Journal of Health Policy and Management (JHPM) is an electronic, open-access, double-blind and peer-reviewed international multidisciplinary and integrative journal, focusing on health policy, health system, and healthcare management. It aims to improve the design and implementation of health policies, health systems, and healthcare management, primarily in low- and middle-income countries. Abstract

Background: In the health insurance system, there are 2 main pillars, namely service quality and financing control. The quality of health services can be identified by measuring the suitability of the service with the agreed clinical pathway. Data from SEAMIC Health Statistics 2011 shows that pneumonia is the 6th cause of death in Indonesia, therefore a good quality clinical pathway is needed. Integrated Care Pathway Appraisal Tool (ICPAT) is one of the instruments that has been validated and can be used to evaluate the content and quality of ICP.Subjects and Method: Study design using qualitative methods and with a case study research design. Population: quantitative data population from all medical records, qualitative data population the researchers involved all officers involved in implementing the clinical pathways. Instrument: List of questions to evaluate the implementation of the use of clinical pathways and ICPAT. Validity: with triangulation techniques. Outcome: compliance assess-ment and recommendations for improvement to maintain quality. Results: Input Aspect, In Dimension 1 What are the Correct Clinical Pathways? it the percentage content was 50%, and the quality was 50%. Dimension 6 (Organizational Role?) in the CP content value is 100% and the quality is 58%. Process aspect, Dimension 2 (Clinical Pathway Documentation Process) CP documentation process in terms of content is 30.43% and quality is 25%. Dimension 3 (Clinical Pathway Development Process) the content is 61.54% and the quality is 41.18%. Dimension 4 (Clinical Pathway Implementation Process) the percentage of content is 60% and the quality is 100%. Dimension 5 (Maintenance Process of clinical pathways) the percentage of the content is 25% and the quality is 38.46%. Abstract

Background: Puskesmas plays an important role in reducing maternal and neonatal morbidity and mortality. Contextual health centers and dimensions of service quality can improve maternal and child health services. This study aimed to analyze the factors that influence patient satisfactionSubjects and Method: A cross sectional study was conducted in Klaten Regency, Central Java. A sample of 200 pregnant women at the community health centers was selected by simple random sampling. The dependent variable was patient satisfaction. The independent variables were reliability, tangible, empathy, responsiveness, and assurance. The data were analyzed by multiple multilevel linear regression using the Stata 14 application.Results: Patient satisfaction was influenced by reliability (b=0.30; 95% CI= -0.48 to -0.13; p= 0.001), tangible (b= 0.28; 95% CI= 0.14 to 0.42; p<0.001), responsiveness (b= 0.17; 95% CI= 0.07 to 0.27; p= 0.001), and assurance (b= 0.16; 95% CI= 0.04 to 0.28; p= 0.007), while empathy has no effect on patient satisfaction (b= -0.02; 95% CI= -0.28 to 0.23; p= 0.837). Health center had strong contextual effect on patient satisfaction with ICC= 36.4%.Conclusion: Patient satisfaction is influenced by reliability, tangible (physical evidence), responsiveness, and assurance, while empathy has no effect on patient satisfaction.Keywords: satisfaction, puskesmas, multilevel linear regressionKorespondensi:  Leny Rahmawati. Abstract

Background: Maintaining hospital quality is the most important thing to consider in hospital service indicators. Patient satisfaction is one of the important factors in maintaining hospital quality. This study aimed to analyzed the relationship between nursing management in patients statisfaction.Subjects and Method: This study is a meta-analysis with PICO, population: inpatients. Intervention: nursing management. Comparison: not using nursing management. Outcome: patients statisfaction. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles are “Nursing Management” OR “Nursing Care” AND “Patient Statisfaction” OR “Statisfaction” AND “Multivariate”. Articles included are full-text English from 2004 to 2021. Articles were selected using a PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 7 cross-sectional study articles from Asia (Indonesia, Iran), Europe (Germany), Africa (Ethiophia), and North America (United States) were reviewed in the meta-analysis. Based on the results of the analysis, it was found that the application of nursing management in hospitals has the possibility to increase patient satisfaction 3.72 times compared to not implementing comprehensive nursing management (aOR= 3.72; 95% CI= 2.40 to 5.78; p<0.001) and the results were statistically significant.Conclusion: Nursing management affect the possibility of patients statisfaction.Keywords: nursing management, patient statisfaction, inpatientsCorrespondence: Titik Anggraeni. Abstract

Background: The development of technology shows a very rapid development, especially in the health sector. One example of the use of technology in the health sector is the electronic health record. Electronic health records provide several benefits, one of which can reduce mortality and readmission rates in patients. The purpose of this study was to estimate the effect of electronic health records on mortality and readmission rates by metaanalysis.Subjects and Method: This was a metaanalysis study using PRISMA flowchart guidelines. The article search process was carried out between 20112022 using databases from PubMed, Google Scholar, ProQuest, Science Direct and Scopus. The PICO formula used is P = patients with asthma. I= using internetbased selfmanagement. C= without using internetbased selfmanagement. O= asthma control. Article searches were performed using the keywords “mhealth” OR “mobile health” OR “telemedicine” AND “self management” AND “asthma control” OR “asthma treatment” AND “asthma control”. The inclusion criteria were full paper articles with randomized controlled trial study design, articles using English, the intervention provided was the application of internetbased selfmanagement, and the outcome was asthma control. Based on the database, there were 9 articles that met the inclusion criteria. The analysis was carried out using Revman 5.3 software.Results: A total of 13 articles spread across 2 continents, namely Asia (Taiwan, Singapore, and South Korea) and North America (South America). Articles reviewed in the metaanalysis showed that electronic health records had an effect on reducing mortality by 0.74 times compared to those without using electronic health records (aOR= 0.74; 95% CI= 0.64 to 0.86; p<0.001). In addition, it was also found that electronic health records had an effect on reducing readmission by 0.77 times compared to without using electronic health records (aOR= 0.77; CI 95%= 0.62 to 0.95; p= 0.010)Conclusion: The application of electronic health records has an effect on reducing mortality and readmission rates.Keywords: Electronic health record, mortalitas, readmissionCorrespondence: Desi Syahbaniar. Abstract

Background: Stroke is a disease that is the third leading cause of death in the world after heart and cancer. Stroke is also a disease that causes serious and permanent disability in the first order in the world. The rehabilitation program or recovery of stroke patients will greatly affect the improvement of functional abilities and independence of post-stroke patients, one of the keys to functional improvement is the intensity of therapy and family involvement in supporting the rehabilitation process. This study aims to analyze the relationship between empowering home-based recovery interventions to improve the functional ability of post-stroke patients, with a meta-analysis of primary studies conducted by previous authors.Subjects and Method: This study is a systematic review and meta-analysis with the following PICO, Population: stroke patients. Intervention: home-based recovery. Comparison: not home based recovery. Outcomes: functional ability. The articles used in this study were obtained from several databases, namely PubMed, Springer Link, Cochrane Database, Hindawi, Elsevier, JBBA, LWW, Google Scholar. Search keywords “home-based” AND “intervention” OR “rehabilitation” OR “exer­cise” OR “therapy” OR “care” AND “stroke” AND “functional ability” OR “functional capacity” AND “Randomized Controlled Trial “ OR “ RCT". The articles included are full-text English and Indo­nesian languages with a study design of a Randomized Controlled Trial from 2000 to 2021 and reporting on the Mean and SD in multivariate analysis. The selection of articles is done by using PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 9 randomized controlled trials from Iran, Turkey, Australia, Taiwan, China, Thailand and the Netherlands were selected for a systematic review and meta-analysis. The data collected showed that the home-based recovery intervention improved functional ability in post-stroke patients compared to usual care (SMD = 2.70; 95% CI = 1.53 to 3.87; p<0.001). Abstract

Background: Telehealth in the setting of patient with stroke or widely known as a telestroke has been researched and practiced in various ways in past decades, but recently with the appearance of pandemic Covid-19 the utilization of telestroke needs to synthesis of the existing literature. This has been reported that there are changes in the utilization of of technology-based health services, includes in the use of telestroke. This study aims to review the utilization of telestroke during this recent pandemic Covid-19. Subjects and Method: This study used scoping review as a method to answer the research question. The study was permormed by searching PubMed, Science Direct, BMJ, and Wiley Online Library databases from 2020 to June 2022. The studies that investigating the uses of telestroke during the period of pandemic Covid-19 were included to this scoping review. The PEOS framework and PRISMA flowchart has been utilized as a method to identifying and selecting the studies, as well as inclusion and exclusion criteria. Results: There were 8 studies that relevant to inclusion criteria and reviewed in this study. The majority of telestroke studies were observational study using retrospective method. Five studies stated that the decreased number of utilization of telestroke during pandemic. Demographic characteristics of the study shown the mean age were >60 years old (87.5%), female patients were more than male (62.5%), primary diagnosed with ischemic stroke (87.5%), severity from mild to severe (62.5%), stroke patients were received thrombolysis (75%), and two studies stated shorter duration for patient to receive treatment.Conclusion: Telestroke has been proven as an effective way in caring patient with stroke. Significant decreased number of telestroke admission was shown from most of the studies due to the Covid-19 while the explanation for this phenomena is still unclear.Keywords: Telestroke, Covid-19, Scoping Review.Correspondence: Nandang DD Khairari. Abstract

Background: Sultan Agung Islamic Hospital Semarang, which is one of the providers in the health sector, has complex and continuous activities which in turn have the potential to cause various impacts and risks, such as work accidents, nosocomial infections, and occupa­tional diseases. Therefore, more detailed obser­vations are needed on the enforcement of occupational health and safety regulations to minimize these impacts. This study aims to determine the application of occupational health and safety hospital regulations to mini­mize work accidents at Sultan Agung Hospital, Semarang.Subjects and Method: This study was con­ducted using a qualitative approach. The key informants in this study were the Director of the Sultan Agung Islamic Hospital totaling 1 person and the supporting informants were the field staff of the Sultan Agung Hospital totaling 17 people. Data collection techniques were carried out by direct observation, question­naires and focus group discussions. Abstract

Background: Health insurance is the answer to the uncertainty of the occurrence of illness and the need for health services. To ensure that the need for health services can be adequately financed, a person or small group of people transfers risk to another party called an insurer/insurer, or an insurance agency.Subjects and Method: This was a systematic review and meta-analysis conducted with PRISMA flow diagrams. Search articles through journal databases including: Google Scholar, MEDLINE/ PubMed, Science Direct and Spinger Link by selecting articles published in 2000-2021 The keywords used are “health care”AND “outpatient” OR “insurance” AND “private health insurance” AND “outpatient” OR “health insurance” AND “private health insurance” AND “utilization of public health care” AND “inpatient service” AND “outpatient service” AND “logistic regression aOR”. The inclusion criteria were full paper articles with retrospective study cohort research methods, the analysis used was multivariate with adjusted Odds Ratio (aOR), the intervention provided was health insurance, the research subjects were private health insurance users. Eligible articles were analyzed using the RevMan 5.3 application.Results: A meta-analysis of 9 articles showed that people with private health insurance were 1.98 times more likely to use outpatient services than those without health insurance, and the effect was statistically significant (aOR = 1.98; 95% CI = 1.39 to 2.81; p<0.001).Conclusion: The use of private health insurance has a statistically significant effect in increasing outpatient use.Keywords: health insurance, private health insurance, outpatient, meta analysis.Correspondence: Sulaiman Putra Nagaring. Abstract

Background: Organizational change is the embodiment of changing organizational environ­ments, structures, cultures, technologies, or people. Changes are planned to make the organization more effective and efficient, but there is still resistance from members of the organization. Negative reactions arise because changes provide pressure, stress, and uncertainty for employees. The goal in the study was to analyze employees' perceptions of commitment, readiness, and resistance during times of change to the implementation of the "MARMER" program.Subjects and Method: The study was conducted at Dr. Iskak Tulungagung Hospital. The selected hospital organization is a hospital that has a program preventing antimicrobial resistance through restriction efficiency (MARMER) for the control of antibiotic resistance. Research design uses quantitative research methods. The population in this study is all health workers at Dr. Iskak Tulungagung Hospital. Data collection through questionnaires. Multivariate data analysis using SEM with SmartPLS.Results: Commitment to change has a significant direct influence on the readiness for change (t-values= 9.95 > 1.96; Standard Coefficiency= 0.53). Commitment to change does not significantly affect change resistance (t-values = 1.22< 1.96; Standard Coefficiency= 0.13). Readiness for change has a negative effect on change resistance (t-values= -5.30 >1.96; Standard Coefficiency= -0.60). Commitment to change negatively and significantly affects resistance to change through readiness to change (t-values = -4.24 > 1.96; Standard Coefficiency= -0.36).Conclusion: Commitment directly affects the readiness of change. Commitment and readiness affect the resistance of change. This study aimed to analyzed the strength of the relationship between hospital administration with quality of services.Subjects and Method: This study is a meta-analysis with PICO, population: patients. Intervention: good hospital administration. Comparison: poor hospital administration. Outcome: quality of services. The articles used in this study were obtained from three databases, namely Google Scholar, Pubmed, and Science Direct. Keywords to search for articles are “Hospital Administration” OR “Good Administration” OR “Hospital Services” AND “Quality of Services” OR “Services” AND “Multivariate”. Articles included are full-text English from 2009 to 2021. Articles were selected using a PRISMA flow diagram. Articles were analyzed using the Review Manager 5.3 application.Results: A total of 8 cross-sectional study articles from Asia, Europe, Africa, and North America were reviewed in the meta-analysis. Based on the results of the analysis, hospital management or good hospital administration has the possibility to increase the quality of service in hospital by 2.61 times compared to poor hospital administration (aOR= 2.61; 95% CI= 1.44 to 4.72; p= 0.002) and the results were statistically significant.
