Abstract

Postoperative pneumonia (POP) is the most common complication in patients after CABG surgery. It increases morbidity, mortality, length of stay, and hospitalization costs. This review aims to identify the factors that influence pneumonia and the microorganisms that cause it in CABG postoperative patients. This scoping review used the framework developed by Arksey and O'Malley (2005). Primary sources of information were searched through five databases: PubMed, Scopus, Science Direct, Oxford, and EBSCO, and 2 search engines: Sage and Google Scholar. Article quality was assessed using the Joanna Briggs Institute critical appraisal checklist. A total of 8 articles with a total of 35235 participants were included in this review. The prevalence of post-CABG POP ranged from 2.4% to 18%. Dominant factors influencing postoperative pneumonia included duration of surgery, CPB time, blood transfusion, mechanical ventilation, LVAD/Tx, smoking history, chronic lung disease, and preoperative malnutrition. Meanwhile, the microorganisms that cause postoperative pneumonia are gram-positive and negative bacteria, fungi, and other bacteria. Infection prevention can be an important point in the CABG postoperative care continuum. By understanding the factors that influence POP, healthcare professionals can optimize patient outcomes, reduce complications, and improve the efficiency of postoperative care.

Abstract

Introduction: Anxiety disorder is a condition that is often found in society in general. Long-term anxiety disorders can cause an increase in the hormone adrenaline resulting in an increase in free fatty acids and blood glucose levels, and also cause the release of the hormone cortisol, which is a glucocorticoid hormone synthesized from cholesterol in the adrenal cortex, which can have a negative impact on cardiovascular, metabolic and respiratory effects. Cortisol is commonly known as the "stress hormone", which is only released during stressful conditions. Research method: This research is a numerical analytic study with a cross sectional method, the sampling method is non-probability sampling with consecutive sampling type, with a total sample of 25 people. Results: There was a significant difference between anxiety disorders and an increase in blood sugar(p=0.001, r =0,625) and cholesterol levels (p0.001, r=0,717). There was no significant difference between anxiety disorders and an increase in cortisone hormone with P= 0.366, r=0,189 .Conclusion: There was a significant difference between anxiety disorders and an increase in blood sugar and cholesterol levels, there was no significant difference between anxiety disorders and an increase in the hormone cortisol.

Abstract

Exclusive breastfeeding in developing countries is still low, especially among working mothers. This study aims to determine the relationship between family support and exclusive breastfeeding for working mothers in hospitals in the Lamongan district. The method in this study used a cross-sectional analytical research design, the sample was working mothers in the Lamongan district hospital who had babies aged 6-24 months and met the inclusion criteria, totaling 120 people using purposive sampling. Data collection was carried out in August–September 2022 using questionnaires, analyzed univariately, bivariate (chi-square), and multivariate (logistic regression). The results showed that family support was significantly related to exclusive breastfeeding (p 0.000; OR 13.8; 95% CI: 4.819-39.381). Maternal characteristics that were significantly associated with exclusive breastfeeding were parity (p 0.034; OR 2.999; 95% CI: 1.086-8.038) and employment (p 0.003; OR 3.76; 95% CI 1.513-9.357). Family support is the most dominant factor related to exclusive breastfeeding for working mothers in hospitals. Mothers who receive good family support have a 13.8 times chance of providing exclusive breastfeeding compared to working mothers who receive less family support. It is recommended for health workers to provide IEC to families to provide optimal support for working breastfeeding mothers.

Abstract

Outpatient nursing services are an important element for hospitals to produce many patients, particularly inpatients. Outpatient nursing services are currently still not optimal. Nurses are responsible for providing good service to increase patient satisfaction. Aim: This case report aims to describe the implementation of planned changes to optimize the outpatient nursing service planning function. Method: The method used is a case report. Data collection uses interviews, questionnaires, and field observations. The results of data exploration are then analyzed using SWOT (strengths, weaknesses, opportunities, threats) and fishbone diagrams, then formulating a plan of action (PoA), followed by implementation and evaluation. Results: The results of the study showed that the priority problem in nursing services was that the implementation of the planning function was not optimal. Making outpatient nursing service guidelines is a strategy for solving problems in developing outpatient nursing services. Implementation of problem-solving uses a theory of change approach by the stages of Kotter's eight-step plan. Recommendation: Kotter's eight-step plan theory of change approach can be continued by hospital management as a form of initiation of service development.
