Abstract

Acute kidney injury (AKI) is a common complication in critically ill patients and is associated with increased length of hospital stay. Early detection of renal dysfunction by measuring the renal resistive index (RRI) using Doppler ultrasound is a non-invasive parameter that reflects intrarenal vascular resistance and has the potential to predict AKI occurrence in critically ill patients. This study aims to compare RRI values between patients with and without AKI  based on severity levels classification from AKIN criteria, and to establish the RRI value as a predictor of AKI occurrence in critically ill patients. This study was an analytical observational study with a prospective cohort design conducted on critically ill patients treated in the ICU of dr. Zainal Abidin Banda Aceh General Hospital from June to August 2025. Data were obtained through Doppler ultrasound RRI examination performed within the first 24 hours and medical record data during treatment. Data were analyzed using ANOVA and ROC curve analysis to determine the cut-off value of RRI as an AKI predictor. Out of the 60 critically ill patients who met inclusion criteria, 30 patients (50%) developed AKI during treatment. There were significant differences (p<0.0001) between RRI and AKI occurrence as well as AKI severity, for both the right and left kidneys. ROC curve analysis identified an RRI cut-off value of 0.7 for predicting AKI, with a Receiver Operating Characteristic Area Under the Curve (AUC) of 0.9628 and CI 95% : 0.9155 – 1.000­, yielding a sensitivity and specificity of 90%. There are significant differences in RRI values between non-AKI patients and AKI patients at stages 1, 2, and 3. RRI value can be used as a predictor for AKI occurrence in critically ill patients.  Keywords: Acute Kidney Injury, Critically Ill, Doppler Ultrasonography, Renal Resistive Index.

Abstract

Undernutrition among children under five is a serious challenge in achieving Sustainable Development Goals (SDGs) Goal 2. In the working area of UPT Puskesmas Enok, especially Rantau Panjang Village, the prevalence of undernutrition still requires innovative interventions. Supplementary feeding (PMT) based on local food with a balanced nutritional menu is expected to be a sustainable solution compared to manufactured processed products. The purpose of the study was to analyze the effectiveness of providing PMT with a combination of a balanced nutrition menu in increasing the body weight of undernourished toddlers in Rantau Panjang Village. Type of quantitative research with a quasi-experiment one-group pretest-posttest design. The research was conducted in Rantau Panjang Village. The population of this study was all toddlers diagnosed with undernutrition in the work area of Community Health Center Rantau Panjang, 42 toddlers. The sampling technique was purposive sampling, obtaining 7 toddlers with an age range of 12-59 months. The results showed that the average body weight of toddlers before the intervention was 9.77 kg (SD = 2.4) and after the intervention increased to 10.38 kg (SD = 2.5), with an average increase of 0.6 kg. Statistical tests showed a p-value of 0.000 (p < 0.05), which means that there is a significant effect of providing a balanced nutrition menu PMT on toddler weight. Puskesmas are advised to adopt this balanced nutrition menu model as a standard operating procedure in handling underweight children. The Rantau Panjang Village Government is expected to integrate the village fund budget to support the sustainability of this program. Keywords: Balanced Nutrition Menu, Toddler, Undernutrition, Supplementary Feeding.

Abstract

Stunting has been a significant public health burden in Indonesia, reflecting chronic malnutrition during the critical first 1,000 days of life. Despite a national decline, local prevalence in rural areas, such as in Langkat District, remains a challenge.  This study aims to analyze interrelated risk factors: maternal knowledge, antenatal care (ANC) visits, iron supplementation, exclusive breastfeeding, and complementary feeding, that contribute to stunting in toddlers. A cross-sectional study was conducted in the working area of the Secanggang Community Health Center, Langkat. A total of 162 toddlers participated in this study. Data were collected through interviews with mothers and anthropometric measurements. Bivariate analysis was performed using the Chi-square test to determine the relationship between variables. The prevalence of stunting was 40.7% (n=66). Statistical analysis showed that all factors studied were significantly associated with stunting (p < 0.05). Mothers' knowledge about stunting was generally low (70.4%). The highest risk was found in infants who did not receive exclusive breastfeeding, where 25.9% experienced stunting compared to those who received exclusive breastfeeding. Inadequate ANC visits (p < 0.05) and poor adherence to iron (Fe) tablets (p < 0.05) also significantly increased the risk of growth failure. Stunting in this population is triggered by the interaction between low maternal literacy and suboptimal antenatal and postnatal care. These findings indicate an urgent need for maternal education programs and breastfeeding support integrated into the primary health care (Puskesmas) framework to reduce long-term growth disorders. Keywords:  Maternal Knowledge, Antenatal Care, Exclusive Breastfeeding, Stunting, Toddlers
