Abstract

This research was conducted at PT. PLN (Persero) ULP Cikande by analyzing employee occupational safety and health (K3) as a form of attention and protection in carrying out their duties. By using the HIRADC method, it is hoped that this research can provide the best solution for employee performance, especially in occupational safety and health (K3). From the results of collecting company work accident data, the risk matrix classification in 2022 obtained 2 variables with low risk, for the high risk level, 2 variables are obtained with a percentage of 40%, and for the medium risk level, 1 variable is obtained with a percentage of 20%. Proposed improvements in this study include changes in training, equipment maintenance, improved work procedures, and more careful risk assessment. With the cooperation and involvement of the entire team, we are confident that the implementation of this change will have a significant positive impact on the safety and welfare of all parties involved in substation inspections.

Abstract

BLKK Al – insan is a company that produces hoe products located in Environment. Karotek RT/RW 001/002.Kalitimbang village Cibeber district Cilegon city. Cilegon City with the main raw material of iron plate and other supporting tools. BLKK Al - Insan is very hopeful in creating products that allow it to compete well in terms of quality in the market and wants a 3% defect standard in its production. In the production process of hoe products BLKK Al - insan has defect problems, namely Distortion, Porosity and Undercut which can cause problems in product quality in the targeted production amount and has a defect percentage of 23% in 4 months. The purpose of this study is to apply the si sigma method (DMAIC) to reduce product defects and determine the factors that cause product defects and provide alternative efforts to improve product quality. Based on the analysis result DMAIC obtained 3 critical to quality that’s is Distorsi, Porosity and Undercat, and obtained the average initial value DPMO 763,8 (2,94) after improvement, the DPMO value becomes 644.4 and the sigma level has increased to 3.02 and the defect percentage value becomes 19% based on the process improve using the kaizen method by proposing to use 5w+1H, Five M Cheecklist and Five Step Plan and developing standard operating procedures for the production process can determine the DPMO value and increase the six sigma value.

Abstract

This research aims to improve the service quality at Selakosa Coffee using Service Quality (Servqual) and Importance Performance Analysis (IPA) methods. As competition in the coffee shop industry increases, providing excellent service has become crucial to attract and retain customers. The study surveyed 90 respondents and analyzed five key dimensions of service quality: tangibility, reliability, responsiveness, assurance, and empathy. The results revealed significant gaps between customer expectations and actual service in three key areas: indoor space size, toilet availability, and cleanliness. These factors were categorized as high-priority issues based on the IPA Cartesian diagram, indicating that immediate improvements are needed. The root causes of these service shortcomings were analyzed using the fishbone diagram, and improvement recommendations were formulated using the 5W + 1H method. The study concludes that by addressing these gaps, Selakosa Coffee can enhance customer satisfaction, reduce complaints, and strengthen its market position. Future studies could explore additional service variables and include a larger sample size for more comprehensive results

Abstract

Ordering raw materials optimally can provide benefits for the company. PT. XYZ places an order for sand 12 (twelve) times in 1 (one) period which causes it to be ineffective and inefficient. Therefore, it is necessary to have a method that is able to control the total cost of raw material inventory in order to ensure a smooth production process. The EOQ (Economic Order Quantity) method is used to identify orders to minimize the total cost of holding inventory, purchase raw materials with each purchase with a minimum cost, and orders that minimize total inventory costs. Thus the EOQ (Economic Order Quantity) method can save the cost of raw material inventory which is able to make a company profitable by applying this method. The results of this study indicate that the EOQ (Economic Order Quantity) method can save raw material inventory costs compared to traditional methods (policy firms). The savings in the cost of raw materials for sand in the EOQ (Economic Order Quantity) method were Rp.8.394.400.

Abstract

This research purpose to calculate the optimal quantity of raw material orders for chicken animal feed and the number of ordering frequencies, calculate the total cost of raw material inventory for chicken animal feed, analyze the most efficient method that must be applied to minimize the cost of raw material inventory at PT Kerta Mulya Saripakan. The method used in this research is the Economic Order Quantity (EOQ) and Just In Time (JIT) methods. After done analysis, the cost of raw material supplies incurred by the company, calculated according to company policy, reached IDR 459,616,202. By using the Economic Order Quantity (EOQ) method, the total inventory cost is IDR 340,449,297. Meanwhile, using the Just In Time (JIT) method, the total inventory costs incurred were IDR. 218,939,956. So a method that can minimize inventory costs at PT. Kerta Mulya Saripakan uses the Just In Time (JIT) method.

Abstract

This study analyzes the raw material inventory control of wood at PT Suryamas Lestariprima using the Economic Order Quantity (EOQ) method to achieve inventory cost efficiency. The results show that the EOQ calculation produces an optimal order quantity of 19.8772 m³ for durian wood, 18.8571 m³ for rambung wood, and 14.0553 m³ for miscellaneous wood, designed to minimize the company’s inventory costs. Additionally, the determination of safety stock at 14.954,5081 m³ for durian wood, 17.101,8334 m³ for rambung wood, and 384,7911 m³ for miscellaneous wood helps the company handle potential demand fluctuations during the lead time. This study also establishes the reorder point as the minimum threshold before reordering, namely 15.038,4718 m³ for durian wood, 17.230,6429 m³ for rambung wood, and 404.1126 m³ for miscellaneous wood. The application of the EOQ method results in inventory cost savings of IDR 5,409,005,289 for durian wood, IDR 2,888,543,167 for rambung wood, and IDR 1,506,367,701 for miscellaneous wood. Based on these findings, it is recommended that the company consider implementing EOQ as a standard in managing inventory to maintain operational efficiency.

Abstract

PT Cahyo Permata Abadi is a company that provides precision measurement solutions and high-quality services for various industries by ensuring accuracy and efficiency in every project, so it is important to pay attention to the K3 aspect in the maintenance activities carried out, one of which is in the field of weighbridge maintenance services. This study aims to analyze the Occupational Safety and Health (K3) risks at PT. Cahyo Permata Abadi in weighbridge maintenance activities. The research method is Job Safety Analysis (JSA). The results of the Risk Analysis study show that several activities have significant potential hazards. In troubleshooting, there is a risk of electric shock and muscle injury with a risk value of 9. Checking the electrical system faces the risk of electric shock and short circuits with a risk value of 8. Other activities such as sensor calibration, platform cleaning, and load testing have moderate risks with potential hazards such as falling from a height, slipping, and being hit by a load. Research suggestions for risk control should be carried out through various safety measures, including the use of Personal Protective Equipment (PPE) such as insulating gloves, safety shoes, and full body harnesses. In addition, safety procedures such as Lock Out Tag Out (LOTO), warning signs, and safe work procedures are also implemented. These controls are designed to minimize potential hazards and ensure worker safety while carrying out these activities.

Abstract

This study aims to optimize the quality control of wood pallet products at PT. Azdhi Kayu Kreasi by using the Root Cause Analysis (RCA) and Statistical Process Control (SPC) approaches. The problem faced by the company is the high level of product defects that can reduce customer satisfaction and increase production costs. The RCA method is used to identify the root cause of product defects, while SPC is used to monitor and control the production process. Data were collected through field observations, interviews, questionnaires, and internal company documentation. The data were analyzed using P charts, Pareto diagrams, and fishbone diagrams. The results showed that the main cause of product defects was wood cracking, which was caused by factors such as poor raw material quality, production process errors, and unsuitable environmental conditions. The implementation of RCA and SPC proved effective in identifying and reducing the causes of product defects. By making improvements to the production process and selecting better raw materials, the product defect rate can be significantly reduced. This research makes a practical contribution to the company in improving product quality and production process efficiency.

Abstract

PT Sulfindo Adiusaha is a chemical company focused on operational efficiency and quality. The main issue faced is frequent machine breakdowns, particularly mechanical seal leakage, which disrupts production processes. This study aims to analyze the causes of damage using the Failure Mode and Effect Analysis (FMEA) method to determine repair priorities. The analysis results show that mechanical seal leakage has the highest Risk Priority Number (RPN) of 504, making it the top repair priority. The main causes of damage include human factors, maintenance methods, materials, and machine wear. Recommendations for improvement include operator training, replacing components with heat-resistant materials, and structured periodic maintenance. These steps are expected to enhance productivity and reduce downtime risks.

Abstract

Every manufacturing company generally aims to achieve the highest product quality to ensure customer satisfaction and maintain market competitiveness. PT. XYZ, a Japanese company specializing in automotive lighting and located in Tangerang Regency, Banten, faces challenges in maintaining production quality, particularly in its main product— motor vehicle lamps. The production process includes injection molding, aluminum metalizing, and assembly. However, defective products, especially those with weld line defects, remain a recurring issue, impacting production efficiency in terms of cost and time. During several months, a total of 588 defective products were recorded out of 148,526 units. This study observed a defect rate of 34% (200 units), which significantly exceeds the company’s quality target of a 2% defect rate. To address this, a structured quality improvement effort is necessary. This research applies the Six Sigma methodology as a quality control tool to identify, analyze, and minimize product defects. The objectives of this study are to provide a general overview of PT. Indonesia Stanley Electric, identify types of product defects, calculate the Defects Per Million Opportunities (DPMO), and propose strategies to achieve zero defect targets. Through this approach, the company is expected to continuously improve product quality in a more efficient and sustainable manner.
