Abstract

Osteoarthritis (OA) is a degenerative joint disease characterized by pain, inflammation, and impaired physical function. Interleukin-1 beta (IL-1β) plays an important role in OA pathogenesis by promoting inflammatory and catabolic processes within the joint. Antioxidants have shown potential in modulating oxidative stress and inflammatory responses associated with OA. This systematic review evaluated the effects of antioxidant administration on IL-1β levels in knee osteoarthritis. A systematic literature search was conducted in five databases (PubMed, Cochrane Library, Epistemonikos, Scopus, and ProQuest) until January 2025, following PRISMA guidelines. Eligible studies included intervention studies involving human or animal models of knee OA, antioxidant administration, and assessment of IL-1β outcomes. Because of substantial heterogeneity among studies, a qualitative synthesis was performed. Risk of bias was assessed using the RoB 2.0 tool for randomized controlled trials and the SYRCLE tool for animal studies. A total of 13 studies were included, consisting of six randomized controlled trials and seven animal studies. Most studies demonstrated reductions in IL-1β levels or expression following antioxidant administration, including curcumin, vitamin E, resveratrol, cucumber extract, and L-carnitine. Several studies also reported improvements in inflammatory markers, pain, and cartilage-related outcomes. Overall, the included studies showed a low risk of bias. Current evidence suggests that antioxidants may reduce IL-1β levels and exert anti-inflammatory effects in knee OA. However, further well-designed clinical studies are needed to confirm their efficacy and safety.

Abstract

Background Androgenetic alopecia (AGA) is the most common form of progressive hair loss. AGA is a polygenic disorder characterized by variability in age of onset, severity, and pattern. Genome-wide association studies (GWAS) have shown that the variant rs1998076 at locus 20p11 may increase risk in Asian and European populations. Association studies of AGA and genetic risk are limited in the Indonesian population. Methods We conducted a cross-sectional pilot study (January-August 2025) in Jakarta, Indonesia, recruiting 100 participants (50 AGA cases and 50 controls). Clinical AGA status was determined by the physician. Anthropometric data, including height, weight, blood pressure, and smoking history, were obtained through measurement and a questionnaire. Genotyping of rs1998076 was performed using allele-specific PCR, and Sanger sequencing was used to validate the results. Results The genotype distribution for rs1998076 was AA (24%), GG (37%), and AG (39%). The allele frequencies for rs1998076 were A (0.435) and G (0.565). However, AGA was not significantly correlated with the rs1998076 genotype (p > 0.05) in multivariate models, although the GG and AG genotypes tended to be associated with a higher risk of AGA than the AA genotype. Conclusions In this pilot study, rs1998076 was not significantly associated with AGA, although the GG genotype showed a non-significant trend toward higher odds. Clinical factors (age, male sex, higher BMI, and hypertension) were correlated. Larger, well-powered studies are needed to clarify the role of rs1998076 in Indonesians.

Abstract

Neonatal jaundice remains the most prevalent clinical condition affecting newborns globally, impacting approximately 60% of term and 80% of preterm infants within the first week of life. For more than five decades, conventional phototherapy has served as the gold standard for preventing kernicterus secondary to severe hyperbilirubinemia. Despite its proven efficacy, the traditional hospital-based phototherapy model presents substantial public health challenges, particularly in developing countries such as Indonesia. Dependence on fixed, high-cost equipment within tertiary facilities often necessitates maternal–infant separation, disrupts early bonding and breastfeeding, and contributes to inequities in healthcare access based on geographic and socioeconomic factors. Technological advancements in Light Emitting Diode (LED) phototherapy blankets represent a paradigm shift toward family-centered neonatal care. By incorporating blue-green light sources into flexible, portable devices, these blankets enable effective treatment while maintaining skin-to-skin contact and direct breastfeeding. This approach supports maternal–infant attachment, enhances lactation success, and may facilitate bilirubin reduction through improved enteral feeding. From a public health perspective, LED phototherapy blankets promote decentralized care delivery by enabling deployment in primary healthcare facilities and remote settings, thereby reducing referral burden and treatment delays. Local innovations such as the BLUI Blanket demonstrate promising clinical efficacy and adherence to thermal safety standards, supporting their role as safe and effective first-line interventions. Furthermore, implementing home-based phototherapy programs for uncomplicated cases can reduce hospitalization costs, minimize the risk of nosocomial infections, and optimize hospital resource allocation. In conclusion, LED phototherapy blankets constitute a transformative innovation that integrates clinical effectiveness, psychosocial well-being, and health system efficiency. Their broader adoption has the potential to enhance equitable neonatal care delivery while preserving the integrity of the mother–infant dyad.

Abstract

Background Polycystic Ovary Syndrome (PCOS) is a complex hormonal disorder commonly found among women of reproductive age, characterized by hyperandrogenism, ovulatory dysfunction, and polycystic ovarian morphology. One of its primary clinical manifestations is hirsutism, which is assessed using the Ferriman–Gallwey Score. Physical activity plays an important role in managing insulin resistance and androgen levels; however, no specific data are available regarding the relationship between physical activity and the severity of PCOS in Ambon City. Therefore, it is necessary to investigate the relationship between physical activity and PCOS severity, as measured by the Ferriman–Gallwey Score, in Ambon City in 2025. Methods This study used an analytical observational design with a cross-sectional approach. The sample comprised 60 reproductive-aged women with PCOS, recruited through consecutive sampling. Physical activity was measured with the IPAQ and categorized as light, moderate, or vigorous. PCOS severity was assessed using the Ferriman–Gallwey Score. Data were analyzed using the Chi-square test. Results Most respondents were 19–30 years old (70%) and 23.3% worked as housewives. The most common level of physical activity was light (53.3%), and the majority of respondents had mild PCOS (60%). A pattern was observed in which vigorous physical activity was associated with more severe PCOS. Chi-square analysis showed a statistically significant association between physical activity and PCOS severity (p < 0.001). Conclusions Physical activity was significantly associated with PCOS severity, with low activity linked to mild PCOS and vigorous activity predominantly linked to severe cases.

Abstract

Background Malnutrition is frequently referred to as a “silent emergency” by UNICEF. Globally, women and children are particularly vulnerable to health issues related to malnutrition. In Indramayu, the prevalence of stunting was reported at 14.72% in the 2021 Indonesian Nutrition Status Study, indicating a potential increase in malnutrition cases. Risk factors for stunting in developing countries include low birth weight, low maternal education, living in rural areas, short birth length, lack of antenatal care (ANC), incomplete immunisation, and lack of exclusive breastfeeding, based on existing literature. Objective: This study aims to identify factors that may contribute to stunting incidence in Pabean Udik Village, Indramayu Regency. Methods This study employed an observational cross-sectional design. The sample consisted of 32 participants, selected using purposive sampling. Results There was no association between gender, child’s age, preterm birth, maternal education, maternal Body Mass Index (BMI), and family income. (p-value > 0.05). Although among children aged 0–2 years, mothers with a BMI classified as malnourished and family incomes < Rp. 2,000,000 had a higher risk of stunting (RR > 1). Conclusions Although no significant associations were found among the variables studied, stunting remains a public health issue in Indramayu. The lack of a significant association may be due to the small sample size. Further research with a larger sample size is needed to explore other potential risk factors, such as nutritional practices, environmental influences, and access to health services, as well as the importance of collaboration with health services to reduce existing risk factors.

Abstract

Background Stroke is a leading cause of premature death globally, and Indonesia has a high prevalence of stroke at 10.9 per 1,000 individuals. Although thrombolytic therapy (rt-PA) is highly effective when administerd within golden period of 4,5 hours. Despite the effectiveness of thrombolytic therapy (rt-PA), its use in Indonesia remains low (<10%) due to significant knowledge gaps. Objectives This study assesses knowledge, attitudes, and practices (KAP) regarding acute stroke and thrombolysis among hospital outpatients. Methods A cross-sectional study was conducted at Hermina Hospital Medan (November 2024–February 2025) with 129 respondents. KAP scores were converted to a 0–100% scale. Differences were analyzed using Mann–Whitney U and Kruskal–Wallis H tests, and relationships were assessed using Spearman’s rho correlation (rs) because the data were not normally distributed. Results Adequate stroke knowledge was reported by 49.6% of respondents, but only 17.8% recognized thrombolysis as a specific stroke therapy. Educational level significantly influenced total KAP scores (p=0.000). Significant positive correlations were observed between knowledge and attitude (rs=0.479, p<0.001) and between knowledge and practice (rs=0.425, p<0.001). Conclusions While outpatients have moderate knowledge, a significant action gap remains, particularly regarding thrombolytic treatment and emergency response. Targeted education for low-literacy groups is essential to improve stroke outcomes.

Abstract

Breast cancer remains the most frequently diagnosed malignancy among women worldwide and continues to represent a major public health challenge. Accurate characterization of breast lesions is essential for early diagnosis and appropriate clinical management. Conventional ultrasound is widely used to evaluate breast lesions and classify them using the Breast Imaging Reporting and Data System (BI-RADS). However, differentiating benign from malignant lesions remains difficult, particularly in BI-RADS categories 3 and 4, which often lead to diagnostic uncertainty and unnecessary biopsies. Shear wave elastography (SWE) has emerged as an adjunct imaging technique that evaluates tissue stiffness and provides additional diagnostic information beyond conventional ultrasound. One characteristic elastography feature is the stiff rim sign, which appears as increased peri-lesional stiffness surrounding a breast mass and may reflect malignant tumor infiltration and stromal reaction. This systematic review aimed to evaluate the diagnostic role of the stiff rim sign detected by shear-wave elastography in differentiating benign from malignant breast lesions and its potential utility in guiding biopsy recommendations for BI-RADS category 3 and 4 lesions. A comprehensive literature search was conducted in PubMed, the Cochrane Library, and ScienceDirect to identify relevant studies assessing SWE and peri-lesional stiffness features in breast lesions. Studies that met predefined inclusion criteria were screened and evaluated, and relevant data on diagnostic performance were extracted for qualitative synthesis. The literature search identified 71 records, of which six studies met the eligibility criteria and were included in the final analysis. Across the included studies, the stiff rim sign demonstrated moderate to high diagnostic performance in differentiating malignant from benign breast lesions. Several studies reported high sensitivity, specificity, and area under the receiver operating characteristic curve when elastography findings were combined with conventional ultrasound parameters. Overall, the stiff rim sign detected by shear wave elastography appears to be a valuable imaging feature that may improve diagnostic accuracy and assist clinicians in guiding biopsy decisions for patients with BI-RADS 3 and 4 breast lesions.

Abstract

Resveratrol is a polyphenolic stilbenoid found in plants such as grapes, berries, and peanuts, widely recognized for its antioxidant, anti-inflammatory, anticancer, antidiabetic, cardioprotective, neuroprotective, and anti-aging properties. Despite extensive preclinical evidence, its clinical translation remains limited by poor bioavailability and inconsistent trial outcomes. This review aims to synthesize current evidence on resveratrol’s mechanisms of action, therapeutic effects across disease models, synergistic drug interactions, and translational challenges, with a focus on its integration into modern integrative medicine. A comprehensive literature search was conducted in PubMed, Scopus, and Web of Science using the terms “resveratrol,” “polyphenol,” “integrative medicine,” and disease-specific keywords. The resuls was Resveratrol demonstrated anticancer effects via apoptosis induction and cell cycle arrest, cardioprotective effects through AMPK/SIRT1/eNOS activation, neuroprotection via Akt/PI3K signaling, and antidiabetic effects through improved insulin sensitivity. Synergistic interactions were identified with metformin (diabetes), statins/aspirin (cardiovascular disease), and chemotherapeutic agents (cancer). Bioavailability and formulation remain major challenges. Conclusion of this study Resveratrol holds significant therapeutic promise in integrative medicine, though robust clinical trials are needed to confirm efficacy and establish safe dosing protocols for clinical application.

Abstract

Background Indonesia is a tropical country exposed to ultraviolet (UV) rays year-round, particularly in Central Sulawesi Province, which lies on the equator. Tojo Una Una Regency is located in this province, and the majority of its residents work as fishermen, who are frequently exposed to UV radiation, a risk factor for pterygium and dry eye. This profession has the highest incidence of pterygium, reaching 15.8%. This study will analyze the relationship between pterygium and dry eye. Methods The study design was cross-sectional with consecutive non-random sampling. A questionnaire was used to assess characteristics. Pterygium was diagnosed through an anterior segment examination by a general practitioner, followed by a double-blind ophthalmologist to assess the degree, location, and whether the pterygium was single or double-headed. The degree and diagnosis of dry eye were confirmed using the OSDI questionnaire. Results The study involved 400 respondents, and the prevalence of pterygium reached 85.25%, with cases predominantly bilateral (66.5%), single-headed (70.25%), female (72.75%), 26-45 age group (49.25%), and those working outdoors (77%). DES cases were found in 54.25%, with the majority being severe DES. A chi-square test showed a relationship between dry eye and pterygium (p<0.001). Conclusions There was a significant relationship between DES severity and pterygium incidence (p<0.001).

Abstract

Colorectal cancer (CRC) is the third most common malignancy but ranks second after lung cancer in terms of mortality, posing a significant public health burden due to its high incidence and death rates. Cancer stem cells play an important role in chemoresistance, tumor progression, and recurrence in CRC. 5-Fluorouracil (5-FU) is a primary chemotherapeutic agent used to kill cancer cells and reduce the risk of recurrence. Resistance to 5-FU remains a major obstacle in CRC treatment, highlighting the need for novel therapeutic strategies targeting cancer stem cells. This review draws on data collected from various electronic databases, covering studies published between January 1, 2015, and December 31, 2024. The search focused on colorectal cancer stem cells (CCSCs) and compared outcomes between monotherapy chemotherapy and chemotherapy combined with phytopharmaceuticals. Preclinical experimental studies published in English and available in full text were included, while case reports, case series, clinical studies, systematic reviews, meta-analyses, clinical guidelines, studies using human tumor tissue samples, and non-English or unavailable full-text articles were excluded. The results were that eight preclinical studies were included, comprising seven controlled experimental studies and one uncontrolled in vitro study. Four studies used 5-FU as a control in combination with different phytopharmaceuticals, including pseudolaric acid B (PAB), morin and MST-312, and diethylaminobenzaldehyde (DEAB), and baicalin. The remaining four studies evaluated phytopharmaceuticals in combination with alternative chemotherapeutic agents, including oxaliplatin with resveratrol, cisplatin with atractylenolide I (ATL-1), and rapamycin with curcumin, or investigated ginsenoside Rg3R as monotherapy. Cellular apoptosis was the primary outcome, while apoptosis-related signaling pathways were evaluated as secondary outcomes. Phytopharmaceuticals as a promising area of research in overcoming chemotherapeutic resistance. The findings reflect a shift in current therapeutic strategies, increasingly favoring phytopharmaceutical-based treatments to reduce drug resistance and enhance the efficacy of chemotherapy with minimal side effects.
