Abstract

Background: During the COVID-19 pandemic in Indonesia, 1086 pregnant women and 302 babies died from COVID-19 infection in 2021. The high mortality rate affecting ANC visits was seen from the decrease in ANC coverage, influenced by knowledge, attitudes, and anxiety. Objective: To see if there is a relationship between social distancing and the incidence of COVID-19. Methods: This study was an analytical study with a cross-sectional design. The sample selection used the snowball sampling method, and 222 samples were obtained which were assessed using an online questionnaire via google form. Results: The 83,3% of respondents did not obey physical distancing and 11,7% of respondents were classified as COVID-19 incidents. Data analysis with Fisher's exact test obtained p value = 0,089 (p> 0,05). Conclusion: There is no relationship between the level of compliance with physical distancing and the incidence of COVID-19 in Medan. Keywords: COVID-19, level of compliance, incidence of COVID-19

Abstract

Background: During the COVID-19 pandemic in Indonesia, 1086 pregnant women and 302 babies died from COVID-19 infection in 2021. The high mortality rate affecting ANC visits was seen from the decrease in ANC coverage, influenced by knowledge, attitudes, and anxiety. Purpose: To determine the relationship between knowledge, attitudes, and anxiety towards ANC visits during the COVID-19 pandemic at Glugur Darat Health Center, East Medan Anxiety. Methods: The analytic observation with cross-sectional study was conducted at Glugur Darat Health Center, Medan Timur District using a total sampling technique that fulfill the inclusion criteria. The sample was third-trimester pregnant women who were recorded as having ANC. Data analysis used Chi-square with alternative fisher's exact test and backward multiple logistic regression Results: From the 66 respondents, it was found that pregnant women who performed ANC well were 46 respondents (69.7%), with high knowledge were 59 respondents (89.4%), with a positive attitude 65 respondents (98.5%), and had anxiety as many as 55 respondents (83.3%). Result data showed that knowledge related to ANC visits (p= 0,023) with OR = 5.6, anxiety related to ANC visits (p= 0,014) with OR = 4,6 and attitude not related to ANC visits (p= 0,303) Conclusion: There is a relationship between pregnant women's knowledge and anxiety about ANC visits and no relationship between pregnant women's attitudes towards ANC visits during the COVID-19 pandemic at Glugur Darat Health Center and the most influencing variable is anxiety.

Abstract

Background: Regardless of recovery in the number of visits from the pandemic to the new normal and post-pandemic era, hospitals need to ensure that patients who have come to the hospital are willing to visit for a second time (repurchase intention). Repurchase intention is influenced by patient satisfaction and quality of services. There are several tools to assess quality of hospital service, such as HEALTHQUAL questionnaire. There has been no research linking HEALTHQUAL, patient satisfaction, and repurchase intention. Objective: This research aims to study the association between HEALTHQUAL and repurchase intention, mediated by patient satisfaction. Methods: This research was a cross-sectional study at a private hospital in Jakarta (December 2022-January 2023). Statistical analysis was performed using SmartPLS software. Results: This study included 100 participants with a mean age of 40 years. Based on the questionnaire results, most of the patients agreed on questions about service quality and satisfaction. Model analysis showed that there was a significant association between HEALTHQUAL, patient satisfaction and repurchase intention (p<0.05; T-statistics>1.96). Moreover, patient satisfaction showed a significant mediator effect towards HEALTHQUAL and repurchase intention (p<0.001, T-statistics>1,96) Conclusion: HEALTHQUAL positively and significantly affects patient satisfaction and repurchase intention. Patient satisfaction also affects repurchase intention positively and significantly. Besides, patient satisfaction partially mediates the effect of HEALTHQUAL on repurchase intention. There is a positive effect of HEALTHQUAL mediated by patient satisfaction on repurchase intention.

Abstract

Background: Acquired immunodeficiency syndrome (AIDS) is a syndrome of disease due to decreased immune system caused Human Immunodeficiency Virus (HIV) and become a global problem, including Indonesia. Adolescents are group at risk for HIV/AIDS. Knowledge is important in determining attitude, attitude is a response to objects , attitude is a predisposing factor that can influence adolescent practice, so good knowledge about HIV/AIDS will help adolescent to make prevention. This study aims to identify of knowledge, attitudes and practice of adolescents about HIV/AIDS in Galang District, Deli Serdang Regency, North Sumatra.Objective: To identify the level of knowledge, attitudes and behavior of adolescents about HIV/AIDS in Galang District, Deli Serdang Regency, North Sumatra.Methods: This descriptive study used a cross-sectional approach conducted at Galang District High School in October 2022. The sample for this study was 104 senior students using cluster random sampling technique. Data was collected using a questionnaire that had been tested for validity and reliability with a Cronbach's Test Alpha value of 0.926; 0.871 and 0.827. Data were analyzed by univariate analysis.Results: The results of this study showed that the majority of respondents were 16 years old (70.2%), female (68.3%), grade 11 (70.2%), in social sciences (59.6%) and received information from the internet/social media (87.5%). The majority of respondents had a sufficient knowledge (57.7%), good attitude (79.8%) and good practice (99%).Conclusion: The majority of students at senior high school Galang District have sufficient knowledge about HIV/ AIDS, have a good attitude about HIV/ AIDS and are not at risk of contracting HIV/ AIDS.

Abstract

Background: Cutaneous candidiasis is a fungal infection of the skin caused by yeast or microorganisms belonging to the genus Candida. It is known that diabetes mellitus is one of the predisposing factors for candidiasis cutis caused by changes in the body's defense system. Diabetes mellitus is a metabolic disease characterized by hyperglycemia resulting from defects in insulin secretion or insulin action. Starting from the autoimmune destruction of pancreatic cells resulting in insulin deficiency to abnormalities resulting in resistance to insulin action. To determine a person's level of glucose control, an HbA1c examination is needed. This test will show the average blood sugar level over the last 90 days and represent a percentage. This test can also be used to diagnose diabetes. Objective: To analyze the relationship between HbA1c levels and cutaneous candidiasis in type 2 diabetes mellitus patients. Methods: This study is an observational analytic study with a retrospective design. To determine the relationship between glycosylated hemoglobin (HbA1c) levels and cutaneous candidiasis in type 2 diabetes mellitus patients. On examination of HbA1c levels it can be said to be controlled < 6.5% and not controlled > 6.5%. Results: The results of this study were taken from 62 patient medical record data according to the inclusion and exclusion criteria, it was found that the majority of patients with type 2 diabetes mellitus were women aged ≥55 years. Based on HbA1c levels, the patients in this study were more dominantly uncontrolled with negative cutaneous candidiasis, 56 (90.3%). The results of Fisher's exact analysis showed p-value = 1.000 (p > 0.05) meaning that there was no significant relationship between HbA1c levels and cutaneous candidiasis. Conclusion: There is no significant relationship between HbA1c levels and the incidence of cutaneous candidiasis.

Abstract

Background: Coronavirus Disease 2019 (COVID-19) is a disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) which causes symptoms of shortness of breath, fever, and cough and is contagious and has infected tens of millions of people worldwide. One of the factors that can lead to the transmission of COVID-19 is the community's non-obedience with the health protocols set by the government, namely 3M.Objective: This study aims to determine the description of community obedience in the working area of the Kabanjahe Public Health Center to health protocols in preventing COVID-19.Methods: This study is a descriptive study with a cross-sectional design. There are 162 respondents who were selected by simple random sampling technique. The research sample was from the population in the working area of the Kabanjahe Public Health Center who were willing to be interviewed. Data were taken using a questionnaire.Results: The results showed that the majority of people in the working area of the Kabanjahe Public Health Center always wash their hands with soap and running water for 40-60 seconds (40,7%) and always use alcohol-based antiseptic liquid (handsanitizer) for at least 20-30 seconds if there is no running water (35,2%), always avoid touching eyes, nose and mouth with unclean hands (50,6%). Always use a mask if you have to leave the house (74,7%) and always use a mask by covering the nose and mouth until the chin reaches (69,8%). Always maintain a minimum distance of 1 meter from other people when outside the house (48,8%) and always avoid crowded places and crowds (55,6%), always take a shower and change clothes after traveling (61,7%), always exercising regularly for at least 30 minutes a day (43,8%), and always getting enough rest for at least 7 hours (67,9%).Conclusion: The majority of the people in the working area of the Kabanjahe Health Center are carrying out health protocols in preventing COVID-19.

Abstract

Background: The use of the antituberculosis drugs isoniazid and rifampin triggers hepatotoxicity. Soursop leaves have hepatoprotective effect because they contain antioxidant compounds that can help inhibit and prevent oxidative damage to the liver. This study aims to determine the effect of soursop leaf extract on SGPT and SGOT levels in rats induced by isoniazid and rifampicin.Methods: This experimental laboratory study used the post-test-only control group design. This study used 25 wistar rats divided into five groups and given a 14-day treatment. Group 1 (negative control) was assigned isoniazidand rifampicin without administering soursop leaf extract. Group 2 (positive control) was given isoniazid and rifampicin by administering hepatoprotector compound. Group 3, 4, and 5 were given isoniazid and rifampicin for 1 hour, and continued with the administration of soursop leaf extract with a dose of 36 mg/day, 72 mg/day, and144 mg/day respectively. The dose of the isoniazid used was 189 mg/day, while the rifampicin dose was 252mg/day. Rat termination was carried out on the 15th day. Blood samples to measure SGPT and SGOT levels were taken through intracardial.Results: The results showed that there was a significant difference in SGPT and SGOT levels between the treatment group and negative control at doses of 72 mg/kg body weight/day and 144 mg/kg body weight/day. There was no significant difference between SGPT levels in the treatment group of dose of 144 mg/kg/day and the positive control group. There were significant differences in the SGOT levels of the treatment groups and the positive control group.Conclusion: Administration of soursop leaf extract can lower SGPT and SGOT levels in rat induced with isoniazid and rifampicin. The hepatoprotective effect of soursop leaves was obtained at a dose of 144 mg/kg body weight/day. Keywords: soursop leaf, isoniazid, rifampicin, SGPT, SGOT

Abstract

Background: The older a person is, the more susceptible they are to many diseases, especially the elderly. Unhealthy diet and lifestyle can affect the immune system of the elderly, making them more susceptible to various diseases, one of which is hypertension. The number of people with hypertension and the death rate due to hypertension and its complications is increasing every year.Objective: To analyze the consumption of high-sodium foods on the incidence of hypertension in the elderly at the Medan Area South Health Center.Methods: This research is an analytic research with a cross sectional approach. This research was conducted at the Medan Health Center in the South area in November 2022–March 2023. The sample in this study were elderly people who suffered from hypertension with a total sample of 68 people. Data taken through a questionnaire is primary data and analyzed univariately and bivariately using the Somers'd test.Results: The characteristics of the respondents in this study were found to be more female (54.4%), aged between 45-59 years (54.4%), and worked with a predominance of self-employed jobs (39.7%). The pattern of sodium intake in this study was more in the high sodium intake category (55.9%). The blood pressure category of respondents in this study had more hypertension grade 1 blood pressure (51.5%). Based on the bivariate test, it was found that more respondents had a high sodium intake category with hypertension degree 1 blood pressure.Conclusion: Based on the Somers' D analysis test, a significant relationship was found between consumption of high-sodium foods and the incidence of hypertension in the elderly at the Medan Area South Health Center (p value 0.000).Keywords: Hypertension, Consumption, Elderly, Sodium

Abstract

ABSTRACT Background : The increasing cases of Tuberculosis (TB) is a concerned health problem to the world, especially Indonesia, which Indonesia is the 2nd most TB cases in the world. The increase in TB cases is accompanied by the increase in Multi Drug Resistant Tuberculosis (MDR TB) cases. Genetic and immunity host factor, inadequate treatment and bacterial mutations affect the infection of MDR TB. Vitamin D has an important role in immune regulatory to eliminate Mycobacterium tuberculosis, as of it can affect the susceptibility to MDR TB. Purpose : The aim of this study was to determine the polymorphism of the Vitamin D Receptor (VDR) BsmI gene in MDR Tuberculosis. Methods : This study was a case control study involving 32 non-MDR TB patients and 32 MDR TB patients. The VDR gene was restricted by the Restriction Fragment Length Polymorphism (RFLP) method using BsmI enzyme and agarose gel visualization was performed. Results : Homozygous variant GG genotype was found higher in non-MDR Tuberculosis (43.75%) than in MDR Tuberculosis (18.8%), and heterozygotes AG genotype was not found in MDR Tuberculosis either in non MDR Tuberculosis. There was no significant relationship between the VDR BsmI gene polymorphism and MDR tuberculosis (p>0.05). Conclusion : This results showed that BsmI VDR gene polymorphism does not affect the susceptibility to MDR Tuberculosis. Kasus TB yang meningkat disertai juga dengan peningkatan kasus Multi Drug Resistant Tuberculosis (MDR TB). Faktor pejamu genetik dan imunitas, pengobatan yang tidak adekuat dan mutasi bakteri, mempengaruhi terjadinya MDR TB. Vitamin D memiliki peran penting dalam regulasi imunitas tubuh untuk mengeliminasi mycobacterium tuberculosis, sehingga dapat mempengaruhi kerentanan seseorang menderita MDR TB . Tujuan : Penelitian ini bertujuan untuk mengetahui polimorfisme gen Vitamin D Receptor (VDR) BsmI pada MDR Tuberkulosis. Metode : Penelitian ini merupakan penelitian case control yang melibatkan 32 penderita non MDR TB dan 32 penderita MDR TB. Gen VDR direstriksi dengan metode Restriction Fragment Length Polymorphism (RFLP) menggunakan BsmI dan dilakukan visualisasi gel agarose. Hasil : Genotif homozigot varian GG lebih banyak ditemukan pada non MDR Tuberkulosis (43,75%) dibandingkan pada MDR Tuberkulosis (18,8%) dan genotif heterozigot AG tidak terdapat pada semua subjek. Tidak terdapat hubungan yang signifikan antara polimorfisme gen VDR BsmI dengan MDR Tuberkulosis (p>0,05). Kesimpulan : Polimorfisme gen VDR BsmI tidak mempengaruhi kerentanan menderita MDR Tuberkulosis. Kata Kunci : Gen VDR, polimorfisme, MDR Tuberkulosis, Non MDR Tuberkulosis, RFLP

Abstract

ABSTRACTS Background: The perceived competence of newly graduated doctors who had undergone clinical practice in their first year could give insight into curriculum accomplishment. Objective: This study aimed to understand alums' perceptions of their performance in tropical infection cases. Methods: Alums who have finished their internship and have been practicing as doctors for less than a year were respondents to this study. We purposefully selected eight respondents based on gender, workplace, and work location. We conducted in-depth interviews by phone. The interview was recorded, and the conversation was transcribed verbatim. Thematic analysis is carried out on transcription by coding and categories. Results: The interview results obtained two significant themes: the perception of competence related to tropical medicine cases and the perception of curriculum implementation. Cases and types of skills influenced perceptions of competence. Respondents perceived curriculum implementation as insufficient and influenced by case exposure, learning methods, and the national final exam. Conclusion: Providing varied cases, improving feedback from clinical supervisors, and initiating early clinical experience are necessary to improve tropical infection competencies. Keywords: perception, medical student, tropical infection ABSTRAK Latar belakang: Persepsi kompetensi alumni yang baru bekerja dapat memberikan informasi mengenai capaian kurikulum. Tujuan: Penelitian ini bertujuan mendapatkan gambaran mengenai persepsi alumni terhadap kompetensinya dalam menghadapi kasus infeksi tropis. Metode: Alumni yang sudah selesai menjalani internship dan telah berpraktek sebagai dokter kurang dari setahun menjadi responden dalam penelitian ini. Terdapat delapan responden yang dipilih dengan teknik purposive sampling, berdasarkan jenis kelamin, tempat kerja, dan lokasi kerja. Pengumpulan data dilakukan dengan wawancara mendalam menggunakan panggilan telepon. Hasil wawancara direkam dan ditranskripsi secara verbatim.Analisis tematik dilakukan terhadap transkripsi dengan cara membuat koding dan kategori. Hasil: Hasil wawancara didapatkan dua tema besar yaitu persepsi kompetensi terkait kasus infeksi tropis dan persepsi mengenai implementasi kurikulum. Kasus dan jenis ketrampilan mempengaruhi persepsi mengenai kompetensi. Responden mempersepsikan implementasi kurikulum infeksi tropis masih kurang mencukupi. Persepsi ini dipengaruhi oleh paparan kasus, metode belajar, dan ujian kompetensi dokter. Kesimpulan: Perbaikan metode pengajaran, memperbanyak kasus, serta umpan balik dari supervisor perlu untuk meningkatkan performa kompetensi kedokteran tropis.
