Abstract

Background:  Tuberculosis (TB) is the most common cause of death in patients with the Human Immunodeficiency Virus (HIV). Understanding CD4 as a predictor of mortality from TBHIV coinfection is critical to improving disease management and minimizing mortality. This study aims to examine the mortality risk of TBHIV-coinfected patients based on CD4 values.Subjects and Method:  Metaanalysis was performed according to the PRISMA flow chart and the PICO model (Population: TBHIV coinfected patients, Intervention: CD4 value <200 cells/µL, Comparison: CD4≥200 cells/µL, Outcome: mortality). The databases used are Google Scholar, PubMed, Scopus, Proquest, and Science Direct. Keywords used (coinfection OR "mixed infection") AND (HIV OR "Human Immunodeficiency Virus") AND (TB OR “tuberculosis”) AND ("CD4 count" OR "T4 lymphocyte") AND “mortality”. The inclusion criteria were full-text articles with cohort studies published in 20122022, articles in English, and multivariate analysis using the adjusted hazard ratio. Analysis was performed with Revman 5.3.Results:  17 articles with cohort design originating from Ethiopia, Khayelitsha, Kenya, Cape Town, Uganda, Brazil, Suriname, Sao Paulo, Guinea Bissau, Myanmar, Durban, Ireland, China, and multicountry studies between Europe and Latin America totaling 24,514 research sample. A meta-analysis of 10 study cohorts concluded that CD4<200 cells/µL had a 2.00 times risk of mortality compared with CD4 values ≥200 cells/µL (aHR=2.00; 95% CI 1.44 to 2.78; p<0.001). A meta-analysis of 7 study cohorts concluded that CD4<100 cells/µL had a 2.40 time risk of mortality compared with CD4 values≥200 cells/µL (aHR=2.40; 95% CI 1.61 to 3.57; p<0.001). A meta-analysis of 7 study cohorts concluded that CD4<50 cells/µL had a 3.12 times risk of mortality compared with CD4 values≥200 cells/μL (aHR=3.12; 95% CI 1.51 to 6.46; p<0.001).Conclusion:  Decreased CD4 values increase the risk of mortality.Keywords:  tuberculosis, HIV, CD4, mortality, meta-analysisCorrespondence: Victoria Husadani Permata Sari. Abstract

Background: Diabetes Mellitus is character­rized by metabolic disturbances due to pro­longed hyperglycemia, causing oxidative stress, which destroys pancreatic β cells. Adjuvant therapy that has antihyperglycemic effective­ness and is required to improve the diameter of pancreatic beta cells, one of which comes from Sechium edule (Jacq.) Swartz has poten­tial as an antihyperglycemic, antioxidant, anti­apop­tosis, cardioprotective, insulin resistance. This study aimed to examine the effectiveness of ethanol extract, Chayote (Sechium Edule (Jacq.) Swartz) fraction, and juice on the pancreatic β-cell diameter of white male rats Wistar strain with type 2 diabetes mellitus.Subjects and Method: This was a rando­mized controlled trial. Sample was Rattus novergius sp. 54 tails. The dependent variable was the diameter of the pancreatic β cells. The independent variables were ethanol extract, Sechium edule (Jacq.) Swartz fraction and juice, at a dose of 50 mg/kg BW, 100 mg/ kg BW, 150 mg/kg BW. The data were analyzed by the ANOVA test.Results: The group of mice induced by Strep­tozotocin 50 mg/kg BW + nicotinamide (120 mg/ kg BW) + HFD and obtained ethanol extract of chayote fruit 150 mg/ kg BW, orally, had the highest pancreatic β cell diameter com­pared to the other groups (Mean= 284.03; SD= 5.15).Conclusion: Sechium edule (Jacq.) Abstract

Background: Knee OA can affect mobility in the elderly causing pain and impaired functional ability. This study aims to analyze the effect of aquatic therapy on functional ability in knee osteo­arthritis patients based on the results of a number of previous primary studies..Subject and Method: This study was conducted using a meta-analysis method with articles searched using the search keywords "knee osteoarthritis" OR "osteoarthritis genu" AND "aquatic therapy" OR "hydrotherapy" OR "water exercise" AND "functional ability" OR "WOMAC" AND "Randomized Controlled Trial” OR “RCT” from PubMed, google scholar and science direct and processed using RevMan 5.3.Results: 9 RCT articles that fit into the inclusion criteria, with a total of 573 samples. Knee OA patients receiving aquatic therapy had a mean WOMAC score of 1.22 units lower than those without aquatic therapy, and the effect was statistically significant (SMD = -1.22; 95% CI -2.03 to -0.41; p = 0.003). In other words, aquatic therapy is effective in improving functional ability in knee OA patients.Conclusion: Aquatic therapy has an effect on increasing functional ability in knee osteoarthritis patients.Keywords: Osteoarthritis, aquatic therapy, WOMAC indexCorrespondence: Wita Hana Puspita. Abstract

Background: Due to its strong association with major morbidity and mortality in the elderly, such as heart attack, stroke, kidney disease, and death, hypertension is currently one of the health problems associated with non-communicable diseases. It is certainly influenced by various vari¬ables, one of which is one’s cardiorespiratory fitness which can affect blood pressure. The study aims to find out if there is a correlation between cardiorespiratory fitness and blood pressure in the elderly.Subjects and Method: The cross-sectional study was conducted in Pepe Village, Kwarasan, Juwiring, Klaten, Central Java. A sample of 31 elderly people was selected by using purposive sampling. The dependent variable was blood pressure. The independent variable was cardiorespiratory fitness. Cardiorespiratory fitness was measured by the Six Minute Walking Test (SMWT). Blood pressure was measured with a sphygmomanometer. The correlation between cardiorespiratory fitness and blood pressure was tested using Pearson correlation and Spearman rho with a 95% confidence interval and p<0.050 statistical meaningfulness.Results: Cardiorespiratory fitness lowered systolic blood pressure by 0.14 mmHg (r= -0.14; p= 0.442) and diastolic blood pressure by 0.55 mmHg (r= -0.55; p= 0.771), but both were statistically insignificant.Conclusion: There was no relationship between blood pressure and physical fitness in the elderly.Keywords: hypertension, blood pressure, cardiorespiratory fitness, six-minute walking test, elderly.Correspondence: Farid Rahman. Abstract

Background: Dysmenorrhea is the most common cause of lower abdominal pain and has a negative impact on women's quality of life resulting in activity limitations. Non-pharmacological management can be used as an intervention in the management of dysmenorrhea. Acupressure is a therapy that is given by massaging or pressing certain points. The purpose of this study was to analyze the effect of acupressure therapy on reducing dysmenorrhea pain.Subject and Method: This study was a meta-analysis with the following PICO, population: Women aged 16-30 years old with primary dysmenorrhea. Intervention: administration of Acupressure Therapy. Comparison: Acupressure therapy was not performed. Results: Decreased pain in primary dysmenorrhea. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles “Acupressure” OR “Acu­pressure Therapy” AND “Sanyinjiao Point (SP6)” AND “Hequ Point (LI4)” AND “Dysmenorrhea” OR “Primary Dysmenorrhea” OR “Menstrual Pain” AND “Randomized Controlled Trial”. The articles included are full-text English with a Randomized Control Trial study design from 2007 to 2022. The articles were selected using PRISMA flow diagrams. Articles were analyzed using the Review Manager 5.3 application.Results: Meta-analysis of 9 randomized controlled trial articles from Asia and Europe concluded that acupressure therapy can reduce pain in primary dysmenorrhea (SMD= -0.66; 95% CI = -0.96 to -0.36; p < 0.001).Conclusion: Acupressure therapy can affect pain reduction in women with complaints of primary dysmenorrhea.Keywords: Acupressure, pain relief, primary dysmenorrheaCorrespondence: Anisya Fajar Rahmawati. Abstract

Background: Brain tumor is a disease that is difficult to treat and causes high morbidity and morta¬lity. One of the clinical manifestations of brain tumors is cognitive impairment which is the most common neurological problem. The aim of this study is to determine the profile of cognitive impairment in patients with brain tumors.Subjects and Method: The design of this study was a retrospective cross-sectional using secondary data from the Neurology Polyclinic of RSUD Dr. Moewardi in January 2021-March 2022. The subject was diagnosed with a brain tumor based on anamnesis, physical examination, and neuroimaging. Cognitive impairment was inferred through the MoCA-Ina test. The analysis used was univariate descriptive analysis, independent T test, Mann-Whitney test, and Pearson correlation test.Results: There were 29 subjects with a mean MoCA-Ina score (17.97). Primary brain tumors (79.3%), more than metastatic tumors. The majority of patients were diagnosed with meningioma (55.2%). This study showed that there were differences in abstraction scores (p=0.015) and total MoCA-Ina scores (p=0.042) between patients with tumors located in the temporal lobe and non- temporal lobe; differences in abstraction scores (p=0.034) and orientation scores (p=0.042) between patients with supratentorial and infratentorial tumors; and differences in memory scores (p=0.028) between patients with and without radiation history. In addition, this study also found an association between the number of lobes affected by brain tumors with attention score (p=0.027; r=-0.409), abstraction score (p=0.004; r=-0.524), orientation score (p=0.021; r=-0.426), and the total score of MoCA-Ina (p=0.018, r=-0.435).Conclusion: There is an association between brain tumors and cognitive impairment which is concluded through the MoCA-Ina test. The clinical manifestations of cognitive impairment in the patient are in accordance with the neuroanatomical function of the brain affected by the lesion.Keywords: Cognitive, Tumor, MoCA-InaCorrespondence: Maria Yosita Ayu Hapsari. Abstract

Background: Covid-19 has been declared a pandemic since March 2020 until now, so the government requires the provision of a covid-19 vaccine for all people. The government's vaccination campaign has received various responses from the public, some people accept and some reject the Covid-19 vaccine. This study aims to determine the relationship between public perception of the acceptance of the COVID-19 vaccine in Surakarta.Subjects and Methods: The cross sectional study was conducted in Surakarta. A sample of 65 people aged >12 years was selected by random sampling. The dependent variable was the willingness to receive the covid-19 vaccine. Tthe independent variable was perception. Data was collected by online questionnaire (google form). Data were analyzed by chi square test.Results: There is a relationship between public perception of vaccination and acceptance of the covid-19 vaccine (OR=5.45 ; 95% CI= 2.57 to 11.53 ; p<0.001)Conclusion: There is a significant relationship between perceptions related to vaccines and people's willingness to receive the COVID-19 vaccine.Keywords: covid-19, acceptance, knowledge, rejection, vaccinationCorrespondence: Annita Viesta Nirmala Dewi. Abstract

Background: Exercise generally improves the quality of life (QoL) in adult populations especially for patients with chronic health problems such as breast cancer. The purpose of this study was to investigate the effect of physical exercise/activity on the quality of life of breast cancer survivors. Subjects and Method: This was a systematic review study that used articles from online data­bases, such as  PubMed, Science Direct, ProQuest dan Medline, which was published from 2000 to 2021. The author used keywords of “physical exercise”, “physical activity”, “quality of life”, “breast cancer survivor”. The dependent variable was quality of life. The independent variable was physical exercise/ activity. As many as 5 articles were selected for this study from 837 articles located in South Korea, Iran, Malaysia, and China.Results: Moderate or high levels of physical activity had positive relationship on the quality of life related to overall physical health among breast cancer survivors.Conclusion: Moderate to high physical activity improve the quality of life among breast cancer survivors.Keywords: physical exercise, physical activity, quality of life, breast cancer survivor Correspondence: Arief Wahyudi Jadmiko. Abstract

Background: Acupuncture therapy is an effective treatment for reducing headache intensity, migraine recurrence, and improving quality of life. This study aims to analyze and estimate the effect of acupuncture therapy on reducing migraine recurrence.Subjects and Method: This study used a systematic review and meta-analysis by following PICO, Population: migraine patients. Intervention: acupuncture therapy. Comparison: no acupuncture therapy. Result: reduction in migraine recurrence. The data used were obtained from scientific research articles from the Pubmed, Google Scholar, Science Direct, and SpringerLink electronic databases with a Randomized Control Trial (RCT) design from 2011 to 2022 which reports effect sizes with Mean and SD. The keywords used in the search for scientific articles are “Acupuncture” AND “Profilaxis Migraine” AND “Migraine Frequency”. The selection of articles was carried out using the PRISMA flowchart. Data were analyzed using Review Manager software version 5.4.1.Results: A total of 9 articles with qualified RCT designs from Asia, America, Australia, and Europe were selected for the systematic review and meta-analysis. The results of the meta-analysis showed that patients with migraine who received acupuncture therapy experienced migraine recurrence 0.31 units lower than those who were not treated with acupuncture (SMD= -0.31; 95% CI= -0.61 to -0.01; p=0.040).Conclusion: Acupuncture therapy reduces migraine recurrence.Keywords: acupuncture, relapse, migraine.Correspondence: Elsa Tursina. Abstract

Background: Condyloma acuminatum (CA) is one of the most common sexually transmitted infections. Several therapeutic modalities that can be used are chemosurgery with trichloroacetic acid and cryosurgery with liquid nitrogen. Recurrence in CA often occurs, so selecting the right therapeutic modality and eliminating the lesion also prevents recurrence. This study aimed to compare the effectiveness of chemosurgery and cryosurgery therapy in patients with CA who are treated at Dr. Moewardi Hospital, Surakarta.Subjects and Method: A retrospective cross-sectional study was conducted at the Dermatology and Venereology Polyclinic, Dr. Moewardi Hospital, Surakarta, from January 1, 2018, to December 31, 2020. The study subjects were 78 patients diagnosed with CA based on ICD-10 criteria, while chemosurgery and cryosurgery procedures were based on ICD-9. The dependent variable was healing time in weeks. The independent variables were chemosurgery and cryosurgery therapy. The data came from the patient's medical record. Data were analyzed using an independent t-test.Results: Based on univariate analysis, most patients were male (67.9%), and the age group was 20-29 years (51.28%). Most of the CA patients work as private employees (46.15%). The most sexual orientation was heterosexual (53.8%), the most common site of lesions was genital (47.4%), and 52.6% were coinfected with HIV. Most CA patients received chemosurgery (70.5%). Bivariate analysis showed that chemosurgery therapy provided faster clinical improvement (Mean= 23.34; SD= 26.45) than cryosurgery therapy (Mean= 28.69; SD= 19.84), and the difference was statistically significant (p= 0.037).Conclusion: Treatment of CA using chemosurgery provides clinical improvement faster than cryosurgery, so that faster treatment time can reduce treatment costs and improve the patient's quality of life.Keywords: chemosurgery, cryosurgery, HPV, condyloma acuminatumCorrespondence: Prasetyadi Mawardi.
