Abstract

Background: Diabetes mellitus management includes dietary restrictions, increased physical activity, an appropriate medication regimen, regular medical follow-up, and periodic metabolic monitoring through laboratory examinations. Adherence of diabetes mellitus patients to therapy that has been indicated and prescribed by physicians will lead to positive therapeutic effects. Purpose: The aim of this study is to identify the relationship between patient knowledge and medication adherence among type 2 diabetes mellitus patients at Simarmata Primary Health Center, Samosir Regency. Methods: This study employed a quantitative research approach with a cross-sectional design. The population consisted of all diabetes mellitus patients who were undergoing routine medical follow-up at the health center, totaling 32 individuals. The sample was selected using a total sampling technique, resulting in 32 respondents. Results: The study findings showed that 56.3% of respondents had good knowledge, with 34.4% demonstrating adherence to medication and 21.9% non-adherence. Respondents with moderate knowledge accounted for 37.5%, of whom 34.4% were non-adherent and only 3.1% adherent. The statistical test resulted in a significance value of Sig. = 0.001, which was lower than 0.005, indicating a significant relationship between knowledge and medication adherence. Conclusion: It can therefore be concluded that there is a significant relationship between patient knowledge and medication adherence among people with type 2 diabetes mellitus. Respondents are encouraged to further improve their adherence to prescribed medication in order to increase the likelihood of successful treatment outcomes and achieve a higher rate of recovery.

Abstract

Working mothers in the health sector, particularly in community health centers, face the challenge of playing dual roles as professionals and homemakers. This situation has the potential to cause psychological fatigue and work stress, which can impact their performance. This study aims to analyze the relationship between psychological fatigue and work stress and their effects on the performance of working mothers at the Alasa Public Health Center (PHC) in North Nias Regency. This study is quantitative and cross-sectional. The sample comprised 36 married women employed at the Alasa PHC. Data were collected using a questionnaire that had been tested for validity and reliability. Data analysis was performed using univariate and bivariate analysis using the chi-square test. The results showed a significant relationship between psychological fatigue and performance (p = 0.0005 < α = 0.05) and a significant relationship between work stress and performance (p = 0.001 < α = 0.05). Mothers who experienced psychological fatigue and high work stress tended to show low performance. This situation is exacerbated by heavy workloads, significant domestic responsibilities, particularly within the family environment, lack of rest time, and minimal social support. This study recommends managerial efforts to identify and manage psychological factors affecting working mothers, including emotional support from superiors, colleagues, and family, adequate rest time, more flexible work schedules, and improved welfare. The implementation of these strategies is expected to reduce psychological fatigue and work stress, thereby significantly and measurably improving the performance of female health workers, particularly working mothers, thereby positively impacting overall performance.

Abstract

Background: Diabetic foot ulcers are a major complication of diabetes mellitus that increase the risk of infection, amputation, and poor quality of life. Adherence to foot care plays an important role in preventing ulcer progression, yet many patients have poor adherence. Objective: This study aimed to examine the association between foot care adherence and diabetic foot ulcer severity among patients with diabetes mellitus at the Asri Wound Care Center Clinic, Medan, Indonesia.   Methods:This analytical cross-sectional study included 52 patients with diabetic foot ulcers selected through purposive sampling from a population of 109 patients. Foot care adherence was assessed using the Nottingham Assessment of Functional Footcare (NAFF) questionnaire, while ulcer severity was evaluated using the Wagner Classification. Data were analyzed using descriptive statistics and Spearman rank correlation. Results: More than half of the participants (53.8%) had poor foot care adherence, and Wagner Grade 1 was the most common ulcer severity (36.5%). Spearman rank correlation analysis showed a very strong negative correlation between foot care adherence and ulcer severity (r = −0.870, p < 0.001), indicating that better adherence was associated with lower ulcer severity. Conclusion: Foot care adherence was significantly associated with diabetic foot ulcer severity. Strengthening structured and continuous foot care education may improve adherence and reduce ulcer severity among patients with diabetes mellitus.

Abstract

Handwashing with soap represents the mechanical sanitization of dermal surfaces using cleansing agents and clean running water to eradicate pathogens, bacteria, and virus strains that potentially trigger various infectious clinical conditions. The primary goal behind promoting handwashing with soap is to suppress the transmission velocity of communicable agents. This empirical investigation was executed to chart the operational correlation between knowledge levels and the behavioral habit of handwashing with soap among learners at Public Elementary School 013 Simangaronsang, Doloksanggul District, Humbang Hasundutan Regency. Methodologically, This study adopts a quantitative approach using a cross-sectional model, drawing from a target population that simultaneously served as the total sample of 69 students from grades IV and V. Primary and secondary data collection was conducted utilizing a structured questionnaire instrument to evaluate the depth of student comprehension and determine the performance of the habit of handwashing with soap variable among the participants. Statistical hypotheses were verified using a bivariate Chi-Square test. The final research outcomes confirmed a highly significant relationship between health awareness and the practical habit of handwashing with soap, supported by an empirical p-value of 0.001. Based on these dynamics, there is an eye-opening need for tangible interventions to revitalize health infrastructure and deploy systematic educational schedules detailing accurate handwashing with soap steps through visual posters and audio-visual media, supplemented by furnishing robust sanitary assets and ensuring a stable supply of clean water within the school premises
