteker di Apotek berdasarkan persepsi preseptorditinjau dari tingkat kepentingan dan resiko kejadian medication error. Penelitian ini adalah penelitian deskriptif ekploratif menggunakan rancangan cross sectional observation menggunakan kuessioner. Subyek penelitian adalah seluruh preseptor PKPA di Daerah Istimewa Yogyakarta sebanyak 22 preseptor, dengan kriteria inklusi: mendapatkan rekomendasi dari organisasi profesi dan bersedia menjadi responden. Kriteria eksklusi adalah preseptor yang mengundurkan diri ketika penelitian berlangsung. Indikator kunci ditetapkan berdasarkan nilai mean rank tertinggi. Hasil penelitian diperoleh 11 indikator kompetensi ditinjau dari tingkat kepentingan (indikator nomor 13; 14; 19;12; 18; 1; 16; 22; 2; 15; dan 33) .dan 11 indikator kompetensi ditinjau dari tingkat resiko kejadian medication error(indikator nomor 14; 16; 12; 13; 22; 9; 15; 8; 23; 17; dan 19).Indikator hasil intersection (irisan) dari kedua kelompok indikator tersebut diperoleh 7 key indicator yaitu indikator nomor 12, 13, 14, 15, 16, 19 dan 22 sebagai key indicatorskompetensi pharmaceutical care pada PKPA di Apotek berdasarkan persepsi preseptor yaitu: menyerahkan obat disertai informasi obat, menyampaikan informasi obat dengan benar, menyiapkan obat dengan benar, melakukan peracikan obat bila diperlukan, membuat etiket obat,melakukan pelayanan informasi obatdan memberikan konseling kepada pasien.

Abstract

Inhaler is the most preferable device to deliver medication in order to treat asthma and Chronic Obstructive Pulmonary Disease (COPD). Incorrect usage of inhaler influences the clinical effectiveness of the delivered drug. A pilot study was conducted to determine the appropriateness of inhaler handling technique among asthma and COPD patients. As many as 92 subjects whom diagnosed with asthma and COPD, aged between 18 to 64 years old were observed in this cross-sectional study. Consenting subjects on inhaler, who attended medical out patients clinic at a primary health care unit in Selangor, Malaysia were asked to demonstrate on how they use the inhaler while an inhaler administration checklist were used to assess each patients inhaler technique. Data was analysed using descriptive statistical methods. Of total 92 patients, a total of 155 devices were used. Only 4 out of 92 patients correctly handles the device (4.3%). In average, a patient made approximately >4 mistakes during pMDI inhalation and >2 mistakes during DPI inhalation. The most common mistake found among pMDI, Turbuhaler and Accuhaler users were unable to hold the breath for approximately 5 seconds in which comprise of ≥80% patients. In conclusion, majority of asthma and COPD patients use their inhaler inaccurately. Patients prescribed by inhalation medications should have routine assessment of their inhaler technique at every visit and corrected if found to be poor.

Abstract

As an effort to boost the issue of family planning program, BKKBN has adopted a policy of IEC (Information Communication and Education) as a promotional effort and community mobilization to increase demand for contraceptives.This study aim was to analyze the role of mass media in influencing attitudes towards participation of family planning. Research was performed by using the Indonesian Demography and Health Survey data (“SKDI” 2007). Sampling location was national-wide which included all 15-49 years old ever-married women and lived in Indonesia. Informan was determined by purposive sampling technique. Total sample was 32,895 women. Data was analyzed by chi-square test. The results showed that 57.4% of ever-married women had a positive attitude towards family planning program and participated in th family planning program with p=0.005 (p value = 0.000). While ever-married women who were exposed to family planning messages through mass media and become participants KB was 58.3%, with p=0.005 (p value = 0.000). The relationship of mass media’s role in influencing attitudes towards family planning participation was observed with the control variables of education level, residence, motivation to have more children and the number of children (alive), each of which showed  a significant relationship with p=0.005 (p value = 0.000). Television is the medium which attract most viewers. Different model is required for the highly educated women. The emergence of other media such as religious leaders, community, doctors, midwives, and family planning cadres can be used as a vehicle for delivering messages and information. It needs a strategy of repetition (redundancing) messages and information, in order to remind people about the importance of the family planning programs. The right design in campaigning and informing the family planning program should be made after considering whether the type of media used is matched witu geographical characteristics, time, and forms of contraception.Keywords: mass media, attitudes, participation in Family Planning Program

Abstract

Prevalensi penyakit diabetes melitus (DM) di provinsi Jawa Tengah sebesar 0,8%, secara keseluruhan adalah 1,3% di Indonesia. Prevalensi untuk Surakarta sebesar 2,8% dan menduduki urutan ketiga di provinsi Jawa Tengah. Konseling dengan alat bantu mkkmkmmklebih efektif dibanding konseling biasa. Konseling dengan lembar informasi penggunaan obat dan telepon membantu dalam memberikan gambaran tentang informasi obat dan cocok untuk pasien dengan regimen terapi serta merupakan salah satu cara mengurangi tingkat kesalahan. Penelitian ini bertujuan untuk mengetahui pengaruh konseling farmasis terhadap kepatuhan, luaran klinik dan kepuasan pasien DM. Penelitian ini merupakan penelitian quasi eksperimental dengan metode pretest posttest with control group design. MMAS-8 digunakan untuk mengukur tingkat kepatuhan dan DMSAT untuk tingkat kepuasan. Pasien dikelompokkan menjadi dua kelompok, yaitu kelompok intervensi (memperoleh konseling dan alat bantu) sebanyak 73 pasien dan kelompok kontrol sebanyak 74 pasien. Penelitian dilakukan di klinik dokter keluarga BPJS Surakarta pada bulan November 2014 sampai Februari 2015. Data dianalisis menggunakan Chi square, uji Wilcoxon dan uji korelasi. Hasil penelitian menunjukkan bahwa pemberian konseling dengan alat bantu meningkatkan kepatuhan dan kepuasan serta meningkatkan ketercapaian luaran klinik dengan tercapainya target kadar gula darah puasa (GDP) dan gula darah post prandial (GDPP) dengan rata-rata penurunan kadar GDP dan GDPP kelompok intervensi sebesar 26,96±28,42 mg/dL dan 26,88±65,49 mg/dL (p<0,05). Tingkat kepatuhan yang tinggi akan meningkatkan tercapainya luaran klinik dan meningkatkan kepuasan terapi, akan tetapi kekuatan korelasinya lemah (r = 0,2-0,4).Kata kunci: konseling, alat bantu, kepatuhan, luaran klinik, kepuasan

Abstract

Persentase kejadian stroke berulang dalam 30 hari pertama setelah serangan adalah 3-10% dan risiko terjadi pada 6 bulan pertama yaitu 8,8%. Pemberian terapi antiplatelet pada pasien stroke iskemik dapat mencegah kejadian stroke berulang. Hasil penelitian terkait efektivitas pemberian terapi antiplatelet kombinasi aspirin dan klopidogrel dibandingkan dengan aspirin tunggal berbeda-beda. Penelitian ini termasuk penelitian observasional dengan metode kohort retrospektif yang dilakukan di RSUP Dr. Sardjito Yogyakarta. Kelompok pertama adalah pasien stroke iskemik yang mendapatkan terapi antiplatelet aspirin tunggal sebanyak 77 subyek. Kelompok kedua adalah pasien stroke iskemik yang mendapatkan terapi antiplatelet kombinasi aspirinklopidogrel sebanyak 70 pasien. Outcome penelitian ini yaitu kejadian stroke berulang dalam kurun waktu 6 bulan setelah kejadian stroke pertama. Data diperoleh dari data sekunder yaitu rekam medik dan follow-up kejadian stroke berulang sebagian dilakukan dengan menghubungi pasien atau keluarga pasien. Stroke berulang terjadi pada 8,6% pasien stroke yang mendapatkan terapi antiplatelet kombinasi aspirin-klopidogrel dibandingkan 13,0% pasien pada kelompok yang mendapatkan terapi antiplatelet aspirin tunggal (RR 1,22; 95%CI 0,807 – 1,850; p=0,391). Kejadian stroke berulang 6 bulan setelah serangan stroke iskemik pertama di RSUP Dr. Sardjito Yogyakarta, antara pasien yang mendapatkan terapi antiplatelet kombinasi aspirin-klopidogrel dan antiplatelet aspirin tunggal tidak berbeda. Kata kunci: stroke iskemik, antiplatelet, kejadian stroke berulang

Abstract

Brand loyalty is an important concept among the competitive market of pharmaceutical industry. Strong brand loyalty willencourage customers to make a commitment on repeat buying and recommend that brand to others. The study aimed to analyze thefactors that affect consumer loyalty toward ulcer medicines, namely Promag and Mylanta.The study was a correlational study. Respondents were 200 pharmacy customers, divided into 100 customers who had been consumed Promag and 100 customers who had been consumed Mylanta. The research instrument used a questionnaire with Likert scale. Data wereanalyzed using multiple linear regression test.The results showed that the brand reputation, brand predictability, brand competence, trust in the company, and brand likingsimultaneously had a significant effect on both the brand loyalty of Promag (R2= 0,962) and Mylanta (R2= 0,931). To sum up, the brandreputation, brand predictability, brand competence, trust in the company, brand liking and loyalty of Promag were higher thanMylanta.Keywords: loyalty, Promag, Mylanta, brand

Abstract

The implementation of INA-DRG for Jamkesmas Class III patient at the hospital was expected to be able to increase the access and quality of health service to the poor in order to realize the optimal health service effectively and efficiently. The aims of this research were to analyze the medical cost of heart failure patients, to investigate the affect of patient’s factors, comorbidities, severity level and the duration of treatment on the cost of heart failure treatment and to investigate the difference of average cost for the heart failure patient’s treatment based on the rate of RSUD Dr. Moewardi with the rate of INA-DRG package. This research was descriptive according to the hospital perspective. The data collection method conducted retrospectively. The research subject was the patients who were hospitalized in class 3 and the member of Jamkesmas at RSUD Dr. Moewardi at period of 2009 who suffered from heart failure and fulfilled the inclusive requirement. The data analysis was done with cross tab and linear regression, the cost average comparison and LOS from INA-DRGs package. The p values of statistic result test among sex, age, number of comorbid, severity level and LOS on the medical cost were 0.851, 0.960, 0.694, 0,0621 and 0.000. The average of heart failure medical cost based on RSUD Dr. Moewardi rate on the level of severity I was Rp. 1,870,231 ± Rp.1,506,661, while on the level of severity II was Rp. 2,382,737 ± Rp. 2,132,236 and on the level of severity III was Rp. 2,513,826 ± Rp. 1,664,057. Real ALOS on the level of severity I was 6.92 days, while on the level of severity II was 7.35 days and on the level of severity III was 8.10 days. The research result shown that there was no relation between the patient’s character, number of comorbid and severity level on the hospital medical cost, while the treatment duration had corelation with the medical cost. The average of heart failure treatment cost on the level of severity I, II, and III was lower compared to INA-DRG package rate. It shown that the hospital succeeds to use medical facilities efficiently so that the tarrif will not surpass it of INA – DRG, while the difference of real ALOS and ALOS INA-DRG was the highest on the level of severity III. It has showed that the hospital has done efficiency and has been able to decrease the duration of the heart failure inpatient treatment at the hospital.Key words: INA-DRG, heart failure, Jamkesmas

Abstract

Rumah Sakit Umum Daerah Kota Semarang merupakan rumah sakit umum kelas B milik Pemerintah Kota Semarang yang telah menggunakan sistem informasi manajemen Medifirst2000 yang berbasis komputer untuk mendukung kegiatan operasionalnya sejak tahun 2007 yang dikelola oleh vendor. Namun pada awal tahun 2012 sistem informasi manajemen dikelola sendiri oleh instalasi sistem informasi manajemen rumah sakit dan telah diterapkan ke semua bagian pelayanan termasuk instalasi farmasi. Tujuan dari penelitian ini adalah untuk mengetahui pengaruh kualitas sistem, kualitas informasi dan kualitas pelayanan terhadap penggunaan sistem di RSUD Kota Semarang, untuk mengetahui pengaruh kualitas sistem, kualitas informasi dan kualitas pelayanan terhadap kepuasan pengguna di RSUD Kota Semarang serta untuk mengetahui pengaruh penggunaan sistem dan kepuasan pengguna terhadap net benefits yang diperoleh. Penelitian ini merupakan penelitian deskriptif analitik kuantitatif dengan metode penelitian menggunakan model kesuksesan sistem informasi DeLone dan McLean. Penelitian dilakukan di RSUD Kota Semarang dengan subyek penelitian adalah staf instalasi farmasi RSUD Kota Semarang. Analisis data dilakukan dengan analisis statistik deskriptif dilanjutkan dengan uji asumsi klasik dan uji regresi linier. Jumlah responden yang digunakan dalam penelitian ini sebanyak 55 responden dengan karakteristik responden mayoritas berjenis kelamin wanita (87,27%), usia 21-28 tahun (41,82%) dengan jenis pekerjaan sebagai assisten apoteker (69.09%). Berdasarkan hasil uji regresi linier dapat disimpulkan bahwa kualitas pelayanan mempunyai pengaruh positif terhadap penggunaan sistem dengan nilai sig.0,000. Kualitas sistem mempunyai pengaruh positif terhadap kepuasan pengguna dengan nilai sig,0,000. Penggunaan sistem dan kepuasan pengguna mempunyai pengaruh positif terhadap net benefits dengan nilai sig. 0,020 dan 0,001. Secara simultan kualitas sistem, kualitas informasi dan kualitas pelayanan mempunyai pengaruh positif terhadap penggunaan sistem dan kepuasan pengguna dengan nilai sig. 0,000. Sedangkan penggunaan sistem dan kepuasan pengguna secara simultan mempunyai pengaruh positif terhadap net benefits yang diperoleh dengan nilai sig. 0,000.Kata kunci: evaluasi, sistem informasi manajemen farmasi, D&M IS Success Model, RSUD Kota Semarang

Abstract

Stroke is the leading cause of death and disability that includes disability mild, moderate and severe need of maintenance costs high. The purpose of this research is to know the characteristics of the patient, the comparison of real costs and fares INA CBG's ischemic stroke inpatient and outpatient in Bethesda HOSPITAL Yogyakarta. This research was conducted with observational analytic approach with cross sectional design based on the perspective of the hospital. The subject of research that meets the criteria of inclusion as much as 96 patients. Criteria for Inclusion include patient JKN, ischemic stroke of the first offensive, onset of less than 24 hours and non of the references. Observation period 1 October 2015 sd 31 March 2016 inpatients were observed 6 months in retrospective. Secondary data research data are sourced on the medical record, IFRS, and Technology Unit of RS. Analysis of statistical data by the method of Mann Whitney and Kruskal Wallis. Based on the characteristics of long hospitalizations, inpatient class there is significant influence (p < 0.05) against ischemic stroke patients real costs of hospitalization. The total real costs RS ischemic stroke of Rp 993,181,170 to 96 111 patients with inpatient visits so that the cost of Rp 8,947,578 per episode of hospitalization. Furthermore the Total cost of stroke ischemic amounting to Rp 139,165,150 with 211 outpatient visits so that the cost of Rp 659,550 per an outpatient episode. Real costs RS ischemic stroke than outpatient rates INA CBG's-2014 have significant differences. Similarly the real costs of hospitalization compared to price of INA-CBG's 2014 have significant differences in class i. In conclusion Fund fares INA-CBG's either outpatient or inpatient insufficient finance the care of patients ischemic stroke in Bethesda hospital Yogyakarta.
