Jurnal Kesehatan Reproduksi first printed version was published in 2014 with ISSN 2302-836X. In 2016, we also have an online journal version with ISSN 2621-461X. Currently, we already use the Online Journal System, requiring all authors to submit their papers online. Afterwards, authors, editors and reviewers will be able to monitor the manuscript processing. This journal is published annually every April, August and December.

Abstract

Background : New cases of COVID-19 continued to emerge due to the new variants. Pregnant women are more susceptible to severe infections.Objective : To compare the effect of COVID-19 infection on maternal and perinatal outcomes in the first and second waves.Method : An analytical observational study with a cross-sectional design was used. Samples were selected by consecutive sampling from the medical record data of RSUP Dr. Kariadi Semarang, Indonesia, with 47 cases during the first wave (1 August 2020 to 14 May 2021) and 47 cases during the second wave (16 May to 30 September 2021). Data were analysed using univariate, chi-square, fisher’s exact, and logistic regression tests with a significant value of p <0,05.Results and Discussion : Pneumonia, ICU admission, and oxygen consumptions were higher in the second than the first wave as 87.23% VS 70.21%; p = 0.044, 36.17% VS 14.89%; p = 0.018, 65.96% VS 12.77%; p < 0.0001 respectively. The severe COVID-19 infection and maternal mortality increased in the second wave (51,06% VS 14,89%; p = 0.009; 29,79% VS 8,51%; p = <0.001). There were no differences in perinatal outcomes between the first and second waves such as fetal distress, fetal growth retardation, low birth weight, nICU admission, and mortality (0.00% VS 8.51%; p = 0.117, 0.00% VS 4.26%; p = 0.495, 12.77% VS 25.53%; p = 0.116, 12.77% VS 21.28%; p = 0.272, 4.26% VS 8.51%; p = 0.677). The incidence of COVID-19 infection in neonates remained low in both waves at 17.02% VS 12.77%; p = 0.562.Conclusion : Even though a high number of severe diseases to maternal deaths were found during the second wave, neonatal COVID-19 infections remained low.

Abstract

Cesarean scar pregnancy (CSP) is a rare but life-threatening complication. It is the abnormal implantation of gestational sac into myometrium and fibrous scar of previous cesarean section. Its incidence is on rising trend due to increase in rate of cesarean section all over the world. A thirty one-year old third gravida presented at five weeks of gestation with complaints of vaginal spotting. She was diagnosed as a case of cesarean scar pregnancy (CSP) on ultrasonography. Conservative management of CSP was done successfully with the injection of systemic methotrexate. CSP pregnancy could be catastrophic, if not managed well in time. Management includes both surgical and medical options. The optimal management of caesarean section pregnancy should be individualized depending upon the hemodynamic status depending on patient’s hemodynamic profile, size of gestational sac, desire for future fertility, and patient’s preference.

Abstract

ABSTRACT Background: Child stunting is a global problem, including in Indonesia. The target of global goals in 2025 is to reduce the prevalence of stunting. The prevalence rate in Demak District in 2019 was 50.23%, making it one of the 60 priority districts for stunting in 2019.Objective: Mobile health application can help pregnant women maintain their health and promote useful information to others.Method: First-year research method started with planning, engineering analysis, and evaluation of digital applications, named "ASTA, (Child Stunting Prevention Application), then the second year implemented the ASTA Application in midwifery care services, with a total of 24 respondents for two control and treatment groups.The results were analyzed with the Wilcoxon and Mann Whitney tests.Results and Discussion : The results of the study in the form of a stunting prevalence ratio of 0.7, indicate that the ASTA application is a factor that can reduce the potential for stunting. Keywords:  Application; ASTA; monitoring; pregnant women; stunting ABSTRAK Latar belakang: Stunting pada anak merupakan masalah global, termasuk di Indonesia. Target tujuan global pada tahun 2025 adalah menurunkan prevalensi stunting. Tingkat prevalensi di Kabupaten Demak pada tahun 2019 adalah 50,23%, menjadikannya salah satu dari 60 kabupaten prioritas stunting pada tahun 2019.Tujuan: Mobile health apps dapat membantu ibu hamil menjaga kesehatan mereka dan mempromosikan informasi yang berguna kepada orang lain.Metode: penelitian tahun pertama dimulai dengan perencanaan, analisis rekayasa, dan evaluasi aplikasi digital, yang diberi nama "ASTA, (Aplikasi Pencegahan Stunting pada Anak), kemudian tahun kedua mengimplementasikan Aplikasi ASTA pada pelayanan asuhan kebidanan, dengan jumlah responden 24 orang untuk dua kelompok kontrol dan kelompok perlakuan. Hasil penelitian dianalisis dengan uji Wilcoxon dan Mann Whitney.Hasil dan Pembahasan : Hasil penelitian berupa rasio prevalensi stunting sebesar 0,7, menunjukkan bahwa aplikasi ASTA merupakan faktor yang dapat menurunkan potensi terjadinya stunting.

Abstract

Background: Puerperal sepsis is one of the five leading causes of maternal mortality worldwide, commonly occurs in low to middle income countries. Higher incidence is observed in rural areas where diagnostic and therapeutic interventions may be limited.Case: A a 21-year-old woman who developed a fever, abdominal pain, and foul-smelling vaginal discharge five days after giving birth. The patient convulsed five times in total before being admitted. She was diagnosed with status epilepticus caused by sepsis-associated encephalopathy and puerperal sepsis, which was successfully treated with antibiotics and supportive care.Discussion: Pregnancy and the puerperium period involve significant changes in maternal physiology and immune function. The Society of Obstetric Medicine Australia and New Zealand (SOMANZ) has proposed Obstetric-modified quick sepsis related organ failure assessment (omqSOFA) for early recognition of maternal sepsis. An score of 2 or higher has predictive value for maternal sepsis.Conclusion: Early recognition, appropriate treatment, and close monitoring are crucial for improving outcomes in patients with puerperal sepsis.

Abstract

Background: Premature birth can increase mortality and disability in infants. Premature birth can be influenced by several types of factors, such as sociodemographic factors. Based on literature studies, there are several contradictory research results regarding the sociodemographic characteristics of premature birth. In addition, there is a surveillance system HDSS Sleman that periodically collects data on demographic transition, health status, and social transition in Sleman.Objective: This research objective is determining the sociodemographic characteristics of premature birth in Sleman Regency.Method: This is a cross-sectional study with total sampling method using HDSS Sleman secondary data from cycle 4 (2018) until cycle 6 (2020).Results and Discussion: The results of this study showed that the total proportion of premature birth in Sleman Regency from 2018 to 2020 was 14.4%. Then, the proportion of premature birth each year from 2018 to 2020 were 18.5%, 13.0%, and 5.8% respectively. After that, the highest proportion of sociodemographic characteristics of the mothers experiencing premature birth were in the category of maternal age <20 years old (20.0%), maternal parity 1 (13.0%), “working” occupational status (15.4%), low educational status (18.8%), and low socioeconomic status (18.0%).Conclusion: It can be concluded that the most common sociodemographic characteristics of the mothers experiencing premature birth were maternal age <20 years old, maternal parity 1, “working” occupational status, low educational status, and low socioeconomic status.

Abstract

Background: Modified Early Obstetric Warning Score (MEOWS) is used to identify obstetric patients at risk of deterioration. MEOWS has long been used in the maternal ward of Dr. Sardjito Central General Hospital. Research in other countries found that MEOWS cards were not filled in completely and some MEOWS cards were not followed up.Objective: to determine the completeness of the MEOWS card and the suitability of the interventions carried out with the MEOWS card instructions.Methods: This study is a mixed method study (quantitative descriptive and qualitative phenomenology).Results: Out of 1285 study subjects, the proportion of complete MEOWS cards was 84.67%. The proportion of appropriate interventions was 95.05%. Appropriate intervention was associated with length of hospitalization ≥ 7 days, p value 0.043, prevalence ratio 0.456 (95% confidence interval 0.242-0.858). Reasons for staff not completing MEOWS cards and not performing appropriate interventions were management factors, lack of knowledge, compliance, vigilance, training and medical records.Conclusion: Most MEOWS cards were completed and appropriate interventions were performed. Appropriateness of MEOWS interventions is likely to reduce the prevalence of length of stay ≥ 7 days. MEOWS completion and interventions are influenced by staff management, knowledge, vigilance and compliance, which can be improved by training and practical use of medical records.

Abstract

Background: Exclusive breastfeeding is one of the world's efforts to prepare healthy future generations from an early age.Objective: This research is to determine the relationship between husband's social support and exclusive breastfeeding for breastfeeding mothers at the Sinorang Community Health Center, Banggai Regency.Method: This research is quantitative research with a cross-sectional approach. Data analysis used in this research used the chi square test.Results and Discussion: There is a significant relationship (p value = 0.000) of husband's informative support with exclusive breastfeeding; there is a significant relationship (p value = 0.047) of husband's emotional support with exclusive breastfeeding; there is a significant relationship (p value = 0.000) of husband's instrumental support with exclusive breastfeeding; There is a significant relationship (p value = 0.024) of husband's appraisal support with exclusive breastfeeding.Conclusion: There is a relationship between social support consisting of informative support, emotional support, instrumental support, appraisal support and exclusive breastfeeding.

Abstract

ABSTRAKLatar Belakang: Respon inflamasi memiliki hubungan yang signifikan terhadap nilai prognostik dari suatu keganasan. Rasio platelet-limfosit (RPL) sebagai salah satu penanda inflamasi pada keganasan sehingga menjadi prediktor terhadap prognosis pasien kanker pada umumnya.Tujuan: Mengetahui hubungan antara RPL pra pembedahan dan respon kemoterapi berbasis platinum pada kanker ovarium epithelial di RSUP DR. Sardjito.Metode: Penelitian kohort retrospektif untuk menilai hubungan antara RPL pra pambedahan dan faktor-faktor risiko terhadap respon kemoterapi berbasis platinum pada pasien kanker ovarium epitelial.Hasil dan Pembahasan: Nilai cut-off RPL pra pembedahan adalah 203 dengan sensitivitas dan spesifitas=76,2%, nilai AUC=0,786, dan akurasi=76,2%. Ditemukan hubungan signifikan antara RPL pra pembedahan dan respon kemoterapi pada pasien kanker ovarium epitelial, p=0,001, RR(CI95%)=3,200(1,815-5,640). Terdapat hubungan yang signifikan antara jenis histologi dan respon kemoterapi, p=0,001, RR(CI95%)=2,384(1,424-3,989).Kesimpulan: Kejadian resisten kemoterapi berbasis platinum pada pasien kanker ovarium epitelial signifikan lebih tinggi pada pasien dengan nilai RPL pra pembedahan ≥203 dan jenis histologi (serosa 2 kali lebih tinggi dan  non serosa 4 kali lebih tinggi yang disertai dengan nilai  RPL ≥203)(efek modifikasi

Abstract

Background: Crash caesarean section is performed because there is a life-threatening condition to the mother or fetus. The duration between a decision is made to perform a caesarean section until delivery the baby is termed decisions to delivery interval (DDI) or response time. Response time of 30 minutes from the decision to baby delivered is considered to prevent complications and worsen maternal conditions.Objective: To assess the relationship between response time in crash caesarean section with maternal outcome at Dr. Sardjito Hospital.Method: This study was cross sectional method using secondary data from the medical record. The subjects were all pregnant women who underwent crash caesarean section and met the inclusion criteria from January 2021 to December 2022 at Dr. Sardjito Hospital.Results and Discussion: There were 38 cases that met the criteria from 114 cases of crash caesarean section during the period January 2021 to December 2022. From the data obtained, response time of £ 30 minutes was 4 (10,5%) cases and response time > 30 minutes was 34 (89,5%) %) cases. There was no significant relationship between the duration of response time and the maternal outcomes of ICU care, blood transfusion, length of hospital stay, or surgical wound infection (p>0,05). From bivariate analysis, maternal outcomes ICU care was statistically significant relationship of preoperative Hb level <8 g/dL (p=0,003; RR 2,500; 95% CI 1,613-3,875) and severe antepartum hemorrhage (p=0,000; RR 9,880; 95% CI 1,484-65,787). From bivariate analysis, severe antepartum hemorrhage was statistically significant against maternal outcomes blood transfusion (p=0,000; RR 2,600; 95% CI 1,307-5,171).Conclusion: There was no relationship between the duration of response time for crash caesarean section based on maternal indications with maternal outcomes (ICU care, blood transfusion, length of hospital stay, and surgical wound infections) at Dr. Sardjito Hospital. There is a significant relationship between severe antepartum hemorrhage with maternal outcomes of ICU care and blood transfusion. Keywords: crash caesarean section, response time, DDI, maternal outcome

Abstract

Background: Limited parental awareness regarding early childhood sex education contributes to the rising prevalence of sexual violence against children. Inadequate understanding of intimate boundaries and insufficient parental guidance place children at higher risk.Objective: To explore parental roles and the strategies employed in introducing and teaching sex education to early-aged children.Method: This research utilizes a qualitative design with a phenomenological approach. Data were obtained through unstructured interviews guided by interview prompts, involving six parents of children aged 2–6 years selected through purposive sampling.Results and Discussion: Findings indicate that parents view essential knowledge about intimate areas, gender distinctions, and modesty as foundational before educating children. Parents act as primary information providers, behavior regulators, communication partners, and interpreters of religious values in delivering sex education. Early sex education fostered responsibility and independence in maintaining personal hygiene and safety of intimate organs. Social support—originating from spouses, family members, teachers, peers, and the children themselves—strengthened parents’ capacity to optimize sex education practices.Conclusion: Parents hold a pivotal role as the initial source of information and behavioral guidance in early sex education. Adequate parental literacy, supported by social reinforcement, promotes children’s responsibility and autonomy in safeguarding their intimate areas.
