Abstract

Background: Indonesia's healthcare system is designed with tiered referrals, involving primary, secondary, and tertiary services that work synergistically to provide optimal care. The pre-referral stage is an important element in this system, where patients receive initial treatment at primary health facilities before being referred to higher levels. This stage aims to ensure proper initial care as well as quick and effective referrals according to the patient's condition. However, the implementation of pre-referral programs often faces obstacles such as limited primary health facilities, lack of coordination between facilities, and difficulty accessing supporting information technology. Objective: to analyze the development of a pre-referral program to improve the effectiveness of patient referrals. Research method: Participatory Action Research (PAR) approach, involving Community Health Center and hospitals under the coordination of the Subang Health Office for six months (November 2023-April 2024). The sample consisted of three Community Health Center and five hospitals selected based on their capacity to handle obstetric cases. Data were collected through a Likert scale questionnaire to measure the feasibility of pre-referral media. Results: The results showed that the pre-referral back sheet had high validity (99.6%) and could be used. Interviews revealed problems such as delays in the referral process due to incomplete medical information and inconsistent documentation standards. Conclusion: The importance of periodic evaluation of this program to ensure its sustainability and effectiveness is emphasized. Health worker feedback as well as evaluation of patient care outcomes should be used for continuous improvement in the obstetric referral system.

Abstract

Background: Digital-based service quality is an encouragement for consumers to establish strong bonds with the company in the long term. Challenges in hospital service quality and the effectiveness of healthcare applications must be addressed to improve patient loyalty and ensure hospital sustainability. Objective: To determine the effect of service quality and application quality on patient loyalty at the Cicendo Eye Center Hospital, Bandung. Methods: The research method used is descriptive verification. The sample of this study used the purposive sampling technique, where the number of samples was 159 people selected based on criteria.  Primary data were collected through questionnaires. Data processing uses regression to test the hypothesis, F-test. Results: The findings show that service quality is rated at a moderate level, application quality is rated at a low level, and patient loyalty at Cicendo Eye Center Hospital, Bandung is also rated at a moderate level. Conclusion: Data gathered under joint testing reveal that measures of service quality and application quality demonstrate a statistically significant positive interaction; each remains influential in its own right. In light of that outcome, the two factors can be said to sway patient loyalty. Given these findings, hospital administrators are urged to reassess their information systems in order to bolster user perception and overall satisfaction..

Abstract

Background: Hypertension is a global health problem that contributes significantly to deaths from cardiovascular disease. Unhealthy lifestyles, including excessive salt consumption, physical inactivity, and obesity, are the primary factors. Without management, hypertension can lead to serious complications such as heart disease, stroke, and kidney failure. Objective: This study aims to investigate, summarise, and identify various scientific evidence related to hypertension risk factors. Methods: This study employed a scoping review approach organised according to the PRISMA protocol. The Population–Concept–Context (PCC) framework was utilised as an initial guideline in formulating research questions and searching for relevant articles. The literature search process was conducted using several databases, including Google Scholar and Scopus. The included articles had to meet the inclusion criteria, which were limited to open-access journals published between 2020 and 2025. From a total of 512 articles found, only 14 met the selection criteria and were suitable for analysis. The data extraction process was conducted by eight people on the 14 selected articles, followed by qualitative data analysis. Results: The findings indicate that factors such as age, gender, length of employment, consumption of salt, sugary drinks, high-fat foods, elevated total cholesterol levels, low vegetable intake, lack of physical activity, stress, and abdominal circumference that exceeds the recommended play a role in increasing the risk of hypertension. Conclusion: These factors trigger fluid retention, insulin resistance, dyslipidemia, and central obesity that together raise blood pressure and accelerate vascular damage.

Abstract

Background: Hypertension is a common chronic disease in the community that requires prevention through a healthy lifestyle and high Self Efficacy to manage it effectively. Ling Tien Kung exercise therapy, a non-pharmacological approach for hypertension, involves body movements focusing on two core exercises: empet-empet anus and jinjit-jinjit. This study aims to assess the impact of Ling Tien Kung exercise therapy on Self Efficacy and blood pressure in hypertensive patients. Methods: The research employs a pre-experimental one-group pre-post test design, with 45 respondents selected through purposive sampling. Conducted twice a week over three weeks, data were collected using the General Self Efficacy Scale (GSES) questionnaire and a blood pressure observation sheet. Results: Analysis with the Wilcoxon Signed Rank Test revealed significant improvements in Self Efficacy and reductions in blood pressure before and after the intervention (p-value = 0.000 < 0.05). Conclusion: Thus, it can be concluded that Ling Tien Kung exercise therapy positively affects Self Efficacy and blood pressure in hypertensive patients, suggesting it can be a valuable complementary therapy for managing hypertension.

Abstract

Background: Hypertension or increased blood pressure is a silent killer disease whose main cause is currently unknown. High blood pressure is the highest case with an incidence reaching 22% of the world's population, or around two-thirds of hypertension sufferers coming from countries with lower-middle incomes. One of the factors causing hypertension or increased blood pressure is poor sleep quality. Purpose: The objective of this  was to identify the relationship between sleep quality and blood pressure in hypertensive patients undergoing hemodialysis Methods: The study was a quantitative survey with a cross-sectional approach in a population of End State Renal Disease patients with hypertension underwent hemodialysis at RSUP. H. Adam Malik, Medan. A total of 57 individuals were sampled using a purposive sampling technique.  The measuring instrument uses a questionnaire and a digital tensiometer. Univariate and bivariate data were obtained from statistical analysis using a computer. Results: Statistical tests used Spearman rank. The outcomes of the study showed the average score of respondents' sleep quality was 13.16, meaning poor sleep quality, and the average systolic blood pressure was 147.26 mmHg, meaning grade 1 (mild) hypertension, and diastolic blood pressure was 93.81 mmHg, meaning grade 1 (mild) hypertension. Statistical tests showed a significant value of p-value = 0.000 (α <0.05), This means there are relationship between sleep quality and blood pressure in hypertensive patients undergoing hemodialysis. Conclusion: It is recommended that health workers improve the quality of their services by providing education to have good sleep quality and controlled blood pressure.

Abstract

Background: Changes in sleep quality as a consequence of the aging process in the elderly can affect sleep quality. So older adults need to get good and appropriate sleep hygiene practices in order to increase sleep efficiency. Purpose: The purpose of this study was to examine the association between sleep hygiene behavior with sleep quality in older adults. Methods: This research is quantitative with the design used being a cross sectional study. The sample used in this research was 49 elderly people. Statistical analysis was performed using the Chi-Square test. Results: the results of bivariate analysis showed a p-value of 0.032 (p<0.05), indicating a statistically significant relationship between sleep hygiene behavior and sleep quality in the elderly. nursing interventions based on sleep hygiene education and modification can be an effective non-pharmacological approach to improving the aging population’s health and overall quality of life. Further research is recommended to explore additional factors such as environmental conditions, chronic health status, and social support that may influence sleep quality in the elderly. Conclusion: The importance of nursing services is that they can implement sleep hygiene practices which can be used as independent interventions by nurses in an effort to increase sleep efficiency in the aging population

Abstract

Myasthenic crisis is an acute exacerbation of myasthenia gravis that results in weakness of the respiratory muscles, potentially leading to acute respiratory failure and the necessity for mechanical ventilation support. Approximately 15-20% of individuals with myasthenia gravis will experience a myasthenic crisis at some point in their lives, making it one of the most dangerous and life-threatening complications if not addressed promptly. Fortunately, mortality rates associated with myasthenic crises are currently decreasing, partly due to the effectiveness of therapeutic plasma exchange (TPE). TPE therapy works by removing pathological antibodies, immune complexes, and cytokines from the plasma and replacing them with replacement fluid. In this report, we discuss the case of a 61-year-old man with myasthenia gravis who was admitted to the emergency department (ED) and then to the intensive care unit (ICU) for one day due to a myasthenic crisis. He was intubated and received mechanical ventilation support, followed by plasma exchange therapy, which resulted in clinical improvement
