Abstract

Currently, there are no suggested psychological interventions or outcomes in the diabetic management guidelines. This case report presents the management of a 66-year-old female patient, Mrs. R, with diabetes mellitus, hypertension, and gastroesophageal reflux disease (GERD) who underwent exploratory laparotomy for retained stone exploration in the common bile duct (CBD). This Case report aimed to explore the implementation of mindfulness in patients post-operative. The patient experienced significant hyperglycemia, with blood glucose levels peaking at 436 mm Hg. Psychological assessment revealed a severe anxiety score of 31, which likely contributed to her unstable glycemic control. Throughout her unstable glycemic control. Throughout her severe condition, the patient was managed with total parenteral nutrition (TPN) due to fasting and green gastric contents, as part of the nursing intervention. Comprehensive nursing interventions, including regular blood glucose monitoring, insulin administration, and psychological support were employed. Mindfulness techniques such as guided meditation and breathing exercises have been introduced to alleviate hospital-induced distress and manage blood glucose levels. Following these interventions, her blood glucose level decreased to 167 mmHg, and the patient reported feeling calmer and more spiritually connected (Hamilton Rating Scale/ HARS 18). This case describes the management of post-surgical diabetic patients, highlighting the benefits of integrating psychological interventions to improve glycemic control and overall patient outcomes. Therefore, psychological interventions, particularly mindfulness, are needed for integration in diabetes management, especially in hospitals.

Abstract

Patient safety is a critical global health issue. Despite solutions from JCI and WHO, 70% of medication errors still occur in many countries, including Indonesia. Objective: To identify factors influencing patient safety implementation by nurses at Sulianti Saroso Infectious Diseases Hospital (SSIDH). Methods: This cross-sectional observational study was conducted at SSIDH in October-November 2024. The population included 150 nurses, midwives, and heads of inpatient wards, selected using purposive sampling. Data were analysed using Kolmogorov-Smirnov, chi-square, and multiple logistic regression (p<0.05). Results: Most respondents were female and held a bachelor’s degree in nursing. Knowledge, attitude, and motivation were not significantly associated with patient safety implementation. Factors significantly influencing patient safety were age, duty room, and tenure (p<0.05). Nurses aged ≥40 years were four times less likely to implement patient safety standards compared to those <40 years (OR=4.04; 95%CI=1.68-9.72). Those working in non-intensive wards were 0.11 times less likely to implement standards (OR=0.11; 95%CI=0.05-0.27). Conclusion: Nurses/midwives should enhance patient safety implementation based on the six patient safety goals. Healthcare providers are encouraged to create programs to support quality improvement and reduce adverse events.

Abstract

Nurse retention in conflict zones remains a critical global issue, significantly influencing the sustainability of healthcare services. Nurses in these areas face numerous challenges, including safety risks, emotional stress, and inadequate compensation, which impact their decision to continue working in such environments. This systematic review aims to identify and analyze factors influencing nurse retention in conflict zones and propose intervention strategies to address these challenges. A systematic review was conducted following PRISMA guidelines. Data were extracted from Scopus, PubMed, Web of Science, and CINAHL databases. Studies focusing on nurse retention in conflict zones were included, with data extraction guided by the PICOS framework. The quality of the included studies was assessed using Joanna Briggs Institute tools. A total of fifteen studies were reviewed, identifying key factors for nurse retention. These include improved working conditions, adequate safety measures, financial incentives (e.g., risk allowances), and emotional support systems. Job satisfaction, influenced by job security and a supportive work environment, was also a significant determinant of retention. Emotional and psychological support systems were found to reduce burnout, thereby fostering nurse retention. A multifaceted approach, incorporating improved working conditions, enhanced financial incentives, and robust emotional support, is essential for retaining nurses in conflict zones. Future research should focus on the long-term impact of these strategies and their adaptation to region-specific needs.

Abstract

The consequences of post-burn neck contractures are considered the greatest challenge in reconstructive surgery. This deformity affects the neck area thereby limiting head movement. These complications can be associated with loss of articulation, cosmetic disfigurement and psychological changes that impair quality of life and result in loss of productivity. Therefore, an appropriate surgical management are essential to obtain optimal results. ALT (anterolateral) free flap can be used to resurface the defect, this technique provides a long pedicle with adequate lumen diameter of blood vessels. The z-plasty technique is useful for redirect a scar into better alignment with a natural skin fold or the lines of least skin tension. Split-thickness skin graft (STSG) is a preferred technique for large wound areas and relatively avascular sites. This study aims to report our experience in reconstructing burnt mentosternal contractures with ALT free flap, z-plasty and STSG. We present a clinical case of a thirty-five years old male with post-burn mentosternal contracture ONAH classification type III, dermogenic type diffuse flexion contracture of the colli region, anterior thorax region, and left and right superior extremity regions, dermogenic type linear contracture of the left and right anterior axillary regions, and burned ear deformity in the left and right auricular due to a history of burns from twenty-four years ago. A contracture release procedure was performed with ALT free flap reconstruction, z-plasty and STSG. After the reconstruction, a pleasing outcome was accomplished in terms of both functionality and esthetics. A severe mentosternal contracture was released, and the limited movement of the neck and both extremities was greatly lessened. Mentosternal contracture reconstruction with ALT free flap, z-plasty and STSG showed a good outcome in this case.
