Abstract

Objective: Cervical cancer is the second most prevalent cancer inwomen around the world and the most common cancer in womencausing death. This study aims to analyze the connection betweeninfection of human papilloma virus (HPV) 16/18 and cervicalchanges in the acceptors of Depot Medroxyprogesterone Acetate(DMPA) Contraceptions and nonacceptors of Depot MedroxyprogesteroneAcetate (DMPA) Contraceptions.Methods: The research was conducted at the Public ServiceInstitution of Dr. Wahidin Sudirohusodo hospital, and privatemidwife clinics for seven months from December 2015 to June 2016.The research design is cross-sectional with. The samples were fortyacceptors of Depot Medroxyprogesterone Acetate (DMPA) and fortynon-acceptors of Depot Medroxyprogesterone Acetate (DMPA)contraception. Prevalence of HPV 16/18 and cervical cytologychanges were examine using the polymerase chain reaction andliquid base cervical cytology.Results: The results showed there was no significant relationshipbetween long-term use of DMPA contraceptives with HPV 16 and 18.There was no significant relationship between long-term use ofDMPA contraceptives with cervical cytology changes. There was nosignificant relationship between HPV 16 and 18 infections with theoccurrence of cervical cytology changes in long-term use of DMPAcontraceptives.Conclusion: The long-term use of DMPA contraceptive does notincrease the risk of HPV 16 and 18 infections. Abstract

Objective: To determine the expression of Phosphatase Regenerating Liver-3 (PRL-3) and E-Cadherin in the epithelial ovarian cancer on various stages and differentiation grades. Method: This was a cross-sectional study design conducted at Obstetrics and Gynecology Department of several teaching hospitals, Faculty of Medicine Universitas Hasanuddin from January to June 2015. The expression of PRL-3 and E-cadherin was assessed immunohistochemically in 40 patients with epithelial ovarian cancer including 15 patients in early stage and 25 patients in advanced stage. We used the Fisher’s exact test with the significance of p0.05). The significant difference was found in the expression of E-cadherin whereas the high expression was shown at early stage than advanced stage (p0.05). This study also pointed out no correlation between the expression of PRL-3 and E-cadherin in epithelial ovarian cancer (p>0.05). Conclusion: PRL-3 overexpression does not decrease E-cadherin expression in epithelial ovarian cancer. Keywords: E-cadherin, epithelial ovarian cancer, PRL-3

Abstract

Tujuan: Mengetahui efektivitas (sensitivitas dan spesifisitas) tes pap pada tes IVA positif sebagai usaha penapisan dua tahap dalam skrining kanker serviks. Bahan dan cara kerja: Uji diagnostik dengan baku emas histopatologi. Merupakan studi deskriptif, desain penelitian potong lintang. Populasi penelitian adalah wanita seksual aktif berusia berusia 25 - 45 tahun, de-ngan kriteria eksklusi: (1) penampakan serviks mencurigai suatu kega-nasan atau memperlihatkan infeksi; (2) pada pemeriksaan tidak dapat dikenali daerah SSK; (3) dan sudah menjalani pengobatan atau tindakan pada serviks atau rahim. Dalam kurun waktu bulan Oktober 2004 hingga Maret 2005 dilakukan tes IVA di puskesmas/balkesmas oleh bidan yang telah mendapatkan pelatihan IVA. Wanita dengan tes IVA positif dilakukan pemeriksaan lebih lanjut di poliklinik kolposkopi Departemen Obstetri Ginekologi FKUI-RSCM yakni tes pap dan kolposkopi, serta biopsi bila ditemukan lesi. Pengolahan dan pembacaan sediaan pap dan biopsi dilakukan di laboratorium Sitologi dan Patologi Obstetri dan Ginekologi FKUI RSCM. Hasil: Telah dilakukan pemeriksaan IVA terhadap 1156 wanita, 1 orang tidak dimasukkan ke dalam sampel penelitian karena secara makroskopik didiagnosis sebagai kanker serviks stadium IIA. Dari 1155 didapatkan IVA negatif 1057 sampel dan IVA positif 98 sampel. Angka positif palsu tes IVA yang dilakukan Bidan sebesar 50%. Sensitivitas pemeriksaan serial dua tahap IVA-Pap berdasarkan histopatologi yang dikelompokkan atas ada/tidaknya lesi intraepitel skuamosa (LIS) adalah 22,45% dan 8,16%, dan spesifisitasnya adalah 93,88% dan 100%. Sedangkan bila hasil histopatologi dikelompokkan atas ada tidaknya lesi intraepitel skuamosa derajat tinggi (LISDT) maka sensitivitasnya adalah 50% dan 25% dan spesifisitasnya adalah 87,23% dan 96,81%. Faktor-faktor risiko yang mempunyai hubungan bermakna dengan temuan lesi prakanker serviks (histopatologi LISDR/LISDT) pada penelitian ini adalah adalah usia saat hubungan seks pertama kali di bawah 20 tahun, menikah lebih dari 1 kali, pengguna kontrasepsi pil, keputihan, merokok, dan wanita PSK. Kesimpulan: Efektivitas pemeriksaan serial dua tahap IVA-Pap lebih baik daripada pemeriksaan satu tahap, dalam hal spesifisitas dan menurunnya angka positif palsu. Rendahnya sensitivitas tes pap pada kasus IVA positif perlu penelitian lebih lanjut, salah satu kemungkinan adalah karena semua lesi pada penelitian ini adalah lesi-lesi kecil (< 10% area SSK), yang mengakibatkan tingginya angka negatif palsu tes pap. Spesifisitas pemeriksaan dua tahap IVA-Pap sangat baik, sehingga bila sensitivitas pemeriksaan serial ini diperbaiki maka dapat digunakan sebagai alternatif penyaring kasus-kasus lesi prakanker yang akan dikirim/ dirujuk untuk tindak lanjut. {Maj Obstet Ginekol Indone 2006; 30-3: 164-74] Kata kunci: tes IVA (Inspeksi Visual dengan Asam Asetat), tes pap, histopatologi, efektivitas, sensitivitas, spesifisitas.

Abstract

Objective: To discussed the ethical aspects, the technique, optimal timing and the possible outcome of selective termination in anomalous twin pregnancy. Case description: A 28-year old primigravida, 21 weeks twin gestation, with giant hygroma coli in one fetus. The normal fetus was compressed by the anomalous fetus with oligohydramnios. The parent decided to undergo selective termination of the abnormal fetus. The procedure perform was intracardiac injection of potassium chloride (KCl). Eight milliliters of KCl were injected into fetal cardiac, followed by bradicardia and asystole of the anomalous twin. A week after the procedure, the normal fetus also died and delivered spontaneously after induction. Conclusion: Selective termination for fetal anomaly in twin pregnancy have some aspects to be considered. Abstract

Objective: To measure and correlate bet-ween elastin expression and elastic fibre thickness in the uterosacral ligaments of women with uterine prolapse. Method: This analytic with cross sectional study was done in January - March 2011. Specimens were obtained from the parafin block of uterosacral ligaments of women having uterine prolapse (prolapse group, n = 16) and the same location in patients with no prolapse having simple hysterectomy, radical hysterectomy or staging laparotomy (control group, n = 16) who came to Dr. Hasan Sadikin hospital and its network. The percentage of elastin positive tissue and elastic fibre thickness were measured by immunohistochemistry from random field readings per sample. The examiner was unaware of sample identity and the patients’ clinical history. Result: The risk factors (age, parity and vaginal delivery) were compared between the control and prolapse groups, and there were no difference of body mass index between the groups. Immunohistochemical staining and morphometric analysis indicated there were significant different beween elastin expression in the prolapse group compare with the control group (p < 0.001). The mean thickness of elastic fibres was 0.938 μm in the group with uterine prolapse and 2.244 μm in the control groups (p = 0.02). Elastin expression was associated with elastic fiber thickness (r = 0.589 and p = 0.016). Conclusion: In this cross-sectional study investigating elastin changes in women with prolapse, there was correlation between elastin expression and elastic fiber thickness in uterosacral ligament of women with uterine prolapse. Abstract

Objective: There were only few studies about the characteristic of vaginal pH among contraception user in Indonesia. This study aimed to determine the effect of contraception on vaginal pH among Indonesian women. Method: This study is a cross-sectional study. The target population was Indonesian women aged 15 - 50 years. The samples consisted of 492 people. Based on the method of contraception, subjects were grouped into 4 groups, subjects with combination hormonal contraceptives, progestin only, condoms/IUDs, and subjects with no contraception or sterile. Vaginal acidity (pH) was examined by Dip- Stick (Merck®). Result: The average age was 30.9 ± 8.27. The methods of contraception the subjects used were combination hormonal contraception (18.0%), progestin-only (29.5%), condom/IUDs (5.1%) and no contraception/sterile (47.4%). Abstract

Tujuan: Meninjau perangai imunopatobiologi penderita endometriosis sebagai modalitas baru untuk menegakkan diagnosis endometriosis tanpa visualisasi laparoskopi. Rancangan/rumusan data: Tinjauan pustaka. Kesimpulan: Endometriosis bisa dilihat sebagai proses inflamasi pelvis dengan perubahan fungsi sel yang berkaitan dengan kekebalan dan jumlah makrofag aktif yang meningkat dalam cairan peritoneum yang mensekresi berbagai produk lokal, seperti faktor pertumbuhan dan sitokin. Meningkatnya sitokin dan faktor-faktor lain dalam cairan peritoneum diikuti dengan meningkatnya faktor-faktor serupa, seperti CRP, SAA, TNF- α, MCP-1, IL-6 dan CCR1, dalam darah tepi pada penderita endometriosis. Monosit CD44+ dan CD14+ meningkat secara bermakna, sementara limfosit T CD3+ dan limfosit B CD20+ menunjukkan pengurangan sedikit tetapi bermakna dalam darah tepi penderita endometriosis. Ini menunjukkan bahwa endometriosis dapat dilihat sebagai penyakit lokal dengan manifestasi, sub-klinis sistemik. Abstract

Objective: To report the congenital abnormalities of müllerian duct and review its literature. Methods: Case report. Case: A sixteen years old unmarried woman came to our policlinic with chief complain of cyclical abdominal pain (VAS 6). She had regular menstruation. In other hospital, she had diagnosed as suffering from ovarian tumor. Our examination showed that the uterus was unicornuate with rudimentary horn. We did explorative laparotomy. We took out the left cornu and left salpingoophorectomy due to hematosalphing and endometrial cyst. The Visual analogue score after surgery was reduced until VAS 2. Conclusion: Congenital abnormalities of the müllerian ducts are relatively common. Comprehensive examination can diagnose this abnormality earlier, thus intervention can be given as soon as possible. Abstract

Objective: To investigate the side effects and acceptance of intracesarean Intrauterine Device (IUD) insertion. Methods: This was a descriptive study. Subjects were all pregnant women who had Cu T380A IUD to be placed after cesarean delivery at Prof. Dr. dr. R. D. Kandou Hospital during the period between August 2016 and September 2016. Results: A total of 52 subjects were recruited in this study.43 (82.69%) subjects were in the age group of 20-30 years. 51 of the subjects accepted the IUD. On the 7th day, 49 (94.23%) and 51 (98.07%) subjects complained of abdominal pain and vaginal bleeding, respectively, while on the 14th day, 48 (92.3%) and 50 (96.1%) subjects had abdominal pain and vaginal bleeding, respectively. There were significant decreases in both complaints on the 14th day, compared to the 7th day. (p&lt;0.05 and p&lt;0.05, respectively). 51 (98.07%) subjects accepted the IUD. There were no significant difference between acceptance on day 7 and 14th (p&gt;0.05). Conclusion: The acceptance of side effects by the acceptor reaches 100%, with the incidence of early postpartum expulsion is 3.8%. The side effects of IUD are minimal. Abstract

Objective: To analyze differences in mean and correlation between content of collagen I and tenascin-C sacrouterine ligaments in patients with uterine prolapse. Method: This type of research is analytic comparative and cross sectional correlation is cut in two research groups. Well conditioned paraffin block of patients uterine prolapse and without uterine prolapse were stained with immunohistochemical staining. The preparation is examined under a light microscope by the Pathology Specialist, assessment of content is done by looking at the distribution and intensity of the color of collagen fibers and tenascin-C. Result: There were significant mean differences between the content of collagen I and tenascin-C sacrouterine ligament in patients with uterine prolapse and without uterine prolapse (p = 0.001). There was no correlation between the content of collagen I and tenascin-C sacrouterine ligament in patients with uterine prolapse (p = 0.780). Conclusion: There was a difference in the average content of collagen I and tenascin-C in the uterine prolapse group compared with no uterine prolapse group. There was no correlation betweenthe content of collagen I and tenascin-C in the uterine prolapse group.
