Abstract

Complete basic immunization occurs if certain vaccines are given to babies; if one dose or more certain vaccines are not given, incomplete basic immunization occurs. The objective of the study is to detect the management of basic immunization, and factors affecting incomplete basic immunization at the Sidomulyo health center in Pekanbaru in 2020. This research was a mixed methodology in which one Qualitative research design confirms 3 informants namely the health center head, immunization section head, and immunization worker. The researcher conducted a deep interview, observation, and reviewing documents. Data analysis was conducted by data reduction, conclusion, and presentation. This used a quantitative case-control study design, the researcher found a population of 286 cases, and a population of 1,308 controls. Each of 205 cases and 205 controls were taken from its population by systematic random sampling. The researcher collected data through interviews using a questionnaire containing close-ended questions and analyzed data by univariate, bivariate, and logistic regression analysis. Qualitative research design found planning and implementation were running well, but monitoring has not been conducted. The case-control study found a causal relationship with each of intellectual development (CI 95%, OR=3.5-10.4), attitude (CI 95%, OR =3.6-8.5), family-supporting (CI 95%, OR= 2.3-8.5), and side effect (CI 95%, OR =1.9-4.4) and basic immunization. It concludes the management of health centers should monitor basic immunization,  and enhance health promotion on intervention concerning intellectual development, positive attitude, family-supporting, and side effect of vaccines to complete basic immunization. Each intervention developed to be suggestions.

Abstract

Capitation Based on Service Commitment Fulfillment (KBPKP) is an adjustment to the capitation rate based on the results of an agreed assessment of individual health service indicators in the form of a commitment to First Level Health Facilities (FKTP) services in order to increase the efficiency and effectiveness of health service delivery in FKTPs. The achievement of the target indicators for the Capitation Based on Service Commitment Fulfillment (KBPKP) at Puskesmas Rumbai Pekanbaru has not been satisfactory. Throughout 2019, most of the KBPKP indicators were in unsafe conditions with a capitation payment range ranging from 90% - 95%. The research objective was to analyze the efforts to achieve the Capitation indicator based on Service Commitment Fulfillment (KBPKP) at the Rumbai Public Health Center in Pekanbaru City in 2019. The method of selecting informants used snowball sampling. Data source qualified and able to present opinions well. The informants in this study were flexible according to the adequacy and suitability of the study. The results showed that the efforts to achieve the Capitation indicators based on the Service Commitment Fulfillment (KBPKP) at Puskesmas Rumbai, Pekanbaru City were not optimal. Commitment to achieve indicator numbers is still lacking. The strategy that has been made is not clear, therefore there is no effort to formulate a clear, structured and systematic strategy. Organizational structure is not followed by a description of the main tasks and functions so that supervision becomes weak, program holder officers do not focus on realizing the plan for achieving the service commitment based capitation indicator (KBPKP) that has been determined as a result efforts to achieve the KBPKP indicator are difficult to realize. It is recommended that efforts be made to make SOPs, a comprehensive, measurable, systematic plan, formulate the main tasks and functions for officers assigned as personnel to realize the target indicators that have been set so that coordination and control of actions in realizing the KBPKP indicators can be carried out properly.

Abstract

Nearly 90% of labor experienced perineal tears, either with or without episiotomy. The incidence of perineal tears occurred almost 90% in normal labor. Red betel leaf or Piper crocatum is known to have chemical properties that have antiseptic and antibacterial effects. The aim of this study was to assess the duration of healing of postpartum mothers' perineal wounds using red betel leaf decoction and anti-septic drugs. The research method was quasi experimental, the population of postpartum mothers with perineal wounds. The sample of the experimental group was given red betel leaf stew as much as 4-5 pieces for 7 days 18 people and the control group 18 people. Samples were taken by consecutive sampling which was conducted in the village of Tanjung Jati Kec. Binjai Kab. Langkat in April-July 2020. The results of this study were the average healing time in the control group was 5.28 ± 0.958 with a Min-Max 3-6 days, while in the intervention group the average healing time was 3.00 ± 1.372 with a Min-Max 2 -5. There was a significant difference (p-value = 0.001) in the length of days of healing between the control and intervention groups. The conclusion of this study is the effect of giving red betel leaf stew (piper crocatum) on the healing of perineal wounds in postpartum mothers.

Abstract

The health laboratory is a public service institution that has an important role in improving public health status. With the increasing demands for health laboratories in improving services, many problems have arisen related to the lack of human resources, budgeting, long bureaucratic flows of disbursement of funds, long process of procuring facilities and infrastructure, and financial management regulations that hinder the smooth running of services. Through the Regional Public Service Agency (BLUD), the technical unit (UPT) health laboratory of Pekanbaru city is expected to have flexibility in managing finances independently, increasing professionalism, entrepreneurship, transparency, and accountability that can improve services to the community. The purpose of this study was to analyze the readiness of UPT health laboratory of Pekanbaru city in the Implementation of BLUD policy in Pekanbaru city in 2020. The study was qualitative. 6 informants were consisting of 5 main informants and 1 supporting informant. The triangulation used were the source, method, and data triangulation. The results showed that readiness in terms of input was still lacking in human resources in finance and administration. From a process point of view, the UPT health laboratory of Pekanbaru city has completed all the substantive, technical, and administrative requirements. And in terms of output, the UPT health laboratory of Pekanbaru city is ready to implement the BLUD policy. It is hoped that the UPT health laboratory of Pekanbaru city will immediately submit an assessment of the implementation of BLUD policies. Becoming a BLUD can fulfill their needs independently with a faster process.

Abstract

Voluntary Counselling and Testing (VCT) is voluntary counseling and HIV testing which aims to help prevention, caret, and treatment of HIV and AIDS. According to Permenkes No.42 in 2017 about triple elimination explain every pregnant woman must be checked for HIV, Hepatitis B, and Sifilis as early as possible Mother to Child HIV Transmission (MTCT) In 2018, in Indonesia the target of HIV testing in pregnant women is that 60% of pregnant women are tested for HIV. However, only 34% of pregnant women conduct examinations, and the Langsat Health Center is the health center with the lowest VCT rate in pregnant women in Pekanbaru City at 12,7% of pregnant women not to check. The purpose of this study was to determine the determinants of the use of Voluntary Counselling and Testing (VCT) services of pregnant women in the work area of Langsat Health Center Pekanbaru City in 2020. This was an analytical quantitative type with a cross-sectional design. The sampling technique was accidental sampling totaling 101 respondents. Variable of the research were knowledge, attitude, stigma, discrimination, support of family, and support from health workers to the utilization of VCT. Analysis of the data used in univariate and bivariate analysis with chi-square test with α = 0,05. The result showed a variable knowledge (p = 0,010. POR = 3,357), stigma and discrimination (p = 0,042. POR = 2,619), family support (p = 0,002. POR = 4,357), and support of health workers (p = 0,004. POR = 3,843) are related to the use of VCT. Suggestion for the health center to actively socialize about VCT and invite cross-sectoral to collaborate in VCT examination in pregnant women can be easily affordable.

Abstract

The implementation of immunization program is a very important element, but still found that the immunization program at health center is not run in accordance with a promise that is in Indonesian Permenkes No. 12 in 2017. This study aims to analyze the program management immunization Hepatitis B-0 in the work area Rumbio Jaya Health Center in Kampar Regency in 2020. This research used is qualitative research with a descriptive approach. This data collection was carried out at the Rumbio Jaya Health Center in Kampar Regency through in-depth interviews involving 7 informants and reviewing documents.The results showed that there was no vaccine management officer, immunization training had not been carried out, planning for syringe, safety box, and cold chain planning was not carried out by the Rumbio Jaya Health Center, unavailability of immunization waste management and timely immunization reporting. It is recommended to Rumbio Jaya Health Center in Kampar Regency should appoint vaccine management officers so that immunization activities can be carried out optimally, it is better to plan syringe, safety box and cold chain needs, it is better to make the imunization reporting at the fifth date the following month. It is recommended to the Rumbio Jaya Health Office conduct minimum immunization training once a year and should provide immunization waste management places.

Abstract

Pharmaceutical service is a direct and responsible service to patients related to pharmaceutical preparations to achieve definite results to improve the quality of life of patients as stated in Permenkes RI number 35 of 2014 article 1 paragraph 3. This research aims to determine the behavior of pharmacy officers regarding SPM (Minimum Service Standards) at the Awalbros Hospital Pharmacy Installation in Ujung Batu. This research is qualitative research with a phenomenological approach. This research was conducted at the Walbros Ujung Batu hospital installation in July 2020. There were six respondents. The results of the study show that the waiting time for finished drug services is following the provisions of hospital management, which is less than 8 minutes or faster than the service time in the provisions of the hospital and the Ministry of Health, the time for presenting the concocted drugs has not been running with the expected target of fewer than 20 minutes. Because of the numbers of drugs formulated and the limited number of workers working on it. There are no errors in administering the drug, this has been going according to the hospital's standard operating procedures, customer satisfaction has not run perfectly such as waiting time for concocted drugs that are not yet on time, prescription writing according to the formulary has been implemented properly and the formulary guideline document is continuously revised, and the implementation of minimum service standards consistently applied during the Covid-19 pandemic.

Abstract

There are many factors that can cause adolescents to experience drug use, including the influence of association, the environment in which they live, joining in with friends, broken home families which are influenced by the lack of attention and communication between parents and children, as well as the pressure experienced during adolescence so they are prone to drug abused. From a field survey conducted by researchers in March 2020 at Penitentiary Rokan Hulu District, in 2018 there were 4 adolescents who were involved in drug problems, while in 2019 there were 18 teenagers (aged 13-19 years) who were detained in Penitentiary The purpose of this study is to analyze the personality and social life of adolescents with their parents and friends for drug abusers in Class II B Penitentiary, Rokan Hulu Regency in 2020. The research method used is qualitative with a phenomenological approach by conducting in-depth interviews with 13 informants about personality analysis and adolescent social life. The research informants consisted of 4 main informants of adolescent drug abusers in prison, and 9 supporting informants. The data analysis in this study was conducted qualitatively. The analysis was carried out by systematically arranging the interview guidelines and then the data was processed and presented the data descriptively. The implementation time of this study was started from April 1, 2020 to July 31, 2020. The results showed that most adolescents who abuse drugs have negative personalities, namely strength weak ego, most of the social relationships between adolescents and their parents are poor and less harmonious, mostly social relations between adolescents and their peers are unhealthy associations. Parents was a role model to their adolescents life, a harmonious family will have a good effect on adolescent interactions.

Abstract

Tuberculosis (TB) is one of the 10 infectious diseases that can cause mortality worldwide. Geographically, the highest TB cases occurred in Southeast Asia (44%). Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis. Indonesia is among the 3 countries that account for two-thirds of TB cases in the world is 8%. The case finding rate in Riau Province was only 39% and in Pekanbaru City it was only 37.5%. The cure rate for TB patients in Indonesia reaches 73.2% and in Pekanbaru it is only 59.9%, which is still below the Ministry of Health's target of 85%. Family support is very important to observe patient reports for regular medication during the treatment period until it recovers. This study aims to see the role of family support in TB control efforts. Type of qualitative research. The research subjects were 1 holder of the TB Health Office program, 1 staff, 2 Puskesmas program holders, 2 family. Data collection was carried out by interview and document search. The results showed that the family support involved was the patient's family. Health workers provide health information to family related to modes of transmission, how to take TB drugs, schedule patient re-visits. The role of family support is still needed to be improved to support TB treatment. It is recommended to use information systems such as SMS, Web or YouTube to increase the role of family support and patient compliance to follow TB treatment procedures. Abstract

Every public health center in Indonesia has an accreditation level as a determinant of quality. Public health management is an effort to improve the quality and quality of health services. Accreditation of public health in Kampar District only 5 (five) public health have been utama accredited, while the others are accredited middle and basic. This shows that it is necessary to evaluate the implementation of public health management as a reference for other public health. This research aims to analyze how the management of the main accredited public health in Kampar District is implemented in 2020. This research used a qualitative method with the Positive Defiance (PD). The number of informants was 14 people consisting of the Head of the public health, the Head of the Administration of the public health and the Head of the Kampar District Health Office. Data collection methods by means of in-depth interviews and document review. In terms of quantity, the main accredited Primary Health Care in Kampar Regency has implemented health center management well, but in terms of quality it still needs to be improved both from human resource management, availability of funds, methods / policies, materials, movement planning and implementation as well as supervision and control so that service quality can be improved. The support from the Kampar District Office greatly affects the performance of the public health in the Kampar district. Public health management is one of the main references in the operation of a public health. the implementation of good public health management in the Kampar district public health also gave good scores to the 5 main accredited public health in the Kampar district.
