Abstract

Ineffective airway clearance is a common problem in pulmonary tuberculosis patients. This condition occurs due to an infectious process that causes an inflammatory response and excessive secretion production. The mediastinal tumor experienced by the patient resulted in the patient's condition getting worse because the lump in the mediastinal area pressed on the airway so that phlegm/secretions were difficult to expel. This study aimed to describe the ineffective management of airway clearance in patients with pulmonary tuberculosis, pleural effusion, and mediastinal tumors. This study uses a descriptive observational research design in the form of a case report using a nursing care approach. The subject of this study was a 34 year old woman with a medical diagnosis of pulmonary tuberculosis, pleural effusion, and mediastinal tumor. The intervention carried out, namely managing airway clearance, was not effective. Some of the actions carried out include semi-fowler position, collaborative administration of 3 lpm oxygen, combivent and pulmicort nebulization 3 times/day, and suctioning. After carrying out nursing procedures for 3 x 24 hours, the patient said he felt more comfortable in the semi-Fowler's position, she felt the shortness of breath was reduced after nebulization but the shortness of breath became worse again sometime later, the patient said he had difficulty expelling phlegm so his shortness of breath did not improve, SaO2 100%, RR 18 times/minute, and wheezing (-). It can be concluded that the interventions can reduce symptoms but cannot solve the problem. Management of ineffective airway clearance in this case cannot be done independently by the nurse. Collaboration is needed by considering the need for surgery and pharmacological therapy.

Abstract

Hypertension is one of the most common diseases in Indonesia, one of which in the province of Gorontalo is included in the top 10 largest diseases. According to the health data profile from the Gorontalo provincial health office in 2022, the level of hypertension in Gorontalo city reached 2,325 cases, or around 4.6%. High cases of hypertension must be balanced with the provision of hypertension drugs, one of which is the calcium channel blocker (CCB) hypertension drug, amlodipine. The purpose of this study was to see the profile of purchasing hypertension drugs from the Calcium Channel Blocker (CCB) Amlodipine group in the community in Gorontalo City, especially in pharmacy and pharmaceutical service facilities. The research method uses a random sampling observation method, namely to see drug purchases in three Gorontalo city pharmacies located in different sub-districts. Purchase tracking was carried out in January, February, and March 2024. The results showed the purchase of amlodipine drugs in the three pharmacies in different amounts. Pharmacy B dominated the purchase of drugs compared to Pharmacy A and Pharmacy C at the 5 mg and 10 mg doses, respectively. Based on the results obtained, these differences are influenced by several indicators, including drug availability, drug completeness, pharmacy strategic location, and customer satisfaction.

Abstract

Background: New normal is a term that has often been discussed recently due to the Covid-19 pandemic. The New Normal is the social habits of society and individual behavior that emerged after the COVID-19 pandemic (Habibi, 2020). The new normal is not only to prevent the transmission of Covid-19, but the new normal is very good for supporting a clean and healthy lifestyle. Children are one of the vulnerable groups in society. The behavioral patterns that exist in children make it difficult to carry out health protocols for washing hands with soap. Objective: This research aims to determine the relationship between knowledge about washing hands with soap in the new era. normal hand washing behavior in Class 4 5 6 students at SDN 2 Kedungwaru. Results: The results of this study show that respondents who have good knowledge all have good behavior with a total of 4 respondents (22%). Meanwhile, respondents who have less knowledge all have poor behavior with a total of 7 respondents (58%). The statistical test results obtained ρ value (0.000) with α (0.05). Conclusion: Therefore, there is a relationship between knowledge about washing hands using soap in the new normal era and hand-washing behavior in students in grades 4 5 6 at SDN 2 Kedungwaru. Children are a group that is quite difficult to enforce when it comes to washing their hands because children assume that their hands are always clean, and when they are given information, it is not always easy for children to grasp and process it.

Abstract

Pleural effusion is a collection of fluid in the pleura caused by the impact of another disease. Treatment for pleural effusion can be provided through the nursing care process with airway management interventions to help minimize shortness of breath in patients. There are nursing actions that can be carried out independently by nurses, namely the semi-fowler or fowler position, administering additional oxygen, and other therapies such as pursed lips breathing. The aim of this research is to determine the effectiveness of the combination of pursed lips breathing and high fowler position in the nursing care process for patients with ineffective airway clearance nursing problems in patients with pleural effusion using the case report method and nursing care approach. The results of the study showed that the combination of pursed lips breathing and the use of the selected high fowler position as high as 90 degrees for 3 days provided comfort and reduced shortness of breath with pleural effusion while being balanced by the provision of pharmacological therapy. Providing pursed lips breathing therapy and high fowler position has the potential to reduce shortness of breath in patients with pleural effusion

Abstract

Chronic kidney failure occurs due to a failure in the body's ability to maintain metabolism and fluid and electrolyte balance resulting in uremia or retention of urea and other nitrogenous waste in the blood. The research aims to synthesize literature about gargling therapy with boiled water, ice cube therapy, and chewing gum to reduce thirst in CKD patients. The method used is content analysis with a literature search from 2020 to 2023 using the keywords Ice Cubes, gargling, chewing gum, CKD To reduce thirst by using ice there were 6 journals, 4 journals using chewing gum, and 1 journal using boiled water. All articles show that there is an effect of gargling therapy with boiled water, ice cube therapy, and chewing gum to reduce thirst in CKD patients. The literature study shows that the use of ice cubes has the most effect on reducing thirst in CKD patients, however, gargling therapy with boiled water and chewing gum are other alternatives.

Endorphin Massage has the effect of stimulating the body to release endorphin compounds which are pain relievers and can create a feeling of comfort, which is a therapeutic action that is useful for improving physical and psychological conditions. The aim of the research was to determine the effect of giving endorphin massage on reducing dysmenorrhea in young women. The research design used was Quasi Experimental with a pre-experimental design, with one group pretest-posttest. to see the effect of Endorphin Massage before and after the intervention. The sample size was 30 respondents. The sampling technique used consecutive sampling which had been adjusted to the inclusion criteria and exclusion criteria. This study used the Wilcoxon statistical test with a significance level of α = 0.05. The results of this study showed a value of ρ = 0.000, where the value of ρ

Abstract

Abstract Introduction: Gorontalo City has 643 cases of tuberculosis in 2022, with the problem of ineffective airway clearance which causes airway obstruction which is often characterized by symptoms of ineffective cough, excess sputum, wheezing and rhonchi. Objective: To determine the effect of active cycle breathing technique on airway clearance in pulmonary TB patients. Method: The type of research used is quantitative. The research design uses a Quasi Experiment One Group Pre test- Post test design, namely a research design that only uses one group of subjects and takes measurements before and after giving treatment to the subjects being studied. The sampling technique was carried out using the Accidental Sampling technique with a sample size of 10 respondents. Data analysis uses univariate and bivariate analysis and the instrument used is a questionnaire. Results: The results of the study showed that the patient's clinical results from ineffective airway clearance problems, after being given the active cycle breathing technique, resulted in effective airway clearance. The statistical test results obtained p-Value = 0.004, which means less than (α = 0.05), indicating that there is an influence of active cycle breathing technique on airway clearance in pulmonary TB patients. Conclusion: The active cycle breathing technique can be a recommended nursing intervention in overcoming the problem of airway clearance in pulmonary TB patients.
