Abstract

Objective : This study aims to assess the effect of the addition of probiotic supplement to the standard therapy on the recovery of Bacterial vaginosis infection. Methods: The research was conducted by taking samples from Obstetrics and Gynecology Department in several teaching hospitals of Faculty of Medicine, Hasanuddin University. This study was an experimental study. Consecutive sampling was conducted on 50 samples, which were then divided into two groups. Group 1 received Metronidazol therapy 2 times a day for 7 days, while group 2 received Metronidazol therapy 2 times a day supplemented with oral Probiotic of once a day for 7 days. The data were analysed by using the chi-square test. Results: There is no significant difference in the recovery of bacterial vaginosis infection between the group that received the probiotic supplementation and the group that did not. Conclusion: The addition of probiotic supplement to the standard therapy of BV infection does not show any signifcant effect. Abstract

Objective: To identify and analyze the differences in the levels of PlGF in preeclampsia and normal pregnancy. Method: This was a cross-sectional observational qualitative study of PlGF in preeclampsia and normal pregnancy. The number of samples in case and control group was 30 in each groups. Result: We found that mean of maternal age in the preeclampsia group was 28.53 years and 25.23 years in the control group. Mean parity in preeclampsia and control group was 2.33 and 1.56, respectively. Mean hemoglobin level in preeclampsia and control group was 11.97 and 11.99, respectively. Mean maternal blood glucose level was 87.0 in the preeclampsia group, and 87.9 in the control group. In the preeclampsia group mean urea concentration was 16.45, while it was 22.78 in the control group. Mean creatinine level was 0.92 in the preeclampsia group and 0.64 in the control group. Mean SGPT and SGOT in the preeclampsia group was 23.36 and 21.97, while in the control group was 29.86 and 26.20. Abstract

Objective: To determine whether HPV L1 gene methylation can be used in triage of precancerous cervical lesions. The main objective is to determine the genotype of HPV in cervical precancerous lesions and to determine the percentage, the sensitivity, specificity, positive predictive value, negative predictive value, and likelihood ratio of DNA HPV L1 methylation in precancerous cervical lesions. Methods: A number of 57 samples of paraffin blocks (FFPE) from precancerous lesions and cervical cancer biopsies in the Department of Pathology Faculty of Medicine-Cipto Mangunkusumo General Hospital that had been re-evaluated by the pathologist, underwent extraction of HPV DNA. The genotypes of HPV DNA were examined using primers GP5 / 6 and specific HPV 16, HPV 18 and HPV 52 probes and analyzed by real time PCR. Sequencing was performed on samples with unknown HPV DNA type that were detected using the specific probes to determine the type of HPV. Bisulfite conversion procedure was then performed for the samples that met the inclusion criteria. Results: There were 30 samples (52.6%) with CIN 1, 12 samples (21.1%) CIN 2, 9 samples (15.8%) CIN 3 and 6 samples (10.5%) of cervical cancer. Most of the samples were 36-45 years (35.1%). Of the total 57 samples, 55 samples were successfully extracted and determined the DNA genotyping of HPV (96.5%). HPV 16 infections both in the form of single or multiple was found to be 76.36%. The samples were mostly dominated by co-infection of HPV16 and 18 (49.1%) followed by HPV 16 (24.6%) and HPV 18 (14.0%). Based on the sequencing results there were other types of high risk HPV infection found: HPV 33, HPV 35, HPV 58 and also undeterminate risk HPV 53 and low risk HPV 54. After several procedures of optimization for methylation examination of HPV DNA L1 there was thin band found in electrophoresis procedure in 8 of 42 samples (19%) of HPV 16 after bisulfite conversion but once it was purified there weren’t any band found so we can not proceed to the stage sequencing. Until now we are still in the stage of optimizing the methylation procedure. Conclusion: HPV 16 infection were most commonly found in the form of single or multiple. Co-infection of HPV 16 and 18 were found in the majority of the samples. There were no significant correlation between HPV type and the severity of cervical lesions. Until now, the examination of DNA methylation HPV L1 already obtained eight samples of HPV 16 with a thin band on electrophoresis but the result could not be concluded because it is still in the process of optimization. Abstract

Objective: To determine whether sub chorionic hematoma increases the risks of spontaneous abortion. Method: Study using prospective cohort design, in which case group was 30 patients with threatened abortion having subchorionic hematoma during 7-20 weeks gestational age (GA), and control group was 30 patients with threatened abortion not having subchorionic hematoma. Diagnosis was confirmed using the aid of ultrasound (USG) (Medison Sondace Live Prime 8000®). The sampling method was consecutive sampling of 7-20 weeks GA pregnancy, with minimum CRL measurements 10 mm, who came to obstetrics emergency room or outpatient clinic at Sanglah General Hospital. Patients were followed until 20 weeks GA by physical and USG examination. If the evaluation results were normal, patients were managed as a normal pregnancy. But if it had the same condition as diagnosis for threatened abortion, patients were re-managed as threatened abortion or according to current diagnosis. Result: The average age of patient, gestational age upon examination at first visit and parity of case and control group were not statistically different (p>0.05). Percentage of spontaneous abortion within the case and control group was 40% and 13.33% respectively. The relative risks of spontaneous abortion were three times higher in the threatened group with subchorionic hematoma (RR=3; IK 95% = 1.09-8.25; p=0.02). Median time span of the occurence of spontaneous abortion in the case and control group was 12 and 16 weeks respectively. Conclusion: The risk of spontaneous abortion in threatened abortion with subchorionic hematoma was 3 times higher than those without subchorionic hematoma. Abstract

Tujuan: Mengetahui korelasi antara gangguan fungsi jantung berupa penurunan fraksi ejeksi dengan kadar endothelin-1 dari darah tali pusat pada pertumbuhan janin terhambat dan normal. Rancangan/rumusan data: Studi korelasi regresi dengan pendekatan potong lintang (Cross-sectional study). Tempat: IGD Lantai III Rumah Sakit Umum Pusat Nasional (RSUPN) Dr. Cipto Mangunkusumo, Jakarta. Bahan dan cara kerja: Tujuh puluh subjek, masing-masing 35 orang untuk kelompok kasus (bayi PJT) dan 35 orang kelompok bayi normal dilakukan pemeriksaan fraksi ejeksi jantung janin dengan ultrasonografi dua dimensi. Setelah bayi lahir dilakukan pemeriksaan kadar endothelin-1 (ET-1) da-rah tali pusat (a. Umbilikalis) dengan metoda ELISA. Korelasi antara fraksi ejeksi jantung janin dengan kadar ET-1 akan diuji secara statistik. Hasil: Umur pasien pada kelompok PJT berkisar antara 19-38 tahun dengan rata-rata 28,31 tahun, umur pasien pada kelompok normal berkisar antara 21-40 tahun dengan rata-rata berusia 28,54 tahun. Abstract

Objective: To evaluate the benefits of adjuvant radiotherapy (ART) based on Kartu Delgado (simple form of Gynecologic Oncology Group (GOG) scoring system) aimed at women with early stage cervical cancer after radical surgery. Method: Fifty patients were enrolled for this study. Twenty one patients from 2011-2012 were given ART following surgery based on their Kartu Delgado score from as follows: score 120 were given ART. Their score and recurrence were compared with 29 patients who were treated in 2009-2010 (based on single prognostic factor). Result: We observed eighteen recurrences for the duration this study. Thirteen patients from the period of 2009-2010 and five patients from the period of 2011-2012. Most recurrences occurred in patients from 2009-2010 with score > 120 but were not designated ART. Two-years recurrence-free survival (RFS) for subjects with score 120 with ART was 64.29%. Conclusion: Adjuvant radiotherapy given based on Kartu Delgado reduced the number of recurrences in women with stage IB-IIA cervical cancer after treated by surgery. Abstract

Objective: To assess the quality of websites providing informationon infertility and its management in Bahasa.Methods: Differences between website types and affiliates wereassessed for the credibility, accuracy and ease of navigation usingpredefined criteria. We used Google search engine with the keyword"infertilitas" and we assessed 50 websites in Bahasa that relates withinfertility.Results: The content credibility for most of the sites has adequatescore with range of score 60 to 80 for 68% sites. Content accuracyfor most of the sites have scores more than 60, with 24% or 12sites with scores 60 to 80 and 44% or 22 sites have scores above80. Abstract

Objective: To analyze HLA-G, Hsp-70, and VCAM-1 proteins expression on trophoblast in women with preeclampsia and women with normal pregnancy, and to assess causality relationship between low HLA-G expression and the increased Hsp-70 and VCAM-1 expression in preeclampsia. Method: Observational analytic study with cross sectional design. Results: Trophoblastic HLA-G expression in pregnant women with preeclampsia is lower (p = 0.01) than its expression in women with normal pregnancy. The expressions of Hsp-70 (p = 0.02) and VCAM-1 (p = 0.00) in preeclampsia are higher, compared to normal pregnancy. Regression analysis showed that low HLA-G is a predictor for the increase of trophoblastic Hsp-70 and VCAM-1. Conclusion: Low trophoblastic HLA-G in is a predictor for endothelial dysfunction in women with preeclampsia. Abstract

Tujuan: Menilai korelasi antara kadar progesteron pada hari penyuntikan hCG dengan reseptivitas endometrium pada hari transfer embrio (TE). Bahan dan cara kerja: Penelitian ini merupakan uji diagnostik dengan desain cross sectional. Dilakukan pengukuran kadar progesteron hari hCG pada 55 pasien yang mengikuti program FIV di Klinik Yasmin RSCM perio-de Januari - November 2008. Nilai referensi reseptivitas endometrium adalah kombinasi antara nilai indeks pulsasi arteri uterina < 3 dan morfologi endometrium klasifikasi C (Gonan dan Casper). Analisis multivariat dilakukan terhadap progesteron sebagai variabel utama dan usia, kadar estradiol, LH serta reseptivitas endometrium hari hCG sebagai variabel perancu. Hasil: Kejadian luteinisasi prematur pada penelitian ini sebesar 45,5% dengan klasifikasi endometrium reseptif sebesar 33,3% (dibandingkan dengan 35,5% pada kelompok non luteinisasi prematur). Tidak terdapat hubungan yang bermakna antara kadar progesteron hari hCG dengan reseptivitas endometrium hari TE (p=0,446). Reseptivitas endometrium hari hCG memiliki nilai area under the curve (AUC) terbesar (0,82; p = 0,001) untuk meramalkan reseptivitas endometrium hari TE dibandingkan dengan usia (AUC 0.67; p = 0,038) dan kadar progesteron (AUC 0,56; p = 0,446). Analisis multivariat mendapatkan variabel reseptivitas endometrium hari hCG dan interaksi antara variabel usia de-ngan kadar progesteron hari hCG sebagai prediktor reseptivitas endo-metrium hari TE (nilai AUC 0,82). Terdapat korelasi positif antara AUC kadar estradiol hari hCG terhadap kejadian luteinisasi prematur (AUC = 0,74). Kesimpulan: Kadar progesteron hari hCG tidak dapat digunakan sebagai prediktor tunggal untuk meramalkan reseptivitas endometrium hari TE. Reseptivitas endometrium hari hCG dan interaksi antara usia dengan kadar progesteron hari hCG merupakan model prediktor yang baik terhadap reseptivitas endometrium hari TE.
