Abstract

Background: Treating STEMI with PCI not only has a positive impact on patients, but also the risk of complications. A complication that can occur is AKI. The aim of this case report is to increase clinician awareness by analyzing the risk of AKI after PCI in order to reduce patient morbidity and mortality rates. Case: A 58-years old-man, with a history of diabetes mellitus, dyslipidemia, and a previous heart attack, came to the emergency department because of chest pain. The patient experienced STEMI and received fibrinolytic therapy. After 3 days of treatment, the condition worsened and the patient experienced shortness of breath (pulmonary edema). The patient was referred to RSUD Dr. M. Soewandhie Surabaya to undergo PCI. Two days after PCI, there were clinical signs of AKI. The patient was prepared for Renal Replacement Therapy (RRT), but the patient experienced cardiac arrest and was declared dead. Discussion: AKI after PCI involves various complex mechanisms. Several factors that can increase the probability of AKI after PCI in this case are a history of diabetes mellitus, previous decline in kidney function, and pulmonary edema. Technique factors such as transfemoral PCI and the amount of contrast volume used also have an influence on the incidence of AKI after PCI and need to be studied further. Risk factors in patients can be used to determine clinical scores, such as the Mehran, ADVANCIS, or ACEF-MDRD scores to predict the probability of AKI after PCI. Conclusion: AKI after PCI is a frequent complication and calculations using clinical scores can be applied to predict this event.

Abstract

Background: MSC, IL-10, TNF-a, IFN-g, PGE2. Background: The high proliferation and differentiation capabilities, paracrine effects and immunomodulatory functions of Mesencymal Stem Cells (MSCs) have been utilized in cellular therapy approaches for infectious and degenerative diseases in the last decade. Paracrine effects MSCs contain growth factors and cytokines, including IL-10, TNF-a, IFN-g and PGE2. There are still debates regarding the role and performance of the levels of paracrine effects of MSCs IL-10, TNF-a, IFN-g and PGE2, so it is necessary to have a model of the performance of these paracrine effects. Objective: To determine the levels of IL-10, TNF-a, IFN-g and PGE2 in basal in-vitro culture conditions, which can be used as a model for MSC immunomodulatory performance. Methods: This research is an in-vitro culture study of MSCs in basal conditions. MSCs were cultured for 21 days and levels of IL-10, TNF-a, IFN-g and PGE2 were examined on the 7th, 14th and 21st days in the culture medium, using the ELISA method. Results: IL-20 was not found in the culture medium until the 21st day, meanwhile TNF-a levels continued to increase until the 21st day. IFN-g was only found on day 7 of culture. PGE2 levels were found to be the highest secreted among the cytokines analyzed, but were not found on day 21. Conclusion: Paracrine effects secreted by MSCs in basal culture conditions due as a response to cell damage that occurs during the duration of culture. The paracrine effects of MSCs can influence each other, so that there can be suppression or induction of secretion of paracrine effects. Keywords: MSC, IL-10, TNF-a, IFN-g, PGE2. Abstrak Latar belakang: Kemampuan proliferasi dan diferensiasi yang tinggi, efek parakrin serta fungsi imunomudulator Mesencymal Stem Cells (MSCs) dimanfaatkan dalam pendekatan terapi seluler penyakit infeksi dan degeneratif dalam dekade terakhir. Efek parakrin MSCs mengandung growth factor dan sitokin, diantaranya IL-10, TNF-a, IFN-g dan PGE2. Banyak perbedaan pendapat tentang peran dan gambaran kadar efek parakrin MSCs IL-10, TNF-a, IFN-g dan PGE2 sehingga diperlukan adanya model gambaran efek parakrin tersebut. Tujuan: Mengetahui kadar IL-10, TNF-a, PGE2 dan IFN-g pada kondisi basal kultur in-vitro, yang dapat digunakan sebagai model gambaran imunomodulator MSC. Metode: Penelitian ini merupakan penelitian kultur in-vitro MSCs pada kondisi basal. MSCs dikultur selama 21 hari dan dilakukan pemeriksaan kadar IL-10, TNF-a, PGE2 dan IFN-g pada hari ke-7, ke-14 dan ke-21 pada medium kultur, dengan menggunakan metode ELISA. Hasil: IL-20 tidak ditemukan pada medium kultur hingga hari ke-21, sedangkan kadar TNF-a semakin meningkat hingga hari ke-21. IFN-g hanya ditemukan pada kultur hari ke-7. Kadar PGE2 ditemukan paling tinggi disekresikan diantara sitokin yang dianalisa, tetapi tidak ditemukan pada hari ke-21. Kesimpulan: Efek parakrin disekresikan MSC pada kultur kondis basal sebagai respon kerusakan sel yang terjadi selama durasi kultur. Efek parakrin MSC dapat saling mempengaruhi, sehingga dapat terjadi supresi atau induksi sekresi efek parakrin. Kata Kunci: MSC, IL-10, TNF-a, IFN-g, PGE2.

Abstract

Latar belakang: Jumlah penduduk lansia di Indonesia tahun 2021 sebesar 10,82%. Jumlah lansia lebih dari 7% total penduduk menunjukkan bahwa Indonesia berada di era ageing population. Diperlukan analisis terkait dengan perilaku pencarian pengobatan pada lansia untuk dapat memastikan kualitas hidup yang baik. Perilaku pencarian pengobatan meliputi sikap dan tindakan yang diambil oleh individu ketika sakit atau mengalami masalah kesehatan. Tujuan: Penelitian ini bertujuan untuk mengetahui gambaran perilaku pencarian pengobatan pada lansia, terutama di Kelurahan Kotabaru, Kecamatan Gondokusuman, Kota Yogyakarta. Metode: Penelitian menggunakan metode deskriptif kuantitatif dengan pendekatan studi cross-sectional. Besar sampel adalah 40 responden. Hasil: Data demografi menunjukkan 72,5% responden masuk dalam elderly (60-74 tahun), 25% masuk dalam kategori old (75- 90 tahun) dan 2,5% dalam kategori very old (> 90 tahun). 55% responden memiliki sikap kurang dan 45% bersikap baik dalam pencarian pengobatan. Aspek tindakan responden menunjukkan 65% bertindak baik dan 35% bertindak kurang baik dalam pencarian pengobatan. Responden dengan pengeluaran Rp 25.000 – Rp 50.000 per hari memiliki rasio tindakan pencarian obat yang baik lebih tinggi signifikan dibandingkan dengan responden dengan pengeluaran Rp 10.000 – 24.999 per hari (X2= 9,707, p= 0,008). Responden dengan fasilitas pelayanan pengobatan skor tinggi memiliki rasio responden dengan tindakan baik lebih tinggi dibandingkan dengan fasilitas pelayanan pengobatan skor sedang (X2= 7,556, p= 0,006). Kesimpulan: Penelitian ini menunjukkan bahwa sebagian besar responden memiliki sikap yang kurang baik dan tindakan yang baik dalam pencarian pengobatan. Pengeluaran per hari responden dan fasilitas pelayanan pengobatan memengaruhi tindakan pencarian pengobatan. Kata kunci: Perilaku pencarian pengobatan, lansia, sikap, tindakan.

Abstract

Background: Several factors reported to be associated with the incidence of acne vulgaris include genetics, race, stress, diet, cosmetics, drugs, and smoking habits. Many studies have been performed to look for the effect of cosmetic use on the incidence of acne vulgaris, but these studies are still limited to observational studies. Therefore, this study aimed to analyze the effect of cosmetic use on the incidence and severity of acne vulgaris through several studies in the 2018-2023 period. Methods: This systematic review was performed by systematically searching for some literatures on Google Scholar with the keywords Cosmetics and Acne vulgaris. The results of this systematic searching found 21 scientific articles in either English or Indonesian. Results: The most of literatures obtained reported that the use of cosmetics, in the form of compact powder, BB cream, and foundation affected the incidence of acne vulgaris. Meanwhile, patients suffered from acne vulgaris, who used compact powder and foundation consistently aggravated acne vulgaris. There is still limited study on other decorative cosmetics such as concealers and primers on the incidence of acne vulgaris. Conclusion: The use of cosmetics such as compact powder, BB cream, and foundation can cause acne vulgaris, furthermore compact powder and foundation can also aggravate acne vulgaris that has occurred.

Abstract

Background: Body dysmorphics are psychological symptoms where sufferers experience an excessive obsession with minimal and even imaginary physical disabilities. One of the factors that cause the appearance of a sense of dissatisfaction with oneself or what is known as a symptom of body dysmorphics is body image. A person with a positive body image will find themselves attractive, both according to themselves and according to others or at least can accept themselves as they are in contrast to someone with a negative body image, they will consider themselves unattractive. The purpose of this study was to determine the relationship between body image and body dysmorphic symptoms in adolescents during the COVID-19 pandemic. Case: This study was observational analytical study with a cross-sectional design. Sample selection using purposive sampling method with a total sample of 100 high school teenagers in Medan Kota District in 2022. Univariate analysis is displayed in tabular form and bivariate assays are performed using chi-square tests. Discussion: The result of this study, 89% of respondents classified as positive body image and 86% did not have symptoms of body dysmorphic. The relationship of body image with body dysmorphic symptoms was analyzed with the Chi-Square test and found a p value = 0.000.Conclusion: there is a relationship between body image and body dysmorphic symptoms in adolescents during the COVID-19 pandemic. Abstract

Background: One of the components of the SPICES approach is Problem-Based Learning (PBL). The Faculty of Medicine at HKBP Nommensen University in Medan has implemented this PBL system in a tutorial activity. Several benefits of the above tutorial activity can be achieved if the tutorial runs effectively. An effective tutorial activity is one where its objectives can be achieved as expected. According to several studies, the tutor and students can influence the effectiveness of the tutorial activity.1,2,3,4 Even Indonesian culture can affect the effectiveness of the tutorial.5 Therefore, the researcher wants to conduct a study on the description of tutor behavior and the effectiveness of tutorial groups at the Faculty of Medicine, HKBP Nommensen University in Medan, Academic Year 2022/2023. Objective: Describe the behavior of tutors and the effectiveness of tutorial groups at the Faculty of Medicine, HKBP Nommensen University in Medan, Academic Year 2022/2023. Methods: This research is a descriptive study and a sample size of 179 students. Data were analyzed using the Chi-square test. Results: There is a relationship between premenstrual syndrome and insomnia among female students at SMA Negeri 2 Medan. The level of tutor behavior in terms of social congruence, cognitive congruence, subject-matter expertise, and the level of tutorial group effectiveness based on students' perceptions at the Faculty of Medicine, HKBP Nommensen University in Medan, Academic Year 2022/2023, is high. Conclusion: There is relationship between premenstrual syndrome with insomnia symptom towards female students of SMA Negeri 2 Medan.

Abstract

Background: Pediculosis capitis is a common head lice infestation among children, particularly in environments with suboptimal hygiene, such as orphanages. A lack of knowledge, attitudes, and preventive behaviors can increase the risk of disease transmission. Objective: To determine the level of knowledge, attitudes, and preventive behaviors regarding pediculosis capitis among children at Hasami Kasih Orphanage, Medan, in 2024. Methods: This study employs a descriptive method with a cross-sectional approach. A total of 15 children who met the inclusion and exclusion criteria were selected using purposive sampling. Data were collected through questionnaires and analyzed descriptively using computer software. Results: The findings show that most respondents were aged 9–13 years (66.7%) and were male (73.3%). The prevalence of pediculosis capitis was 26.6%, occurring exclusively in female respondents. The majority of respondents had good knowledge (66.7%) and very positive attitudes (66.7%) toward prevention. However, preventive behavior was still in the moderate category (80%). Conclusion: The children in this orphanage generally have good knowledge and attitudes regarding pediculosis capitis prevention. However, preventive behavior still needs improvement. Further research is needed to identify other factors influencing the occurrence of pediculosis capitis and to examine the relationship between knowledge, attitudes, and preventive behaviors in infestation cases.

Abstract

Background: Anxiety disorders are a type of mental health condition that is common in society. One of the most common types of anxiety is social anxiety (SA), which is a condition in the form of a real and persistent fear of social situations. Nearly 70% of people had received the vaccine during the Covid-19 pandemic and had begun to actively conduct out their activities outside, which led to an increase in social anxiety. Objective: The purpose of this research was to characterize social anxiety in senior high school students in the Medan City District in 2022 during the Covid-19 pandemic. Methods: This study used a cross-sectional design and was descriptive in nature. There was 130 respondents in senior high school at Medan Kota District in 2022 who were chosen by the cluster selection technique. Results: In this study obtainbed 130 respondents, the level of social anxiety in senior high school adolescents at Medan City District were mild 45.4%, moderate 50,8% and severe 3.8%. Conclusion: The majority level of social anxiety in senior high school adolescents at Medan City District during the Covid-19 pandemic were mild to moderate in both male and female genders. Abstract

Background : Acute myeloid leukemia (AML) is known to confer complications mainly due to their underlying disease or chemotherapy. Although they are at high risk for a wide variety of hemorrhagic and thrombotic complications, thrombotic vascular complication as an initial manifestation is less common. Cerebral involvement in patients with AML is associated with a five times higher mortality risk. Case : 54-year-old male patient, presenting with left-sided hemiparesis, fever, decreased appetite and laboratory findings showed leukocytosis (40,990/mm3) and thrombocytosis (814,000/mm3). Computed tomography showed a right infarct in the middle cerebral artery territory. The diagnosis of AML emerged during investigated. While conservatively managed, he developed a loss of consciousness and was admitted to the intensive care unit. Discussion : In a case series of 24 patients, thrombotic events were the first clinical manifestation of leukemia in only 3.4% of cases. The association of thrombocytosis with AML is unusual, a few cases have been described with chromosome 3 abnormalities, however, we could not perform a genetic analysis of this case. Pathogenic mechanism in several studies demonstrated a causal relationship between elevated platelet count are associated with an increased risk of vascular occlusion and leukocytosis in the acute phase of the disease produces hyperviscosity causing blood stasis. Conclusion : Thrombotic events in the cerebrovascular system in early cases of AML may occur possibly due to hyperviscosity, and this may increase the risk of death. Abstract

The medial longitudinal arch is a part of the plantar foot with clinical significance. Three variations of the medial longitudinal arch consist of normal, low, and high arches. These variations are caused by intrinsic factors such as gender. Failure to form the normal medial longitudinal arch leads to poor posture and balance. Variation in the shapes of the medial longitudinal arch can be assessed using footprints, namely the arch index. This study aims to identify the variations in the medial longitudinal arch between young adult males and females. The research is descriptive-analytic with a cross-sectional study design. The subjects are young adults aged 18-22 years. Sampling using the simple random sampling technique with a sample size of 54 participants. The medial longitudinal arch shape was evaluated using the footprint method with the arch index. The majority of participants (72.2%, 39 participants) were female, according to the results. The most age group was 21 years old 23 participants (42.6%). The highest average arch index (0.29) was in the male participants. The most prevalent normal medial longitudinal arch shape was observed in females 28 participants (71.8%). In males, the low arch was most common among 11 participants (73.35%), while the high arch was found in 5 female participants (12.8%). The Mann-Whitney statistical test showed p < 0.05. This study concludes that the medial longitudinal arch forms vary between young adult males and females.
