Abstract

Peripartum cardiomyopathy (PPCM) is a potentially life-threatening pregnancy-related condition that typically occurs during the peripartum period. It is characterized by impaired function of the left ventricle and heart failure. Preeclampsia is recognized as a risk factor for PPCM and can coexist with this condition. However, there is limited available data that explains the variations in treatment approaches between cases of peripartum cardiomyopathy with and without preeclampsia. Hence, the objective of this study is to examine the distinctions between PPCM and PPCM accompanied by preeclampsia, encompassing their molecular, pathogenic, and pathophysiological aspects, as well as their treatment and management strategies. These factors could potentially play a role in influencing the treatment of patients diagnosed with peripartum cardiomyopathy. A thorough review of existing literature was conducted between June and August 2023. Articles were retrieved from medical databases including PubMed, EBSCOhost, Medline, SAGE, JSTOR, and ScienceDirect, utilizing keywords such as "Peripartum Cardiomyopathy," "PPCM," "Preeclampsia," "Biomolecular," and "Treatment." This study encompassed original articles written in either English or Indonesian, focusing on a ten-year time frame. Articles categorized as literature reviews, systematic reviews, and/or meta-analyses were excluded from consideration in this study. The Results of this review, In instances of PPCM accompanied by preeclampsia, the secretion of sFlt1 from the placenta is notably elevated compared to cases of PPCM alone. PPCM and PPCM with preeclampsia exhibit distinct pathogenic and pathophysiological characteristics. Patients with concentric left ventricular remodeling (PPCM with preeclampsia) may not derive the same degree of benefit from angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, or beta-blockers as those with eccentric left ventricular remodeling (PPCM without preeclampsia). The Conclusion of this review, PPCM and PPCM with preeclampsia are differentiated by underlying biomolecular factors, pathogenesis, and pathophysiology. Furthermore, they respond differently to treatment and have different approaches to management.

Abstract

Introduction: Blood transfusion can cause infectious complications through transfusion of microbes present in asymptomatic donor blood and/or contamination of stored blood products such as Hepatitis B, Hepatitis C, HIV, syphilis, malaria, Cytomegalovirus (CMV) infection. The risk of infection increases with the amount of blood products transfused.  Case presentation: A 5-month-old boy who had never been to malaria endemic area with jaundice and abdominal enlargement since two months ago, fever since one month ago, and icteric sclera. Data from laboratory results shows anemia, leukocytosis, hyperbilirubinemia, elevated liver enzyme, positive IgM and IgG anti-CMV. Blood smear evaluation:  Trophozoite and ring forms of malaria parasites. Abdominal USG: Type-I Biliary atresia, Choledochal cyst, splenomegaly, ascites, and right pleural effusion. Discussion: An acute Transfusion-transmitted CMV infection in high-risk patients can have severe complications such as billiary atresia, myocarditis, retinitis, encephalitis, or encephalopathy. Leucodepleted blood products for transfusion can reduce the risk of infection. Thick and thin blood smears which is the gold standard for diagnosing malaria, cannot be used for donor screening. Asymptomatic malaria infections may remain undetected. Conclusion: Blood transfusion screening for infectious diseases is still very limited for Hepatitis B, Hepatitis C, Syphilis, and HIV. It is necessary to think about blood transfusion screening for other infectious diseases, such as CMV and malaria, especially in endemic areas, to prevent the occurrence of transfusion-transmitted infection.

Abstract

Background Breast cancer is the most frequently diagnosed cancer worldwide, including in Indonesia, with the highest cancer survivors in Central Java. Mastectomy is one of the procedures in breast cancer treatment which result in major physical and mental alteration, possibly causing Body Image Distress (BID), risking them for depression and suicide. Expressive Writing Therapy (EWT) is an intervention that has been proven to reduce BID in breast cancer patients. Objective: This study aimed to analyze the impact of providing EWT on reducing post-mastectomy breast cancer patients Methods This is an experimental study using a two-group pretest-posttest with CG design conducted on 46 respondents at the oncology surgery clinic at a General Hospital in Central Java. Sample obtained using purposive sampling technique. BID scores were measured using the Body Image Scale (BIS). Data were analyzed in univariate, bivariate, and multivariate analysis using SPSS. Results Data analysis using Mann-Whitney Test showed a significant difference in BID scores reduction between intervention group (IG) and control group (CG) (p=0.00, significance p<0.05). Multivariate analysis using Linear Regression Test showed a significant influence of EWT on reducing BID scores (p=0.00). Conclusions Expressive Writing Therapy (EWT) significantly reduces BID scores in post-mastectomy breast cancer patients in a General Hospital in Central Java, Indonesia.

Abstract

Background Adolescents face numerous obstacles, including physical, emotional, and social changes that can have an impact on their self-esteem and mental health. Inability to deal with these difficulties can result in emotional disorders such as depression, anxiety disorders, and stress. Low self-esteem, or an individual’s image of oneself, is one of the elements that might precipitate the start of emotional disorders. The aim of this study is to determine the relationship between self-esteem and emotional disorders in adolescents at senior high school. Methods An observational analytic analysis with a cross-sectional design is used in this study. The study was carried out on SMAN 23 Jakarta students, and involved 131 respondens, which selected using consecutive random sampling methods. The RSES (Rosenberg Self-Esteem Scale) questionnaire was used to assess self-esteem, and the DASS-21 (Depression Anxiety Stress Scale) was used to examine emotional disorders. Results The study's findings were evaluated using the Pearson chi-square test, and no significant relationship was found between grade level and emotional disorders such as depression (p=0.691), anxiety (p=0.969) and stress (p=0.313). Furthermore, there was no significant relationship between body mass index and emotional disorders, such as depression (p=0.962), anxiety (p=0.257), and stress (p=0.975). However, a significant relationship was found between gender and emotional disorders, including depression (p=0.011), anxiety (p=0.000) and stress (p=0.009). Moreover, there was also a significant relationship between self-esteem and emotional disorders, including depression (p=0.002), anxiety (p=0.029), and stress (p=0.002). Conclusions This study found that there was no relationship between grade level, body mass index, and emotional disorders. However, a significant relationship was identified between gender, self-esteem, and emotional disorders  among SMAN 23 Jakarta students.

Abstract

Background Sepsis and septic shock are global health problems with high morbidity and mortality rates. The ability to identify sepsis and septic shock with tools available in health facilities in Indonesia is an important step in reducing the morbidity and mortality of this disease. One of the tests that can be performed is a complete blood count such as the number of leukocytes, platelets, hemoglobin levels, and neutrophil-lymphocyte ratio (NLR). The costs incurred for complete blood counts are low and generally available in all health facilities. Methods This study is an analytical observational study with a cross sectional research design using medical record data of sepsis and septic shock patients in the PICU of RSUP Dr. M. Djamil Padang in 2020 - 2022. Univariate analysis is presented in the form of a frequency distribution table and bivariate analysis using the Chi Square test. Results The results showed that the most common diagnosis was septic shock (74.5%), male (57.5%), age under one year (38.3%), and death outcome (59.6%). Bivariate analysis found that there was a difference in platelet count between sepsis and septic shock (p=0.004) and there was a difference in NLR between sepsis and septic shock (p=0.024). Conclusions The conclusion of this study is that there are differences in platelet count and NLR between sepsis and septic shock patients in the PICU of RSUP Dr. M. Djamil Padang.

Abstract

Myopia or near-sightedness is the most common ophthalmologic disorder in the world. Myopia prevalence has significantly increased over the last decade, especially in the pediatric population. During the COVID-19 pandemic, governments worldwide implemented home quarantine and school closures to contain and mitigate the virus’s spread. This resulted in increased screen time, more time doing near-work activities, and decreased time spent outdoors. This study was to compare myopia progression in children before and during the COVID-19 pandemic and examine pandemic-related lifestyle risk factors for myopia progression. Methods of this study were A comprehensive search of Ovid MEDLINE and PubMed for potential articles was conducted following the PRISMA 2020 guidelines. The data extracted from each included studies included epidemiologic data on myopia in children, changes in spherical equivalent (SE), and the amount of outdoor activity, screen time, and near work before and during the COVID-19 pandemic home quarantine. This review included six studies, comprising two retrospective cohort studies, three prospective cohort studies, and one cross-sectional study. Epidemiological data showed increased pediatric myopia cases and a greater SE difference during the COVID-19 pandemic. During the pandemic, it was reported that children spent more time on screens, engaging in near-work activities, and less time outdoors. Multiple logistic regression analyses consistently showed increased screen time as a significant risk factor for myopia in children during the pandemic, while outdoor time and time spent doing near-work activities were not statistically significant. Conclusion of this study where Myopia progression in children was more rapid during the COVID-19 pandemic, with increased screen time identified as a risk factor. More research is needed to further investigate other pandemic-related risk factors for myopia in children.

Abstract

Dermoid and epidermoid cysts are prevalent benign orbital tumors in childhood, representing 3-9% of pediatric ocular tumors. Occasionally, dermoid cysts can attain significant size, leading to cosmetic deformities. A precise surgical approach to dermoid cysts can lead to considerable cosmetic improvement for the patient. A 19-year-old female presented with swelling in the outer corner of her left eye, accompanied by minor discomfort in the affected area. Upon ophthalmologic examination 1.5x1 cm masses were detected in the upper left eyelid near the lateral canthus Clinical diagnosis of Dermoid cyst left eye upper eyelid made. Subsequently, a surgical excision under general anesthesia was scheduled. During the procedure, the cyst was successfully excised while preserving the cyst wall integrity. The conclusion is imperative for ophthalmologists to recognize the necessity of precisely performing surgical excision of these cysts to achieve favorable functional and aesthetic outcomes.

Abstract

Background The need for CT Scan radiology examinations, which offer more detailed image results than X-rays, continues to increase. Head CT scans are the most frequently performed examinations, reaching 60%. Objective This study aims to evaluate the effective dose distribution of radiation to the tissues involved in head CT-Scan examinations at RS Hermina Bitung and assess its compliance with the regulations set by the Indonesia Diagnostic Reference Level (I-DRL). Methods This study involved 100 patients, consisting of 50 men and 50 women. Data in the form of Dose Length Product (DLP) values were collected from each patient and converted to effective dose using a radiation dose calculator. Results The results showed that 6 out of 100 samples exceeded the dose limit set by the Nuclear Energy Regulatory Agency (BAPETEN). Factors that affect the effective dose include exposure, body weight, object thickness, scan length, age, tissue weight, tissue sensitivity, FOV, and pitch. Nonetheless, the overall average effective dose of the patients was still within the I-DRL. Conclusions Factor such as gender can affect the effective dose received by patients. The 75 percentile value of the effective dose of non-contrast head CT-Scan examination at Hermina Bitung Hospital is in accordance with the provisions of the I-DRL. The radiation dose calculator used can be applied in Indonesia, but still requires increased information literacy regarding its use.

Abstract

Blepharitis is an eyelid infection that increases in incidence with age. This infection can cause anterior and posterior blepharitis and is caused by bacteria, viruses, fungi, and parasites. The most common bacteria that cause blepharitis is Staphylococcus. Blepharitis that is not caused by infection is allergic blepharitis. The pathophysiology of anterior blepharitis is the occurrence of bacterial colonization that triggers an excessive inflammatory response, while in posterior blepharitis it is mainly due to a decrease in the quality of the meibomian glands which causes dry eye and causes friction on the conjunctiva and cornea, causing inflammation. Symptoms of blepharitis are discomfort on the eyelid's surface such as itching, burning, stinging, watery eyes, and others. Risk factors for this infection include seborrheic dermatitis, acne rosacea, contact allergies, diabetes mellitus, chemical irritation, poor hygiene, low socioeconomic status, cosmetics, and the elderly. Diagnosis can be made based on anamnesis and ophthalmological examination which a general practitioner or eye specialist can perform. Based on the diagnosis, further management is given to avoid complications. The management that can be given is maintaining eyelid hygiene, warm compresses, not rubbing the eyes, and giving topical antibiotics (in anterior blepharitis) which can be added with systemic antibiotics (posterior blepharitis). Complications that can occur are eyelash abnormalities, lacrimal punctum malposition, eyelid malposition, recurrent hordeolum, keratitis, dry eye, keratopathy, corneal ulcers, and corneal opacities that can cause impaired visual function.

Abstract

Background Intestinal tissue damage is one of the effects of increased reactive oxygen species (ROS) in hyperlipidemia. Cells damaged by hyperlipidemia experience changes in the biomarker malondialdehyde (MDA). The use of jicama extract and kefir alone has shown the ability to lower cholesterol levels. This study aims to determine the effect of the combination of jicama extract and kefir milk as a synbiotic drink on MDA levels in the intestines of hyperlipidemic rats. Methods Twenty-five male Wistar rats were divided into 5 groups, consisting of negative control (K-) and positive control (K+) groups and three treatment groups (P1, P2, P3). Hyperlipid induction using quail egg yolk. The dose of quail egg yolk was 5ml/200grBW. The intervention group was given a synbiotic drink with a dose of 5ml/200grBW with 3 types of kefir milk and jicama extract combination formulations. Intestinal MDA levels were measured after termination and the data were analyzed using statistical software. Results The level of MDA in intestine (nmol/ml) were 11.78±0.21 (K+), 0.73±0.05 (K-), 5.38±0.28 (P1), 3.02±0.14 (P2), 1.59±0.11 (P3). The analysis of the results of the MDA level showed significant differences in all groups after being given synbiotic drinks (p <0.001). Conclusions Combination of jicama extract and kefir as synbiotic drinks significantly affect the intestinal MDA levels of hyperlipidemic rats.
