Abstract

Post-partum depression (PPD) is a post-natal disorder that not only impacts the mental health of the mother but also has long-term effects on the development and well-being of the child and family. PPD is a preventable disorder, starting from interventions at primary health care facilities. As one of the developing countries with a PPD incidence of 22% per 100,000 births, PPD is considered an urgent public health issue in Indonesia. In practice, evaluation, and improvement of services at primary health care facilities are needed. This study aims to identify and adapt international intervention strategies to reduce post-partum depression prevalence in Indonesian primary health care. A scoping review analysis method was used, involving previous studies on activities from pregnancy to post-partum that can prevent and reduce post-partum depression. The reviewed articles employed Randomized Controlled Trials or Randomized Clinical Trials (RCT) methods with specific inclusion and exclusion criteria. Data were collected from relevant sources, and a synthesis was conducted on 16 articles that met the inclusion criteria. The results show that direct psychological support activities (through health professionals or groups) and indirect support using digital platforms and applications are effective in reducing depression. Strategies such as digital technology, direct support programs, physical therapy, pharmacological therapy, community involvement, and strengthening health services, particularly post-partum psychological counseling, are recommended to enhance prevention. This research supports the expansion of evidence-based strategies that can be implemented in primary healthcare facilities to address post-partum depression.

Abstract

The community that adheres to the naturalistic concept assumes that a person suffers from illness due to the influence of the environment, food, lifestyle, and imbalance in the body. Meanwhile, the community that adheres to the personalistic concept assumes that the emergence of illness is caused by the intervention of an active agent that can be a non-human creature (ghost, spirit, ancestor, or evil spirit). This research aims to identify the perception of health and illness of the Jember community based on the theory of transcultural nursing in the scope of personalistic and naturalistic. This research is a descriptive study with a case study and a qualitative type of research. Data were obtained by conducting interviews with in-depth interview guide instruments. There were four main participants in this study and two additional participants. The steps in conducting research are preliminary studies, data collection, and conclusions. This research uses the Colaizzi method analysis technique. The study results show that the community believes that illness is caused by a person's biological condition, which is influenced by environmental conditions, adequate nutrition, lifestyle, body imbalance, and congenital diseases (naturalistic). The treatment method based on the naturalistic concept allows the community to get a cure for their illness by seeking treatment from modern medical health services, such as going to a doctor or buying medicine at a shop. People who understand and comprehend the meaning of illness and the causes of illness will determine their choice of treatment, whether naturalistic or personalistic.

Abstract

This research focuses on the pillars of primary care transformation by increasing primary services' capacity and capability, especially Diabetes Mellitus (DM) care. The study aims to produce a recording form containing simple, interprofessional records of DM patient care that can be used by various professions and can be applied in multiple primary health facilities. The design was developed through a development research approach with stages of form modification, expert and practitioner review, revision, trial, focus group discussion, and final revision. The trial was carried out by Community Health Center (Puskesmas) nurses in services at the Puskesmas through home visits for DM patients, Posbindu services, and outpatient care. Data from expert opinions, user opinions, and observations of the forms tested were analyzed qualitatively using the Technology Acceptance Model concept approach. The resulting form consists of two pages containing primary patient data and daily progress notes in tabular form. Research participants showed positive beliefs, attitudes, and behavior toward the forms being tested—the actual use of the form on an average of 3 patients per participant. The completeness was 90 percent filled. They expressed their intention to use the form further and recommended widespread dissemination to implement it in practice. The form is acceptable and ready to be applied for outpatient care, health monitoring, and home care for DM cases. Recommendations can be given to policymakers and health service providers to implement in services. Apart from that, the form can be further developed in electronic form by information technology developers.

Abstract

Successful breastfeeding significantly impacts the health of both mothers and infants. This study aims to analyze factors affecting breastfeeding success through a quantitative design incorporating bivariate and multivariate analyses. Data were collected from 90 breastfeeding mothers and analyzed using statistical tests. The results reveal that maternal age is a significant factor (p-value=0.007); mothers aged 20–35 are 1.6 times more likely to breastfeed successfully than those younger than 20 or older than 35. Maternal education also plays a critical role (p-value=0.002); mothers with higher education levels are 2.3 times more likely to succeed in breastfeeding than those with primary education. Employment status is another significant factor (p-value=0.013); non-working mothers are 1.8 times more likely to breastfeed successfully than working mothers. Parity also influences success (p-value=0.029); multiparous mothers are 1.2 times more likely to succeed than primiparous mothers. These findings highlight the need for targeted interventions. Educational programs should prioritize mothers with primary or secondary education and those employed. Support mechanisms for working and primiparous mothers are crucial to address breastfeeding challenges. Policies such as extended maternity leave and workplace lactation facilities are also recommended to foster breastfeeding success. In conclusion, maternal age, education, employment status, and parity significantly impact breastfeeding outcomes. Tailored programs and supportive policies can improve breastfeeding rates and enhance maternal and infant health.

Abstract

Low Back Pain (LBP) is discomfort due to physiological changes during pregnancy that will have an impact on the quality of life of pregnant women, such as decreased physical mobility, reduced sleep quality, and increased psychological stress. The high prevalence of LBP in pregnancy ranges from 24% to 90%. Therefore, it is necessary to strive for pain management that is easy to do, has minimal side effects for pregnancy, and can be done independently by pregnant women, with minimal equipment and low costs. Using a gym ball in pelvic rocking exercises can stretch the pelvic muscles and increase levels of the beta-endorphin hormone, which can reduce pain. This study aims to analyze using a gym ball in pelvic rocking exercises to minimize the pain scale of low back pain in pregnancy. This research design is a quasi-experiment with a Pre-test, Post-test, and Nonequivalent Control Group Design. The sampling technique in this research was Purposive sampling, with 30 respondents divided into two groups. The instrument used was the Visual Analog Scale (VAS) to assess the level of the LBP pain scale. Data analysis used the t-test. The results of statistical tests show a p-value of 0.000 (p-value<0.05), so it can be concluded that using a gym ball in pelvic rocking exercises affects the LBP scale in pregnancy.

Abstract

Stunting is still one of the most pressing global health problems. Father involvement in childcare is needed to ensure optimal child growth and development and free the child from stunting problems. Based on transcultural nursing theory, this study analyzes factors related to father involvement in preventing toddler stunting. The study used a cross-sectional approach with a multistage random sampling technique in Jember Regency with a sample of 852 respondents. A questionnaire was used to collect data. The study was conducted for 2 weeks, from August 1, 2024, to August 15, 2024. Statistical tests were carried out using Spearman Rho with a significance of 95% (α <0.05). Statistical analysis showed that technology factors (p-value=<0.001, r score=0.297), religion, spirituality, and philosophy (p-value=<0.001, r score= 0.170; kinship and social (p-value=<0.001, r score= 0.378),  cultural values, beliefs, and lifestyles (p-value=<0.001, r score=0.204); politics and law(p-value=<0.001, r score=0.360);  economics (p-value=<0.001, r score=0.370)—were positively correlated with father involvement in preventing stunting in toddlers. Meanwhile, biological variables (p-value=0.054, r score=0.237) and education level (p-value=-0.067, r score=0.052) did not correlate significantly with the father's involvement in preventing toddler stunting. Current research shows that father involvement in preventing stunting in toddlers is beneficial. Public health nurses can use these results to create health promotion initiatives that encourage fathers to be more involved in child care and prevent childhood stunting. The results of this study can be used to develop health promotion programs to increase father involvement in improving stunting prevention.

Abstract

Complications that occur during pregnancy are one of the causes of high maternal mortality, but these complications can be treated and prevented. Proper care before giving birth and adequate antenatal care can be supported by the presence of teaching media for pregnant women, including pregnant women with hypertension. Teaching media is effective in improving knowledge and self-care skills and helping understand pregnant women and pregnant women with hypertension. The small number of study articles on teaching media for self-care for pregnant women and pregnant women with hypertension made researchers interested in conducting a literature review on this topic. This study aims to determine the effectiveness of teaching media for self-care for pregnant women and pregnant women with hypertension. This research method is a literature review with a narrative review design. A comprehensive literature search was conducted using electronic databases, namely Pubmed, CINAHL, and Google Scholar. The author obtained 24 articles that were based on the research objectives. The results of the study, the teaching media used in self-care for pregnant women and pregnant women with hypertension are educational modules, power points, booklets, videos, animated videos, pamphlets, m-Health technology, mobile applications, social networks such as WhatsApp, Telegram, and Instagram. In conclusion, self-care teaching media can improve self-care in pregnant women, including pregnant women with hypertension because the teaching media is adjusted to the needs of pregnant women, the material is for self-care, the language is easy to understand, and the media used is interesting.

Abstract

Most hypertension sufferers are not very strict in controlling themselves and managing their blood pressure to prevent complications. One of the significant risk factors for heart disease is hypertension. This study aims to ascertain if hypertension patients' degree of self-management and the prevalence of coronary heart disease are related. This specific type of research uses a cross-sectional study design and is quantitative. Purposive sampling is employed. The Slovin formula is used to compute the sample. The study's total sample was 109 outpatients with hypertension from the internal medicine polyclinic of Dr. Moewardi Surakarta Hospital. This study used the HSMBQ (Hypertension Self-Management Behavior Questionnaire) questionnaire by Nargis Akther to measure the level of self-management of hypertension with coronary heart disease. The data analysis used was a univariate test, and the risk level was calculated using the Framingham Risk Score and the chi-square bivariate test. Results show that the study's p-value=0.048 (<0.05) suggests a significant correlation between hypertension patients' degree of self-management and the prevalence of coronary heart disease. Most individuals fall into the category of having an adequate level of self-management. The majority of the 76 people (69.7%) with intermediate levels of self-management had a minor risk of coronary heart disease. The results of the study showed a significant relationship between the incidence of coronary heart disease and the level of self-control in hypertensive patients. Promotive and preventive efforts through educational approaches and structured interventions are essential to improve the quality of self-control in hypertensive patients.

Abstract

Proper and standard healthcare services will provide patient satisfaction in evaluating healthcare performance. Based on interviews conducted in December 2023 in the Surgical Inpatient Room at RSUD DR. H. Abdul Moeloek, Lampung Province, with 10 respondents, 6 respondents (60%) they were stated that some nurses did not explain the actions to be taken and were slow to respond to patient complaints. This study aims to determine the relationship between nurse services and communication with patient satisfaction in the surgical inpatient room of RSUD Dr. H. Abdul Moeloek, Lampung Province. This study was conducted in March-April using a quantitative method with a descriptive-analytic design through a cross-sectional approach. The study population was 83 respondents selected through accidental sampling (accidental sampling, also known as convenience sampling, is often considered a weak sampling technique because it does not ensure that the sample is representative of the larger population). Data collection instruments used questionnaires with dependent variables of patient satisfaction and independent variables of nurse services and communication. Data analysis used the Chi-square test. The results showed that 94% (78) of respondents stated that nurse communication was always good, and 96.4% (80) stated that nurse services were good. The correlation results showed a p-value=0.000<0.050, indicating a significant relationship between nurse services and communication with patient satisfaction. It is recommended that nurses continue to provide good, prompt, friendly, and always smiling services to patients.

Abstract

Various factors are thought to hinder the success of TB treatment, including failure of therapy due to non-compliance as one of the main factors. The solution is expected to be the development of an android-based pulmonary tuberculosis patient treatment monitoring information system application (SISFOTBPARU). This study aims to assist the operational health officers of the Kampung Sawah, Satellite, and Kedaton Bandar Lampung Health Centers in monitoring the treatment of TB patients at their Health Centers. Based on the results of the black box testing that had been carried out, the application could run according to the development design. In the UAT test using a list of questions for each application user using Likert's summed rating scaling calculation technique, for users of Kampung Sawah, Satellite, and Kedaton Health Center Officers in Bandar Lampung, 76.0% of users stated that it was very appropriate and 24.0% stated that it was proper that the menus displayed on the application met the expectations of health center officers. In the Testing of the Application for Patient Users of Kampung Sawah, Satellite, and Kedaton Health Centers in Bandar Lampung, 78.7% of users stated that it was very appropriate, and 21.3% stated that it was proper that the menus displayed on the application made it easier for patients to report their treatment activities. This study proves that the criteria for success include the web and Android TB patient treatment monitoring applications used by health workers at Kampung Sawah, Satellite, and Kedaton Health Centers and TB patients.
