Abstract

Background: Systemic lupus erythematosus (SLE) is one of the pregnancy complications which cause an increase in fetal and maternal complications which are mainly caused by placental damage due to chronic inflammation and apoptosis. The rate of apoptosis can be assessed by the high expression of caspase-3 which is the majorexecutioner caspases of apoptosis. The use of Mesenchymal Secretome Stem Cell for SLE therapy has an anti-inflammatory and anti-apoptotic role. This study aimed to examine the effect of mesenchymal secretome stem cell therapy toward caspase-3 expression in pregnant mice with pristan-induced lupus model.Subjects and Method: This study was a randomized controlled trial. This study was conducted at the Prodia ProStem Laboratory, Laboratory of the Faculty of Veterinary Medicine of Airlangga University, and Anatomical Pathology Laboratory, Faculty of Medicine, Airlangga University. A total of 14 healthy BALB/C female mice strain aged 6-8 weeks, with a bodyweight of 20-30 grams, were divided into two groups, (1) 7 pregnant mice who received pristan therapy only, and (2) 7 pregnant mice who received pristan therapy and mesenchymal secretome stem cell. The dependent variable was the caspase-3 expression. The independent variable was the administration of mesenchymal secretome stem cell therapy. The caspase-3 expression was measured by an immunoreactive score. The data were analyzed using independent t-test.Results: After giving mesenchymal secretome stem cell therapy, the mean of the caspase-3 expression in the treatment group (Mean=1.86; SD=0.66) was lower than the control group (Mean=3.30; SD = 1.42), with p=0.031.Conclusion: Mesenchymal secretome stem cell therapy is effective for reducing caspase-3 expression in the placenta of pregnant mice with pristan-induced lupus model. Keywords: Caspase-3 expression, lupus, placentaCorrespondence: Febrian Andhika Adiyana. Abstract

Background: Patients with Type 2 Diabetes Mellitus have a high risk of serious complications. Complications resulting from diabetes can be minimized and prevented by implementing self-care behavior. The study revealed that one of the factors that affect self-care activity is family support. The correlation of family support with self-care activity has been extensively investigated in several primary studies with several contradictions in the results. This study aimed to determine the correlation between family support and self-care activity in people with type 2 diabetes mellitus using the meta-analysis method.Subjects and Method: This study was a systematic review and meta-analysis. The meta-analysis was carried out by systematically reviewing articles published from 2010 to 2020 from the Pubmed, Google Scholar, DOAJ, and Springer Link databases using search keywords, namely ("Family Support" OR "Social Support") AND ("Self Management "OR" Self Care ") AND" Diabetes Mellitus Type 2". Article searches were carried out using PICO. The study population was people with type 2 diabetes mellitus. Intervention/exposure was getting family support compared to not getting family support, and the outcome was diabetes mellitus self-care. The search for articles was carried out for one month. The articles included in this study were full-text articles with a cross-sectional design. Articles were reviewed using the PRISMA flow diagram guidelines. Articles analyzed using Revman 5.3 Software. Conclusion: People with type 2 diabetes mellitus with good family support will increase self-care activity 2.22 times compared to people with type 2 diabetes mellitus who do not get family support. Abstract

Background: Extended-spectrum β-lactamase (ESBL) is an enzyme produced by certain Escherichia coli and Klebsiella pneumoniae. ESBL can induce theantibiotic resistance for betalactam antibiotics such as penicillin, cephalosporin, etc. Frequent utilization of carbapenem may induce the risk for carbapenem resistant against ESBLs producing microorganisms. This stud aimed to communicate Aminoglycocides as an alternative antibiotic therapy against urinary tract infection in children.Case Presentation: We present a case of 4 years child who was admitted to the state hospital of Madiun with main complaint of 10 days high fever accompanied by phimosis appearance. Laboratory results based on urine culture show the ESBL positive Escherichia coli. Results: We have used aminoglycoside for treating the patient. Clinical presentation in this patient start showing an improved clinical response and negative bacteriuria in the second urine test after the aminoglycoside therapy. Conclusion: Aminoglycosides such as gentamicin can be considered to be one of the best options for treating an ESBL infection beside carbapenem in order to reduce the carbapenem trend In clinical setting of the urinary tract infection caused by ESBL Eschericia coli. Keywords: Aminoglycosides, Carbapenem, Extended-spectrum β-lactamase, Gentamicin, urinary tract infectionCorrespondence: Muhammad Pradhika Mapindra. Abstract

Background: Myeloperoxidase (MPO) levels have been shown to have prognostic values for death events in acute myocardial infarction (AMI) patients. Increased levels of high sensitive troponin I (hs Trop I) are associated with poorer prognosis in AMI patients. This study aimed to determine the association between levels of MPO and hs troponin I at admisson on myocardial contractility in AMI patients.Subjects and Method: This was a prospective observational analytic study, conducted at Dr. Moewardi General Hospital, Surakarta, from November1, 2018 to January 31, 2019. The study subjects were patients who were diagnosed with AMI. A total of 23 patients were included in this study; 15 patients with ST elevation myocardial infarction (STEMI) and 8 with non ST elevation myocardial infarction (NSTEMI). The dependent variable was myocardial contractility. The independent variables were MPO and hs Trop I. Blood tests for MPO and hs Trop I was measured at admission and the left ventricular ejection fraction (LVEF) was measured when predischarge by the Simpson method to examine myocardial contractility. The data were analyzed by Pearson correlation run on SPSS 22.  Results: hs Trop I (r= -0.53; p= 0.009) and MPO (r= -0.15; p=0.487) decreased LVEF.Conclusion: Hs Trop I at admission has a correlation with myocardial contractility, and no correlation of MPO at admission with myocardial contractility in AMI patients. Keywords: Myeloperoxidase, High sensitive Troponin I, Acute myocardial infarctionCorrespondence: Trisulo Wasyanto. Abstract

Background: Polycystic Ovarian Syndrome (PCOS) is an endocrine disorder in women characterized by increased levels of androgens, ovulatory dysfunction, and Polycystic Ovarian Morphology (PCOM), as well as a combination of classic clinical features. Women with PCOS experience changes in the endometrium and endometrial receptivity markers such as VEGF (Vascular Endothelial Growth Factor). VEGF increases the permeability and dilation of endometrial blood vessels which play an important role in implantation. This study aimed to analyze the difference of VEGF expression between women with PCOS and normal women.Subjects and Method: This study was a case-control study design conducted at the polyclinic of the Department of Obstetrics and Gynecology, Dr. Moewardi Hospital, Surakarta, Central Java, in July 2020. There were 30 infertile women with PCOS (diagnosis based on Rotterdam criteria, consensus 2003) and 30 women without PCOS as the sample of this study. The dependent variable was VEGF expression. The independent variables were age, BMI, family history with PCOS, menarche, occupation, and education. VEGF expression was obtained from an endometrial biopsy on days 19-24 of periods. The differences in VEGF expression (in %) were analyzed by the Mann-Whitney test. VEGF determinants were analyzed by a multiple linear regression. Results: The mean of VEGF expression in women with PCOS (Mean=64.33; SD=44.43) was higher than in women without PCOS (Mean= 7.90; SD=8.73) and the result was statistically significant (p<0.001). PCOS (b=42.09; 95%CI=64.26 to -19.93; p<0.001) and obese (b=38.63; 95%CI=-63.44 to -13.82; p=0.003) increased VEGF expression. Conclusion: VEGF expression in the endometrium of women with PCOS is higher than normal women. Keywords: VEGF, endometrium, Polycystic Ovarian SyndromeCorrespondence: Ricky Bernadi Saputra.
