Abstract

Introduction: India, the country with the highest prevalence of diabetes, has between 62,4 and 77,2 million diabetics and pre-diabetics. The purpose of this study was to investigate the biological correlation between sex hormone profile and type 2 diabetes in Indian men. Methodology: In this case-control study, total of 181 diabetic cases and 181 healthy controls were enrolled as per WHO norms. Along with clinic-demographical data, fasting blood glucose, Postprandial blood glucose, HBA1c, insulin, lipid profile and Testosterone, LH, FSH were measured and compared. Results: A non-significant difference was observed [p=0.7831] between different ages among the study population's case and control groups. The FPG, HbA1c, total cholesterol, LDL, triglycerides were significantly elevated in cases as compared to controls, except HDL and SHBG, showing non-significant differences. The spearman correlation between Testosterone and different parameters, and all the correlations showed a significant negative correlation. However, Testosterone Vs. Testicular Volume (ml) [r=0.2981], Testosterone Vs. HDL cholesterol(mmol/l) [r=0.04884] and Testosterone Vs. Calculated Free Testosterone (mmol/L) [r=0.007494] respectively shows significant positive correlation. Conclusion: Type 2 Diabetes male patients had lower testosterone levels. As a biomarker for Type 2 Diabetes, it is possible to measure changes in serum testosterone levels.

Abstract

The Diagnostic and Statistical Manual of Mental Disorders (DSM-5) defines an anxiety disorder as an excessive feeling of fear that occurs in a person that interferes with daily activities. Generalized anxiety disorder causes a person to feel anxious about a wide range of situations and issues. Patients often experience anxiety, feelings of fear and constantly on edge, difficulty concentrating, difficulty sleeping, muscle tension, and frequent irritability. Based on data from the Indonesian Ministry of Health in 2007, there were 450 million people who had to live with mental disorders. The prevalence of adult individuals (aged 18 years and over) and the elderly, there were 11.6% who experienced emotional disorders, such as anxiety and depression. This case report discusses a 20-year-old woman with the main complaint of unclear anxiety, not knowing what to worry about, floating between things she worries about is death, fear of having a serious illness, breakup, fear of losing her job as a content creator, and feeling of being blamed that keeps on coming. Management in the form of giving SSRI antidepressants and psychodynamic psychotherapy is done to try to reinterpret feelings of guilt that often arise as a result of fixation in one phase of their growth and development.

Abstract

Background: The diaphragm is a dome-shaped respiratory muscle located near the lower ribs, the exact location is below the chest. Diaphragmatic rupture is a medical condition caused by blunt or penetrating trauma, which often leads to herniation of the abdominal viscera into the thoracic cavity. Case: We presented a case in which a 39-year-old man complained of left chest pain and abdominal pain radiated to his back, after involed in a traffic accident. On physical examination, the left chest was left behind when breathing, a dull sound was found on percussion, with diffuse abdominal tenderness. Thoraco Abdomen CT-scan was performed and it concluded that there was a rupture of the left posterior diaphragm with a diameter of 15 cm which caused herniation of the gastric and intestines to the left of thoracic cavity, accompanied by a collapse of the left lung due to blunt trauma. The patient underwent an explore laparotomy with the aim of repairing his diaphragmatic rupture, and splenectomy procedure to remove the affected spleen. Conclusion: Surgery is a mandatory action in order to repair the diaphragm. The choice of the approach to be used, either laparotomy or thoracotomy depends on other organ abnormalities, acute or latent cases, stable or unstable condition of the patient and also depends on the experience of the operator.

Abstract

BACKGROUND: Anaemia and abnormal left ventricular (LV) geometric pattern are common findings in Chronic Kidney disease (CKD) patients. OBJECTIVES: To assess LV geometric pattern and its relationship with anaemia among CKD patients. METHODS: A cross sectional study of 163 subjects (102 and 61 CKD subjects with and without anaemia respectively). Echocardiography determined the LV geometric pattern while packed cell volume (PCV) levels determined anaemia. RESULTS: The mean age of subjects with and without anaemia was 54.04 ± 14.47 and 54.92 ± 15.67 years respectively (p = 0.717) while the prevalence of LVH among the two groups was 68.8% and 57.9% respectively (p = 0.174). The most frequent LV geometry in both groups was concentric LVH (53.8% and 43.9% respectively). Prevalence of LV systolic dysfunction was 45%, higher among anaemic subjects (58(61.7%) vs 10(17.5%)) p < 0.001. There was a strong negative correlation between PCV and left ventricular mass index (r = -0.345, p = 0.001) among anaemic subjects, but weak positive correlation among patients without anaemia (r = 0.001, p = 0.993). CONCLUSION: Anaemic CKD patients had a high prevalence of abnormal LV geometry with significant contribution from anaemia. Early management of anaemia may thus improve cardiovascular outcomes.

Abstract

Background: Irritant contact dermatitis (ICD) is an occupational disease that involves non-immunological inflammatory mechanisms of the skin, resulting from a response to exposure to irritants, physical, or biological. ICD disease often occurs in car wash employees who are exposed to laundry soap containing sulfate. Most of the factors that cause ICD are the length of exposure to chemicals or from the workers themselves such as knowledge, use of personal protective equipment, and personal hygiene. The purpose of this study was to determine the effect of using personal protective equipment (PPE) and exposure to sulfate content of car wash soap on the incidence of ICD in car wash employees in the city of Malang with and without a history of skin disease. Method: The research method uses an analytic observational research type with a cross sectional design. The samples studied were 84 respondents who were car wash employees in the city of Malang, both those who had a history of skin diseases (39 respondents) and those who did not have a history of skin diseases (45 respondents). Data were analyzed using univariate and bivariate tests with SPSS application. Results: The results of the chi square test showed that there were significant differences in risk factors (p < 0.05) on the incidence of ICD. Conclusion: The conclusion of this study is there is an effect of the use of PPE and exposure to sulfate content on the incidence of ICD in car wash employees in the city of Malang with and without a history of skin diseases.

Abstract

Background & Aim: Enteric fever is a major public health concern around the world and endemic in low- and middle-income countries like, India. Typically, it spreads through contaminated food or water. Salmonella Typhi proliferate and spread throughout the bloodstream seeding multiple organs in the body. Incidence data of culture confirmed Typhoid cases is approximately 377per 100000 population with an approximate case fatality rate of 1% in India. The management of cases are hampered due to emerging the drug resistance of isolates because of rampant and misuse of antibiotics. This study investigates to analyse the current pattern of antibiotic susceptibility among Salmonella isolates from cases of enteric fever seen at ESIC Medical College & Hospital, Faridabad. Material & Methods: This is a retrospective, cross-sectional study. Blood cultures from patients with suspected enteric fever from Jan 2017 to Dec 2019 were included. The blood cultures were processed using the BacT/Alert automated blood culture system. Results: During the study period, a total of 4064 blood culture specimens were received from the patients suspected for fever. Out of a total of 2717 culture positive samples, 373 (13.73%) were found positive for the growth of Salmonella enterica [Salmonella typhi 87.13% (325/373), Salmonella paratyphi A (12.86%)], confirming the enteric fever. Conclusion: Treatment with appropriate antimicrobial drugs is crucial for patients with typhoidal infections and the antimicrobial susceptibility of such isolates must be reported as soon as possible.

Abstract

Introduction: Dermatophytes are pathogenic fungi that cause cutaneousinfection of human and animal and grow exclusively on the stratumcorneum, nail and hair. Methods: The present study was conducted on 160 samples from clinicallydiagnosed onycomycosis patients, further subjected to culture from nailsamples of patients attending dermatology OPD of a tertiary care hospital,Delhi from January 2016 to December 2018 Results: In a soy protein culture, a substantial proteolytic activity was seen, which was secreted by Trichophyton rubrum, Trichophyton mentagrophytes and Microsporum canis. This proteolytic activity was 55–75% inhibited by O-phenanthroline, which authenticate that metalloproteases were secreted by all three species. A consensus probe was constructed on previously characterized genes, which encodes metalloproteases (MEP) of the M36 fungalysin family in Aspergillus fumigatus, Aspergillus oryzae and M. canis. From genomic libraries of T. rubrum, T. mentagrophytes and M. canis, T. rubrum & T. Mentagrophytesa five-member MEP family was isolated and also secretes MCP A & MCPB, MEP 3, MEP4, IL, CS, MS AND DPP V of the M14 family accordingto the MEROPS proteolytic enzyme database. Conclusion: This study shows that Metalloprotease, Metallocarboxypeptidase, Isocitrate Lyase, Citrate Synthase, Malate Synthase and Dipeptidylpeptidase V are expressed in Trichophyton Rubrum Isolates from Dermatophytosis Patients.

Abstract

Background: Maternal Mortality Rates (MMR) and Infant Mortality Rates (IMR) are two of the indicators on the success of health programs in Indonesia. Jember has become the district with the highest rate of maternal and infant deaths throughout 2020-2021. Methods: This research assessed the contributing factors of MMR and IMR in Puskesmas Kencong, Jember Regency. This study uses a qualitative descriptive research design. Data in this study were taken by conducting interviews to fill out questionnaires to mothers who had given birth at least once and the Coordinating Midwife and Head Midwife of PONED (Basic Emergency Neonatal Obstetrics Services) at the Puskesmas Kencong. Then the data from the questionnaires and interviews will be processed and then explained in the narrative. Results:. Based on data from questionnaires filled out by 37 respondents, as well as questions posed to the midwife, Puskesmas Kencong has fulfilled the requirements needed as a PONED Health Center according to PONED Guidelines. Conclusion: The PONED Health Center at Kencong Health Center has a low prevalence of MMR and IMR.

Abstract

Background: Health inequality often occurs among disadvantaged population groups, including migrants. Currently, the migrant population does not always receive adequate health services. In addition, the health care system is not optimized for migrants, especially in terms of language, access, genomic data and the expertise of medical personnel. The health condition of these migrants is a global problem that needs attention if countries want to meet the Sustainable Development Goals (SDGs) targets by 2030. Purpose: to map the latest existing research on the topic of migrant health, both qualitative and quantitative. Methods: narrative literature review and literature search were carried out using an electronic database with the automatic selection feature used in the electronic database according to the specified inclusion criteria. Results: Obtained an overview of inequality in [1] access to health services for migrants in general, [2] utilization of health services and health-seeking behavior, [3] health literacy, [4] health services related to the COVID-19 pandemic, and [5] maternal and child health services. All authors agree on this division of groups. Conclusion: Migrants who do not have documents or are illegal, skin color (black migrants) who migrate to western countries, languages ​​that are not the same, and do not understand their own health conditions due to lack of health education due to language barriers, these factors are obstacles for migrants to achieve equality in countries where migrants have migrated.

Abstract

Background: Coronavirus is the major pathogens at human respiratory system. The antibody is a response infected patients largely not clearly. We need to understanding of antibody responses to great diagnosis and treatment studies. Objective: In this article the objective is to describe that kinetics of IgM and IgG SARS-CoV-2 in recovery patients. Method: Cohort study used a total 19 subjects who had negative evaluation RT-PCR after confirmed. IgG and IgM of SARS-CoV-2 were detected by IFA (Immuno fluorosence assay) used serum. Serum were collected 3 times after 1 month, 2 month and 4 month negative evaluation RT-PCR We profiled the serological responses (IgM and IgG) to SARS-CoV-2. Result: Majority the IgM SARS-CoV-2 post evaluation RT-PCR were very low after 1,2,4 month negative evaluation. IgG SAR-CoV-2 patients post negative evaluation RT-PCR decreased after 4 months. The level IgM and IgG level increase at first week and decrease after 12 weeks. Conclusion: IgM level lower than IgG level overtime. Quantitative IgG and IgM detection could be point of diagnosis and manifestation.
