Abstract

Diabetes Mellitus is a chronic metabolic disorder characterized by hyperglycemia resulting from defects in insulin secretion, insulin action, or both. Health promotion plays a pivotal role in enhancing patient knowledge and self-management behaviors. Among various educational tools, posters are considered effective media due to their ability to convey complex health messages visually and concisely. This study aimed to evaluate the effectiveness of poster-based health promotion interventions in reducing blood glucose levels among patients with Diabetes Mellitus at the Bandar Khalifah Public Health Center. This research employed a quantitative method utilizing a quasi-experimental design with a one-group pretest-posttest approach. A sample of 52 respondents was selected using a random sampling technique. Data were analyzed using the Paired Sample t-test to determine the statistical significance of the intervention. The statistical analysis revealed a significant difference in blood glucose levels before and after the intervention, with a p-value of 0.006 (p < 0.05). These findings indicate that the implementation of visual health education significantly contributed to the reduction of blood glucose levels among the respondents. It is concluded that poster-based media is an effective tool for supporting glycemic control in patients with Diabetes Mellitus. Therefore, it is recommended that healthcare providers utilize and develop diverse visual health promotion media regularly to sustain public awareness and foster long-term healthy behaviors.

Abstract

Diabetic foot ulcer is a common and serious complication of diabetes mellitus that requires effective wound management to accelerate healing and prevent further complications. Objective: This study aimed to evaluate the effectiveness of moist wound healing using hydrogel in improving wound healing outcomes among patients with diabetic foot ulcers at Asri Wound Care Center Medan. Methods: This study employed a case study design with purposive sampling. A total of five patients with diabetic foot ulcers stage 4 were included, consisting of three patients in the treatment group and two patients in the control group. Wound healing outcomes were assessed using the Bates-Jensen Wound Assessment Tool (BWAT) over seven wound care sessions. Results: The treatment group demonstrated a more significant improvement in wound healing compared to the control group. BWAT scores in the treatment group decreased from 43 to 24 (R1), 40 to 20 (R2), and 39 to 20 (R3), indicating marked reductions in necrotic tissue, slough, biofilm, and exudate, along with improved wound base and granulation tissue formation. In contrast, the control group showed smaller reductions in BWAT scores, from 40 to 34 (R4) and 41 to 33 (R5), reflecting a slower healing process with residual necrotic tissue and exudate. Conclusion: Moist wound healing using hydrogel was more effective than conventional wound care in accelerating the healing of diabetic foot ulcers, as evidenced by greater BWAT score reductions and improved wound characteristics. These findings support the use of hydrogel as an effective dressing in diabetic wound management.

Abstract

Diabetic foot ulcer is one of the most serious complications in individuals with diabetes mellitus, resulting from peripheral neuropathy, impaired vascularization, and chronic infection, which collectively delay the wound healing process. Proper wound management is essential to prevent severe outcomes such as amputation. Methods: This study employed a case study design to provide an in-depth clinical description of wound healing progression in patients with stage 4 diabetic foot ulcers. Purposive sampling was used to select three patients treated at Asri Wound Care Center Medan. Zinc cream was applied as an adjunct therapy for six weeks, with treatment administered twice weekly. Wound healing was evaluated using the Bates-Jensen Wound Assessment Tool (BWAT) and clinical observations. Results: All patients demonstrated observable improvements in BWAT scores, with reductions from 37 to 22, 40 to 26, and 39 to 24, respectively. Clinical observations indicated improved wound healing indicators, including increased granulation tissue, reduced exudate, and enhanced epithelialization. Conclusion: The application of zinc cream as an adjunct therapy may support the wound healing process in patients with stage 4 diabetic foot ulcers. However, due to the limited sample size and the absence of a control group, the findings should be interpreted cautiously, and further studies with larger samples and comparative designs are recommended.

Abstract

Endometriosis is a chronic, estrogen-dependent gynecological disorder affecting women of reproductive age and is associated with persistent pain, organ dysfunction, and infertility. Despite the availability of various treatment options, its management remains challenging due to complex pathophysiology and variable responses to therapy. This literature review aims to summarize recent evidence on management strategies for endometriosis and to highlight their clinical implications. A descriptive literature review was conducted using a systematic search of four databases which are PubMed, Scopus, EBSCOhost, and ScienceDirect for articles published between 2020 and 2025. Following the PRISMA 2020 guidelines, an initial search identified 45,014 records. After screening titles and abstracts and assessing full-text eligibility, six primary studies met the inclusion criteria and were included in the final synthesis. The included studies were analyzed thematically and categorized into five main therapeutic approaches: hormonal therapy, immunological therapy, antioxidant therapy, surgical therapy, and traditional or complementary therapy. Overall, these strategies demonstrated potential benefits in reducing pain, improving fertility-related outcomes, and lowering postoperative recurrence when applied in appropriate clinical contexts. Rather than indicating a single optimal treatment, the findings emphasize the importance of individualized and multimodal management tailored to symptom severity, reproductive goals, and treatment tolerance. This review contributes to the existing literature by integrating diverse and emerging management strategies into a cohesive framework that supports clinical decision-making. Clinically, the findings underscore the value of patient-centered care, careful monitoring of treatment effects, and interdisciplinary collaboration, including psychosocial support, to optimize long-term outcomes for women with endometriosis.

Abstract

Cognitive decline is often considered a natural condition in the elderly. However, this disorder can cause forgetfulness, difficulty concentrating, and communication difficulties, impacting daily activities and quality of life. One non-pharmacological intervention that can be used to improve cognitive function is the traditional game of Dakon. This study aimed to determine the effect of Dakon on the cognitive function of elderly people at the Turusgede Rembang Community Learning Center (PPSLU). The study used a quantitative pre-experimental design with a one-group pre-post test and a cohort approach. The study population consisted of 30 elderly people with cognitive decline, and 28 were selected using a purposive sampling technique. The research instruments included the Mini Mental State Examination (MMSE) questionnaire and the Dakon game standard operating procedures (SOP). Data analysis was performed using the Wilcoxon test with a significance level of α=<0.05. The results showed a ρ value of 0.000<0.05, thus accepting H₁, indicating that Dakon significantly improves cognitive function in the elderly. Most respondents experienced improved cognitive scores due to their ability to follow the game stages well, concentrate, and cooperate during the intervention. One respondent experienced no improvement in cognitive function, influenced by factors such as advanced age, low education level, lack of social interaction, and health conditions such as hypertension. It can be concluded that the traditional Dakon game is an effective non-pharmacological intervention in improving the cognitive function of the elderly at the Turusgede Rembang PPSLU.

Abstract

Patients undergoing surgery often experience anxiety due to concerns about surgical outcomes, pain, and potential complications. Family support plays a vital role in helping patients cope with anxiety before surgery. This study aimed to determine the relationship between family support and anxiety levels among preoperative hernia reponibel patients in Nyi Ageng Serang Room 1 at RSUD Sekarwangi. A quantitative cross-sectional design was employed. The sample consisted of 32 preoperative patients with reducible hernia, representing the total population treated in the study setting during the data collection period. Patients selected using a total sampling technique. The research instruments used were the Multidimensional Scale of Perceived Social Support (MSPSS) to assess family support and the Hamilton Anxiety Rating Scale (HARS) to measure anxiety levels. Data were analyzed using the Chi-Square test with a significance level of p < 0.05. The results revealed a significant relationship between family support and patients’ anxiety levels (p = 0.000). Patients who received strong family support predominantly experienced mild anxiety (87.5%), while those with low family support tended to experience moderate to severe anxiety. These findings indicate that greater family support is associated with lower anxiety levels among preoperative patients. This study contributes evidence that family support is a significant determinant of preoperative anxiety among patients with reducible hernia in a regional hospital setting. The findings enrich nursing science by highlighting the importance of family-centered care as a contextual psychological support strategy in preoperative management, particularly in non-tertiary hospitals.

Abstract

Pain is a common clinical problem that can be managed through both pharmacological and non-pharmacological approaches, including progressive muscle relaxation. This study aimed to examine the effect of progressive muscle relaxation technique on pain among patients in the Fatmawati Ward of RSUD Sekarwangi. A pre-experimental study with a one-group pretest–posttest design was conducted involving 25 respondents selected through purposive sampling, consisting of patients experiencing mild to moderate pain. Progressive muscle relaxation was administered once daily for approximately 15–20 minutes over the intervention period. Pain intensity was measured using the Numeric Rating Scale (NRS) before and after the intervention, and data were analyzed using the Wilcoxon Signed Rank Test. The results showed that prior to the intervention, most patients experienced moderate pain (84%), while after the intervention all patients (100%) reported mild pain. Statistical analysis revealed a significant difference in pain levels before and after the intervention (p = 0.001; < 0.05). These findings indicate that progressive muscle relaxation is effective in reducing pain among hospitalized patients. This technique can be considered a complementary nursing intervention in hospital pain management, and future studies are recommended to involve larger samples, control groups, and longer follow-up periods to evaluate its long-term effectiveness and broader clinical applications.

Abstract

One of the significant physical changes experienced by adolescent girls is menstruation. Almost every adolescent girl experiences a menstrual disorder, namely dysmenorrhea. Dysmenorrhea is a gynecological disorder caused by an imbalance of the hormone progesterone in the blood, resulting in excessive prostaglandin production that triggers pain during menstruation. The intensity of dysmenorrhea can vary from mild, moderate, to severe. In addition to physical effects, menstrual pain may also cause psychological symptoms, such as uncontrolled emotions. With good emotion regulation, adolescent girls are able to manage, control, and express their emotions appropriately according to their menstrual condition. The purpose of this study was to determine the relationship between emotion regulation and the intensity of dysmenorrhea pain in adolescent girls at SMAN 3 Malang. This study used a quantitative method with a correlational approach and a cross-sectional design. Data were collected using questionnaires distributed via Google Form. The study involved 244 respondents who were students of SMAN 3 Malang in grades X, XI, and XII. Data analysis was conducted using the Spearman test. The results showed a p value of 0.002 (α< 0.05) with a correlation coefficient of r=-0.194. These findings indicate a significant negative relationship between emotion regulation and dysmenorrhea pain.

Abstract

Hypertension is a chronic “silent killer” that affects over 1.4 billion people globally. Besides physical symptoms, hypertensive patients often experience anxiety, especially when accompanied by headaches. This study aimed to identify anxiety levels among hypertensive patients with headache and their implications for holistic nursing care. A descriptive quantitative study with a cross-sectional design was conducted at Puskesmas Ngawi Purba from February to May 2024. Forty-three hypertensive patients aged 35–55 years experiencing headaches were selected by accidental sampling. Anxiety was measured using the Hamilton Anxiety Rating Scale (HARS) and pain using the Numeric Rating Scale (NRS). Most respondents were female (69.8%), elderly (60.5%), and obese (44.2%). Moderate anxiety was found in 30.2% of participants, with 79.1% having stage 2 hypertension and 62.8% reporting moderate pain. Hypertensive patients with headache mostly experience moderate anxiety. Holistic nursing care addressing both physical and psychological aspects is essential to enhance blood pressure control and patient well-being.
