Abstract

Therapeutic Drug Monitoring (TDM) is a clinical practice to increase the effectiveness of therapy and minimize the risk of adverse drug reactions (ADR). The purpose of this study is to describe the implementation of TDM, and whether it is in accordance with the Regulation of the Minister of Health of the Republic of Indonesia (Permenkes) Number 72 of 2016 concerning pharmaceutical service standards at hospitals and the evaluation of TDM services conducted by pharmacists on inpatients. This research covers a descriptive study with a collection of retrospective data from TDM documents of inpatients during the period of January–May 2016 at X Hospital, Tangerang. The implementation of TDM by pharmacists based on the standards was 73%. The number of patients who met the inclusion criteria were 50 patients. Based on the diagnosis of the disease, patients with the potential of receiving TDM were patients with hypertension (10 patients; 20%), congestive heart failure (9 patients; 18%), and diabetes mellitus (8 patients; 16%). Whereas patients with both potential drug interaction and prescribed with more than 10 drugs was 29 patients (58%), patients with both potential drug interaction and diagnosed with three types of diseases was 19 patients (38%). The implementation of TDM in accordance with pharmaceutical service standards requires the competence of clinical pharmacist pharmacists with 2 years work experience. Potential drug interactions can be controlled by prioritizing TDM based on the number of drugs, complications of the disease and type of disease.

Abstract

The plant of Benalu Batu (Begonia medicinalis) from the North Morowali regency has been used empirically and is known to have anticancer activity. The study aims to determine the profile of the activity of lymphocytes cell proliferation of extracts and fractions of Benalu Batu in vitro as well to measure the correlation of total flavonoid content against the stimulating index of lymphocytes cell proliferation. The extraction of simplicia by using the maceration method with methanol solvent. The liquid-liquid partition method is used in the fractionation of methanol extracts with hexane, ethyl acetate, and water solvents, sequentially. Test of lymphocytes cell proliferation using MTT reduction (3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide) method with concentration series 10, 20, 50, and 100 μg/mL. The total of flavonoids content of extracts and fractions were determined by using colorimetric method. The stimulation index (IS) is used in measuring the activity of lymphocytes cell proliferation. The test results showed that the methanol extracts, hexane fraction, ethyl acetate, and water may increase the proliferation of lymphocytes in the entire test concentration. The concentration of test influences on increasing the stimulation of the proliferation of lymphocytes cells. A test concentration of 100 μg/mL has the highest stimulation index on each test sample and its stimulant index value of hexane fraction, ethyl acetate fraction, methanol fraction and water fraction were 10.12, 6.56, 4.82, and 4.17, respectively. Total flavonoids content and stimulation indices have very low correlation (r = 0,082) and concentrations of 10 μg/mL have the highest correlation coefficient (r2 = 0.18). The results showed that the hexane and ethyl acetate fractions can be developed as immunostimulant materials for the anticancer supportive therapeutic agents despite having a very low correlation to the flavonoid levels.

Abstract

Papaya seeds has many benefits in medicine. One of the benefits that have been studied is it’s antibacterial activity against Staphylococcus aureus. This study aims to determine the phytochemical compounds of papaya seed ethanol extract and their antibacterial activity against Staphylococcus aureus bacteria. The ethanol extract of papaya seeds was obtained through maceration method using 95% ethanol as a solvent. Maceration was carried out by weighing 85 grams of simplicia powder, macerated using 500 mL of 95% ethanol then left for 4 days while stirring occasionally, then filtered to produce filtrate and residue. The remaining residue is then soaked again (remaserated) with 250 ml of 95% ethanol and left for 2 days. After 2 days, the sample is filtered to produce filtrate and residue. All filtrates were evaporated using a rotary evaporator to obtain a thick extract. Phytochemical screening results showed the presence of flavonoids, alkaloids, tannins, saponins, and phenolic compounds in ethanol extracts of papaya seeds. The antibacterial activity test of papaya seed ethanol extracts towards Staphylococcus aureus was carried out through in vitro by disc diffusion method with several concentrations. Antibacterial test showed that concentrations of 2%, 4%, and 6% of papaya seeds ethanol extracts was moderate antibacterial activity and at concentrations of 8% and 10% the antibacterial activity was classified as strong against Staphylococcus aureus.

Abstract

Javanese turmeric is one of the Indonesia’s authentic plant that has many benefits, antimicrobial effect is one of them. Curcuminoid and xanthorrhizol contents are responsible for it's antimicrobial activity. Acne vulgaris is one of skin’s disorder that caused by bacteria, the major cause is Propionibacterium acnes. This study aimed to compare antimicrobial activity difference between nanoparticle and extract gel. Javanese turmeric was extracted and then formulated into nanoparticles. Javanese turmeric extract and it's nanoparticles were then formulated in gel preparation. Gel preparations were evaluated physically (organoleptic, homogeinity, spreadibility, and viscosity and rheology), chemically (pH value), and tested for their antimicrobial activity. Gel evaluations showed that both formula (nanoparticle and extract gel formula) did not show any discoloration and still homogeny. Extract gel formula had viscosity about 1540–1600 Ps, while nano extract gel had viscosity in the range of 487.63–501.01Ps. Extract gel formula had spreadability in the range of 51.75-53.35 mm, while nano extract gel has viscosity in the range of 65.125-72.75 mm. Extract gel formula had pH value in the range of 5.98–6.02, while nano extract gel had pH value in the range of 5.88–5.93. Extract gel formula has inhibition zone in the range of 10.0 mm–10.3 mm, while nanoextract gel has inhibition zone in the range of 13.0 mm–13.4 mm. Based on statistical paired sample t-test analysis, nano extract gel formula was signifi cantly different from extract gel formula in antimicrobial activity.

Abstract

Beta-adrenergic receptor blockers (β-blockers) are an appropriate treatment for patients with hypertension who have concomitant ischemic hearat disease, heart failure, obstructive cardiomyopathy, aortic dissection or certain cardiac arrhythmias. Taya (Nauclea subdita (Korth) Steud) is a local natural potential of Kalimantan that is empirically believed to have antihypertensive activities. This study aims to determine the activity of β-adrenergic receptor inhibitors of stem bark water extract in male wistar rats. Tests were carried out on male Wistar rats by the Non Invasive Blood Pressure method using the CODA® tool. Test animals were divided into 5 groups: normal group, induction group (Epinephrine 1.2 µg/kgBW), 100 mg/kgBW extract group, 200 mg/kgBW extract group, and 4,5 mg/kgBW atenolol group. The induction group showed an increase in systolic blood pressure, diastolic blood pressure, and heart rate that were significantly different from the normal group, the 100 mg/kgBW extract group, the 200 mg/kgBW extract extract group, and the 4.5 mg/kgBW atenolol group (p<0,05). The 100 mg/kgBW extract group was not significantly different from systolic blood pressure, diastolic blood pressure, and heart rate with the normal group, the 200 mg/kgBW extract group, and the 4.5 mg/kgBW atenolol group (p> 0.05). The results of this study showed that taya bark water extracts 100 mg/kgbw had inhibitory activity of β-adrenergic receptors in male wistar rats.

Abstract

Curcuma zedoaria has a high antioxidant activity and potential to be made a topical antioxidant preparation. Emulgel is a combination of O/W emulsion with a gel base, and it has many advantages, including convenience in use. In this study, an emulgel formulation was made using Sepigel 305® with various concentrations (3%, 4%, and 5%). The emulgel formulation, the ethanol extract of C. zedoaria, was added with concentrations 1%, 2%, and 5%. The preparation was evaluated for physical quality and antioxidant activity using the DPPH method. In the selected formula, had been observing the accelerated stability tests were carried out for three months. Ethanol extract of C. zedoaria has antioxidant activity with IC50 49.72 ± 0.32 ppm. The best emulgel preparation is F2, which contains Sepigel 305® 4% and extract 2%. The F2 emulgel formulation was found to be stable physicochemical properties (organoleptic, homogeneity, viscosity, flow properties, dispersion ability, and emulsion type test) upon the accelerated stability tests, and pH met the requirements with antioxidant activity gave IC50 135.8 ppm. Ethanol extract of C. zedoaria can be made emulgel preparations using Sepigel 305®, physically and chemically stable, and has an excellent antioxidant activity for the three months of storage.

Abstract

Brown seaweed has been known as a fucoidan source that has various biological activities. Fucoidan could be obtained by various extraction methods. This study aims to extract, characterize and to evaluate the antioxidant activity of crude fucoidan extracted from Sargassum cinereum. Extraction was carried out by refluxing at 100oC for 4 and 5 hours and then precipitated with ethanol. The crude fucoidan was brown powder. The yield with 4 hours extraction time was 2.78% which did not differ significantly from the 5 hours extraction time. Phytochemical screening of crude extract showed that beside fucoidan there were flavonoids, saponins, steroids, triterpenoids, and tannins. The extraction with duration of 4 hours and 5 hours had sulfate content of 16.31% and 11.22%, total carbohydrate content were 22.94% and 22.85%, and in the FTIR spectrum showed OH absorption bands of carbohydrates, fucose, sulfates and uronic acid. Testing of antioxidant activity using the DPPH (1,1-diphenyl-2-picrylhyrazyl) free radical inhibition method produced IC50 values ​​of 721.9 µg/ml and 749.9 µg/ml respectively and it was concluded that crude fucoidan of Sargassum cinereum had weak antioxidant activity.

Abstract

Indonesian medicinal plants have been used as traditional medicine (herbal medicine), standardized herbal medicines or phytopharmaca. Kratom leaf ethanol (Mitragyna speciose Korth) has Escherichia coli bacterial activity with a concentration of 5%. Kratom leaf extract granules. Ethanol extract of kratom leaf (Mitragyma specioca Korth) has activity against male white mice (Mus musculus L) as antidiarrheal and know the time used for granules of kratom leaf ethanol extract (Mitragyma specioca Korth) and the dosage which gives activity to white mice (Mus musculus L) as antidiarrheal and know the time used for granules of kratom leaf ethanol extract (Mitragyma specioca Korth) and the dosage which gives activity to white mice (Mus musculus L) as antidiarrheal, Kratom leaf extract granule was tested to white male mice with a concentration without active substance (blank) formula 1 as a negative control and using active ingredient kratom leaf extract formula II using a concentration of 0.4%, Formula III concentration 0.5 % and formula IV with a concentration of 0.6% using loperamid HCL as a positive control Method of randomized block design (True Experimental Post Only Control Group) 6 hours of observation time with variations seen were organoleptic, consistency, frequency and stool weight. kratom 0.4% has antidiarrheal activity with a time of 330 minutes.

Abstract

Drug planning at Pluit Hospital is carried out using a consumption method that is based on previous use. The purpose of this study was to determine the investment value and use of drug supplies based on a combination of consumption methods with ABC, VEN and Reorder Point (ROP) analysis. This research is a qualitative and quantitative descriptive study with retrospective data collection during the period May-October 2016 at Pluit Hospital. ABC analysis results based on the investment value of the total group A drug items have a percentage of 69.90%; group B 20.07% and group C 10.03%. ABC analysis results based on the use of group A 69.95%; group B 20.04% and group C 10.01%. VEN analysis results for group V 3.24% absorbed investment of 5.15%; group E 60.10% absorbed investment 76.56% and group N 36.66% absorbed investment 18.29%. Analysis of drug planning based on ABC and VEN methods has high, medium and low investment values, usage values ​​and NIK. While the analysis of drug control based on the method of Lead Time, Safety stock and Reorder Point guarantees the availability of drugs on an ongoing basis.
