Abstract

Drug abuse has emerged as a major and deeply concerning challenge, particularly due to the high incidence of relapse among abusers. One of the crucial factors to consider in preventing relapse is self-control. This study aims to identify the relationship between self-control and the tendency for relapse among drug abusers undergoing social rehabilitation at Sentra Insyaf Medan. This research employed a quantitative analytic method with a correlational design, involving 98 respondents selected through purposive sampling based on specific inclusion criteria. Data were collected using two questionnaires: a self-control questionnaire and a relapse tendency questionnaire developed based on indicators of addictive behavior, relapse history, and intention to reuse drugs. Data analysis was conducted using the Spearman Rank correlation test. The results showed that 39.8% of drug abusers had low self-control and 41.8% exhibited a high tendency for relapse. A significant negative relationship was found between self-control and the tendency for relapse, with a p-value of 0.001 (< 0.005) and a correlation coefficient (r) of -0.564, indicating that higher self-control is associated with a lower tendency for relapse. These findings highlight the importance of strengthening rehabilitation programs to support the development of self-control, emotional regulation, and decision-making skills in order to prevent relapse among drug abusers.          Keyword: Self-Control, Relapse Tendency, Drug Abusers, Rehabilitation

Abstract

The increasing prevalence of emotional and behavioral problems among teenagers necessitates effective, early intervention. The World Health Organization recommends schools as key sites for mental health promotion. However, the sustainability of these programs is often a significant challenge. Empowering teachers to handle students' emotional and behavioral issues is a critical strategy to improve program sustainability. This study aimed to evaluate the impact of a school-based mental health program on teachers' abilities to manage students' emotional and behavioral problems. This study utilized a one-group pretest-posttest design. A total of 60 teachers from SMP Negeri 1 Tanjung Morawa participated as the sample. Teachers' abilities were measured using a questionnaire before (pretest) and after (posttest) the implementation of the school-based mental health program. Data were analyzed using a dependent t-test to compare the mean scores. A significant improvement in teachers' abilities was observed after the program. The mean ability score increased from 50.13 at pretest to 75.20 at posttest, representing a mean difference of 25.07 (a 50.01% increase). This increase was statistically significant (p < 0.001, with an alpha of 0.05). The school-based mental health program was effective and had a substantial influence on improving teachers' abilities to handle students' emotional and behavioral problems. These findings support teacher empowerment as a viable strategy for implementing and sustaining mental health programs in school setting. Abstract

Modern nursing faces increasing challenges, including depersonalization, burnout, and the fragmentation of care delivery. In response, the Theory of Communion-in-Caring provides a transformative framework that re-centers nursing on its relational, moral, and spiritual dimensions. The objective of this review is to critically examine the theory’s conceptual, methodological, and practical contributions to nursing science. A qualitative content review was conducted on the first edition of the book, focusing on its philosophical assumptions, theoretical foundations, methodological proposals, practical applications, and future directions. Thematic analysis guided the data organization, using conceptual clarity, methodological rigor, contextual relevance, and interdisciplinary potential as key evaluative criteria. The review highlighted the theory’s grounding in Filipino cultural values and global relational ethics, positioning caring as a sacred communion of mutual presence and transformation. The theory promotes methodological pluralism, including phenomenology, narrative inquiry, and mixed methods that align with its relational ontology. It offers concrete strategies for nursing education, clinical practice, and administration, with promising interdisciplinary and community-based applications. Despite challenges in measurement and implementation, the theory presents a culturally authentic and philosophically coherent model of care. In conclusion, the Theory of Communion-in-Caring makes a timely contribution by restoring dignity, empathy, and relational meaning in nursing, offering a valuable paradigm for both local and global healthcare contexts. Abstract

Well-being is a unique phenomenon perceived by nurses. Nurses have their own definition of well-being based on their work experience. This study aimed to examine the nurses’ workplace well-being from the perspective of Indonesian staff nurses within hospital settings. A qualitative interpretive descriptive design was employed in this study. Participants were recruited from a university hospital in Medan, Indonesia, comprising 10 staff nurses representing the emergency department, intensive care unit, and inpatient wards. Data were collected through a focus group discussion and analyzed using thematic analysis. Data analysis identified three overarching themes that encapsulate staff nurses perspectives on workplace well-being: the ideal concept of nurses workplace well-being, contributing factors to nurses’ workplace well-being, and nurses expectations of hospital administrators to enhance nurses workplace well-being.  Staff nurses hold distinct expectations regarding their workplace well-being, which are shaped by personal and professional factors. Several contributing elements influence their experiences, including organizational support, workload, and interpersonal relationships. The findings indicate that Indonesian staff nurses perceive workplace well-being as a multifaceted and challenging issue. They highlighted their hopes and expectations concerning workplace well-being across psychological, physical, financial, inter-professional relationship, and hospital facilities dimensions. It is imperative that nurse managers and hospital administrators give careful consideration to these concerns and implement evidence-based, targeted strategies to mitigate the challenges that adversely affect nurses’ well-being. Abstract

The main problem among adolescents related to menstruation is the lack of knowledge about Menstrual Health Management (MHM) and poor water sanitation. This study aims to determine the effectiveness of MHM on the knowledge and personal hygiene behavior of elementary school students in Medan. The research employed a quasi-experimental design with a one-group pretest-posttest approach. The population is all elementary school students in grades 4 and 5. The sample included 67 female and 43 male students, totaling to 110 participants. Data were collected using a questionnaire administered before the intervention (pretest), followed by a health education program consisting of lectures, discussions, and role plays, supported by LCD/PowerPoint presentasions, leaflets, and videos, and then after the intervention (posttest).The sampling technique used was total sampling. Dependent t-test analysis was utilized to assess the data. The results showed a significant difference in respondents’ knowledge before and after the intervention. For attitudes, the mean difference was 3.440 with a p-value of 0.000, indicating a significant effect of the intervention on respondents’ attitudes. Specifically, for male students, the mean difference in knowledge was 7.721 (p = 0.000), and the mean difference in attitudes was 1.628 (p = 0.000), confirming a significant improvement in both knowledge and attitudes after the intervention. Therefore, it is recommended that health workers implement MHM education programs to improve adolescent reproductive health status. Abstract

In 2023 Indonesian Health Survey reported that 0.6% of Indonesian women had their first pregnancy at ages 10-14, and 25.8% at ages 15-19. Medical records from the MCH Clinic at Putri Ayu Community Health Center in Jambi City stated around 164 adolescent pregnancies (ages 14-19) over the past five years (2020-2024), but only 120 births were recorded. This was a cross-sectional study using secondary data from the MCH Clinic and Independent Midwife Practices in the working area of Putri Ayu Community Health Center in Jambi City from January 01, 2020 to December 31, 2024. This study aims to analyse factors associated with premature births among adolescents in the working area of the Putri Ayu Community Health Centre in Jambi City. The study sample included all 120 pregnant adolescents (ages 14-19). Data were analysed using univariate, bivariate, and multivariate. Univariate analysis was presented as a frequency distribution, and bivariate analysis used a chi-square test to examine the relationship between all characteristic variables and premature birth. Multivariate analysis used logistic regression to determine which variable had the most significant influence on premature birth. The chi-square results indicated that all determinants (maternal age at pregnancy, maternal nutrition during pregnancy, frequency of antenatal care, and history of abortion) were significantly related to premature birth (p-value = 0.05). The logistic regression analysis showed that maternal age at pregnancy was the most dominant factor influencing premature birth, indicated by the highest odds ratio (OR = 7.034), while nutritional status was not statistically significant in relation to premature birth (p-value= 0.309). Abstract

Preeclampsia is a condition with increased blood pressure in pregnant women that occurs after 20 weeks of gestation. Maternal behaviour in pregnancy care is important to study to prevent the risk of preeclampsia, such as knowledge, attitudes and actions. This study aims to identify the relationship between the behaviour of pregnant women in knowing hypertension with the incidence of eclampsia in pregnant women in the working area of Puskesmas Kampung Baru Medan. A quantitative, descriptive-correlational design was employed using a cross-sectional approach. Data were analyzed using frequency distribution and percentage tables. The data in this study are presented in the form of frequency distribution tables and percentages. The findings revealed that the majority of respondents demonstrated adequate knowledge (55%), positive attitudes (62.5%), and appropriate preventive actions (82%). A moderately strong correlation was found between maternal knowledge and the incidence of preeclampsia (p = 0.00; r = 0.398). However, no significant associations were observed between attitudes (p = 0.204) or preventive actions (p = 0.117) and preeclampsia occurrence These results underscore the need to enhance maternal knowledge regarding pregnancy-related hypertension to effectively prevent preeclampsia. Abstract

A patient with Chronic Kidney Disease Stage V presented to the Emergency Department (ED) with severe shortness of breath, caused by pulmonary edema resulting from non-compliance with fluid restriction. The patient and her husband consented to the use of a mechanical ventilator and dialysis; however, they declined to authorise cardiac and pulmonary resuscitation in the event of cardiac arrest or to establish a Do-Not-Resuscitate (DNR) order. This presents a dilemma, as patients with requiring ventilatory support typically necessitate ICU admission; nevertheless, patients with DNR orders are generally not admitted to ICU. The situation worsened when the extended family opposed the DNR Order established by the patient and her husband. This case report aimed to provide solutions for nurses addressing ethical dilemmas concerning the determination of DNR orders. This case report follows the CARE Case Report Guidelines. Following immediate and continuous care in the emergency department and intensive care unit, the patient demonstrated significant improvement. The nurse resolved ethical issues by organizing a family conference that included the patient’s family and care team, discussing the treatment plan on the basis of the most recent assessment results. Nurses’ proactive engagement of multiple stakeholders in patient care within the hospital effectively addresses Do-Not-Resuscitate order-associated ethical dilemmas. To effectively address ethical conflicts, nurses should improve their emphatic communication skills, prioritizing patient interests. Abstract

Teens involved in the juvenile justice system are particularly vulnerable to anxiety disorders due to recurrent exposure to adverse experiences and lack of psychological support. This systematic review aimed to evaluate the efficacy and implementation of Cognitive Behavioral Therapy (CBT) for reducing anxiety among justice-involved adolescents across diverse international contexts. Comprehensive searches were conducted across PubMed, Scopus, PsycINFO, ScienceDirect, and Google Scholar for studies published up to August 2025 involving adolescents aged 10–18 with experience in juvenile justice settings. Cohort, quasi-experimental, randomized controlled trials (RCTs), and pre–post intervention designs were included, and methodological quality was assessed using the Joanna Briggs Institute (JBI) Critical Appraisal tools. Ten studies across the US, Germany, Nigeria, Turkey, Thailand, China, and Sweden met the inclusion criteria. Most reported significant reductions in anxiety, particularly in group-based or trauma-focused CBT interventions. Protocol adherence, practitioner skill, and cultural adaptation were identified as critical success factors, while implementation challenges included resource limitations and staff turnover. In conclusion, CBT demonstrates robust potential as an effective and adaptable intervention for anxiety reduction among justiceinvolved youth, especially when culturally tailored and trauma-informed. Further studies should focus on long-term outcomes and context-specific adaptation for resource-limited settings such as Indonesia. Abstract

Spousal loss is one of the most profound life events in old age, often accompanied by psychological challenges such as loneliness, depression, and reduced self-esteem. Self-esteem plays a vital role in older adults’ mental health, influencing their ability to adapt to aging and cope with bereavement. This study aimed to examine the relationship between spousal loss and self-esteem among older adults. A correlational quantitative design with a cross-sectional approach was employed. A total of 87 older adults were recruited using purposive sampling. Data collection was conducted from March to April 2023 using the Rosenberg Self-Esteem Scale (RSES) questionnaire. Data were analysed using univariate analysis and bivariate analysis with the chi-square test. The average age of the respondents was 66 years. Most participants were female, of Javanese ethnicity, Muslim, residing in Malang City, had low educational attainment (elementary school), worked in the private sector, and were in stable health condition. The analysis revealed a significant association between spousal loss and self-esteem among older adults (p-value < 0.0001). Self-esteem in older adults is associated with the presence of a spouse. Those who still have a living spouse tend to exhibit higher levels of self-esteem. Psychosocial interventions, as well as family and community support, are essential to enhance self-acceptance and self-esteem among older adults who have experienced spousal loss.
