Abstract

Cholelithiasis is a common condition in gastroenterology that affects 10% to 15% of adults. It is commonly known as gallstones and results from the precipitation of substances in the bile. Meanwhile, prostatic hyperplasia is an enlargement of the prostate gland that can cause various urinary tract symptoms, including bladder outlet obstruction and dysuria, which significantly affect the patient's quality of life. This case reports a 54-year-old man who was admitted to the hospital with complaints of shortness of breath and swollen legs 1 day before admission. After 2 days of hospitalization, the patient felt pain in the right abdomen. So an abdominal ultrasound examination was performed and found thickening of the wall and a picture of stones in the gallbladder and an enlarged prostate. Actions taken include a cholecystectomy surgery for gallstones in the gallbladder and observation for the development of symptoms of prostate enlargement. The patient's prognosis was good, indicating a good outcome or positive recovery.

Abstract

Nephrolithiasis, or kidney stones, is a condition that can cause severe pain and serious complications if not properly managed. This case report discusses a 64-year-old female patient presenting with a chief complaint of stabbing pain in the lower back radiating to the stomach for the past month. Ultrasonographic examination revealed bilateral kidney stones. This report aims to describe the disease progression, diagnostic evaluation, and management of a patient with bilateral nephrolithiasis. Abstract

Streptococcus suis (S. suis) is a significant zoonotic pathogen that can cause severe infections in humans, primarily manifesting as meningitis, septicemia, and endocarditis. This review focuses on the pathophysiology of S. suis meningitis, emphasizing the bacterium's mechanisms for crossing the blood-brain barrier (BBB) and eliciting a robust inflammatory response. The review highlights the virulence factors, including collagenase-like protease and capsular polysaccharides, that facilitate the pathogen's evasion of the immune system and contribute to the severity of the disease. Notably, the prevalence of S. suis infections remains a critical public health concern, particularly in regions with high pig consumption, where occupational exposure increases the risk of human disease. The manuscript also evaluates current diagnostic techniques, comparing traditional methods, such as cerebrospinal fluid culture, with advanced molecular diagnostics, including polymerase chain reaction (PCR) and next-generation sequencing (NGS). These newer methods demonstrate enhanced sensitivity and specificity, crucial for timely and accurate diagnosis. Furthermore, emerging innovations in diagnostic technology, including point-of-care testing and biosensors, hold promise for rapid identification of S. suis, potentially mitigating morbidity associated with the disease. This comprehensive review highlights the urgent need for enhanced diagnostic capabilities and public health strategies to reduce the burden of S. suis meningitis, promoting interdisciplinary collaboration to improve disease prevention and management.

Abstract

Human sperm undergo physiological and biochemical changes when present in the female reproductive tract, known as capacitation. The release of cholesterol from the plasma membrane, which increases membrane fluidity and permeability to bicarbonate and calcium ions, plasma membrane hyperpolarization, protein phosphorylation changes, protein kinase activity, increased bicarbonate (HCO3-), intracellular pH, Ca2+ concentration, and cyclic adenosine monophosphate (cAMP) levels are all biochemical changes connected with the capacitation process. Sperm capacitation can be induced by various factors, one of which is Reactive Oxygen Species (ROS). Physiologically, spermatozoa produce ROS as a byproduct of aerobic respiration metabolism. Due to cytoplasmic insufficiency, human sperm structurally have few intracellular antioxidant enzymes. Therefore, sperm are more vulnerable to their own generated ROS. Interestingly, in large quantities, ROS can cause oxidative stress that may lead sperm towards apoptosis; however, if present in adequate amounts, it can induce the capacitation signaling pathway in sperm. The main focus of this paper is to discuss the role of ROS in inducing capacitation signaling pathway in human sperm.

Abstract

Introduction: Osteoarthritis (OA) is a degenerative joint disease that causes pain, inflammation, and impairment in physical functions. Antioxidants, which neutralize free radicals, have shown potential in modulating inflammatory responses associated with OA. Vitamin E can suppress IL-1β-induced toxicity of chondrocytes, preserving cartilage integrity. However, further investigation is needed to determine their efficacy in diverse populations and conditions, such as osteoarthritis (OA). Methods: The search was conducted across five databases: PubMed, Cochrane, Epistemonikos, Scopus, and ProQuest, as well as EBSCO, up to January 2025. Eligibility criteria included data availability in a compatible language, inclusion of intervention studies, involvement of animal or human samples with knee osteoarthritis, utilization of antioxidants for efficacy outcomes, and consideration of at least the interleukin-1 beta parameter. Results: The study thoroughly evaluated 13 studies, removing duplicates and conducting an initial screening of abstracts, and assessed their demographics and clinical characteristics. Six studies were evaluated using the RoB 2.0 tool for randomized clinical trials, and seven studies were assessed using the SYRCLE tool for animal trials. All studies have a low risk of bias. Conclusion: Antioxidants are effective in treating knee osteoarthritis (OA) compared to a placebo, but dosage is crucial for optimal results. Further safety studies are needed.

Abstract

Dapsone hypersensitivity is a form of drug allergic reaction involving multiple organs. This article reports the case of a man who experienced dapsone hypersensitivity after undergoing treatment for multibacillary leprosy with the dapsone regimen. The patient was diagnosed with dapsone hypersensitivity based on the presence of a skin eruption in the form of erythroderma and involvement of various internal organs. Hypersensitivity to dapsone is potentially fatal. Limited facilities for risk factor screening become a challenge in preventing the incidence of dapsone hypersensitivity.

Abstract

Plasmodium vivax remains an important cause of malaria, especially in children who are at higher risk of complications such as anemia and thrombocytopenia. We report a 15-year-old girl with intermittent fever, cough, and weakness. Laboratory tests showed severe thrombocytopenia and leukocyte abnormalities, and peripheral blood smear confirmed Plasmodium vivax infection. The patient was treated with dihydroartemisinin–piperaquine and primaquine, leading to gradual clinical improvement and clearance of parasitemia. This case highlights that P. vivax malaria in children can mimic other febrile illnesses and cause significant hematologic changes. Early diagnosis and appropriate therapy are essential to prevent complications and relapse. The successful use of dihydroartemisinin–piperaquine with primaquine in this patient demonstrates the importance of radical cure and careful clinical monitoring in pediatric malaria management. Abstract

Background: Androgenetic alopecia (AGA) is the most common form of progressive hair loss. This disease is known as a polygenic disorder characterized by variability in age of onset, severity and pattern. Genome-wide association studies (GWAS) have shown that variant rs1998076 at locus 20p11 potentially increase the risk factors in Asian and European populations. Association studies between AGA and genetic risks are limited in Indonesian population. Methods: We conducted a cross-sectional pilot study (January-August 2025) in Jakarta, Indonesia, recruiting 100 participants (50 AGA cases and 50 controls). Clinical AGA status was determined by physician. Anthropomorphic data such as height, weight, blood pressure, and smoking history were obtained by measurement and questionnaire. Genotyping of rs1998076 was done by allele-specific PCR method and Sanger sequencing were used to validate the results. Results: Genotype distribution of rs1998076 was AA (24%), GG (37%), and AG (39%). The frequency of allele A of rs1998076 is 0.435, while allele G is 0.565. However, AGA was not significantly correlated with the rs1998076 genotype (p > 0.05) in multivariate models, although GG and AG genotypes showed a tendency for a higher risk of AGA compared to the AA genotype. Conclusion: In this pilot study, rs1998076 was not significantly associated with AGA, although the GG genotype showed a non-significant trend toward higher odds. Clinical factors (age, male sec, higher BMI and hypertension) showed correlations. Larger sample size and well-powered studies are needed to clarify the role of rs1998076 in Indonesians. Abstract

ABSTRACT Background: Cardiac disorders with Unstable Haemodynamics often require intensive care in the intensive care unit (ICU) for monitoring and advanced medical interventions. The length of hospitalisation and prognosis of patients in the ICU are important indicators in evaluating the efficiency of health services. Methods: The design of this study was descriptive analytic with a retrospective approach, using medical record data of cardiac patients admitted to the ICU of Hermina Ciruas Hospital from January 2024 to December 2024, there were 138 patients who met the inclusion criteria. Data obtained included age, gender, comorbidities, disease diagnoses, average length of stay, mortality rates. Data were described in tabular form. Results: Of 138 cardiac patients in the ICU of Hermina Ciruas Hospital, it was found that the age group < 65 years was the largest number of respondents (81.2%), males dominated around 62.3% of the total patients. The most common medical diagnosis was ischaemic heart disease, which was around 68.8% with comorbidities of hypertension as much as 62.3% followed by diabetes mellitus 27.54%. and the results of the chi-square independence test informed that there was a significant association between the scoring/classification of the severity of heart disease and the length of stay in the ICU (p = <.001). Conclusion: The severity of heart disease affects the length of ICU stay. Keywords: Length of stay, Severity level of heart disease, Intensive care Unit

Abstract

Background: Anti-obesity drugs are associated with adverse side effects; therefore, non-pharmacological approaches without side effects, such as regulating dietary patterns and engaging in physical activity, are more strongly recommended for obesity management. Consequently, supportive therapies such as pulsed electromagnetic field (PEMF) exposure are needed to maximize the recovery potential of individuals with obesity. Obesity leads to an abnormal increase in the number and size of white adipose tissue through the differentiation of new adipocytes (adipogenesis). Objective: This study aimed to determine the molecular effects of PEMF exposure by examining the expression of ZFP423 and PPARG in obese mice. This research was conducted as an effort to contribute to the reduction of obesity prevalence. Methods: This experimental study used male C57BL/6J mice induced to become obese through a high-fat diet. The mice were exposed to PEMF for 2, 7, 14, and 21 days. The parameters assessed included the expression of ZFP423 and PPARG using qRT-PCR. Results and Discussion: C57BL/6J mice developed obesity following high-fat diet administration, as indicated by a Lee index value > 0.310. The expression levels of ZFP423 and PPARG genes relative to the control gene β-Actin showed a decrease after 2 days of exposure. ZFP423 expression increased again after 7, 14, and 21 days of exposure, whereas PPARG expression continued to decline at 7, 14, and 21 days of exposure. Conclusion: PEMF exposure reduced ZFP423 expression in obese mice exposed for 2, 7, 14, and 21 days compared to obese mice without exposure. In addition, PEMF decreased PPARG expression in obese mice across all exposure durations when compared to non-exposed obese mice, although these differences were not statistically significant.
