Abstract

Maternal mortality remains a major challenge in Indonesia, particularly due to limited access to health services and low awareness of the importance of regular pregnancy check-ups. The use of information technology in the health sector, such as telemedicine, has shown great potential in improving health services, including for pregnant women. This study aims to examine applications for monitoring the health of pregnant women with chronic energy deficiency (CED), which can support medical personnel and pregnant women in monitoring health effectively. The research design is a Systematic Literature Review of articles or journals published in 2019-2024, accessed on Google Scholar, PubMed and Scopus with the Prisma Flow technique. This study found that telemedicine and digital health applications have great potential in improving access and quality of maternal health monitoring. The Android-based system allows for effective monitoring of nutritional status, vital signs, and communication between pregnant women and medical personnel. The use of digital technology-based health applications can be an effective solution to support monitoring the health of pregnant women so that it can help reduce maternal mortality rates (MMR) and infant mortality rates (IMR) in Indonesia through wider and more efficient access to health information.

Abstract

Mamuju, as one of the cities in West Sulawesi, Indonesia, is highly prone to earthquakes. Recently, on January 15, 2021, Mamuju was struck by a major earthquake with a magnitude of 6.2, accompanied by hundreds of aftershocks. The disabled community, particularly students in Special Schools (SLB), faces unique challenges during disasters. Various physical, sensory, or cognitive limitations necessitate a tailored approach to disaster preparedness education. The School Watching approach can enhance awareness and collaboration among stakeholders, including local governments, educational institutions, and organizations focused on disability issues. Objective to improve earthquake preparedness among disabled students at SLB Mamuju through the implementation of the School Watching approach, aiming to create an inclusive, adaptive, and resilient school environment in disaster situations. This study is a quantitative research with a quasi-experimental design, using a pretest and posttest control group approach, conducted from May to July 2024. The study population consisted of SLB students in Mamuju Regency, with a total of 40 respondents divided into two groups using the total sampling technique. The intervention group of 20 students received disaster preparedness education and the School Watching approach for earthquake preparedness twice, each session lasting 60 minutes. The control group was only provided with earthquake preparedness material for 60 minutes without the School Watching approach. Before delivering the material, an initial measurement was conducted to evaluate the students' preparedness and knowledge related to earthquakes, as well as after the material delivery. Descriptive statistical tests were used to summarize the characteristics of the respondents and the pre-test and post-test scores. Subsequently, paired t-tests were conducted to compare the pre-test and post-test results. The School Watching approach had a significant impact on disaster preparedness among disabled students at SLB Mamuju. This was demonstrated by a significant difference between the intervention group and the control group. The School Watching program proved effective in improving earthquake preparedness among disabled students. This is evidenced by a significant difference in the pre-test and post-test results of the intervention group compared to the control group.

Abstract

Healthcare delivery in archipelagic regions faces significant challenges, including limited access, inadequate infrastructure, and a shortage of healthcare professionals. Achieving universal health coverage in these regions requires innovative strategies and approaches. This study aims to identify regulations, challenges, and provide recommendations to improve access to healthcare services in archipelagic regions. A literature review method was employed, utilizing two databases, Google Scholar and ScienceDirect. Keywords used include “Pelayanan Kesehatan di Kepulauan” OR “archipelago healthcare” OR “health services in islands”. From 515 articles with publications from 2019 – 2024 initially retrieved, only seven were analyzed after screening using the PRISMA diagram based on predetermined criteria. The findings reveal that common challenges across archipelagic regions include geographical conditions, infrastructure limitations, and shortages in healthcare workforce quantity. Innovative efforts, such as telemedicine, hospital ships, health boats, and village health posts, have been implemented in several countries with similar geographical characteristics and proven effective in overcoming these access barriers. In conclusion, despite the complex challenges in archipelagic regions, existing regulations and various implemented efforts have improved healthcare access, fulfilling community health needs. These initiatives contribute to accelerating the equitable delivery of sustainable and high-quality healthcare services.

Abstract

Nursing managers face challenges in implementing Quality Improvement (QI) programs for pressure injury prevention in hospitals. Objectives: To identify problems faced by nurse managers in implementing QI programs and to develop problem-solving solutions. Mini-project on improving the quality of care room management and analysis of results and implementation gaps based on literature review. Data collection through interviews, observations, and questionnaires in the internal medicine inpatient ward. Data were analyzed using the 5 Whys technique and fishbone diagram. Problem-solving solutions used the FOCUS PDSA approach. It was found that the direction and control functions of the quality improvement program in efforts to prevent pressure injuries were not optimal. The implementation of solutions in the form of improving pressure injury data management, staff re-education, making mini posters, and evaluating the impact of pressure injuries on costs. The evaluation results showed that all at-risk patients had received preventive interventions and no new cases of pressure injuries were found. Strengthening the direction function of the head of the room through staff re-education, poster installation, and effective monitoring can improve the implementation of the QI program in preventing pressure injuries in hospitals.

Abstract

Electronic medical records (EMR) are an information technology innovation that has been adopted in various health facilities to improve operational efficiency and quality of health services. In addition to its benefits, the use of EMR also presents a number of challenges for doctors in hospitals. This study aims to evaluate the positive and negative impacts of EMR use on doctors in hospitals. This study used a scoping review method using the PRISMA guideline, and produced 13 articles that met the inclusion criteria related to the impact of EMR on doctors in hospitals. The results of the study showed that EMR had a positive impact on improving efficiency, quality of care, and patient safety, but also increased the administrative burden. The use of EMR outside of working hours increases the risk of burnout and decreases satisfaction with work-life balance. In addition, doctor-patient interaction can be reduced, and concerns about data privacy reduce patient openness. Hospitals need to optimize workflows, provide comprehensive and ongoing training, delegate administrative tasks to non-clinical staff and develop more user-friendly EMR systems. This is important to ensure that EMR technology can support improved quality of care without sacrificing doctor well-being and interaction with patients.
