Abstract

Abstract Background : Type 2 DM is DM with hyperglycemia due to a combination of insulin action resistance, insulin secretion and excessive or inadequate glucagon secretion. High adherence to medication is one of the factors that determine the success of the DM type 2 control process. Objective : This study aimed to analyze the factors that influence adherence to taking antidiabetic drugs in the Muarasoma Health Center Work Area in 2021. Methods: This study is an analytical study using a cross-sectional approach which was conducted on 77 patientswith type 2 DM according to the inclusion and exclusion criteria in the working area of the Muarasoma HealthCenter. Knowledge level, medication adherence and family support were measured using the Diabetes Knowledge Quiestionnaire (DKQ-24), Morisky Medication Adherence Scale (MMAS-8) and the Hensarling Diabetes Family Support (HDFS) which have been validated. Results: The factors influencing medication adherence in patients with type 2 DM were gender with p value = 0,002), education level with p value = 0,007, income with p value = 0,015, level of knowledge with p value =0,047, and family support with p value = 0,043). Factors that are not associated with medication adherence in patients with type 2 DM were DM duration with p value = 0,059 and drug regimen with p value = 0,056 Conclusion: The factors that influence medication adherence in patients with type 2 DM at the Muarasoma Health Center are gender, level of education, level of knowledge, income, and family support.

Abstract

Background: The spread of COVID-19 has been increasingly widespread since the easing of Large-Scale Social Restrictions (PSBB). This increases community activities outside the home, especially office employees who have started working, so that the office becomes the largest source of transmission clusters. Breaking the chain of spreadof COVID-19 requires a good balance between knowledge, attitudes, and preventive measures for COVID-19. Individual knowledge about COVID-19 based on appropriate information is the most important domain in shaping individual behavior. It is hoped that this good knowledge can influence every individual in showing a positive attitude and good action in preventing and controlling COVID- 19.Objective: This study aimed to determine the relationship between the level of knowledge of the campus staff of the Universitas HKBP Nommensen Medan on the attitudes and actions to prevent Coronavirus Disease 2019 (COVID-19).Methods: This study was an analytic study using a cross sectional design. The number of samples is 86 people who were selected using purposive sampling method. The data obtained are primary data from questionnaires that have been filled out by the research sample.Results: The results of this study showed 86 respondents, good knowledge level (97.7%), positive attitude (90.7%) and good action (84.9%). In the bivariate analysis, it was stated that there was a statistically significant relationship between knowledge and preventive measures for COVID-19 (p=0.021).Conclusion: Good knowledge was expected to have a good impact on prevention and transmission of COVID-19. Abstract

Background: Hand hygiene is very important because human hands are often contaminated with microorganisms, so that it becomes an intermediary for the entry of disease-causing microorganisms into the body, especially Escherichia coli and Staphylococcus aureus. The use of hand sanitizer has shown an increase in society. Hand sanitizers are available with various types of packaging, dosage forms, variations in the composition of active ingredients and their carriers at quite competitive prices.Objective: To determine the antibacterial effect of various hand sanitizers available in the market and their comparison with antiseptic handwash soap.Methods: Antibacterial effectiveness test using the Kirby Bauer method by measuring the diameter of the clear zone produced on Muller Hinton media by diffusion discs tested with four hand sanitizer products available in the market compared with antiseptic hand soap as positive control and akuades as negative control.Results: Various hand sanitizer products containing 70% alcohol showed moderate to strong inhibition against E. coli and S.aureus. Analysis of differences in the activities of various hand sanitizer products using the KruskalWallis non-parametric method showed a difference with p= 0.003 against E. coli and p=0.002 against S.aureus. Alcohol denatures bacterial proteins so that it interferes with the metabolic processes of bacterial cells which causes bacterial cell death.Conclusion: Hand sanitizer is effectively used to inhibit the growth of E. coli and S.aureus bacteria.

Abstract

Background: The evidence of an occurrence is increasingly developing with the presence of silent witnesses, this type of witness required an expert to be able to explain the occurrence, one of which is a doctor. A doctor must meet the characteristic criteria in order for a doctor to explain a case. Objective : This study aimed to describe characteristics of doctors who are called as experts in the Medan District Court (DC) and Lubuk Pakam DC. Methods : This research was a descriptive qualitative study using a cross-sectional. The number of samples is 145 doctors who were selected using the total sampling method. Primary data from interviews with related doctor, and secondary data in the form of case data presenting them as experts. Result : The results of this study showed 145 doctors, (58,1%) abuse cases in Medan DC, (38,1%) sexual abuse cases in Lubuk Pakam DC, (37,7%) male doctors in Medan DC and (33,3%) Lubuk Pakam DC, (37,7%) the age group 25-34 years in Medan DC and (33,3%) 35-44 year in Lubuk Pakam DC, (25,0%) the duration of doctors was 6-10 year in Medan DC and (23,8%) 31-35 year in Lubuk Pakam DC, Bhayangkara TK II Hospital in Medan DC and Lubuk Pakam DC, general practitioner (57,3%) in Medan DC and doctors forensic (38,1%) in Lubuk Pakam DC, 1 doctors who compensated (0,7%). Conclusion : There were still many doctors who haven’t received reimbursement when presented as experts at the trial. Key Words : Doctor, Expert, Reimbursement.

Abstract

Background: Subramanian stated, countries with vaccination coverage of 75% of the population have a higher increase in COVID-19 cases compared to countries with vaccination coverage of 10% of the population. Prevention of COVID-19 is not enough just by vaccination, the 5M health protocol should still be carried out.Objective: The purpose of this study was to determine the relationship between COVID-19 prevention behavior (5M) before and after vaccination with COVID-19 vaccination in the community at H. Adam Malik General Hospital Medan. In addition to 5M's behavior, the frequency of AEFI COVID-19 was also seen.Methods: This is a cross sectional study with a survey approach. This research was conducted at the H. Adam Malik General Hospital Medan. The population is people 18 years and over who have not and have had 1st and 2nd vaccinations for COVID-19. Consecutive sampling techniques were used, as many as 200 people. Measuring with a questionnaire, 3 times, namely pre-1st vaccination, pre-2nd vaccination, and post-2nd vaccination. Non-parametric comparative analysis using the Friedman test, and Post Hoc analysis on mask wearing behavior.Results: Significant differences were found between the behavior of wearing masks pre1stvaccination nd pre2nd vaccination with p 0.008. The results of the analysis with the Friedman test on the behavior of wearing masks showed a significant relation, with p value 0.016. The most common AEFI COVID-19 that respondents complained about was muscle pain, as many as 81 people (40.5%).Conclusion: The trend shown is the increase in non-compliant behavior in carrying out 5M behavior, in each measurement.

Abstract

Background: Blood pressure is a very important factor in the body's circulatory system. If there is an increase ordecrease in blood pressure it will affect the balance in the body. High blood pressure, also known as hypertension, affects many people around the world, including in Indonesia itself. One of the factors that affect blood pressure is physical activity.Objective: This study aimed to determine the association between physical activity and blood pressure of students in the Faculty of Medicine, Universitas HKBP Nommensen Medan.Methods: This study was an analytic study with a cross-sectional design. Total respondents was 90 students who were selected using purposive sampling method. This study used primary data, namely a questionnaire to fill in the history of physical activity and direct blood pressure measurements. The association between physical activity and blood pressure was analyzed using the Chi-Square test.Results: In this study, it was found that the majority of respondents were classified as active (74.4%) and had normal blood pressure (74.4%). The results of the Chi-Square test found that there was no association between physical activity and blood pressure in students of the Faculty of Medicine, Universitas HKBP Nommensen Medan (p = 0.240).Conclusion: There is no association between physical activity and blood pressure in students of the Faculty of Medicine, Universitas HKBP Nommensen Medan, during the COVID-19 pandemic.

Abstract

ABSTRACT Background: Infections and antimicrobial resistance remains a major public health worldwide. Therefore, studies regarding the natural therapeutic agent that is safe for humans and specific against bacteria are getting more interest. Objective: This study aimed to investigate the effectiveness of combination red ginger and garlic extract against Streptococcus pyogenes. Methods: This was a laboratory experimental study. The red ginger and garlic extract obtained by maceration extraction method using 70% ethanol and antibacterial activity test was conducted by disk diffusion method. Results: : One way Anova test showed that red ginger extract in 75% and 100% concetrations have significant difference with the control. Garlic extract in 75% and 100% concentrations have significant difference with the control. The higher concentration result wider inhibition zona hambat both in red ginger and garlic extract. Otherwise, red ginger- garlic combination extract showed no significant difference with the control. The widest mean inhibitioin was shown in R50-G50 extract combination which is 2.45mm (±0.3). Post hoc LSD test showed that red ginger 100% that has significant difference with red ginger 25%, so did garlic extract. There was no significant difference between red ginger-garlic combination groups. Conclusion: Red ginger and garlic extract has no synergic effect against Streptococcus pyogenes.

Abstract

ABSTRACT Background: COVID-19 is a disease caused by Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) which is rapidly transmitted in humans. Systemic inflammation will cause changes in leukocytes in the circulation which can lead to neutrophilia and lymphocytopenia. In COVID-19, the neutrophil-lymphocyte ratio (NLR) can be used as a predictor of disease severity in the early stages. An increase in NLR can reflect an increased inflammatory process and can predict severity. This study aims to determine relationship between NLR and the severity of COVID-19 patients at Martha Friska Hospital Medan in 2021. Methods: The design of this research is observational analytic with cross sectional method. Samples was taken from medical records of COVID-19 patients at Martha Friska Hospital Medan from January-March 2021 with consecutive sampling method and obtained 74 samples. This research was done in November 2021. The analysis of the data processed by the univariate frequency distribution table and the SPSS version 25 program was carried out bivariately using the Chi Square test. Results: From 74 COVID-19 patients, RNL <3.13 obtained 29 people (93%) with mild severity, 2 people (7%) moderate severity and no patients with severe-critical severity. COVID-19 patients with RNL 3.13 is 6 people (14%) with mild severity, 20 people (46%) with moderate severity, 17 people (40%) with severe-critical severity. This study showed there was a relationship between NLR and the severity of COVID-19 patients (p=0.000). Conclusion: There is a relationship between NLR and the severity of COVID-19 patients. Keywords : COVID-19 severity, neutrophil-lymphocyte ratio ABSTRAK Latar Belakang : COVID-19 adalah suatu penyakit yang disebabkan oleh Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) yang bersifat cepat menular sesama manusia. Peradangan yang terjadi secara sistemik dapat menyebabkan perubahan leukosit dalam sirkulasi yang dapat membuat perubahan pada neutrofilia dan limfositopenia. Rasio neutrofil-limfosit (RNL) pada pasien COVID-19 dapat digunakan sebagai prediktor keparahan penyakit pada fase awal infeksi penyakit. RNL yang meningkat dapat diartikan sebagai proses peningkatan inflamasi. Penelitian ini bertujuan untuk mengetahui hubungan antara RNL dengan derajat keparahan pasien COVID-19 di Rumah Sakit Martha Friska Medan tahun 2021. Metode : Penelitian ini memiliki desain analitik observasional dengan metode cross sectional. Sampel penelitian diambil dari data rekam medik pasien COVID-19 di Rumah Sakit Martha Friska Medan dari Januari-Maret 2021 dengan metode consecutive sampling dan didapatkan 74 sampel. Penelitian dilakukan pada bulan November 2021. Analisis data yang diolah dengan tabel distribusi frekuensi secara univariat dan dengan program SPSS versi 25 dilakukan secara bivariat menggunakan uji Chi Square. Hasil : Dari 74 pasien COVID-19, nilai RNL 3,13 didapatkan 29 orang (93%) dengan derajat keparahan ringan, 2 orang (7%) derajat keparahan sedang dan tidak didapatkan pasien dengan derajat keparahan berat-kritis. Pasien COVID-19 dengan RNL 3,13, sebanyak 6 orang (14%) derajat keparahan ringan, 20 orang (46%) dengan derajat keparahan sedang, 17 orang (40%) derajat keparahan berat-kritis. Penelitian ini menunjukkan terdapat hubungan antara RNL dengan derajat keparahan pasien COVID-19 (p=0,000). Kesimpulan : Terdapat hubungan antara RNL dengan derajat keparahan pasien COVID-19. Abstract

Background: Intracerebral hemorrhage is associated with a high mortality and disability rate, 40% of which happened in non-traumatic ICH patients. Putamen hemorrhage is the most common cause of ICH and strongly associated with hypertension. Until now, bilateral putamen hemorrhagic stroke is still a rare case. Case: Male, 54 years old, admitted to the ER for being unconscious 1 hour earlier. The patient had a history of hypertension but did not receive any treatment. The patient also had a history of schizophrenia and was taking Clozapine, Trifluoperazine, Sertraline, and Dimenhydrinate. Patient was presented with GCS 112, blood pressure 176/104 mmHg, SpO2 98%. Other physical examinations were within normal limits. Head CT scan showed acute ICH in the right putamen (vol ±7.8cc) and left putamen (vol ±20.7cc). Conservative management was given, but there was no neurological improvement. On day 14, complications of pneumonia occurred and the patient later died due to cardiopulmonary arrest. Discussion: Spontaneous bilateral putamen hemorrhagic stroke is a rare type of ICH and loss of consciousness is the main manifestation of the condition. Hypertension is the most common etiology of bleeding, but the exact mechanism is still unknown. Mortality in patients with spontaneous bilateral putamen bleeding is very high and to date, conservative therapy is the main treatment for patients with these conditions. Conclusion: Spontaneous bilateral putamen hemorrhagic stroke is a rare case with a high mortality rate and so far, the main treatment for this case is conservative management. Abstract

Background: The decidual cast represents a condition which the entire endometrial lining is shed in intact condition through the vagina. This phenomenon is infrequent and has been linked to various factors, including ectopic pregnancy and the use of exogenous progesterone. In the current report, we present a case finding of ectopic pregnancy upon discovery of decidual cast.Case: Female, 36 years old, came with complaints of pain in the pelvic area accompanied by patches of bleeding from the genitals since 1 week ago. On the 8th day since the complaint was felt, the patient complained of severe pain accompanied by discharge of tissue from the genitals. On histopathological examination of the tissue, a decidual cast was found in the absence of chorionic villi. The pregnancy test was positive and on ultrasound examination we found a mass with a cavity measuring 1.55 x 1.53 cm in the left fallopian tube so that the diagnosis of tubal ectopic pregnancy was established and a laparoscopic procedure was carried out. Discussion: The decidual cast is a proliferation of the endometrial lining that is released through the vagina in an intact form. This finding of decidual cast is often found in ectopic pregnancies and is associated with exposure to exogenous progesterone. The release of the decidual cast through the undilated cervix will cause severe pain referred to as membranous dysmenorrhea. The differential diagnosis of this decidual cast finding is the occurrence of abortion. However, the results of histopathological examination of the tissue obtained a decidual cast without chorionic villi so that the possibility of abortion can be removed.Conclusion: The identification of a decidual cast is frequently linked to the occurrence of an ectopic pregnancy. Therefore, its presence should alert clinicians to consider the possibility of an ectopic pregnancy and conduct further investigations.
