Abstract

Maintenance emerges as a crucial intervention to ensure the sustained reliability of machinery. The AMC 36-800/MA AN SHAN lathe, a piece of equipment subject to extensive operational demands, frequently encounters breakdowns, thereby obstructing the seamless execution of maintenance activities for facilities. This investigation seeks to pinpoint the components responsible for the failures and malfunctions afflicting the lathe above. Employing the methodologies of Failure Mode and Effects Analysis (FMEA) and Fault Tree Analysis (FTA), this study successfully identifies the components at the epicentre of the machine's vulnerabilities. The analysis culminates in delineating critical components by calculating Risk Priority Numbers (RPN), unveiling a pivotal RPN value of 309.57. Moreover, it isolates two components whose failure modes surpass the critical RPN threshold: the Drive Motor and the Fixed Head. The genesis of these failures can be attributed to the escalating operational intensity, elevated engine temperatures, and a maintenance regimen that is both sporadically scheduled and suboptimal. Keywords: Lathe AMC 36-800/Ma AN Shan, Failure mode, FMEA, FTA, RPN

Abstract

UKM Gudang Dompet is a company that produces wallets. The number of orders will affect employee performance because the number of targets achieved will increase the workload for employees, causing fatigue. To prevent excessive fatigue felt by workers in the production division at the Dompet Warehouse, it is necessary to measure workload. The Full Time Equivalent (FTE) method measures working time, which is based on working time, measuring the time needed to complete a particular task and then converting it into an FTE index value. The cardiovascular load (CVL) method measures physical workload based on comparing pulses. Working pulse with a maximum pulse. This research showed that worker fatigue occurred in the packing section, indicated by a CVL value of more than 30%, so FTE calculations were carried out to determine energy consumption. Based on energy consumption calculations, the results obtained are an additional rest time of 32.85 minutes in the packing section. Keywords: workloads; full-time equivalent; CVL; work system design.

Abstract

Ant-sugar SMEs are developing in the Indragiri Hilir Regency. This industry uses the sap of hybrid coconut trees to make sugar. To maintain sustainability, SMEs require assistance with quality from upstream to downstream. Based on this, the research attempts to ascertain the mechanism of the sugar supply chain, identify the elements that lead to flaws, and suggest ways to enhance the chain's quality. Identify supply chain conditions using the Vorst method and identify the root causes of problems using the FTA method and quality improvement proposals based on evaluating potential failure levels using the FMEA method. The research results show that the supply chain members are Farmers, SMEs and Final Consumers. The factors causing defects in coconut sap water and ant sugar are human and environmental. The proposed improvement at the farmer level is that the cracked wood preservative must be measured correctly before taking coconut sap water. In contrast, at the SME level, workers pay attention to the ongoing production process and carry out periodic checks when boiling the sap water so that the maturity level is perfect and stir optimally so as not to cause failure. In the crystallization of ant sugar.

Abstract

PT. ISP is a manufacturing company that focuses on the production of springs for vehicles in the industrial sector. The products produced include leaf springs and snail springs, which are produced via cold and hot processing methods. PT. ISP collaborates with Mitsubishi Steel Manufacturing, Japan, using a license from that company. One of them is Moveable Driven, which is a type of spiral spring that is produced through a cold process. PT. ISPs face challenges related to defects that arise during the production process. This research aims to assess the level of defects, identify factors that contribute to product defects and suggest improvements. Seventools method for identifying factors that cause defects in products. The research results based on the Pareto diagram show that the most dominant type of defect is Outside Diameter (+) with a total of 3,197 pcs with a percentage of 44.9%, so defects need to be the main focus for improvement. On the P Chart, there is 1 data that is out of control below the Lower Control Limit/LCL limit in the 9th data. After revision 1 there is no data out of control. In this way, the factors causing Outside Diameter (+) defects using fishbone include Man Factor, Measurement Factor, Machine Factor, Process Capability, Method Factor. The condition of internal defects is still within control limits. So improvements need to be made so that defects meet the targets that have been set.
