Abstract

Spinal tuberculosis (TB) is the most common form of skeletal TB, with the thoracolumbar junction being the most frequently affected region, while cervical spine TB remains relatively rare, accounting for only 3% to 5% of cases. Historically, non-operative treatment was the only option in the pre-antibiotic era. However, with the introduction of effective combination chemotherapy in the early 1950s, the mortality rate for spinal TB significantly decreased to 1%–3%. Despite advancements in medical therapy, surgical intervention remains crucial in cases involving neurological complications and kyphosis prevention, with instrumentation playing an integral role in managing spinal pathologies. This study aims to evaluate the effectiveness of surgical therapy versus conservative therapy in the management of tuberculosis spondylitis through a systematic review and meta-analysis following PRISMA guidelines and the PICO format. The research includes randomized controlled trials (RCTs), observational, quasi-experimental, and case-control studies, while studies not directly assessing treatment effectiveness were excluded. A comprehensive literature search using PubMed, Sage Journal, The Lancet, and ScienceDirect initially retrieved 1,499 publications. Through a rigorous three-stage screening process, only 10 articles published between 2014 and 2024 were selected for final analysis. The findings indicate that both conservative anti-TB drug therapy and surgical treatment are safe and effective, particularly for early-diagnosed patients with fewer affected vertebrae. Conservative therapy provides good clinical and radiological outcomes, while surgical intervention offers advantages such as thorough debridement, spinal cord decompression, and improved spinal stabilization. The implications of this study suggest that treatment selection should be tailored based on disease severity and patient-specific conditions. Further research should focus on long-term comparative studies, minimally invasive surgical techniques, and biomechanical modeling to optimize treatment strategies for spinal TB..

Abstract

Low back pain is a frequently seen problem in the society and causes loss of productivity. In recent studies, inflammatory mediators and inflammation itself has an efficient role in pain mechanism. We aimed to investigate the association between the neutrophil to lymphocyte ratio level as an inflammatory biomarker with pain severity patients with chronic low back pain. A retrospective analysis was conducted on medical records of low back pain patients between Januari until December 2024 in Neurology Department of Bhakti Rahayu Denpasar Hospital. Seventy-three medical record patients between 27-81 years old were included in the study. The neutrophil-to-lymphocyte ratio (NLR) is calculated by dividing the absolute neutrophil count by the absolute lymphocyte count based on blood samples. The patients were divided into two groups based on pain severity. The average age of the participants was 56,63 years (±15,56) (p= 0,077), with 40 males (54,85) and 33 females (45,2%) with average neutrophil count 5,99 x 109/L (±1,45) (p= 0,200) and lymphocyte count 1,75x 109/L (±0,39) (p= 0,093). NLR cut-off was 3,55 (±0,72) (p= 0,200). Higher NLR correlated with age (r = 0.243, p = 0.038) and pain score (r = 0.394 p < 0.001). High NLR increased the risk of 33, 43 LBP patients experiencing moderate pain (p= <0.001). NLR may be used as a simple and reliable premise independent predictor of pain severity in patients with chronic low back pain

Abstract

Interior design is not only limited to arranging the room to make it look attractive. There are fundamental reasons why we need interior design, which must be able to create a room that is harmonious, aesthetic, meets the demands of its activities and the psychological demands of its community so that the room is functional. The furniture and equipment facilities of the pharmaceutical laboratory have been standardized. For color standardization does not yet exist, it is necessary to analyze the color in the interior design of the pharmaceutical laboratory through a total ergonomics approach to be able to provide comfort and focus for staff. The method used is descriptive analysis method. Color psychology is a theory of color studies related to human psychology. To balance the room, it must also be considered the appropriate color implemented in interior design to achieve balance and harmony in the room. Designing the interior of a pharmaceutical laboratory is not just about implementing the colors we like, but must have sufficient meaning so that it can have an impact on behavior, mood, cognition, focus, and general association. For this reason, it is considered important to study the total Ergonomics approach in order to get the optimal improvement solution. The results of the analysis obtained colors that are suitable to be applied to the interior of the pharmaceutical laboratory include neutral colors of white, light gray, and may slightly apply black as an accent. Besides there are several companion colors to be combined with neutral colors including white heron, crystalline, windmill wings, golden straw, thunder. By implementing these colors, it can provide comfort and focus for laboratory staff to improve performance and productivity.

Abstract

TB is caused by Mycobacterium tuberculosis (M. tb). Tuberculosis (TB) prevalence is increasing in developed nations and continuing to cause significant mortality in low and middle income countries. When M. tb disseminates to the vertebral column, it is called Pott’s disease or spinal TB. The frequency, symptoms, and severity of the disease range by the location of the spine and the region of the affected vertebrae. The delay in establishing diagnosis and management results in complications such as spinal cord compression and spinal deformity. Tuberculous spondylitis is the most dangerous form of musculoskeletal tuberculosis because it can cause bone destruction, deformity and paraplegia. The spread of tuberculosis infection will cause inflammation in paradiscs, and causes progressive bone destruction and then will make vertebral collapse and deformity arising shaped kyphosis (posterior angulation) called gibbus. The role of surgical debridement and fusion with TB is controversial, and good results may be obtained with either medical treatment or surgery. Surgery may be more appropriate when definite cord compression is documented or for complications such as abscess or sinus formation or spinal instability.

Abstract

This study aims to analyze the relationship between hemoglobin (Hb) levels and early mobilization on surgical wound healing in post-sectio caesarean section patients. The research design uses a quantitative approach with a cross-sectional analytical observational method. The study was conducted at Metta Medika Padangsidimpuan Hospital on 33 patients who were selected using the purposive sampling technique. The data was analyzed by the Chi-Square test to determine the relationship between variables. The results showed that the Hb level ≥ 11 g/dl was related to better healing of surgical wounds compared to Hb < 11 g/dl (p = 0.013). In addition, rapid and normal early mobilization significantly improved wound healing compared to slow mobilization (p = 0.025). This study concluded that Hb levels and early mobilization had a significant relationship with wound healing in post-sectio caesarean section. These findings can be used as a reference in postoperative management to improve the quality of patient recovery.

Abstract

The availability of platelets in the Indonesian Red Cross (PMI) Blood Transfusion Unit (UDD) has been significantly high, with 767,680 bags collected in 2019, increasing to 957,397 bags in 2021. The dengue fever outbreak alongside the COVID-19 pandemic led to a nearly twofold increase in the demand for concentrated platelets (TP) in hospitals from 2021. However, platelet additive solution (PAS), which can extend platelet shelf life, is not yet utilized in Indonesia. This study aims to investigate whether TP obtained using the buffy coat leukofiltered pooling method and stored with PAS can improve platelet quality during storage. A series of laboratory tests were conducted to evaluate platelet count, pH levels, and aggregation function using ADP inspection, monitored over a storage period from day zero to day seven. The results indicate that the addition of PAS effectively maintains platelet quality, pH stability, and platelet function throughout the storage duration. This is attributed to the nutrient-rich composition of PAS, which contains glucose, sodium acetate, sodium chloride, phosphate, magnesium, and potassium, ensuring the preservation of platelet integrity. The findings highlight the potential benefits of PAS implementation in Indonesia to enhance platelet storage efficiency and availability, particularly during periods of high demand. Future research should explore the optimal PAS formulation for different storage conditions, cost-effectiveness, and clinical efficacy in transfusion settings to support broader adoption of this technology in blood banking.

Abstract

The most common regional anesthesia procedure used in various operations, especially operations on the lower abdomen and lower extremities, is spinal anesthesia. Urinary retention is a urination disorder, where the flow of urine is weak, not flowing smoothly, and there is a feeling of something remaining and dissatisfaction when urinating, resulting in discomfort. Urinary retention is one of the common complications that occurs after spinal anesthesia. This study aims to determine the comparison of the incidence of urinary retention after spinal anesthesia with lidocaine 2% 60mg and bupivacaine 0.5% 10mg. the test results showed that the p-value = 0.500 (p>0.05), so H1 was rejected and H0 was accepted, which means there is no significant relationship between the incidence of urinary retention and the anesthetic drugs lidocaine or bupivacaine. There is no significant difference regarding the incidence of urinary retention during spinal anesthesia between lidocaine 2% 60 mg and bupivacaine 0.5% 10 mg in postoperative patients at PKU Muhammadiyah Gamping Hospital.

Abstract

Phacomorphic glaucoma is a secondary angle-closure glaucoma caused by lens thickening due to cataract formation process, potentially leading to increased intraocular pressure and a risk of permanent optic nerve damage, resulting in visual impairment. This report discusses a case involving a 57-year-old female presenting with blurred vision and pain in one eye, diagnosed with phacomorphic glaucoma with senile immature cataract based on history taking, and ophthalmological examination. The findings revealed decreased visual acuity, lens opacity in the left eye with a positive shadow test, increased iop, and shallow anterior chamber. the patient initially received medical therapy to lower intraocular pressure, followed by definitive treatment, which consisted of a combination of phacoemulsification with intraocular lens implantation and trabeculectomy. postoperative results showed symptom and visual acuity improvement, although the prognosis for visual function depends on the degree of optic nerve damage prior to treatment. primary management of phacomorphic glaucoma involves cataract extraction, which causes pupil block, that is often combined with trabeculectomy to effectively control intraocular pressure.

Abstract

Resistance training has grown in importance for managing oxidative stress, particularly in adults, as it enhances muscle development and overall health. However, it also leads to the formation of reactive oxygen species (ROS), which can result in cellular oxidative damage and potentially hasten the aging process. This review assesses the role of astaxanthin, a powerful antioxidant, in counteracting oxidative stress during resistance training, with a focus on its effects on aging. This study employs a narrative review methodology, evaluating peer-reviewed publications from 2019 to 2024. Keywords used include "Aging," "Antioxidant," "Astaxanthin," "Oxidative Stress," and "Adults." Twenty-five articles were selected for in-depth analysis. The findings suggest that astaxanthin supplementation notably decreases oxidative stress markers, particularly plasma malondialdehyde (MDA) levels, in individuals undergoing resistance training. On a molecular level, astaxanthin activates the Nrf2 pathway, which is responsible for regulating antioxidant enzyme production and supports mitochondrial biogenesis through the PGC-1α pathway. This dual mechanism not only reduces oxidative damage but also enhances energy metabolism, helping maintain cellular function and delaying age-related decline. In conclusion, incorporating astaxanthin supplements into resistance training routines offers promising potential for reducing oxidative stress and promoting healthy aging in adults.
