Abstract

Pelayanan puskesmas perlu ditunjang dengan pendanaan kesehatan, hal ini didukung dalam Permenkes Nomor 87 Tahun 2019 diberikan dana dekonsentrasi yang bersumber dari APBD disebut dengan Bantuan Operasional Kesehatan (BOK), berdasarkan data yang diperoleh implementasi pengelolaan dana BOK Puskesmas Koto Baru selama dua tahun terakhir terdapat penurunan realisasi seperti tahun 2019 dengan anggaran BOK Rp. 742 realisasi Rp. 678 (91 %) lalu tahun 2021 sebesar Rp. 957 juta realisasi Rp. 623 juta (65%) lalu kebijakan pemerintah tentang persen pendanaan BOK mengalami perubahan sebanyak dua kali tahun 2022 menyesuaikan kondisi pandemi Covid 19. Tujuan dari penelitian ini untuk mengetaui gambaran refocusing implementasi pengelolaan dana BOK di Puskesmas Koto Baru. Jenis penelitian kualitatif dengan pendekatan studi kasus. Hasil dari penelitian ini didapatkan adanya kualifikasi pendidikan yang belum di miliki puskesmas terhadap tenaga tetap dan kontrak, terjadi keterlambatan pelaporan dan pencairan pendanaan serta perhitungan rumus intensif UKM yang rumit sehingga tidak terealisasi, pengawasan BOK hanya dilakukan sekali oleh pihak dinkes serta tidak ada pelatihan khusus pengelola BOK.

Abstract

Indonesian National Population and Family Planning Board defines youth as unmarried individuals aged 10-24 years. Globally, every year there are more than 1,2 million adolescent deaths due to preventable or treatable health problems and that were occurred due to lack of adequate information or limited access to health services. The purpose of this review to conclude and explore related to adolescent reproductive health services. This research is a systematic review using the search engines Pubmed, Science Direct and Google Scholar. Search result through search engines found as many as a 5 relevant articles and analyzed them. This review found two themes: the determinants of adolescent reproductive health services, including knowledge, perceptions, and needs; and barriers to adolescent reproductive health services, including facilities, costs, staff, lack of privacy and confidentiality, social culture and policies.

Abstract

Andir is one of district that has the highest HIV and AIDS cases in Bandung. According to Bandung AIDS Comission, it is confirmed that 224 PLWIH and estimated that 4,235 key populations, including sex workers, LGBTQ+, people who inject drugs live in the district. Young people in Andir has become on of the most risky population of HIV transmittion. The knowledge of sexual reproductive health, drug abuse and HIV prevention among these young population needs to be improved in order to decrease new HIV cases. The purpose of this rapid general assessment is to obtain a program design model that is right on target and according to the needs of youth.The method used in this assessment was to use a quantitative and qualitative approach involving 63 teenage respondents and 7 stakeholder informants. The results of the assessment revealed: 60% of adolescents understand reproductive health, 72% of respondents understand drugs, 60% of adolescents understand HIV and AIDS; the desired form is through social media; 89% of respondents had been involved in youth activities, 79% had been involved in planning an activity, and 62% of respondents had made decisions in activities; Facilities and infrastructure owned by the District and Kelurahan are Youth Space, Musrembang, community funds, Jurnal Ners Universitas Pahlawan ISSN 2580-2194 (Media Online)corporate concern funds; The facilities owned by the Puskesmas are funds sourced from the BLUD (Regional Public Service Agency), BOK (Health Operational Assistance), APBD (Regional Revenue and Expenditure Budget), and cooperation with other parties (sponsorship).The results of the assessment recommendations for youth and stakeholders include: regular information on reproductive health, drugs, HIV and AIDS through social media with youth involvement (meaningful youth involvement); Optimizing government facilities, community, CSR, BLUD (Regional Public Service Agency), BOK (Health Operational Assistance), APBD (Regional Revenue and Expenditure Budget), as well as the collaboration of academia, business, community, government and media (Pentahelix).Keywords: content, formatting, article

Abstract

Senile cataracts, prevalent in the elderly, especially in developing areas with limited healthcare access, pose significant challenges in geriatric patient management. This case study focuses on Mrs. S, a 66-year-old Indonesian woman with a mature-stage senile cataract, illustrating the difficulties in diagnosis and treatment in resource-constrained settings. Her symptoms, initially a white fog-like view, quickly worsened, highlighting the urgency of early cataract detection and intervention to prevent severe vision impairment. Examination revealed a uniformly cloudy lens in her right eye, leading to a diagnosis of Mature Stage Senile Cataract. The treatment plan included extracapsular cataract extraction with Intraocular Lens implantation. This case emphasizes the importance of early diagnosis in managing cataracts, the potential for less invasive surgical options with timely intervention, and the consequences of delayed diagnosis on patient well-being and treatment complexity. Early recognition and treatment are crucial in preserving visual function and minimizing complications associated with advanced cataracts, particularly in resource-limited healthcare settings.

Abstract

Herpes zoster (HZ) adalah reaktivasi virus varicella-zoster yang menetap laten di dalam ganglia (dorsal root ganglion) setelah serangan varisela. Komplikasi dari HZ yang sangat mempengaruhi kualitas hidup dan Kesehatan secara keseluruhan adalah post-herpetic neuralgia (PHN). Hal ini lebih sering terjadi pada lansia dan bila terapi terlambat diberikan. Telah ditemukan bahwa hipertensi dikaitkan dengan peningkatan risiko HZ. Secara bersamaan, HZ juga meningkatkan risiko terjadinya kejadian kardiovaskular. Laporan kasus ini bertujuan untuk menerapkan pendekatan dokter keluarga yang holistik dan komprehensif, dan melakukan penatalaksanaan berbasis Evidence Based Medicine dengan pendekatan Family Approach pada seorang pasien bernama Tn. M berusia 64 tahun dengan herpes zoster dan hipertensi tidak terkontrol. Prinsip yang digunakan dalam pendekatan kedokteran keluarga adalah Mandala of Health. Pada pasien dilakukan intervensi medikamentosa bertujuan untuk mengurangi keluhan dan intervensi non medikamentosa berupa edukasi kepada pasien dan keluarga mengenai faktor risiko penyakit, pencegahan, pola makan pasien. Penegakan diagnosis dan penatalaksanaan pada pasien ini telah dilakukan secara holistik, patient centered, family approach dan berdasarkan beberapa teori maupun penelitian terkini. Penatalaksanaan yang dilakukan secara holistik telah memberikan hasil pada Tn. M berupa penyembuhan lesi herpes zoster dan juga penurunan tekanan darah menjadi dibawah < 140/90.

Abstract

This case report details the management and implications of a 13-year-old male patient who presented with Epidural Hematoma (EDH) and Subdural Hematoma (SDH) following a traumatic brain injury sustained from a fall. The patient's treatment involved decompressive craniotomy to relieve intracranial pressure and reduce the risk of further brain damage, followed by cranioplasty to reconstruct the cranial vault. The case highlights the complexities involved in pediatric neurosurgery, especially in managing EDH and SDH, which are common yet critical conditions in traumatic brain injury cases. These hematomas, while distinct in their location and nature, share a common etiology in trauma and require prompt and effective surgical intervention. The importance of decompressive craniotomy in such cases is emphasized, particularly for alleviating increased intracranial pressure and minimizing additional cerebral injury. However, the potential postoperative complications, including cerebrospinal fluid leaks, infections, and seizures, necessitate vigilant postoperative care and monitoring. The epidemiology of EDH and SDH in the context of traumatic brain injury underscores their prevalence and the significant impact they have on the pediatric population. The successful management of this case, with both immediate surgical intervention and comprehensive postoperative care, illustrates the effectiveness of current treatment protocols. This case also sheds light on the critical need for a multidisciplinary approach in the long-term rehabilitation of pediatric patients, ensuring optimal recovery and development post-injury. In conclusion, this report contributes to the understanding of the management of traumatic brain injuries in children, emphasizing the significance of timely surgical intervention, careful monitoring for complications, and the importance of a holistic approach to patient care for favorable outcomes.

Abstract

This comprehensive review examines the complex human immune response to malaria, a significant global health challenge caused by Plasmodium parasites. The innate and adaptive immune systems play pivotal roles in defending against malaria, with mechanisms involving various immune cells like dendritic cells, natural killer cells, eosinophils, basophils, T cells, and B cells. These cells operate in a dynamic interaction, recognizing and responding to the parasite at different stages of its life cycle. Our review methodologically analyzed recent studies and literature on the immune response to malaria, focusing on the roles of different immune cells and the production of cytokines and antibodies. We also explored the epidemiology of malaria, with particular attention to regions like Indonesia, where climate, geography, and socio-economic factors influence transmission dynamics. The findings highlight the innate immune system's crucial role in early pathogen detection and response, particularly through PAMPs recognition by PRRs such as TLRs and scavenger receptors. Additionally, the adaptive immune response's complexity, including anti-sporozoite antibodies and T cell immunity, is emphasized, particularly in recognizing parasite-exported antigens and developing memory responses for long-term immunity. The immune response's intricacy, coupled with the challenges in vaccine and therapy development due to the parasite's complex life cycle and varying epidemiological patterns, underscores the need for continued research and innovation in malaria immunology and public health strategies. This review contributes to a deeper understanding of the immune mechanisms against malaria and the ongoing efforts to control and eradicate this pervasive disease.

Abstract

Demam Berdarah Dengue (DBD) merupakan masalah kesehatan yang tersebar luas, terutama di daerah tropis dan subtropis seperti daerah Gembong. Studi ini menyelidiki pengetahuan, sikap, dan perilaku masyarakat mengenai pencegahan DBD, menyoroti perlunya peningkatan kesadaran dan tindakan proaktif. Desain penelitian adalah penelitian deskriptif dan dilaksanakan pada tanggal 24 Agustus 2023 di Kampung Hauan, Desa Tobat, Kecamatan Balaraja, Tangerang, Banten. Penelitian ini melibatkan 25 responden lokal berusia 18-64 tahun yang dipilih melalui non-random purposive sampling. Variabel difokuskan pada pengetahuan dan perilaku mengenai DBD. Penelitian ini mengungkapkan kesenjangan pengetahuan yang signifikan di kalangan responden tentang DBD, karakteristik vektornya, habitat perkembangbiakannya, dan strategi pencegahan seperti program 3M plus. Kelemahan sikap dan perilaku juga diamati, termasuk penolakan terhadap tindakan pencegahan dan kurangnya partisipasi dalam kegiatan pengendalian vektor. Studi ini juga mengidentifikasi tantangan dalam aksesibilitas layanan kesehatan dan pendidikan terkait penyakit. Intervensi pendidikan dan strategi penyebaran informasi yang inovatif, terutama untuk berbagai latar belakang pendidikan, diperlukan untuk mengatasi permasalahan ini. Hasil penelitian ini menekankan perlunya pendekatan komprehensif yang menggabungkan pendidikan kesehatan, peningkatan layanan kesehatan, dan keterlibatan masyarakat dalam pengendalian lingkungan untuk memerangi DBD. Mengatasi kesenjangan pengetahuan dan mendorong perubahan perilaku secara efektif sangat penting dalam mengurangi kejadian DBD dan meningkatkan kesejahteraan masyarakat.

Abstract

Preauricular abscesses, particularly in pediatric patients, are a clinical challenge often associated with congenital anomalies like preauricular pits. These conditions necessitate a comprehensive approach to diagnosis and management, emphasizing the importance of understanding their etiology, clinical presentation, and treatment strategies. A 15-month-old male presented with recurrent preauricular abscesses. Notably, he had congenital preauricular pits present since birth. The clinical presentation included localized swelling, erythema, and tenderness in the preauricular area. The history of recurrent infections and physical examination findings led to the diagnosis of preauricular abscess secondary to infected preauricular pits. The management of preauricular abscesses in pediatric patients involves a dual approach. Initially, appropriate antibiotic therapy is administered to address the acute infection. In cases of recurrence, as in our patient, surgical intervention for the removal of the pit or fistula is considered to prevent future infections. This case highlights the importance of recognizing congenital anomalies and tailoring treatment plans to address both immediate and long-term concerns. This case underscores the need for meticulous clinical assessment and a comprehensive treatment plan in managing pediatric preauricular abscesses. Recognizing the role of congenital preauricular pits in recurrent infections is crucial for effective management. The dual approach of medical and surgical intervention can significantly improve outcomes in pediatric patients with this condition.

Abstract

Menyusui adalah proses pemberian air susu ibu (ASI) kepada bayi. Pemberian ASI pada bayi sangat bermanfaat bagi pertumbuhan dan perkembangan bayi khususnya pemberian ASI pertama yang berwarna kekuningan (kolostrum). Tujuan penelitian : adalah untuk mengetahui hubungan tingkat pendidikan dan pengetahuan ibu menyusui dengan pemberian kolostrum pada neonatus di Puskesmas Gunungkencana Tahun 2023. Metode penelitian : penelitian ini menggunakan desain cross sectional, yang dilakukan pada bulan Desember 2023 di Puskesmas Gunungkencana. Jumlah sampel sebanyak 50 orang, yang diambil dengan teknik total sampling. Variabel dependen yaitu pemberian kolostrum dan variabel independennya yaitu tingkat pendidikan dan pengetahuan yang dianalisis secara univariat dan bivariat dengan menggunakan uji chi square. Hasil Penelitian : Secara uji statistik diperoleh sebagian besar responden berpendidikan rendah atau lulusan SD dan SMP sebanyak 54%. Responden dengan pengetahuan cukup sebesar 52%, dan sebagian besar responden tidak optimal dalam pemberian kolostrum yaitu sebesar 54%. Bersdasarkan hasil bivariat menunjukkan terdapat hubungan yang bermakna antara tingkat pendidikan dan pengetahuan ibu menyususi dengan pemberian kolostrum pada neonatus (p=0,000).
